An AI reviewer can sound confident while missing a CORS failure, race condition, memory leak, null dereference, or broken state mutation. The proposed fix is not another chatbot: it is an open-source workflow that makes an AI reviewer produce line-level evidence, pass deterministic checks, validate hypotheses against runtime errors, attack its own security fixes, and survive a second independent review before merge.

Mohamed Saleh described this approach in a September 6, 2026 Dev.to article as a set of prompts and mandatory gates. It is a practical design proposal, not an independently benchmarked product. You can adapt the pattern to backend, web, mobile, or desktop repositories, but you should measure its false positives, missed defects, and review time in your own codebase.

What is this unforgiving AI QA system?

It is a strict prompt-and-workflow suite for making an AI coding assistant behave more like a hostile QA engineer than a friendly pair programmer. The workflow does not grant approval because the code looks plausible. A finding must be tied to an exact file path and line number, required checks must be completed in order, and unsupported claims invalidate the review.

The source describes four open-source, stack-specific variants: a backend and cloud auditor with 19 phases, plus web, mobile, and desktop auditors with 13 phases each. The phase counts are the author’s design figures, not a certification or a test result. The project is intended to be forked and adapted rather than installed as a physical appliance or treated as a replacement for your existing CI system.

  • Read Before You Buy — No Video Output: These adapters support charging and USB 2.0 data transfer, but cannot transmit video signals. Except for standard USB webcams (which use USB data only), they are not compatible with HDMI/DisplayPort cables, video-capable USB-C hubs, or docking stations with video output.
  • Convert USB-A Ports to USB-C: Designed to connect USB-C earphones, cables, flash drives, card readers, and other USB-C accessories to standard USB-A ports. Plug-and-play with no drivers or software required.
  • Aluminum Alloy Housing: Built with a sturdy aluminum alloy shell that aids in heat dissipation and protects against daily wear and scratches. Designed to maintain a stable and secure connection.
  • Compact & Travel-Friendly: The ultra-compact design allows the adapter to stay plugged into your device without blocking adjacent ports or adding bulk, reducing wear and tear on your original USB ports.
  • 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.

Its central idea is a two-part pattern:

  • Deterministic checks establish facts through compilation, type checking, AST or linter analysis, and mutation testing.
  • Adversarial model review generates failure hypotheses, attempts to demonstrate counterexamples in an isolated sandbox, and records evidence for each conclusion.

That separation matters. A language model can suggest where a race condition might exist, but a compiler, test runner, mutation score, or runtime event can confirm or reject parts of the hypothesis without relying on the model’s confidence.

Why does AI say my code looks great when it is broken?

Most AI code reviewers optimize for a useful-looking response, not for proving that a claim is true. They infer intent from the diff, recognize familiar patterns, and often stop after finding no obvious syntax error. A review can therefore be articulate and still miss failures that appear only under a particular request order, input shape, device, deployment configuration, or production load.

Several conditions make this more likely:

  • The model sees source text but no live telemetry, so it cannot know that a promise rejects in production or that a memory footprint grows over time.
  • It tends to accept the author’s stated goal and may treat a plausible implementation as evidence that the goal was met.
  • Security fixes are reviewed as finished solutions unless the prompt explicitly asks for bypass attempts.
  • A single pass can anchor on its first interpretation and ignore a defect that a different reviewer would notice.
  • Without a precise citation requirement, vague warnings and vague approvals are difficult to audit.

The unforgiving workflow changes the incentives. The reviewer must either show a reproducible path to a defect or label the hypothesis unproven. “Looks safe” is not a sufficient result.

How does the workflow combine deterministic tests with adversarial reasoning?

Run objective checks before asking the model to interpret ambiguous behavior, then make the model test its own theories rather than merely list them. A useful division of labor looks like this:

Layer Typical work Required output Merge implication
Deterministic gate Compilation, type checks, AST or linter checks, and mutation testing Machine-readable pass, failure, or score with the command and revision recorded A failed required gate blocks advancement
Adversarial hypothesis Search for race conditions, null dereferences, unhandled rejections, resource leaks, and similar failure modes Specific hypothesis linked to code locations and a proposed counterexample An unsupported concern is not treated as a confirmed defect
Sandbox verification Attempt to trigger the counterexample in an isolated environment Reproduction steps, observed result, logs, or an explicit “not reproduced” result Evidence determines severity; confidence alone does not
Runtime cross-check Compare static findings with production or staging telemetry Matching or conflicting runtime events, with their time window and service context Deployment decisions include observed behavior, not just source inspection

The implementation details above are an example design described by Saleh, not an independently measured benchmark. Teams should keep the deterministic commands in CI and give the model only the outputs and source ranges it needs to reason about. That makes the review reproducible and limits the chance that a model silently invents a successful test.

