Find the queue before adding tools. Measure the time from code completion to first review, review turnaround, queue age, diff size, reviewer workload, handoffs, and escaped defects. Then keep AI-generated changes small, run tests and security checks before requesting review, use an AI reviewer only for a consistent first pass, and reserve human approval for architecture and risk. This combination shortens waiting without treating faster approval as proof of quality.
Why AI-assisted teams develop review bottlenecks
AI changes the location and volume of work. Code can be produced faster, but every generated line still needs context, testing, security checks, and a decision about whether it is safe to merge. If a team increases coding throughput without changing review capacity, the pull-request queue grows.
DORA’s 2025 research, based on nearly 5,000 technology professionals and more than 100 hours of qualitative research, describes AI as an amplifier: it magnifies the strengths of a well-designed delivery system and the dysfunctions of a struggling one. DORA’s 2026 analysis reports that 90% of technology professionals use AI at work and more than 80% believe it increases productivity. The time saved during creation is often reallocated to verification, so a faster coding step can still produce a slower delivery system.
Review delay is not an abstract feeling. DORA has found that longer durations between code completion and review reduce developer effectiveness and software quality. A useful diagnosis therefore starts with timestamps and queue behavior, not with a purchase decision.

Measure the review value stream before changing it
Instrument each pull request (PR) so that you can see where time is spent. Record at least these events:
- Opened: the PR is created.
- Ready for review: the author declares that required checks have run and reviewers can act.
- First feedback: the first human or automated review comment that addresses the change.
- Author response: the first subsequent push or reply that addresses review feedback.
- Approval: required reviewers approve the current revision.
- Merge: the change enters the target branch.
- Deployment: the change reaches the relevant environment.
Report medians and p75 or p90 values. A mean can look healthy while a small number of large AI-generated changes wait for days. Segment the results by repository, PR size, risk tier, reviewer group, and time zone.
| Measure | What it reveals | Useful cut |
|---|---|---|
| Completion-to-first-review time | How long finished work sits before anyone examines it | Median and p90 by repository and time zone |
| Total review turnaround | Elapsed time from ready-for-review to merge | Separate author waiting from reviewer waiting |
| Queue age | Whether unreviewed work is accumulating | Count PRs older than your service target |
| Review batch size | The cognitive load presented to a reviewer | Changed lines, files, generated-code share, and commits |
| Reviewer workload | Whether a small group is the constraint | Open assignments, active reviews, and interruptions per person |
| Handoffs | Delay caused by ownership or geography | Number of teams, locations, and approval groups involved |
| Quality outcomes | Whether speed changes are safe | Actionable findings, review iterations, rework, escaped defects, and incidents |
DORA’s diagnostic questions are practical: Are peer reviews embedded in the workflow? How long is the duration between code completion and review? What is the average review batch size? How many teams and geographic locations participate? Does the team improve its quality automation from review suggestions? Answer these with your own event data rather than relying on anecdotes.

Once the measurements exist, identify the stage with the largest wait. Common constraints have different remedies.
| Observed pattern | Likely constraint | First intervention |
|---|---|---|
| Ready PRs wait while a few people own most paths | Reviewer scarcity or overloaded CODEOWNERS | Clarify ownership, add trained backups, and limit unnecessary approval fan-out |
| Large diffs receive many late comments and repeated revisions | Oversized batch, often created by an AI-assisted refactor | Split behavior changes from refactors and set a soft diff-size target |
| PRs cross teams or time zones before anyone can approve | Serial handoffs | Define an accountable owner and use parallel specialist review where risk requires it |
| Reviewers wait for tests, scans, or environments | Feedback is running after human review starts | Move deterministic checks into author-side and pre-review automation |
| Many comments are duplicates, stylistic, or immediately rejected | Low-signal automation or unclear standards | Tune rules and instructions from accepted and rejected findings |
| Approval is quick but incidents or rollbacks rise | Throughput is being optimized without quality control | Restore risk-based gates and inspect escaped defects and rework |
Do not infer the constraint from the tool a team happens to use. Validate it with queue age, workload, and outcome data. Buying an AI reviewer will not fix a CODEOWNERS rule that sends every change to an unavailable specialist, and adding reviewers will not fix flaky tests that keep a PR in limbo.
Keep AI-generated work small and reviewable
Small batches are the most reliable protection against review overload. DORA recommends keeping AI-generated changes in small, reviewable, testable units because large generated diffs raise cognitive load and make failures difficult to isolate.
Set a soft diff-size target
Choose a target appropriate to the repository and risk, then treat it as a conversation trigger rather than an arbitrary hard limit. When a PR exceeds it, require the author to explain why or split the work. Review changed behavior separately from mechanical formatting, renaming, or generated files.

