Implanted speech neuroprostheses can translate brain activity into text or synthesized speech, but they remain investigational technologies rather than approved medical products. The main risks come from cranial surgery, a permanent electrode-and-connector system, skin interfaces, possible hardware revision or removal, uncertain long-term signal quality, and errors in the decoded words. Small clinical studies have reported encouraging safety findings, yet they are too limited to establish population-wide complication rates or guarantee durable communication.
What kind of “speech implant” is this?
This article concerns an implanted cortical brain-computer interface (iBCI): electrodes placed on or in the brain record neural activity associated with attempted speech, and an external computer converts those signals into text or synthetic voice. The goal is communication for people who cannot reliably use their natural voice, not restoration of normal vocal-cord function.
An iBCI is different from an auditory brainstem implant (ABI). An ABI stimulates hearing pathways near the brainstem to provide auditory sensations for selected people with severe hearing loss. It does not decode attempted speech from the motor cortex. ABI adverse-event reports therefore cannot be used as complication rates for cortical speech implants.

Not as routine medical devices. A Nature Reviews Bioengineering review published in 2024 identified 28 clinical trials involving 67 implanted participants across 21 research groups between 1998 and 2023. It concluded that no implanted brain-computer interface had been approved by a regulatory agency for the medical-device market during the review period.

That status affects both access and risk. A participant generally enters a research protocol with a specific eligibility rule, surgical plan, decoder, follow-up schedule and data-use agreement. Results from one investigational device should not be assumed to apply to another electrode design, another disease or a future commercial product.
Risks from the operation itself
Implantation requires cranial surgery. Depending on the design, electrodes and leads are placed beneath the skull or through the skull, and a connector may pass through the scalp to an external cable. Neurosurgical and anesthesia risks can include:
- Incisional pain, swelling and wound-healing problems
- Bleeding or a blood clot
- Infection involving the wound, skull or deeper tissues
- Seizure, stroke or other neurological injury
- Cerebrospinal-fluid leakage
- Anesthesia complications and cardiopulmonary complications
- Need for another operation if the wound, electrode or connector causes a serious problem
The available studies do not provide one universal percentage for each risk. Surgical approach, underlying disease, medications, age, immune status and the trial’s definition of an adverse event all change the estimate.

A 2023 New England Journal of Medicine report described an implantation operation lasting about five hours. The participant left the hospital on postoperative day 3. No serious adverse events occurred in the reported implantation and follow-up period. Nonserious events included incisional pain and a temporary increase in pre-existing limb spasticity. This is a result in one reported participant and does not establish that every speech implant operation will have the same course.

Skin, connector and infection problems
A percutaneous system has a long-term opening through the skin. The pedestal and cable can rub against tissue, collect moisture or become a route for organisms. Possible consequences include irritation, drainage, wound separation, local infection, deeper infection and treatment with antibiotics or surgery.
In the BrainGate feasibility safety study, the most common device-related adverse event was “skin irritation around the percutaneous pedestal.” The study followed 14 adults implanted from 2004 through 2021 and accumulated 12,203 days of safety experience. It recorded 68 device-related adverse events, including six serious adverse events. Investigators reported no intracranial infections, deaths, explantation-required safety events or permanent disability attributable to the investigational device in that safety analysis.
Those findings are reassuring for the participants and system studied, but they are not a guarantee for every implant. A small, carefully selected research cohort cannot reveal the frequency of uncommon infections in the wider population. Ask how the trial defines a suspected infection, who pays for treatment, and what happens if antibiotics do not resolve it.

Hardware failure, revision and explantation
An iBCI is a system rather than a single component. Electrodes, electrode wires, feedthroughs, the scalp connector, external cables, amplifiers and decoding computers can each fail, loosen or become impractical to use. A connector can be damaged by accidental pulling; an electrode may produce poorer signals; and software or an external computer may require replacement or recalibration.

A malfunction may lead to a revision operation. Removing an implant (explantation) can also require surgery and carries its own risks, including bleeding, infection, scarring and neurological injury. The BrainGate safety publication found no explantation-required safety event under its one-year safety definition, but that observation should not be generalized to every design, longer follow-up period or future clinical setting. Before enrollment, request the protocol’s criteria for revision and removal, the expected recovery, and a plan for device disposal and data retention.
How long do speech implants last?
There is no established, universal service life for a cortical speech implant. Mechanical survival, skin tolerance and useful neural control are different questions. A device may remain physically intact while the signals become harder to decode, or a decoder may continue working while disease progression makes communication less reliable.
A seven-year New England Journal of Medicine report illustrates this distinction. At-home use eventually stopped because control became unreliable. Neural-signal amplitude declined, and computed-tomography imaging showed progressive brain atrophy. Investigators found no technical malfunction and considered disease-related neurodegeneration the likely explanation, while acknowledging that alternative explanations were plausible. The case shows why a successful initial implantation does not promise permanent benefit.
Long-term performance can also be affected by changes at the electrode–tissue interface, cortical movement or scarring, progression of the underlying condition, decoder drift, the need for recalibration, and dependence on an external computer or caregiver. A trial should explain how often calibration is expected, whether the user can perform it independently, and what support is available if signal quality falls.

