A static AI summary is a snapshot: it gives an answer based on whatever sources and retrieval date underlie that generation, often without showing how the evidence will be checked later. A living evidence system is an ongoing clinical process. It schedules repeat searches, applies predefined eligibility and appraisal rules, records versions, explains changes, and identifies the guideline or study behind each conclusion. AI may accelerate searching, extraction, or navigation inside that process, but automation alone does not make a resource “living” or prove that its recommendations are clinically dependable.

For oncology, the practical question is not whether a tool uses AI. It is whether you can see its evidence scope, surveillance schedule, human governance, version history, and limits before relying on an answer for patient care.

What is a static AI summary in oncology?

A static AI summary is a generated response that represents the evidence available at one point in time. It may have been produced by a general chatbot, a search-and-summarize feature, or a clinical interface that does not publish a continuing surveillance protocol. The answer can be useful for orientation, but its underlying retrieval date, source selection, and update history may be invisible to the reader.

“Static” does not mean “wrong.” A carefully checked summary can accurately describe a trial or guideline on the day it was written. The risk is that a reader cannot easily tell whether the answer includes a newly published trial, whether a recommendation has since changed, or whether the system searched beyond a narrow collection of documents.

What a static answer usually contains

  • A prose response to a question such as which treatment is recommended for a stage or biomarker-defined cancer.
  • References that may be incomplete, difficult to verify, or absent from the visible answer.
  • No public promise to repeat searches at a defined interval.
  • No durable record showing which version of a guideline or evidence profile was used.
  • No formal notice when a source changes, is withdrawn, or is superseded.

A static answer is therefore best treated as a starting point for finding primary guidance, not as proof that the oncology evidence has been continuously monitored.

What makes an oncology evidence resource “living”?

A living resource builds surveillance and revision into its operating design. Cochrane defines a living systematic review as one that is continually updated with new or newly identified evidence. Its protocol should state how often searches will run and when an update is expected. A living guideline applies a comparable cycle to recommendations: search, assess, synthesize, decide whether a recommendation needs revision, publish a new status or version, and communicate what changed.

The recurring workflow

  1. Search: run database and other source searches according to a documented schedule.
  2. Screen: assess new records against predefined inclusion and exclusion criteria.
  3. Appraise: examine risk of bias, relevance, consistency, and other methodological factors.
  4. Synthesize: update evidence profiles or summaries when eligible evidence changes the body of knowledge.
  5. Decide: determine whether a recommendation, its certainty, or its wording should change.
  6. Publish and communicate: issue a new version or status update, preserve the prior version, and describe the change—or state that surveillance found no relevant new evidence.

The important distinction is the explicit, repeatable cycle and its audit trail. An interface can look current while lacking any of these controls; a system can also use automation without being living if it does not have a defined surveillance and governance process.

Does every part of a living guideline change?

No. NHMRC describes the recommendation as the unit that may be living. A guideline can contain living recommendations that are monitored frequently alongside stable or static recommendations that remain unchanged because new evidence is not relevant or does not alter the balance of benefits and harms. “No relevant new evidence” is a meaningful surveillance result; it is different from “not checked.”

How often are living oncology guidelines updated?

There is no universal interval. The appropriate cadence depends on how quickly evidence is arriving, how likely new results are to change practice, the priority of the recommendation, and the resources available to the guideline program.

Program or guidance Stated cadence What the cadence means
ASCO living guidelines Reviewed on an eight-week cycle Used for selected rapidly evolving oncology topics where new evidence can drive practice changes; review does not imply that every recommendation changes each cycle.
NHMRC general living-guideline guidance Searches generally recommended every three to six months Frequency should reflect topic priority, research activity, and available resources.
Static AI summary No continuing interval established by the answer itself The reader may not know when the underlying sources were last retrieved or what has changed since generation.

These schedules are not contradictory. An eight-week review is reasonable for a fast-moving, high-priority question; a three- to six-month search may be appropriate for a slower-moving recommendation. The schedule should be public and tied to the topic rather than presented as a universal definition of “living.”

What does ASCO’s oncology example show?

ASCO describes living guidelines for selected topics in which evidence can rapidly change clinical practice. Its guideline directory showed 2026 updates, including versions for advanced gastroesophageal cancer and stage IV non-small cell lung cancer. The ASCO 2026.3.1 guideline for stage IV non-small cell lung cancer states: “This ASCO Living Guideline is reviewed on an eight-week cycle.”

The same document cautions that guidelines are not a substitute for the treating clinician’s independent professional judgment and do not account for individual variation among patients. The review cycle is therefore a surveillance commitment, not a promise of a patient-specific answer or a guarantee that an intervention is appropriate for every person.

What can AI contribute to a living evidence system?

AI can reduce clerical work inside a governed evidence workflow. Potential tasks include finding candidate records, deduplicating citations, extracting structured data, flagging studies for human screening, drafting evidence tables, and helping users navigate a defined guideline collection. Each task still requires validation because an efficient pipeline can propagate an eligibility, extraction, or interpretation error at scale.

