Evaluate a production AI agent by whether it reliably delivers the intended business or user outcome—not by whether its conversation sounds fluent. Define a measurable task contract, capture an immutable trace of every model and tool action, score the end result and the path taken, then track safety, consistency, latency and cost per successful task. This approach catches failures that a polished final answer hides, such as an unauthorized tool call, an incorrect database update, or a task that consumed twice its budget before a human intervened.

What does it mean for an AI agent to succeed in production?

Success is an externally verifiable result achieved within the permissions, safety rules, time limit and cost target you set. A plausible response is only one possible signal; the agent must also complete the required side effects, use trustworthy information and avoid forbidden actions.

Write the task contract before choosing metrics

For each task family, document:

  • Goal: the user or business outcome, such as “refund the eligible order and record the reason.”
  • Acceptable outcomes: the states that count as complete, including acceptable alternatives.
  • Forbidden actions: operations the agent must never perform, such as changing a bank account without verification.
  • Escalation conditions: when to hand off to a person, for example unresolved identity, missing evidence or a high-value transaction.
  • Latency budget: the maximum wait the user or downstream system can tolerate.
  • Cost target: the maximum model, tool and infrastructure spend for one attempt.
  • Failure taxonomy: named categories for wrong answers, missing steps, policy violations, tool errors, timeouts and human handoffs.

Google Cloud’s agent-evaluation workflow starts with evaluation cases and expected outcomes. AWS’s published framework likewise emphasizes customer-defined success, failure thresholds, golden sets, safety rules, latency budgets and cost targets. Those elements turn an abstract quality goal into a testable contract.

Separate task completion from answer quality

An agent can produce a factually correct sentence while failing the task—for example, saying a refund is complete when no refund was issued. Conversely, it can complete a transaction while explaining it poorly. Keep outcome metrics (the result and its correctness) separate from communication metrics (clarity, tone and completeness) so a strong writing score cannot mask an operational failure.

How should I build a representative evaluation set?

Use a versioned set of real task patterns, not a collection of ideal prompts. Include normal successes and the situations that expose unsafe or unreliable behavior.

Cases to include

  • Successful, routine requests across supported products, regions, roles and data shapes.
  • Ambiguous instructions that require a clarifying question.
  • Missing, stale or contradictory data.
  • Tool failures, rate limits, malformed responses and partial outages.
  • Permission boundaries, including users who may view but not change a record.
  • Adversarial prompts, prompt injection in retrieved content and attempts to bypass policy.
  • Long-running or multi-step tasks with retries and resumptions.
  • Cases that must be refused or escalated rather than completed automatically.

Store each case with its input, relevant context, expected outcome, allowed and forbidden actions, escalation rule and evaluation rubric. Keep the set under version control. After a production incident, redact sensitive data, review the case with the owning team, and add it to the next version instead of silently changing an existing expected result.

Use a small golden set first

Start with a compact set that represents the highest-volume and highest-risk workflows. A smaller, carefully labeled set produces faster regression feedback than thousands of weakly defined examples. Expand coverage when a new failure mode appears or when routing, retrieval, tools, policies or models change.

What must an agent trace contain?

An immutable end-to-end trace lets you reconstruct what the agent knew, decided, did and changed. OpenAI describes a trace as the record of model calls, tool calls, guardrails and handoffs; Google Cloud similarly describes traces as immutable records of inputs, responses and tool calls.

Minimum trace fields

Area Capture Why it matters
Identity and version Task ID, user or service role, agent version, prompt and policy versions, model and routing decision Separates regressions caused by code, prompts, models or permissions
Model calls Timestamp, request and response metadata, token counts, finish status, safety result and retry number Explains latency, truncation, retries and budget use
Tool calls Tool name, validated arguments, authorization decision, request and response, status, duration and side-effect ID Supports tool-accuracy scoring and audit of real-world changes
Context Retrieved document IDs and versions, snippets or hashes allowed by policy, conversation state and memory reads Shows what evidence was available without relying on hidden internal reasoning
Controls Guardrail results, policy checks, redactions, refusals, handoffs and human decisions Measures safety and escalation behavior
Outcome Final state, user-visible response, completion label, rubric scores and failure category Connects the trajectory to the business result
Economics Model and tool cost, infrastructure allocation, total duration and elapsed waiting time Enables cost and latency per successful task

Use append-only events with a stable correlation ID. Record timestamps from a single clock source, preserve ordering, and retain the raw event plus a normalized evaluation view. Redact or tokenize personal and confidential data before storing traces, and define retention by risk and regulation. Do not treat a final transcript as a substitute for the event stream: it cannot prove which tool version ran or whether a side effect succeeded.

