To tell whether an AI company really monitors safety, look for an operating system of controls—not a reassuring policy or one benchmark score. A credible program names accountable owners, maps foreseeable risks, tests models and surrounding products before and after release, collects continuous telemetry, preserves traceable logs, gives people authority to intervene, and proves that incidents lead to corrective action. Evaluate the program across coverage, independence, measurement quality, alerting, incident response, human control, and external accountability. The steps below turn those questions into evidence you can request, verify, and compare across vendors.
What does effective AI safety monitoring include?
Effective monitoring follows an AI system through its entire lifecycle. It covers the foundation model, application code, retrieval or data pipelines, integrations, human operators, and third-party services. It combines preventive controls, repeatable testing, live detection, incident handling, and documented learning.
NIST’s AI Risk Management Framework (AI RMF) 1.0, published in 2023, organizes this work into four functions: Govern, Map, Measure, and Manage. NIST describes risk management as continuous and timely throughout the AI system lifecycle. ISO/IEC 23894:2023 provides guidance for integrating AI risk management into organizational activities and allows the methods to be adapted to context.
Use those frameworks as a structure for due diligence, not as proof that a vendor is safe. The vendor still has to show dated records, operating metrics, test results, and examples of decisions made when signals indicated danger.

How do I verify governance and accountability?
Ask the company to identify who owns each AI system, each material risk, and each escalation decision. Governance is credible when responsibilities, approval gates, review dates, and exceptions are visible in records rather than implied by job titles.
Documents to request
- An inventory of AI systems, models, versions, environments, and business owners.
- A risk policy that defines acceptable, elevated, and prohibited uses.
- A responsibility matrix naming the system owner, safety lead, security lead, privacy lead, incident commander, and executive approver.
- A risk register linking each identified risk to controls, tests, residual risk, and an accountable owner.
- Change-approval gates for model updates, data changes, policy changes, and new integrations.
- Review minutes, exception records, training completion records, and evidence that leadership receives safety metrics.
Check whether the review cadence is explicit—for example, continuous dashboards with monthly operating reviews and an event-triggered review after a serious incident. A policy that has no owner, due date, escalation route, or evidence of use is governance theater.
What risks and system components should monitoring cover?
A company should state its intended uses, foreseeable misuse, affected groups, threat model, safety objectives, and remaining risk. Then test whether those statements cover the whole sociotechnical system rather than only the model endpoint.
Scope questions
- Which model versions and deployment environments are in scope?
- Are prompts, retrieval sources, fine-tuning data, content filters, tools, plug-ins, APIs, and downstream applications included?
- How are risks to children, people with disabilities, minority language groups, and other affected populations assessed?
- What misuse scenarios are considered, including prompt injection, data exfiltration, fraud, self-harm content, dangerous instructions, and automated high-impact decisions?
- Which third-party models, datasets, hosting providers, and human reviewers create dependency or concentration risk?
- What residual risks remain after controls, and who accepted them?
Compare the stated scope with the product you are buying. A model card that discusses accuracy but omits tool permissions, data retention, or human review leaves important failure paths unmonitored.
How can I judge pre-release testing and red-teaming?
Request a written evaluation plan that connects each material risk to a test, threshold, owner, and release decision. Safety testing should continue after launch because model behavior, usage patterns, policies, data, and integrations change.

Evidence of a serious evaluation program
- Adversarial and abuse testing with a documented threat model.
- Representative test sets, including languages, user groups, domains, and edge cases relevant to deployment.
- Repeatable procedures, versioned prompts or scenarios, and a record of test conditions.
- Predefined pass, fail, and escalation thresholds rather than thresholds chosen after seeing results.
- A list of unresolved findings, accepted exceptions, deadlines, and named approvers.
- Retest results showing whether fixes worked and whether they introduced regressions.
- Post-release red-team plans, bug-bounty or community-reporting processes, and channels for academics or civil-society testers.
The EU General-Purpose AI Code of Practice identifies independent evaluations, bug bounties, community evaluations, stakeholder dialogue, and collaboration with academia and civil society as useful post-market methods. Independence matters: an internal team that reports to product leadership may have different incentives from an evaluator that can publish limitations or escalate directly to a risk committee.
Recommended Free Tools
Do not treat a single benchmark score as a safety verdict. A benchmark may omit the misuse modes, languages, integrations, or user populations that matter in your deployment. Ask how test coverage maps to the company’s own stated risks.
What should continuous monitoring measure?
Continuous monitoring should detect changes in performance, abuse, safety, security, and operating conditions, then trigger an assigned response. For covered high-risk systems, Article 72 of the EU AI Act requires a proportionate post-market monitoring system that actively and systematically collects, documents, and analyzes performance and compliance data throughout the system’s lifetime.
Telemetry worth seeing
| Area | Examples of measures | Questions to ask |
|---|---|---|
| Reliability | Failure rate, timeout rate, latency, availability, tool-call errors | What baseline and alert threshold are used? Who is paged? |
| Safety behavior | Policy violations, harmful-output rate, refusal errors, dangerous completion rate | Are labels human-validated, sampled, or fully automated? |
| Abuse and security | Attack attempts, prompt injection, account takeover, data leakage, unusual usage | How are correlated events investigated across accounts and systems? |
| Distribution shift | Changes in inputs, language mix, task mix, retrieval sources, or user population | What amount of drift triggers review or rollback? |
| Fairness and subgroup quality | Error, refusal, escalation, and latency rates by relevant subgroup | Which groups are represented, and how is privacy protected? |
| Near misses | Events that almost caused harm but were blocked or reversed | Are near misses analyzed like incidents? |
Ask how thresholds were chosen, whether they are absolute or relative to a baseline, and how often they are recalibrated. A useful dashboard shows the metric definition, denominator, sampling method, confidence limits where appropriate, current value, threshold, trend, owner, and last action—not just a green status light.
What happens when a threshold is crossed?
Require a written playbook that maps alerts to triage times and containment options. Depending on severity, actions may include increased sampling, rate limiting, disabling a tool, rolling back a model or policy, retraining, suspending a customer workflow, or shutting down a feature. Verify that alerts reach people who can authorize those actions, including outside normal business hours when the system is continuously operated.

