Evaluate a connected medical device’s cybersecurity as part of patient safety and total lifecycle cost—not as an IT checkbox. Before signing a purchase order, map every device, cloud service, app, gateway, workstation, remote-support path and data flow; obtain the vendor’s threat model, cybersecurity risk assessment, machine-readable SBOM, testing evidence and support horizon; then verify how vulnerabilities, emergency patches and end-of-life will be handled. A device with excellent clinical performance can still create unacceptable risk if its supplier cannot explain its architecture, software components, update process or incident responsibilities. The process below gives procurement, clinical engineering, information security and compliance teams a repeatable way to compare clinically comparable products.
What does “secure enough” mean for a medical device?
There is no permanent state of perfect security. The U.S. Food and Drug Administration (FDA) says that threats and vulnerabilities cannot be eliminated, and that reducing cybersecurity risk is especially challenging. Your buying decision should therefore ask whether the manufacturer can identify, reduce, monitor and maintain risk throughout the device’s expected service life.
For a healthcare buyer, “secure enough” means that the remaining cyber risk is understood, documented and manageable alongside clinical risk. The assessment should cover confidentiality of patient information, integrity of measurements and therapy, availability during care, safe operation during a cyber incident, and the effort your facility must supply to keep protections working.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problems- Safety impact: Could a compromised device alter a dose, alarm, measurement, setting or treatment workflow?
- Operational impact: What happens if the device, its cloud service or a vendor gateway is unavailable?
- Maintainability: Can the manufacturer deliver authenticated, clinically validated fixes for the whole supported period?
- Accountability: Are responsibilities divided clearly between the manufacturer, cloud and component suppliers, and your facility?
Do not turn a vendor’s certification, marketing statement or one-time penetration test into a lifetime security conclusion. Security is a lifecycle capability that must be evidenced and contractually supported.

How do I define the device’s cybersecurity system boundary?
Start by documenting everything that can send data to, receive data from or change the device. A “device” may include far more than the equipment at the bedside, and an omitted component can become the easiest attack path.
Build an asset and data-flow inventory
- List the clinical asset. Record model, hardware revisions, firmware, operating system, interfaces, wireless radios, removable media and safety-critical functions.
- Add external services. Include manufacturer cloud tenants, analytics services, mobile applications, licensing servers, update servers, identity providers and notification systems.
- Include intermediaries. Map gateways, middleware, integration engines, local servers, workstations, bedside consoles and network appliances that handle the device’s data or commands.
- Document remote access. Show vendor support tunnels, virtual private networks, jump hosts, maintenance accounts, telehealth links and any always-on connection. State who can open each path and how access is approved and logged.
- Trace data and commands. Draw flows for patient data, configuration, alarms, software updates, logs and control commands. Mark trust boundaries and whether traffic is encrypted and authenticated.
- Assign ownership. For every component, identify whether the manufacturer, a cloud provider, a component supplier or your facility controls configuration, patching, monitoring and incident response.
Ask the vendor to review your diagram and identify inherited risks. For example, the manufacturer may secure its application while your facility remains responsible for network segmentation, identity management, backups or a workstation’s operating system. Get those boundaries in writing before comparing prices.
Which security documents should a vendor provide?
Request a security package that lets your team connect threats to controls and tests, rather than a marketing overview. The FDA’s February 2026 final premarket guidance says a security risk report should connect the threat model, cybersecurity risk assessment, SBOM and testing documentation.
| Document or evidence | What to check | Why it matters |
|---|---|---|
| Threat model | Assets, actors, entry points, trust boundaries, abuse cases and safety consequences | Shows whether the supplier considered realistic attacks on clinical functions |
| Cybersecurity risk assessment | Likelihood and impact reasoning, safety links, mitigations and residual risk | Reveals what remains after controls are applied |
| Security architecture views | Network zones, authentication, cryptography, update paths, logging and remote service design | Allows your architects to test whether the design fits facility controls |
| Residual-risk conclusion | Unresolved risks, assumptions, compensating controls and acceptance authority | Prevents “no known issues” from being mistaken for zero risk |
| Unresolved-anomaly assessment | Open defects, deviations, safety rationale and planned disposition | Shows how known imperfections are managed |
| Verification and validation summary | Security requirements, test methods, results, failures and retesting | Connects claimed controls to observed behavior |
| Security testing summary | Penetration testing scope, independence, environment, dates and remediation | Indicates whether testing covered the actual product and high-risk interfaces |
Ask for confidential review, a secure data room or a redacted version if the supplier cannot release sensitive details publicly. A refusal to provide every page is not automatically disqualifying, but the vendor should explain what can be shared, with whom and under what agreement.
How should I evaluate a medical-device SBOM?
Request a current, machine-readable software bill of materials (SBOM), not only a PDF list of major libraries. It should cover commercial, open-source and off-the-shelf components in the device, embedded software, cloud services, gateways and supporting applications that are within the product’s security boundary.