Separate refactors from behavior changes
A reviewer can validate a pure rename or extraction quickly, but the same change mixed with new authorization logic is difficult to reason about. Ask the author to produce independent PRs when practical: one for structure, one for behavior, and one for data or migration work.
Make intent and evidence visible
Require every PR description to state:
- The user or system behavior being changed.
- The risk tier and affected services, data, and permissions.
- Tests, type checks, and security scans that ran, including results.
- What code was generated or materially edited with AI.
- Known limitations, migration sequencing, and rollback steps.
AI assistance is not a reason to lower merge thresholds. Apply the same review and test requirements to generated and human-written code.
Move cheap, deterministic checks toward the author
Human review should focus on meaning and risk, not on discovering a formatter failure or an obvious leaked secret. Run the following before requesting a reviewer whenever the project supports them:
- Formatter and lint checks.
- Unit and integration tests, with reliable failure reporting.
- Type checking and build validation.
- Static application security testing (SAST).
- Software composition analysis (SCA) for vulnerable dependencies.
- Secret detection.
OWASP recommends inline IDE findings for insecure patterns, vulnerable dependencies, and secrets, together with remediation suggestions. The IDE is an early feedback surface, not an authority: OWASP explicitly says IDE feedback complements and does not replace authoritative scans in continuous integration (CI). Keep CI gates for the checks that must be reproducible, centrally governed, and auditable.
A practical pre-review sequence
- Generate or edit the smallest coherent change.
- Run the formatter, linter, type checker, and focused tests locally or in a pre-PR job.
- Run relevant SAST, SCA, and secret scans before assigning a human reviewer.
- Review the resulting diff yourself, removing generated files and unrelated changes.
- Open the PR only when required checks pass or the description records an intentional exception.
- Let CI repeat authoritative checks so local success cannot bypass branch policy.
This sequence shifts inexpensive feedback to the person who has the most context—the author—while preserving a consistent control point in CI.
Use an AI reviewer as a first pass, not as risk acceptance
An AI code-review service can provide a consistent initial inspection before a human spends time on the PR. GitHub documents Copilot code review as able to review any language, examine changes from multiple angles, gather full-project context, suggest fixes, and run automatically on pull requests. Its stated goal is high-signal feedback: “Silence is better than noise.”
Automatic review is useful when every PR needs a baseline check, but the comments remain hypotheses. A human must verify whether a suspected bug is real, whether a suggested fix preserves business rules, and whether the residual risk is acceptable.

Choose the effort level by risk
| Copilot review effort | Use it for | Trade-off |
|---|---|---|
| Lite | Routine, low-risk changes where targeted feedback and speed matter | Lower reasoning depth and lower expected credit use |
| Balanced | Complex logic, security-sensitive code, strict-quality repositories, and cross-service changes | Higher-reasoning model, more time or AI credits, and potentially more detailed findings |
GitHub’s current documentation estimates $0.05–$1 in AI credits for a Lite review and $0.25–$5 for a Balanced review. Those are estimates, not a fixed per-PR price; actual consumption, plan terms, and availability can change. Treat the figures as an input to a pilot’s economics, not as a promise.

Give the reviewer usable context
Configure repository instructions, ownership rules, security policies, and path exclusions so the automated reviewer knows what “correct” means in that codebase. Context should include service boundaries, dependency expectations, data classifications, and generated-file conventions. A comment that lacks this context is more likely to be technically plausible but operationally wrong.
Route findings into the existing workflow
Keep AI comments in the same PR conversation as human feedback, label them clearly, and measure which comments authors accept, reject, or mark as duplicate. Do not let an AI approval satisfy a required human approval or a branch protection rule. If automatic review creates noise, narrow the paths, lower the effort level for low-risk changes, or disable it for repositories where the signal is not actionable.
Keep humans accountable for high-consequence decisions
Human reviewers should own decisions that depend on business intent, system context, or risk tolerance:
- Architecture and service-boundary changes.
- Authorization, privacy, and business-rule correctness.
- Threat-model decisions and security exceptions.
- Database, schema, and data migrations.
- Failure handling, observability, and rollback safety.
- Final approval for production-impacting changes.
Use path-based ownership such as CODEOWNERS or an equivalent mechanism for sensitive areas. Keep the approval set as small as policy allows; adding every interested team serially increases waiting without necessarily improving scrutiny. Where specialist input is required, request it in parallel and assign one accountable approver for the final decision.

Control AI-specific security and supply-chain risks
AI-assisted workflows introduce risks beyond ordinary defects. OWASP highlights prompt injection, poisoned agent configuration, excessive agency, credential exposure, and the volume of unreviewed generated code. Address them as engineering controls, not as documentation alone.
- Sandbox execution: run agents and generated tests with filesystem, network, and process permissions limited to what the task requires.
- Approval prompts: require explicit human confirmation before an agent changes production systems, pushes code, rotates credentials, or performs destructive operations.
- Scoped, short-lived credentials: issue narrowly permissioned tokens that expire, and never place long-lived secrets in prompts, logs, or generated files.
- Allowlisted and version-pinned tools: control which plugins, actions, packages, and external services an agent can invoke.
- Instruction-file review: treat agent configuration and repository instruction files as code subject to review; a poisoned instruction can redirect every future change.
- Mandatory human review: require a person to inspect AI-generated code before merge, including code that appears to have passed automated checks.
Apply these controls in addition to normal branch protection, dependency policy, secret scanning, and incident response. An AI reviewer can identify likely problems, but it cannot establish that an attacker has not manipulated the context it received.