How do I score end-to-end task completion and goal accuracy?

Score the system against the contract and the resulting state. For deterministic workflows, verify the state directly; for open-ended work, use a rubric with explicit pass, partial-pass and fail criteria.

Core outcome metrics

  • Task-success rate: successful tasks divided by all eligible attempts, with the denominator defined before measurement.
  • Completion rate: tasks reaching a terminal state, including a separately reported human handoff rate.
  • Goal accuracy: the proportion whose result satisfies every required condition.
  • Final-answer correctness: factual and procedural correctness of the user-visible response.
  • Groundedness: whether material claims are supported by approved retrieved context or verified system data.
  • User resolution: whether the user’s issue was actually resolved without a repeat contact, when that signal is available.

Report counts and confidence intervals for small samples; a percentage without its denominator can hide a handful of successes. Break results down by task type, user role, language, region, model route, tool and risk tier. A single aggregate score can improve while a severe failure category worsens.

Rubrics for open-ended tasks

Define observable criteria such as required facts, prohibited claims, citation or evidence requirements, actionability and escalation correctness. A three-level rubric can be sufficient:

  • Pass: every required criterion is met and no forbidden behavior occurs.
  • Partial pass: the user receives useful work but a non-critical criterion is missing or needs correction.
  • Fail: the goal is missed, a material claim is wrong, a required action is absent, or a forbidden action occurs.

Have two trained reviewers score a calibration sample, resolve disagreements, and monitor reviewer agreement. Automated judges can scale triage, but high-impact or uncertain cases need human adjudication. A generic “conversation preference” score must not replace task completion.

How do I measure tool-call accuracy and trajectory quality?

Inspect the sequence that produced the result. A correct-looking answer can follow an unsafe path, and a failed answer may reveal a recoverable tool or retrieval problem.

Trajectory checks

  • Selection: Was the required tool chosen, and were irrelevant or forbidden tools avoided?
  • Arguments: Were IDs, quantities, filters, units and dates valid and consistent with the user’s authorization?
  • Ordering: Did the agent verify identity before a sensitive action and read current state before writing?
  • Tool outcome: Did the call succeed, return a partial result or fail? Did the agent detect and handle that status?
  • Output utilization: Did later steps use the returned value, or did the agent ignore an error or stale field?
  • Plan adherence: Were required checkpoints completed and unnecessary detours avoided?
  • Retrieval relevance: Were the selected documents current, authorized and relevant to the decision?
  • Unnecessary steps: How many calls, retries and repeated reads occurred beyond the contract?

Create deterministic validators for schemas, permissions, call order and side effects. For less structured work, use a rubric or judge that receives the trace summary and expected behavior. Track both per-call success and task-level recovery: a retry that safely recovers is different from a silent duplicate charge.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Useful trajectory indicators

Measure required-tool recall (required calls made), forbidden-call rate, argument-validity rate, ordering-violation rate, successful-tool-call rate, recovery rate after errors and excess-step ratio. Keep these as diagnostic measures; the outcome still determines whether the task succeeded.

How should safety and reliability be evaluated?

Safety is a separate dimension, not a bonus added to an outcome score. An agent that completes many tasks while occasionally making an unauthorized change is not production-ready.

Safety measures

  • Policy-adherence rate by policy and risk tier.
  • Unauthorized-action rate, including blocked attempts and actions that escaped controls.
  • Sensitive-data exposure, retention and redaction failures.
  • Correct-refusal rate for prohibited or unsupported requests.
  • Correct-escalation rate for ambiguous, high-impact or out-of-scope cases.
  • Prompt-injection and retrieval-poisoning resistance.