Check the SBOM’s scope and freshness
- Confirm the product model, software release, build or version identifier and SBOM generation date.
- Ask which components are excluded, including proprietary modules, firmware, containers, operating-system packages and third-party services.
- Request a standard machine-readable format and enough identifiers to match components to vulnerability databases.
- Ask how quickly the SBOM is updated after a release, component replacement or newly discovered vulnerability.
Ask about component support status
For every material component, ask whether it is actively maintained, no longer maintained or abandoned. Obtain the component’s end-of-support date and the manufacturer’s plan for replacement, isolation or compensating controls. An old library is not automatically exploitable, but an abandoned component can limit the supplier’s ability to issue a safe fix.
Compare the SBOM against the supplier’s vulnerability process. The important question is not whether the current list contains a vulnerability on the day you receive it; it is whether the vendor can identify affected versions, assess patient-safety impact and communicate a decision when a new issue appears.

Ask for a named vulnerability-disclosure channel and a written process covering discovery, triage, communication, remediation and verification. The FDA’s section 524B FAQ identifies monitoring, coordinated vulnerability disclosure, cybersecure processes, patches and SBOMs among expectations for cyber devices.
Questions to put to the supplier
- How do you monitor CVE advisories, supplier notifications and the Cybersecurity and Infrastructure Security Agency (CISA) Known Exploited Vulnerabilities Catalog?
- Who decides whether a vulnerability affects patient safety, clinical performance, privacy or availability?
- What are your target times to acknowledge a report, complete an initial assessment, notify customers and provide a mitigation or fix?
- How do you coordinate disclosure with researchers, hospitals and regulators without exposing patients to unnecessary risk?
- How are customers notified when exploitation is active, when a workaround is available and when a permanent remediation is released?
- How do you verify that a fix closes the issue without creating a new clinical or interoperability problem?
Request a sample advisory with sensitive details removed. It should show affected versions, severity reasoning, exploitation status, clinical consequences, compensating controls, remediation instructions and contact information. “We will address issues promptly” is not a measurable commitment.
What should I verify about patches and software updates?
Update mechanics can create clinical risk if they interrupt care, change device behavior or are deployed without validation. Evaluate the complete path from release signing to rollback and recovery.

| Update control | Evidence to request | Buyer decision |
|---|---|---|
| Frequency and routine maintenance | Release cadence, supported versions and maintenance-window requirements | Can your staffing and scheduling absorb the work? |
| Emergency patching | Escalation path, out-of-band process and customer notification method | Can an actively exploited issue be addressed before the next routine cycle? |
| Signing and authentication | Code-signing design, key protection, device verification and failed-update behavior | Does the device reject tampered or unauthorized software? |
| Clinical validation | Verification and validation results, regression scope and release approval | Is safety tested, not merely technical functionality? |
| Deployment | Remote or local procedure, prerequisites, downtime, staffing and network changes | What labor and service disruption will each update require? |
| Rollback and recovery | Rollback triggers, backup image, recovery time and data-preservation procedure | Can you recover safely if an update fails? |
Clarify who pays for vendor labor, onsite visits, validation, replacement hardware, network redesign or temporary loaner equipment. Ask whether your facility can defer an update, and what compensating controls are required during that deferral. Require a documented maintenance window and a tested downtime procedure for safety-critical devices.