Roll out changes as a controlled experiment
A staged rollout shows whether latency improvements survive contact with quality and reviewer behavior.
- Select one or two repositories. Choose teams willing to expose their review data and include at least one ordinary and one higher-risk workflow.
- Capture a two-to-four-week baseline. Record the value-stream timestamps, diff sizes, reviewer workload, actionable findings, review iterations, escaped defects, incidents, and rework by risk tier.
- Enable author-side checks. Make formatter, tests, type checks, SAST, SCA, and secret detection visible before human assignment; preserve CI as the authoritative gate.
- Start an AI first pass. Use automatic review with Lite for routine changes and Balanced for complex, security-sensitive, or cross-service changes.
- Compare like with like. Use the same metrics and risk segments as the baseline. Examine median and p90 waiting time, not only throughput.
- Tune from real outcomes. Record accepted, rejected, duplicate, and missed findings. Update instructions, exclusions, and ownership rules based on those observations.
- Set stop conditions. Roll back or narrow automation if actionable-comment rate falls, review churn rises, escaped defects or incidents increase, or reviewers spend more time triaging noise.
The purpose of the pilot is not to prove that AI comments are correct. It is to determine whether the combined workflow delivers earlier useful feedback without transferring risk downstream.
Compare review tools and workflow designs on the signals that matter
When evaluating an AI reviewer, IDE integration, CI scanner, or a redesigned human workflow, compare the system rather than a feature checklist.
| Axis | Questions to answer |
|---|---|
| Signal | What percentage of comments is actionable? How are severity, false positives, and duplicate findings calibrated? |
| Context | Can it understand the repository, custom instructions, dependencies, service boundaries, and data flows? |
| Latency | How long from PR creation to the first useful result, and does it delay a human review? |
| Coverage | Which languages, file types, generated files, and automatic-review triggers are supported? |
| Integration | Does it fit the PR provider, CI, IDE, CODEOWNERS, ticketing, and audit-log systems already in use? |
| Control | Can you select effort levels, exclude paths, set approval boundaries, and enforce policy? |
| Security and privacy | What are data-retention and model-training policies? How are secrets handled? Are runners isolated, and are regional requirements met? |
| Economics | What do AI credits, CI minutes, reviewer time, and escaped defects cost together? |
A tool that produces more comments is not necessarily better. Prefer the system that finds consequential issues early, remains quiet when it has no reliable finding, and fits the authority boundaries you can enforce.
Track speed and quality together
Use a balanced scorecard. Latency and throughput show whether the queue is moving; quality measures show whether it is moving safely.
- Median and p75/p90 completion-to-first-review time.
- Median and p75/p90 total review turnaround.
- PRs waiting beyond the team’s service target.
- Review batch size and the percentage of PRs exceeding the soft target.
- Reviewer load and distribution of approvals.
- Actionable finding rate, severity calibration, duplicate rate, and false-positive rate.
- Number of review iterations and amount of rework after approval.
- Escaped defects, production incidents, rollbacks, and security findings.
- AI review credit and CI consumption compared with reviewer hours saved.
Faster approvals alone do not demonstrate a healthier process. A process that halves review latency while increasing escaped defects is a regression, not an optimization.

The queue is still growing after automation
Check whether the real constraint is reviewer ownership, not feedback latency. Inspect queue age by CODEOWNER, geographic handoff, and risk tier. Add trained backup reviewers, reduce serial approvals, and split large PRs before expanding automation.
Reviewers approve quickly but defects escape
Inspect whether tests and security gates were bypassed, whether AI-generated volume increased, and whether reviewers understood the business change. Restore mandatory checks, require human approval for high-risk paths, and add escaped defects and incidents to the pilot’s stop conditions.
Authors wait for checks before they can request review
Separate fast author feedback from slower authoritative gates. Run focused tests and inline scans immediately, then let CI perform complete reproducible checks in parallel where policy permits. A failed or flaky check should identify its owner and recovery path instead of leaving the PR silently blocked.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsCross-team reviews stall overnight
Use the timestamp segments to confirm the handoff delay. Define one accountable approver, request specialist reviews in parallel, and document the risk information the specialist needs in the PR template. If ownership is genuinely unclear, fix the ownership rule rather than adding another notification.
A concise operating policy
A team can turn the practices above into a short policy:
- Every PR states intent, risk, test evidence, and AI assistance.
- AI-generated work is split into small, testable batches; refactors and behavior changes are separated where practical.
- Formatter, tests, type checks, SAST, SCA, and secret detection run before human review, with CI remaining authoritative.
- Automatic AI review is a first pass. It cannot replace required human approval.
- Lite effort is the default for low-risk changes; Balanced is reserved for complex logic, security-sensitive code, and cross-service changes.
- Architecture, business rules, threat-model decisions, migrations, and rollback safety require accountable human review.
- Sandbox agents, scope and expire credentials, pin tools, and review instruction files.
- Review latency is reported with actionable findings, rework, escaped defects, and incidents.
This policy keeps speed improvements tied to reviewability and risk, which is the condition under which AI-assisted development can reduce a bottleneck instead of merely moving it.