What does “Citation Or It Didn’t Happen” require?

Every material assertion must identify the exact file path and line number that supports it. If the reviewer cannot cite a location, it must not present the statement as an established finding.

  1. Ask the reviewer to state the claim in one sentence, such as “a request can use an expired authorization result.”
  2. Require a path and line range for the source that creates the condition and, where relevant, the code that consumes it.
  3. Request the triggering sequence, input, timing, or state transition that would make the condition observable.
  4. Have the reviewer classify the claim as confirmed, reproduced, not reproduced, or unproven.
  5. Reject any item whose citation points to a nearby but unrelated line, a generated file, or code that is not executed by the affected path.

A good citation is more than an address. It connects the line to a control-flow or data-flow explanation and to an observable consequence. For example, a null check at one line is not proof of safety if another branch reaches the dereference without that check. The reviewer should cite both branches and explain the input that selects the unsafe one.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Line references are revision-sensitive. Store the commit identifier with the report, and regenerate citations after an automatic formatter or merge changes line numbers. Otherwise a once-valid review can become impossible to audit.

How do mandatory phase gates stop the review from skipping hard work?

A phase gate makes the reviewer print a completion checklist before it can advance. The model is not allowed to jump from a quick scan to an approval simply because the diff is small or familiar.

A practical gate record contains:

  • the phase name and the commit being reviewed;
  • the commands or observations completed;
  • findings with file paths, line numbers, and evidence status;
  • items that could not be tested, with the reason;
  • the explicit decision to proceed, stop, or request a human decision.

The backend and cloud variant is described as covering 19 strict phases, including Zero Trust controls, chaos engineering, OWASP API Top 10 concerns, and advanced forensics. The web, mobile, and desktop variants each contain 13 phases tailored to their failure modes. Those numbers should be treated as workflow coverage targets, not as a guarantee that every phase catches a defect.

Gates are useful only when they are enforceable. Put the checklist in the pull request or CI artifact, fail the job when a required phase is missing, and prevent an approval label from being added by the same unverified model that produced the report.

How is runtime evidence added with Sentry?

The workflow’s “Real-World Sentry Validation” step cross-checks static hypotheses against runtime errors before deployment. Static analysis might predict an unhandled rejection in a code path; telemetry can show whether that path is actually failing, where, and under which release.

  1. Record the static hypothesis with its cited path, lines, affected release, and expected symptom.
  2. Query the relevant Sentry project for matching exceptions, rejected promises, performance events, or release regressions.
  3. Compare timestamps, stack traces, environment, request route, device, and release identifier. A similarly named error from another service is not confirmation.
  4. Label the result as corroborated, contradicted, or not observable in the available telemetry.
  5. Use the result to prioritize a fix or a targeted reproduction; do not treat the absence of an event as proof that the bug is impossible.

Telemetry has blind spots. Sampling, disabled instrumentation, privacy filters, short retention, and untested environments can hide a real failure. The workflow therefore treats runtime data as an additional evidence source, not as a replacement for tests or a reason to ignore a plausible defect.

How does the hacker-mindset rule test a proposed security fix?

Rule 6 requires the reviewer to try to bypass its own security recommendation. The first pass might propose input validation, an authorization check, a safer default, or a stricter content policy. The red-team pass then assumes the fix is incomplete and searches for an alternate route.

Ask the attacker persona to examine:

  • alternate endpoints, methods, or content types that bypass the guarded path;
  • different encodings, casing, duplicate parameters, and boundary values;
  • replay, race, privilege-confusion, and time-of-check/time-of-use scenarios;
  • client-side checks that are not repeated on the server;
  • error paths, background jobs, migrations, and administrative interfaces that use the same data.

Require a concrete bypass attempt in a sandbox or test harness. If it fails, record what blocked it and where that control lives. If it succeeds, the original fix is incomplete and the review should cite the bypass path rather than simply raising the severity label.

This rule is deliberately uncomfortable: the model must challenge its own earlier answer. It does not turn an AI into a penetration tester, and it cannot authorize testing against systems you do not own. Keep experiments isolated and within your approved security scope.

Why require a fresh-eyes second pass?

“Fresh-Eyes Re-Analysis” asks for a second review performed as a different reviewer. The second pass should receive the code, test results, and relevant evidence, but not inherit the first reviewer’s conclusion as an unquestioned fact.

Use a distinct instruction set or model persona and ask it to:

  • find a defect the first pass missed;
  • challenge the strongest claim in the existing report;
  • verify that every critical finding still maps to executable code;
  • look for regressions introduced by the proposed patch;
  • identify evidence that is stale, contradictory, or based on an untested assumption.

