A two-tier AI code-review gate sends every pull request through a fast, lower-cost pass, then routes changes with credible security, logic, data, or service-boundary risk to a slower, higher-reasoning pass. The pattern can reduce expensive model usage without leaving routine changes unreviewed, but it is a team-designed policy—not a proven universal standard. Define explicit escalation signals, log why each pull request was routed, keep tests and static analysis in CI, and measure useful findings, false positives, cost, latency, and missed escalations before trusting the gate.

What is a two-tier AI code-review gate?

The gate is a routing policy with two review efforts. Tier 1 applies a light, fast analysis to every change. Tier 2 applies deeper reasoning or more extensive analysis when the diff could affect security boundaries, complex behavior, data integrity, or communication between services.

The distinction is comparable to the effort choices in GitHub’s current Copilot code-review documentation: Lite is described as a standard, fast, targeted review, while Balanced sends a pull request to a higher-reasoning model for longer analysis. GitHub specifically names security-sensitive code, multi-service pull requests, and repositories with strict quality standards as cases for Balanced. That is product guidance, not an independent engineering standard or proof that the same routing policy will improve your results.

“Scary” should therefore mean “matches a documented, reviewable signal,” not “looks large.” A small authorization change can be consequential, while a large mechanical rename may be low impact. Line count by itself is not a dependable risk measure.

What should Tier 1 do for routine changes?

Tier 1 should provide a quick second set of eyes for localized, comparatively low-risk work. It complements deterministic checks rather than replacing them.

Keep the light pass narrow and actionable

  • Ask the model to identify likely correctness defects, obvious regressions, and missing tests in the changed code.
  • Require file-and-line references and a short explanation that a developer can verify.
  • Tell it to avoid speculative style comments and to distinguish a confirmed defect from a question.
  • Return a predictable status, such as “findings,” “no findings,” or “needs human review,” so the result can be collected by CI.

Run ordinary engineering checks regardless of tier

Unit and integration tests, linters, type checks, dependency scanners, and static analysis belong in the normal CI path for every pull request. An LLM may notice a behavior that a rule-based tool misses, but it can also miss deterministic failures or invent a concern. Treat the light model as an additional signal, not as a replacement for build and test protection.

Examples that often remain in Tier 1

  • A localized UI copy change with no authorization or data-flow effect.
  • A mechanical rename that passes compilation and tests.
  • A small test-only change confined to an existing fixture.
  • A documentation update that does not alter generated configuration or executable examples.

These are starting examples, not a universal allow-list. Repository owners should define what “localized” means for their architecture.

Which pull requests need a stronger AI code review?

Escalate when a change could create a high-impact failure even if the diff is short. Use multiple signals where appropriate, and record the signal that caused the escalation.

Security and identity boundaries

  • Authentication, authorization, session handling, permission checks, token issuance, or policy evaluation.
  • Changes to encryption, secrets handling, network exposure, sandboxing, or input validation at a trust boundary.
  • Updates to access-control middleware or shared security libraries.

Data handling and migrations

  • Schema migrations, backfills, retention rules, or code that transforms sensitive data.
  • Changes to serialization, deserialization, validation, or data-export paths.
  • Writes that can affect billing, balances, audit records, or irreversible user state.

Cross-service and interface changes

  • API, event, queue, RPC, or message-schema changes consumed by another service.
  • Changes that update a client and server in different repositories or alter compatibility guarantees.
  • Deployment, routing, or configuration changes that affect several services.

Complex or high-impact logic

  • Concurrency, retry, caching, scheduling, transaction, or consistency logic.
  • Pricing, entitlement, fraud, ranking, or other calculations with material user or business impact.
  • Changes in components with a history of incidents or frequent rollbacks.

GitHub’s documentation uses security-sensitive code, multi-service pull requests, and complex logic as examples for a higher-effort review. Your team-authored list can add local signals, but the available evidence does not establish a validated universal score or numeric threshold. Avoid presenting any threshold as scientifically calibrated.

How do I implement the routing policy?

Start with an explainable ruleset, then refine it from observed misses and unnecessary escalations. A human must be able to override the automatic decision in either direction.