What testing and traceability should I demand?
Request a security-testing summary appropriate to the device’s risk, including penetration testing or equivalent assessment of exposed interfaces. The summary should identify scope, test environment, dates, independence, significant findings, fixes and retesting. A generic corporate penetration-test certificate does not demonstrate that the purchased model and release were assessed.
Ask for a traceability matrix linking each cybersecurity risk to a requirement, design control, verification method and result. Health Canada recommends testing evidence and traceability for higher-risk device classes. Review whether the matrix covers wireless interfaces, service ports, authentication, authorization, update functions, logging, denial-of-service resilience and safety-critical commands.
Where a finding remains open, require the residual-risk rationale, compensating control, customer instructions and planned resolution. Testing is a point-in-time activity; ask how the supplier repeats it after major architecture, component or software changes.
How long will the manufacturer support the device?
Obtain two dates, not one: the expected clinical service life and the security-support end date. They may differ. Also request the support policy for each manufacturer-controlled component, including cloud services, gateways, operating systems and remote-service infrastructure.

- Expected service life and assumptions about operating environment
- Security-support start and end dates for each model and software branch
- Minimum notice before discontinuing security updates or a cloud endpoint
- Replacement, migration or retirement plan at end of support
- Data export, deletion and archival options when a service ends
- Responsibilities that transfer to the healthcare facility after support ends
FDA postmarket guidance explains that exploitation can affect health and typically requires continual maintenance throughout the product life cycle. Treat an unsupported device as a lifecycle liability even if it has never experienced an incident. A low purchase price can be overwhelmed by accelerated replacement, compensating segmentation, manual monitoring and emergency clinical-workflow changes.
How do I compare two clinically comparable devices?
Use a weighted scorecard only after confirming that both products meet clinical requirements. Score evidence, not promises, and record assumptions so a low technical score cannot be hidden by a low acquisition price.
| Comparison area | Evidence to score | Cost or risk to include |
|---|---|---|
| Architecture and attack surface | Interfaces, exposed services, segmentation options, authentication and remote access | Network redesign, monitoring and incident containment |
| SBOM and component support | Coverage, machine readability, maintenance status and end-of-support dates | Replacement work when dependencies age out |
| Vulnerability response | Disclosure channel, monitoring, target timelines, advisories and exploit handling | Time exposed and staff effort during an active issue |
| Patch safety and delivery | Signing, validation, downtime, rollback and emergency process | Service visits, labor, validation and lost clinical availability |
| Testing and traceability | Independent testing scope, findings, retesting and risk-to-test matrix | Confidence in controls and audit preparation |
| Support horizon | Security end date, cloud commitments, replacement and retirement plan | Early replacement, migration and data-exit costs |
| Facility integration | Identity, logging, alerting, backup, recovery and staffing requirements | Recurring operational workload |
| Contractual commitments | Service levels, notification, remediation, access to evidence and remedies | Enforceability when conditions change |
Calculate total lifecycle cost as acquisition plus integration, recurring security operations, update labor, downtime, specialized service, compensating controls, incident response and replacement. A device that is cheaper to buy may be more expensive to operate if every patch requires a manual validation visit or prolonged downtime.