Merge the two reports by evidence, not by vote. Two identical unsupported approvals do not equal proof, while one reproducible failure is enough to stop a merge. A human should resolve disagreements about risk, business impact, and whether a mitigation is acceptable.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What does each stack-specific variant check?

The variants share the evidence and gate rules but focus on different production risks.

Variant Phases described Coverage emphasized in the source Questions it should answer
Backend & Cloud Auditor Skill 19 Zero Trust, chaos engineering, OWASP API Top 10, and advanced forensics Can an identity, API path, dependency, or failure mode violate the service’s trust boundaries?
Web App QA & Security Auditor Skill 13 Core Web Vitals, XSS, CSRF, CSP, state mutations, and SEO readiness Does the browser experience remain secure, responsive, crawlable, and consistent across state changes?
Mobile App QA Tester Skill 13 iOS and Android crashes, memory leaks, and local SQLite or Firebase integrity What happens across lifecycle changes, constrained memory, offline transitions, and local-data updates?
Desktop App QA Tester Skill 13 IPC security for Electron or Tauri, DLL-injection prevention, and distribution readiness Are privileged processes, update paths, packaged assets, and inter-process messages protected?

Choose the variant that matches the deployed surface, not the language used in the repository. A TypeScript desktop application still needs desktop IPC checks, while a web dashboard backed by an API may need both web and backend phases.

How can a team implement the workflow step by step?

Start with a narrow, auditable rollout instead of asking an AI to inspect every historical pull request.

  1. Define the merge boundary. Decide which branches, repositories, environments, and data the workflow may inspect. State who can override a failed gate.
  2. Choose the stack variant. Select backend/cloud, web, mobile, desktop, or a combination. Remove phases that do not apply and document the reason rather than silently skipping them.
  3. Wire deterministic checks first. Run compilation, type checks, AST or linter rules, and mutation testing in CI. Save their outputs with the commit.
  4. Give the reviewer a constrained task. Supply the diff, relevant files, test output, and phase checklist. Instruct it to cite exact paths and lines and to mark unproven ideas explicitly.
  5. Require sandbox reproductions. Provide a disposable environment where the model can run targeted tests without touching production data or credentials.
  6. Add runtime validation. Compare high-risk hypotheses with Sentry events and record whether telemetry supports, contradicts, or cannot observe them.
  7. Run the red-team pass. For every proposed security control, attempt an approved bypass through alternate inputs, routes, states, or processes.
  8. Run fresh-eyes analysis. Give a second reviewer the evidence and ask it to challenge both findings and proposed fixes.
  9. Publish the gate record. Attach the checklist, citations, test outputs, reproductions, telemetry comparison, and unresolved questions to the pull request.
  10. Measure locally. Track confirmed defects, false positives, escaped defects, review time, and overrides. Use those measurements to tune prompts and phase requirements.

Keep credentials, customer data, and proprietary source out of prompts unless your approved tooling and retention policy allow them. An adversarial workflow is still an AI workflow and inherits the access, privacy, and supply-chain risks of the systems running it.

Correctness and state

  • Null, empty, malformed, duplicate, and boundary inputs.
  • State transitions that can be interrupted, retried, or applied twice.
  • Concurrent requests, stale reads, lost updates, and ordering assumptions.
  • Unhandled promise rejections, exceptions crossing process boundaries, and partial failures.

Resources and resilience

  • Connections, files, timers, subscriptions, workers, and caches that are not released.
  • Retries without limits, unbounded queues, and timeouts that leave inconsistent state.
  • Behavior during dependency outages, slow networks, process restarts, and degraded services.

Security and data handling

  • Authentication and authorization on every relevant route and operation.
  • Validation performed on the server or privileged process, not only in a client.
  • Secrets, tokens, personal data, logs, backups, and error messages crossing trust boundaries.
  • Bypass attempts against the proposed fix, including alternate encodings and execution paths.

Delivery and operability

  • Migration compatibility, rollback behavior, feature-flag combinations, and configuration defaults.
  • Useful logs, actionable alerts, and release identifiers that let telemetry be matched to code.
  • Tests that would fail if the suspected defect were reintroduced, including mutation tests where practical.

Each checklist item still needs an evidence status. A list of questions without executions, citations, or observed results is only a prompt, not QA.

How is this different from an ordinary AI code review?

The distinction is procedural. Ordinary review may ask an assistant for risks and accept a polished summary; this workflow makes proof, gates, runtime evidence, self-attack, and independent review explicit requirements.