  1. Define the two contracts. Document the Tier 1 model or effort, expected response time, required output format, and the conditions that invoke Tier 2. Document what Tier 2 adds, such as a higher-reasoning model, a larger context window, or additional analysis passes.
  2. Map repository signals. Identify paths for authentication, authorization, migrations, secrets, public APIs, event schemas, and incident-prone components. Keep this mapping in version control so changes to the policy receive review.
  3. Inspect the pull request. Collect changed paths, dependency or schema metadata, labels, and whether multiple services or repositories are involved. Do not use raw line count as the sole decision.
  4. Apply the rules. If any high-risk signal is present, route to Tier 2; otherwise run Tier 1. If metadata is unavailable or the classifier is uncertain, choose the safer route or require a reviewer decision.
  5. Run CI checks in parallel or before completion. Keep tests, linters, type checks, and static analysis mandatory. A passing AI review must not bypass a failing deterministic check.
  6. Log the decision. Store the selected tier, triggering signals, model or effort setting, timestamps, token or credit usage when available, and whether a human overrode the route.
  7. Require verification. A developer or reviewer checks each actionable finding against the code and tests. Tier 2 is an aid to review, not an approval authority.
  8. Review the rules periodically. Sample false escalations, missed high-risk changes, incorrect findings, and changes in latency or spend. Update the policy when architecture or incident patterns change.

A minimal, explainable decision sketch

signals = detect_signals(pull_request)
if human_override:
    tier = human_override
elif signals.security_boundary or signals.data_migration:
    tier = "deep"
elif signals.cross_service or signals.complex_logic or signals.incident_prone:
    tier = "deep"
else:
    tier = "light"
record(tier=tier, signals=signals)
run_review(tier)

The exact detector can use path ownership, labels, dependency graphs, or a small classifier, but keep the final reason legible. “Model score 0.73” is not an adequate explanation unless a reviewer can see which repository fact produced it.

How should Tier 2 differ from Tier 1?

Tier 2 should spend additional reasoning where a plausible mistake has a larger blast radius. It should not merely repeat the same prompt at a higher price.

Increase context and adversarial attention

  • Include relevant callers, interfaces, migration order, policy definitions, and failure-handling code.
  • Ask for abuse cases, authorization bypasses, race conditions, rollback hazards, and compatibility breaks.
  • Request explicit assumptions and evidence for each finding, with a severity and a suggested verification step.
  • Where practical, use a second pass to challenge the first pass rather than duplicating identical output.

Partition very large changes carefully

Some workflows partition a large diff by risk and assign specialized reviewers. Closedloop documents this style of risk partitioning, while Qwen Code documents selectable effort levels and multi-pass review workflows. These are documented product or workflow patterns, not independent evidence that any particular partitioning strategy improves accuracy. If you partition, preserve cross-partition context so an interface break is not hidden between two apparently safe chunks.

Keep humans in the decision loop

A security or data-migration escalation should identify the owner who can validate the finding, test the rollback plan, and decide whether the residual risk is acceptable. The model can prioritize attention; it cannot accept operational or compliance responsibility.

How much does a two-tier review cost?

Costs have at least two components: model interaction and the compute used to gather context or run tools. GitHub describes both components for its hosted code-review workflow.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Review effort GitHub’s documented AI-credit estimate What affects the estimate
Lite $0.05–$1 USD per review Generally rises with pull-request size and repository custom instructions
Balanced $0.25–$5 USD per review Generally rises with pull-request size and repository custom instructions; uses a higher-reasoning model for longer analysis

These are current estimates from GitHub’s documentation as accessed on 2026-10-25, not a quote for every provider or a guaranteed rate. GitHub says the estimates exclude GitHub Actions minutes and may change as models evolve. Larger hosted runners are billed at a higher per-minute rate; self-hosted runners do not consume GitHub Actions minutes. Those billing details apply to GitHub’s workflow and should not be generalized to another service.

Calculate spend for your own gate

Track light and deep model usage separately, then add the CI infrastructure attributable to context gathering and tool execution. A simple monthly estimate is:

monthly AI cost = (light reviews × light unit cost)
+ (deep reviews × deep unit cost)
monthly total = monthly AI cost + attributable CI minutes cost

Use your provider’s actual credit or token records when available. Report the proportion of pull requests escalated, median and tail latency, retries, and any human time spent triaging findings; a low model bill can still be expensive if noisy output slows reviewers.