Speech-specific limitations and harms
Decoding is probabilistic. The system can omit a word, substitute a different word, stop responding or produce output the user did not intend. Even a fast interface may require confirmation, correction or a backup communication method.
- Communication frustration: Repeated errors can make conversations slower and tiring, particularly when the user is fatigued or the signal is weak.
- Clinically important misunderstanding: A caregiver or clinician could act on an incorrect output if it is treated as perfectly accurate. Medication instructions, consent and emergency reports require independent confirmation.
- Privacy exposure: Neural recordings and decoded language may reveal sensitive information. Data can pass through implanted hardware, bedside equipment, research servers and clinician interfaces. Ask who can access raw signals, decoded text, audio, logs and derived models; how long they are stored; and whether data can be withdrawn.
- Loss of independence: If communication depends on a paired computer, network, caregiver or frequent recalibration, a battery failure, software update or unavailable helper can interrupt access.
- Uneven performance: Evidence from a small number of participants does not show that one decoder works equally well across speakers, languages, diseases, accents, environments or conversation partners.
These systems should therefore be viewed as assistive communication tools under evaluation, not as a guaranteed replacement for natural speech or for established augmentative and alternative communication support.
What the main safety studies actually show
| Report | Participants and period | Observed findings | How to interpret them |
|---|---|---|---|
| BrainGate feasibility safety study, Neurology, 2023 | 14 adults implanted from 2004 to 2021; 12,203 days of safety experience | 68 device-related adverse events, including six serious events; skin irritation around the percutaneous pedestal was most common; no intracranial infection, death, explantation-required safety event or attributable permanent disability was reported | A selected research cohort and a defined safety analysis, not a universal complication rate |
| Speech neuroprosthesis report, New England Journal of Medicine, 2023 | One described implantation; approximately five-hour operation and discharge on day 3 | No serious adverse events; incisional pain and transient worsening of pre-existing limb spasticity were reported | Useful detail about one procedure and follow-up, not a prediction for all patients |
| Seven-year ALS report, New England Journal of Medicine, 2024 | One long-term participant | At-home control became unreliable; signal amplitude declined; CT showed progressive brain atrophy; no technical malfunction was found | Demonstrates that useful control can decline for biological or disease-related reasons even without hardware failure |
Why auditory brainstem implant reports must not be transferred to speech iBCIs
A 2024 analysis in Neurochirurgie examined 55 qualifying ABI reports in the U.S. Food and Drug Administration’s MAUDE spontaneous-report database. The reports included 27 descriptions of failure to provide hearing benefit, 10 of implant failure or malfunction and six of non-use. Postoperative reports included local skin infection (three), cerebrospinal-fluid leak (three), elevated intracranial pressure (one), wound dehiscence (one), seroma requiring drainage (one) and meningitis (two). Thirty-five reports involved full explantation.

These are counts of reports submitted to a spontaneous-report system, not incidence rates. They concern auditory brainstem implants, which have a different target, surgical context and patient population. They should not be combined with the BrainGate or speech-neuroprosthesis findings and should not be used to estimate the risk of a cortical speech implant.

How an implanted option compares with non-implanted communication support
The right comparison is not simply “implant versus no implant.” A research team and an independent clinician should examine the practical trade-offs across the full communication plan:
| Decision axis | Implanted cortical speech neuroprosthesis | Non-invasive or assistive alternatives |
|---|---|---|
| Invasiveness | Requires cranial surgery and an implanted electrode/connector system | No brain surgery; may include screens, switches, eye tracking, muscle signals or other interfaces |
| Maintenance | Skin care, hardware checks, external computer support and possible recalibration | Usually external equipment maintenance, charging and software support |
| Reversibility | Removal may require another operation | Can generally be stopped or replaced without explant surgery |
| Communication output | Decoded text or synthesized speech can contain unintended words or omissions | Performance depends on the selected assistive method and the user’s residual movement, vision or hearing |
| Privacy and governance | Requires decisions about neural recordings, decoded language and model data | Still requires device and communication-data safeguards, but does not record implanted cortical signals |
| Availability | Limited to research protocols and their eligibility criteria | Established assistive options may be available through clinical services, subject to local assessment and funding |
Questions to ask before joining a trial
- What exact electrode, connector and decoder are being implanted, and where are they placed?
- Which complications have occurred in this protocol, and how are minor, serious and device-related events defined?
- What are the expected risks of bleeding, infection, seizure, stroke, cerebrospinal-fluid leak and anesthesia for this operation?
- How will the team monitor the scalp opening or pedestal, and who should be contacted for redness, drainage, fever, worsening pain or neurological symptoms?
- What is the plan for a failed lead, damaged connector, persistent infection or explantation? Who covers those procedures?
- How often will calibration, software updates or in-person visits be needed, and what happens if the signal deteriorates?
- What accuracy, delay and error information has been measured in people with the same condition, and what backup communication method will remain available?
- Who owns the raw neural recordings, decoded text and trained models? Can data be shared with other researchers, and can consent be withdrawn?
- What happens when the study ends: continued access, equipment return, long-term follow-up and support for disease progression?
- Can an independent neurosurgeon, neurologist, rehabilitation specialist or speech-language professional review the decision with you?
When to seek urgent medical help after implantation
Follow the trial’s emergency instructions rather than waiting for a routine visit. Urgent assessment is appropriate for rapidly increasing headache, new weakness or confusion, seizure, fever, spreading redness, pus or wound separation, clear fluid leaking from the incision, repeated vomiting, sudden loss of function, or any change that the research team identifies as an emergency. The exact response depends on the implant and the person’s medical condition.
What patients should take away
Speech-decoding brain implants offer a promising route to communication for some people, but the technology is still investigational. The meaningful risks are not limited to the operation: a chronic skin interface can become irritated or infected, hardware may need revision or removal, decoding errors can have privacy and safety consequences, and useful control may decline as signals or the underlying disease change. Small studies can document what happened to their participants, including encouraging outcomes, but they cannot establish a single risk percentage or a guaranteed lifespan for every implant. Any decision should be based on the specific trial protocol, a clear explantation and data-governance plan, realistic expectations about errors and maintenance, and independent medical advice.
