If an AI agent changed, sent, deleted, bought, or published something you did not approve, first prevent further impact without destroying useful evidence. Pause the agent or restrict the tool it used when it is safe to do so; preserve logs and approvals; identify what changed and what the agent could access; then recover and tighten permissions before resuming. The right controls depend on the agent, the connected service, and the action—there is no universal stop button or rollback.
1. Contain the agent without causing more damage
Use the provider’s pause, stop, disable, or disconnect control to prevent repeated actions. If the agent is operating through a connected service, limiting or disconnecting that integration may be more targeted than shutting down the entire workflow.
Before stopping a workflow abruptly, consider what it is doing and what depends on its current state. A sudden stop can disrupt a business process or leave incomplete work. Choose the least disruptive action that prevents further harm, and involve the system or business owner if the consequences are unclear. NIST identifies interventions to constrain and monitor agent access as a security measure, but does not prescribe one control for every product (NIST, January 12, 2026).
Revoke credentials when access may be exposed
If compromise or unintended access is plausible, revoke or rotate the service token, API key, or delegated account the agent used. Check whether it has other credentials or integrations that could enable the same action. If the problem appears limited to overly broad permissions rather than exposed credentials, restrict those permissions while preserving access needed for safe containment. NIST identity guidance discusses credential revocation and granular authorization (NIST NCCoE concept paper, February 2026).

Escalate high-impact events promptly
Use the affected service’s security controls and escalation route if its account may be compromised. Notify the relevant organizational owner or incident-response contact promptly if the action involved money, personal data, safety, regulated records, or an external recipient. Which people must be involved depends on the system and organization; there is no universal reporting contact or deadline established here.
2. Preserve evidence before clearing history or reconnecting
Keep records that can help establish what happened and what the agent was authorized to do. Before deleting history, resetting the agent, or reconnecting integrations, preserve available:

- Logs and audit records, including timestamps and time zone.
- The agent identity and the human account or delegated authority associated with it.
- Tool and integration calls, action inputs and outputs, and permission changes.
- Relevant instructions, approval records, and policy checks.
- A factual timeline of the event and the containment steps you took.
Store records through your organization’s approved incident process and limit access. Do not collect or redistribute secrets unnecessarily. NIST’s concept paper raises the need for verifiable records of agent actions and intent; its incident-handling guidance also emphasizes analyzing incident-related data (NIST NCCoE concept paper, February 2026; NIST SP 800-61 Rev. 2).
3. Determine the action’s scope
Reconstruct the sequence from the agent’s history and the connected services’ audit records—not just the visible conversation. NIST notes that agent systems can take autonomous actions affecting real-world systems, and that access to diverse data, tools, and applications creates security challenges (NIST, January 12, 2026).

- What happened? Identify exactly what was changed, sent, disclosed, purchased, deleted, or published, and whether it can still be altered or recalled.
- Where did it happen? List affected accounts, services, records, recipients, and systems.
- Did it continue? Check whether the action repeated or triggered downstream workflows after the first occurrence.
- What could the agent reach? Review connected tools, permissions, and credentials, including access not visible in the conversation.
- What authorization was expected? Check whether a human approval, policy check, or other boundary was meant to apply, and whether it was recorded.
Scope determines whether this is a contained mistake or an incident needing a broader response. If impact is significant or the affected resources are unclear, coordinate with system owners and the security team rather than assuming the visible action is the whole event.

Reverse or correct the change only when doing so is safe and technically valid. For deleted or damaged data, restore from a known-good state if needed. Coordinate restoration with system owners: later work may depend on the changed state, so an automatic rollback can cause additional disruption.

After a correction, verify the affected systems and records, then monitor for recurrence. NIST SP 800-61 Rev. 3 places response and recovery within cybersecurity risk management; it does not set a universal rollback procedure for AI-agent actions (NIST SP 800-61 Rev. 3, April 2025). It is the current revision and supersedes Rev. 2.
5. Reduce risk before enabling the agent again
Do not restore the previous level of autonomy until the cause and the safeguards are clear. Review whether the agent’s access matched the task, and update the setup where possible:

- Use appropriately scoped, managed credentials rather than shared or long-lived credentials where the service supports them. NIST’s September 2026 guidance warns that broad API keys and long-lived bearer tokens can put agent owners and downstream applications at risk (NIST Cybersecurity Insights, September 2026).
- Require explicit human authorization or review for consequential actions, such as external messages, purchases, publication, or destructive changes.
- Make audit records identify the agent, the human authority delegated to it, and the action taken.
- Test revised controls in a low-impact setting before restoring autonomy.
- Establish who can disable the workflow and who decides when it is safe to resume.
NIST’s February 2026 identity concept paper raises least privilege, delegated authority, human-in-the-loop authorization, and auditability as design concerns. It is a concept paper, not a finalized mandatory standard.
When to treat the event as an incident
An unauthorized action is not automatically a cyberattack. It may be a mistaken instruction, a control failure, excessive access, or compromised credentials. Use your organization’s incident-response process when the impact, exposure, recurrence, or uncertainty warrants investigation. NIST SP 800-61 Rev. 3, finalized in April 2025, is the current NIST incident-response publication (NIST CSRC).