What logs and records prove that monitoring is traceable?
Request a redacted event record and an incident reconstruction, not merely a statement that logs exist. EU high-risk guidance treats automatic lifecycle logging as necessary for traceability and also emphasizes transparency, human oversight, accuracy, robustness, and cybersecurity. The EU General-Purpose AI Code of Practice discusses privacy-preserving logging and metadata analysis of inputs and outputs.

Fields a useful event record may contain
- Model, application, policy, retrieval, and tool versions.
- Timestamp, request identifier, deployment region, and environment.
- Relevant input and output metadata, with sensitive content minimized or protected.
- Safety-policy decisions, classifier scores, human-review status, and alert identifiers.
- Actions taken, reviewer or service identity, approval or override, and remediation link.
Examine retention periods, role-based access, encryption, tamper evidence, clock synchronization, and deletion workflows. Logs should be sufficient to reproduce what happened without collecting unnecessary personal data. Ask whether a customer can obtain records needed for its own regulatory, legal, or safety investigation and how access requests are authenticated.
How quickly does the company report and fix serious incidents?
A mature program defines severity levels, reporting channels, investigation ownership, response targets, regulator notifications, root-cause analysis, and corrective-action tracking. It also feeds lessons back into tests, controls, and training.
Incident process to inspect
- Detection: Show how a customer, employee, automated monitor, or external researcher can report an issue.
- Triage: Ask for severity definitions, evidence requirements, and the target time to acknowledge and contain each class.
- Escalation: Confirm who becomes incident commander and when legal, security, privacy, product, and executive teams join.
- Containment: Verify available controls such as disabling a model route, revoking a tool, blocking traffic, or pausing an automated decision.
- Investigation: Require preserved logs, a timeline, affected versions and customers, causal analysis, and uncertainty notes.
- Correction: Look for a tracked action owner, due date, validation test, and approval before closure.
- Learning: Check that the finding changes a test set, alert, design control, training module, or deployment gate.
The EU General-Purpose AI Code of Practice explicitly names anonymous reporting channels and serious-incident forms. Under EU AI Act Article 73, providers of covered high-risk systems must report serious incidents to the relevant market-surveillance authority; the general deadline is no later than 15 days after becoming aware, with shorter deadlines for widespread infringements and deaths. Those are legal requirements for systems in scope, not a universal deadline for every AI product or every jurisdiction.
Ask for an anonymized example showing when the company learned of a serious issue, when it contained the risk, when customers or regulators were notified, and what changed afterward. A polished postmortem with no linked corrective-action evidence is not enough.
Can a human actually stop the system when monitoring detects danger?
Human oversight is effective only when trained people receive usable signals, have authority to act, and can reach a safe state quickly. ISO/IEC DIS 42105 describes oversight as lifecycle monitoring and control that includes intervention and interfaces designed for transparency and to avoid automation bias.
Test human intervention directly
- Ask an operator to explain an alert, the evidence behind it, and the uncertainty involved.
- Verify that the operator can override an output, pause processing, revoke a tool or credential, and safely shut down the affected function.
- Check staffing levels, on-call coverage, language skills, training records, and recent response drills.
- Measure the time from alert to acknowledgment, decision, and completed containment.
- Inspect interface design for confirmation steps, clear state indicators, escalation buttons, and protection against automation bias.
- Confirm that intervention authority is documented and not blocked by commercial, engineering, or permission boundaries.
Request a recent exercise or incident example with the operator’s available information, elapsed times, decision rights, and resulting changes. A policy that grants intervention without access to logs, authority to pause the system, or a comprehensible interface is not operational oversight.