How do I evaluate whether the gate works?

Compare the routing policy with a defined baseline, such as the current single-effort review, on a representative set of pull requests. The available evidence supports this measurement approach but does not show that this exact two-tier gate wins.

Measure useful issue recall and false positives

Use human-reviewed findings as the reference. GitHub’s ReviewBench repository describes an open, reproducible benchmark built from findings reviewed by people on real pull requests, evaluating useful-issue identification and false positives. It is a method for evaluation, not proof of this routing design.

  • Useful-issue recall: the share of real defects or security issues in the reference set that the workflow surfaces.
  • False-positive burden: the share or count of findings that are irrelevant, incorrect, or unverifiable.
  • Severity-weighted misses: whether the gate misses high-impact issues even when overall recall looks acceptable.

Measure coverage and routing correctness

  • Confirm every pull request receives at least Tier 1 unless a documented outage or override applies.
  • Confirm every triggered escalation actually receives Tier 2.
  • Sample changes labeled low risk to look for missed security, data, or interface signals.
  • Record why humans overrode the route and whether the override corrected a false escalation or a missed risk.

Measure usability, latency, and cost

Ask reviewers whether findings are specific enough to verify, whether severity is understandable, and whether the escalation reason makes sense. Track median and worst-case completion time, model credits or tokens, CI minutes, and the time reviewers spend dismissing noise. A policy that finds more issues but delays every merge may not be useful for your delivery process.

What are the main failure modes?

Over-escalation

If broad path rules send most pull requests to Tier 2, the gate becomes an expensive single-tier system. Narrow the rule, distinguish read-only from write paths, or require a second signal for noisy categories.

Under-escalation

If a new service or endpoint is not represented in the routing map, risky changes can receive only the light pass. Add ownership and interface metadata, and review misses after incidents or near misses.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

False confidence from a clean AI result

GitHub’s official documentation warns: “Copilot is not guaranteed to spot all problems or issues in a pull request. Sometimes it will make mistakes.” Treat “no findings” as an absence of detected issues, not a security or correctness guarantee.

Context loss

Truncated diffs, generated files, hidden repository instructions, or partitioned reviews can conceal the dependency that makes a change risky. Make the context policy explicit and fail safely when required metadata is missing.

Unverifiable findings

Reject comments that lack a location, impact explanation, and reproduction or test idea. A finding that cannot be checked quickly increases reviewer burden without improving assurance.

Untracked policy drift

Architecture changes can invalidate path lists and ownership rules. Version the policy, review it like code, and include its changes in the same metrics dashboard as model output.

After enabling the gate, review a fixed sample of both tiers each week. Look specifically for missed high-risk changes, repeated false positives, and pull requests whose latency or cost is abnormal. Expand the signal set only when the added rule has an owner, an explanation, and a way to measure its effect.

Vendor pages describe related capabilities: Harness advertises risk-based grouping and required AI checks, while Qwen Code and Closedloop document effort or partitioning patterns. These descriptions explain how products can be configured; they are vendor statements, not controlled comparisons demonstrating that one implementation is superior.

Frequently Asked Questions

Can a small pull request still require the deep review tier?

Yes. A short change to an authorization check, migration, public interface, or retry policy can have a large blast radius. Route on the affected boundary and behavior, not on the number of changed lines.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Should the model choose the review tier by itself?

Use automated detection to propose a route, but keep deterministic rules and a human override. A reviewer should be able to escalate an apparently routine change or downgrade a false escalation with the reason recorded.

How often should routing rules be recalibrated?

Set a regular review cadence, such as after each incident review and at a scheduled quarterly policy review. Recalibrate sooner when services, ownership metadata, or deployment architecture changes.

What should happen when the deep reviewer is unavailable?

Fail closed for the signals your team classifies as critical: block merge or require an explicitly named human reviewer. For lower-severity escalations, define a documented fallback and record that Tier 2 was unavailable rather than silently treating Tier 1 as equivalent.