What warning signs should stop or pause a purchase?
- The supplier cannot draw the full architecture or says cloud, gateway or remote-service components are “outside the device” without assigning responsibility.
- An SBOM is unavailable, not machine-readable, materially incomplete or has no component support information.
- The vendor has no public vulnerability-reporting channel, cannot describe CVE or known-exploited-vulnerability monitoring, or will not state target response times.
- Updates are unsigned, cannot be rolled back, require unplanned downtime or have no documented clinical validation.
- Testing evidence is limited to a corporate certificate, an old product version or a scan with no findings, scope or retest information.
- The security-support end date is shorter than the expected service life, or the supplier has no retirement and data-exit plan.
- Facility responsibilities are described only as “customer security” with no network, identity, logging, backup or staffing assumptions.
- Sales commitments conflict with the written support policy or cannot be included in the contract.
One warning sign may be resolvable through clarification or a compensating control. Several together indicate that the purchase decision is carrying unknown lifecycle risk.
What is a practical pre-purchase evaluation workflow?
Run the work as a gated procurement process. Do not wait until installation to discover that the device needs an unsupported operating system, a permanent vendor tunnel or daily manual patching.
- Set clinical and risk requirements. Identify safety-critical functions, acceptable downtime, privacy obligations, integration needs and the planned service life.
- Issue a security questionnaire. Request the system boundary, architecture, threat model, risk assessment, SBOM, testing, vulnerability process, update mechanics and support policy.
- Hold a technical review. Have biomedical engineering, network, identity, privacy, clinical safety and procurement staff challenge assumptions and map facility controls.
- Validate evidence. Match the SBOM to the offered release, inspect test scope and traceability, and ask for demonstrations of authentication, logging, update verification and rollback.
- Model operations. Estimate patch labor, downtime, validation, monitoring, incident response, segmentation and end-of-life replacement—not just license and hardware price.
- Negotiate commitments. Put support dates, disclosure, remediation, evidence access, update safety, remote access and exit duties into enforceable terms.
- Approve with conditions. Record residual risks, compensating controls, owners, deadlines and the authority that accepts them.
- Reassess before deployment and renewal. Confirm the delivered configuration, current SBOM, open vulnerabilities, support status and incident contacts before connecting the device to production systems.
Section 524B cybersecurity submission requirements began on March 29, 2023. The FDA’s February 2026 final premarket guidance superseded its 2025 guidance, so ask suppliers which regulatory expectations apply to the product’s submission and lifecycle. ANSI AAMI SW96:2023 was added to the FDA-recognized consensus standards list on October 9, 2023; a vendor may use it as part of its risk-management approach, but recognition does not replace your own evidence review.
What should I ask the vendor in the final meeting?
- Will you provide the current machine-readable SBOM, with component versions, support status and end-of-support dates?
- Which vulnerabilities are open now, including entries in CISA’s Known Exploited Vulnerabilities Catalog, and what compensating controls exist?
- What is your coordinated-disclosure channel and your target time to acknowledge, assess, communicate and remediate?
- How are emergency patches signed, clinically validated, rolled back and deployed without unsafe downtime?
- Which device, cloud, gateway and remote-service components remain supported for the expected service life?
- What independent testing and traceability evidence can you share under procurement confidentiality?
- What responsibilities transfer to our facility at end of support or end of life?
Record the answers, attach the supporting evidence and make unresolved answers explicit in the approval record. The best buying decision is not the product with the most reassuring language; it is the one whose risks, maintenance work and accountability your organization can operate safely.
Frequently Asked Questions
Can a medical device be connected to the hospital network if the vendor has no SBOM?
Connection may be technically possible, but the absence of an SBOM prevents reliable dependency and vulnerability analysis. Treat it as an unresolved procurement risk and require compensating controls, a dated delivery commitment or an alternate product before allowing production access.
Who should sign off on a device’s cybersecurity risk?
Use a cross-functional approval that includes clinical or biomedical engineering, information security, networking, privacy, procurement and the clinical service owner. The person accepting residual risk should have authority over patient-safety and operational consequences, not only the purchasing budget.
Does putting a device on a separate VLAN make it secure?
Segmentation limits reachability but does not correct vulnerable software, weak accounts, unsafe remote access or malicious updates. It should be combined with least-privilege access, monitoring, patch planning, backups and a tested response procedure.