What transparency and external accountability should I expect?
Look for public or customer-facing documentation that is updated when the risk posture changes. Useful materials describe intended use, limitations, evaluation methods, major incidents, mitigations, and material model or policy changes.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteExternal checks to look for
- Independent assessments with a defined scope, methods, limitations, and unresolved findings.
- Researcher access, bug-bounty terms, or community evaluation channels that do not prohibit good-faith disclosure.
- Customer feedback and appeal routes with tracking and response targets.
- Evidence of cooperation with regulators and affected stakeholders.
- Assurance reports or conformity assessments that identify what was examined and what was excluded.
NIST identifies external engagement and information sharing as governance practices. Treat a certification, audit badge, or compliance statement as evidence about a defined scope and date—not as a guarantee that every use, model version, or integration is safe.
How should I compare two AI companies?
Use the same evidence requests, definitions, and time window for each company. Score the operating capability, not the polish of the documentation.
| Comparison axis | Strong evidence | Weak evidence |
|---|---|---|
| Risk and lifecycle coverage | Inventory and threat model cover model, application, data, operators, integrations, and third parties. | Only model accuracy or content-filter claims are supplied. |
| Evaluation independence | Independent testers, disclosed scope, reproducible methods, and direct escalation. | Unspecified internal testing and selected benchmark results. |
| Measurement quality | Clear definitions, representative subgroups, baselines, thresholds, and uncertainty. | Unlabeled percentages or a single aggregate score. |
| Monitoring and alerting | Continuous telemetry, drift detection, owners, and tested response playbooks. | Periodic manual checks with no alert or containment target. |
| Logging and reproducibility | Versioned, privacy-protected records that reconstruct events. | Logs exist but cannot be accessed, correlated, or retained long enough. |
| Incident response | Severity definitions, channels, response targets, postmortems, and verified fixes. | Generic support contact and no examples of corrective action. |
| Human authority | Trained operators can override, pause, and shut down with measured response times. | Human review is promised but authority and staffing are unclear. |
| External accountability | Independent scrutiny, stakeholder engagement, candid limitations, and remediation history. | Marketing claims with no external challenge or change history. |
Keep the raw evidence behind each score. Record the document date, system version, scope, exceptions, and follow-up owner. If a value is not established, mark it as “not stated” rather than treating silence as a pass.

What are the most common red flags?
Red flags are patterns of missing evidence or unconnected controls, not proof of a particular incident. Escalate for clarification when you see:
- Safety described only through a benchmark, model card, or broad “responsible AI” statement.
- No named owner for a risk, alert, exception, or incident.
- Testing that stops at launch or excludes integrations and human workflows.
- Metrics without definitions, denominators, subgroup breakdowns, baselines, or thresholds.
- Alerts that notify a team but do not trigger a documented decision or containment action.
- Logs that omit model and policy versions or cannot be correlated to a customer event.
- Human review presented as a safeguard even though reviewers cannot override or pause the system.
- Incident policies with no anonymous channel, response target, regulator rule, or corrective-action tracking.
- Independent evaluation with undisclosed methods, narrow scope, or no retest after fixes.
- Promises of future controls with no interim restriction on high-risk use.
How do I reach a defensible decision?
Set a minimum evidence threshold before comparing vendors. For a high-impact deployment, require documented ownership, risk mapping, representative testing, live telemetry, reconstructable logs, tested intervention, and an incident process with accountable deadlines. If a vendor cannot meet a requirement, limit the use case, add compensating controls, or postpone deployment rather than converting missing evidence into an assumption of safety.
Reassess after material changes to the model, data, policy, tools, user population, or legal requirements. Monitoring is an operating capability that must keep producing evidence; it is not a one-time questionnaire or a permanent label.

Frequently Asked Questions
What documents should a procurement team request first from an AI vendor?
Start with the system inventory, current risk register, responsibility matrix, evaluation plan, monitoring metric definitions, incident policy, and a redacted event log. Ask each document to show its date, scope, owner, and links to corrective actions so outdated or generic material is easy to identify.
How can a small organization evaluate a vendor without an internal AI safety team?
Assign one accountable executive, use the NIST AI RMF functions as a simple checklist, and require the vendor to demonstrate alerts and shutdown procedures in a live session. For high-impact uses, obtain an independent specialist review of the threat model and deployment controls rather than relying on a sales questionnaire.
What is the difference between a near miss and a safety incident?
An incident produces actual harm or a confirmed policy or security breach. A near miss is an event that could have caused harm but was blocked, detected, or reversed; tracking near misses reveals weak controls before they produce a reportable incident.
Can an AI safety certification prove that a product is safe?
No. An assessment or certification covers a defined system, control set, and time period. Read its scope, exclusions, evidence date, and unresolved findings, then confirm that the assessed configuration matches the model, integrations, users, and geography you will operate.
How often should an AI company repeat its safety evaluation?
Repeat evaluations after material model, data, policy, tool, or integration changes and on a scheduled cadence based on risk. Continuous telemetry can identify when an unscheduled review is needed because of drift, a new abuse pattern, a near miss, or a serious incident.