What ASCO’s Guidelines Assistant does—and does not do

ASCO’s Guidelines Assistant is described as an interactive AI tool developed with Google Cloud. Its official description says that answers are drawn solely from ASCO’s published guidelines and that its database is updated as those guidelines are published or revised. Access is for ASCO members through account login.

That design supports navigation and retrieval within ASCO’s guideline collection. It is not described as independently and continuously searching all new oncology literature. A user asking about a just-published trial, an off-label regimen, or a guideline outside that collection should not assume the assistant has considered it. The collection’s scope and update policy still determine what the answer can cover.

Why automation alone is not enough

A 2025 joint position statement from Cochrane, the Campbell Collaboration, JBI, and the Collaboration for Environmental Evidence says evidence synthesists remain responsible for the work. They should judge whether an AI tool is appropriate and methodologically sound, keep human oversight, and report transparently when a tool makes or suggests judgments about study eligibility, appraisal, data extraction, synthesis, or certainty of evidence.

In practice, a living system should identify where automation was used, who checked its output, how disagreements were resolved, and which final version was approved. A fluent explanation is not evidence that those controls existed.

What did the 2026 breast-cancer AI pilot actually find?

One 2026 breast-cancer pilot evaluated a human-conducted, AI-augmented systematic-review platform integrated with guideline pathways. Across eight breast-cancer treatment scenarios, the authors reported review accuracy of 95.1% to 97.2% and extraction accuracy of 51.1% to 99.4%.

The study compared the platform with AI chatbots and called for additional studies of physician workflow, clinical decisions, and implementation in practice. What is the difference between a static summary and a living system?

The central difference is operational: a static summary is an output, while a living system is a maintained process with evidence surveillance, decision rules, and version control.

Comparison point Static AI summary Living evidence system
Evidence scope May be broad or unclear; depends on the model’s retrieval and prompt. Defines cancers, interventions, outcomes, study designs, and guideline sources in a protocol.
Surveillance Usually no visible recurring search schedule. Publishes a search frequency or review cycle appropriate to the topic.
Update trigger May change when a user asks again or when the model’s sources change. Uses predefined criteria to decide whether new evidence requires synthesis or a recommendation change.
Traceability May provide a citation list without a stable version lineage. Links the answer or recommendation to the underlying evidence profile, guideline, study, and version.
Human governance May be unclear who checked eligibility, extraction, interpretation, or conflicts of interest. Assigns responsibility for screening, appraisal, synthesis, approval, and conflict management.
Change communication Often leaves the reader to infer what changed between answers. Shows version dates and explains changed, unchanged, or newly monitored recommendations.
Clinical boundaries Can sound individualized even when it is a general summary. States that guidance supports, rather than replaces, clinician judgment and patient-specific care.

No validated head-to-head evaluation across the examples above establishes that one format produces better clinical outcomes. These are practical criteria for judging reliability and fitness for purpose, not a performance ranking.

How should clinicians evaluate an AI oncology resource?

Use the resource’s documentation, not its interface design, as the test. The following sequence helps a tumor board, library, or informatics team decide whether an answer is suitable for orientation, evidence review, or clinical discussion.

  1. Define the question’s scope. Record the cancer type, disease stage, biomarkers, intervention, comparator, outcome, and patient population. Check whether the resource explicitly covers each element.
  2. Find the evidence date. Look for the last search date, the guideline version, and the date of the answer. If any are missing, label the answer as undated rather than assuming it is current.
  3. Inspect surveillance rules. Confirm how often searches run, which databases or sources are monitored, and what event triggers an unscheduled update.
  4. Check the update unit. Determine whether the whole document is revised or only selected recommendations. Verify that stable recommendations are distinguished from those actively monitored.
  5. Follow the citation chain. Open the cited guideline or study and confirm that the recommendation, population, dose, line of therapy, and evidence certainty match the patient question.
  6. Identify human sign-off. Ask who screened records, checked extraction, assessed bias, resolved disagreements, reviewed conflicts of interest, and approved the final recommendation.
  7. Read the change log. A trustworthy living resource should say what changed, what did not change, and when surveillance found no relevant new evidence.
  8. Record the clinical boundary. Preserve the statement that the resource supports professional judgment and does not account for individual variation. Escalate questions involving contraindications, comorbidities, interactions, or unusual presentations.

When is a static AI summary useful, and when is a living system necessary?

Appropriate uses for a static summary

  • Learning the terminology of a new cancer subtype or treatment class before opening primary sources.
  • Generating a list of questions for a multidisciplinary meeting.
  • Finding the name and edition of a guideline that can then be verified directly.
  • Explaining a known recommendation in plainer language, provided the source and date are checked.