Comparison axis Typical lightweight AI review Unforgiving workflow
Evidence for claims May offer general explanations or approximate locations Requires exact file-path and line-number citations; unsupported claims are invalid
Order of operations Model reasoning may happen before tests Deterministic gates run before or alongside model analysis and must be recorded
Runtime telemetry Often absent from the prompt Static hypotheses are cross-checked with Sentry events before deployment
Security testing Reviews the suggested mitigation Attempts an approved bypass of the reviewer’s own fix
Independence One pass is commonly treated as sufficient A fresh-eyes second pass challenges the first report
Technology coverage General advice may ignore platform-specific risks Separate backend/cloud, web, mobile, and desktop phase sets target their stacks
Effectiveness evidence Confidence is often anecdotal The source proposes measurement but reports no independent defect, false-positive, benchmark, or controlled-comparison results

What evidence exists, and what is still unknown?

The available account is a first-person description by Mohamed Saleh, published on Dev.to on September 6, 2026. It explains the workflow mechanics and invites readers to fork and try the open-source project. It does not publish an independent defect-detection rate, false-positive rate, benchmark, controlled comparison with ordinary code review, or reproducible test report.

That limitation changes how to adopt it. Treat the workflow as a disciplined experiment and collect your own baseline. Compare escaped defects and confirmed findings against your current process, record how often reviewers mark a hypothesis “not reproduced,” and watch for review fatigue caused by gates that add little signal. Do not claim that the suite outperforms human review or another AI system until you have measured that in a comparable setting.

How do you troubleshoot a review that is too noisy or too weak?

Different symptoms point to different workflow failures.

  • Many findings have no valid citations: narrow the prompt to executable files, require a path and line range before allowing prose, and reject generated or vendored code unless it is in scope.
  • The model reports every theoretical risk: require a triggering input and a sandbox attempt, then separate “confirmed” from “unproven” instead of escalating every possibility.
  • Gates pass while production still fails: check that the test environment exercises the deployed configuration, compare release identifiers in Sentry, and inspect telemetry coverage and sampling.
  • The red-team pass repeats the first answer: provide a fresh context, hide the original conclusion until the second pass has produced its own hypotheses, and assign it a specific bypass objective.
  • Reviews take too long: run cheap deterministic checks first, scope adversarial analysis to changed and reachable code, and reserve the full phase set for high-risk changes.
  • Developers bypass the process: make the evidence artifact automatic, define a small number of non-negotiable gates, and document the human override path rather than pretending overrides will never happen.

When is this workflow worth adopting?

It is most useful when a plausible-looking defect would be expensive to discover after release: APIs with authorization boundaries, stateful web applications, mobile apps with local data, or desktop applications with privileged processes. It is less attractive for a tiny, isolated change whose deterministic tests already provide strong coverage and whose failure cost is low.

Adopt it incrementally. Begin with citation enforcement and deterministic gates, add sandbox reproduction for high-risk hypotheses, then introduce runtime cross-checking, red-team review, and fresh-eyes analysis. This staged approach lets you see which controls find real defects before making every pull request carry the full 19- or 13-phase burden.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The practical verdict is straightforward: an AI reviewer becomes more trustworthy when it must prove each claim, pass visible gates, confront runtime evidence, attack its own fix, and survive an independent second look. The workflow described by Saleh supplies that discipline, but your own measurements must determine whether it improves decisions in your repository.

What remains a human responsibility?

Humans still define acceptable risk, authorize security testing, protect sensitive data, choose production safeguards, and decide whether a known issue can ship. They also have to validate that the cited code is actually reachable, that telemetry represents the affected users, and that a proposed fix preserves business behavior.

An AI can accelerate hypothesis generation and repetitive verification. It cannot take ownership of an outage, a privacy violation, or an unsafe release. Keep a named engineer accountable for every gate that can block or permit deployment.

Frequently Asked Questions

Can this workflow run without Sentry?

Yes. Sentry is the runtime-validation example in the design, not a prerequisite for the citation, deterministic, sandbox, red-team, or fresh-eyes stages. If you use another telemetry system, record equivalent release, environment, stack-trace, and timestamp context.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Does a 19-phase review mean every pull request needs 19 separate jobs?

No. The phase count describes the backend/cloud auditor’s coverage, not a required CI topology. You can group compatible checks, run expensive phases only for high-risk changes, and document any phase that is intentionally deferred.

How should line citations be handled after code is reformatted?

Tie the report to a commit identifier and regenerate the review when formatting or generated-code changes move lines. A citation that no longer resolves to the reviewed revision should be treated as stale evidence.

Can the reviewer test production systems while attempting a bypass?

It should not. Perform red-team attempts only against systems and data you are authorized to test, preferably in an isolated sandbox with synthetic credentials and fixtures. Production telemetry can be consulted separately under your access and privacy policies.

What metrics show whether the workflow is helping?

Track confirmed defects, escaped defects, false-positive findings, time from pull request to decision, mutation-test results, runtime corroboration, and the rate of human overrides. Compare those measures with your previous review process over a defined period instead of relying on approval sentiment.