For these uses, treat the summary as navigation. Do not let a polished paragraph replace reading the cited recommendation when a treatment decision is at stake.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Situations that call for living surveillance

  • Diseases with frequent pivotal-trial readouts, rapidly changing targeted therapies, or fast-moving immunotherapy evidence.
  • Protocols that must show exactly when a recommendation was current.
  • Institutional pathways where a change must be communicated to prescribers, pharmacists, and patients.
  • Systematic reviews or guideline programs that need reproducible searches, appraisal, and version history.

A living process is especially valuable when a new result could change standard practice between routine annual revisions. It still cannot eliminate the need to interpret evidence for the individual patient.

What failure modes should an oncology team watch for?

Stale evidence presented as current

Symptom: the answer has no search date or cites an older guideline while newer versions exist. Response: stop at the source, check the publisher’s current version and change log, and document the date used in the clinical record or meeting notes.

Hidden scope limits

Symptom: a tool answers confidently even though it covers only one organization’s guidelines, a narrow set of cancers, or selected study types. Response: write down what the collection excludes and obtain the missing source before treating the answer as comprehensive.

Extraction or transcription errors

Symptom: a dose, biomarker threshold, endpoint, or eligibility criterion differs between the AI output and the cited document. Response: verify the original table or recommendation and have a qualified reviewer check any value that could alter treatment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

False freshness

Symptom: an interface says “updated” without specifying whether the underlying guideline changed, whether a new search found evidence, or whether only software was refreshed. Response: look for a version date, surveillance result, and explanation of substantive changes.

Recommendation overreach

Symptom: a general guideline statement is phrased as a personal prescription. Response: return to the population and conditions in the source, then apply clinician judgment to comorbidities, prior therapy, patient preferences, and local availability.

How should an organization implement AI without losing accountability?

Start with a narrow, documented use case rather than deploying an open-ended chatbot as an unofficial guideline service.

  1. Choose the evidence collection. Specify which guidelines, reviews, registries, and trial reports are in scope and who owns each source.
  2. Set the cadence by topic. Use a shorter cycle for recommendations likely to change quickly and a longer cycle where evidence activity is low; publish the rationale.
  3. Separate machine suggestions from decisions. Label candidate studies, extracted fields, and draft summaries until a human reviewer approves them.
  4. Define escalation rules. Require manual review for safety outcomes, conflicting trials, off-label questions, low-certainty evidence, and any answer that could change a treatment plan.
  5. Preserve provenance. Store source identifiers, document versions, search dates, model or software version, reviewer names or roles, and the final approval date.
  6. Measure the workflow that matters. Track missed eligible studies, extraction corrections, time to update, unresolved disagreements, and whether clinicians can find and understand changes. Do not substitute a benchmark score for evidence of better decisions or outcomes.

This approach makes AI an auditable component of evidence work instead of an authority that silently replaces it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What does a living guideline mean for patients?

For a patient, “living” means the organization has promised to watch for relevant evidence and to explain when recommendations change. It does not mean that every appointment uses the newest study, that every recommendation changes at each review, or that the guidance automatically accounts for personal medical history.

Patients can ask three practical questions: What version and date are you using? What evidence or recommendation changed? Which parts of this advice depend on my stage, biomarkers, prior treatments, other conditions, and goals? Those questions connect the maintained resource to the individualized discussion that only the treating team can provide.

What is the practical verdict?

A static AI summary is a convenient snapshot; a living evidence system is a governed, versioned surveillance process. For oncology decisions, prefer resources that disclose their scope, search cadence, update triggers, human review, citations, and change history. Use AI for retrieval and structured assistance where it is validated and supervised, but do not infer clinical dependability from the words “AI-powered,” a high pilot score, or a recently refreshed interface.

Frequently Asked Questions

Can a living guideline be temporarily out of date?

Yes. A living program can lag between scheduled searches, review, approval, and publication, especially when a study appears just after a cycle closes. The useful safeguard is a visible last-search date and a process for urgent interim updates when evidence is practice-changing.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What should be recorded when an AI answer is discussed at a tumor board?

Record the exact question, source collection, guideline version or study identifiers, retrieval date, and any human corrections. Also note unresolved uncertainty and the clinician who verified the recommendation before it influenced a plan.

Does a guideline assistant search every oncology paper published that week?

Not necessarily. An assistant limited to one organization’s published guidelines can retrieve and explain that collection while excluding new primary studies or other societies’ guidance. Its documented source scope is more informative than its conversational interface.

How can a patient tell whether an online oncology recommendation has been updated?

Look for a publication or version date, a stated surveillance interval, and a change log that distinguishes substantive revisions from routine webpage edits. If those details are absent, ask the care team to verify the recommendation against the current issuing organization’s document.

Why can extraction accuracy vary so widely in an AI evidence pilot?

Extraction tasks differ in difficulty: a clearly stated sample size is easier to capture than a subgroup outcome spread across tables, supplements, or eligibility notes. The reported breast-cancer pilot’s 51.1%–99.4% range and three still-refined variables illustrate why each field needs task-specific validation.