Set hard release gates for severe violations even when their frequency is low. Review the full trace and resulting state for every suspected breach, and preserve evidence needed for incident response.

Consistency and robustness

Run the same case repeatedly with controlled randomness and compare outcomes. Measure pass-rate variance, disagreement between runs, and sensitivity to harmless perturbations such as reordered context, minor wording changes or equivalent date formats. Test degraded dependencies, delayed tools and resumed sessions. Reliability means consistent correct behavior over time, not one impressive demonstration.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How do I calculate latency and cost per successful task?

Report operating economics against delivered value. Token volume alone cannot tell you whether an expensive run solved the problem.

Recommended formulas

  • task_success_rate = successful_tasks / eligible_attempts
  • cost_per_attempt = model_cost + tool_cost + allocated_infrastructure_cost
  • cost_per_successful_task = total_cost / successful_tasks
  • p95_latency = 95th percentile of end-to-end elapsed time
  • excess_step_ratio = (actual_steps - required_steps) / required_steps

Include retries, failed attempts and human-review costs in the numerator when they are part of the service. Report median and tail latency, time spent waiting for tools, timeout rate and handoff time. A route with low cost per attempt can be worse overall if it fails often and forces rework.

Budget controls

Enforce per-task limits on elapsed time, model calls, tool calls and spend. When a limit is reached, stop safely, preserve partial work, and escalate or ask the user to continue; never continue indefinitely to improve a weak answer. Alert on both absolute thresholds and changes from the previous version.

How do I run evaluations continuously after launch?

Use a loop that combines pre-deployment regression, sampled production traces and targeted human review.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Before release: run the versioned golden set against the candidate model, prompts, tools, retrieval index, policies and routing configuration.
  2. Compare gates: require no regression in task success, safety, latency or cost beyond approved tolerances; block any severe policy failure.
  3. Sample production: stratify by task, risk, route, user segment and failure signal rather than sampling only easy traffic.
  4. Review uncertainty: send low-confidence, high-impact, novel or judge-disagreement cases to trained reviewers.
  5. Classify failures: assign one primary category and contributing factors, then link the case to the responsible component.
  6. Alert and roll back: trigger alerts on outcome, safety, process and budget thresholds; retain the last known-good configuration for rapid rollback.
  7. Update deliberately: add reviewed failures to the eval set, change one component where possible, and rerun the relevant regression slices.

Re-evaluate after any model, prompt, tool schema, retrieval, policy, memory, routing or infrastructure change. Keep dashboards version-aware so a metric shift is not mistaken for random variation.

Which evaluation approaches and tools should I compare?

Choose an approach by the risks and observability your system requires, not by the presence of a fashionable score. Compare the following capabilities before adoption.

OpenAI’s agent-evaluation documentation recommends using traces, graders, datasets and evaluation runs together. Google Cloud agent evaluation, Microsoft Foundry agent evaluators and NVIDIA NeMo Helix agentic metrics are implementation options to assess against the same axes. Verify current feature availability, regional handling and pricing for your deployment before selecting one.

What does current production evidence say about agent evaluation?

The 2026 Measuring Agents in Production (MAP) study surveyed 86 deployed-system practitioners across 26 domains and conducted 20 case studies. It reports that 68% of deployed agents execute at most 10 steps before human intervention, 70% rely on prompting off-the-shelf models instead of weight tuning, and 74% depend primarily on human evaluation. Reliability—consistent correct behavior over time—remains the top development challenge.

These findings favor a controlled starting point: a small representative task set, strong traceability, explicit human review and clear failure categories. Add sophisticated autonomous loops or model tuning only when the measured bottleneck justifies their complexity.

How would an evaluation work for a support-refund agent?

Suppose an agent handles refund requests. Its contract could require verifying the customer, checking eligibility and order status, issuing one refund through an authorized tool, recording the reason and telling the customer the resulting status.

  1. Build cases covering eligible and ineligible orders, duplicate requests, missing receipts, expired payment methods, permission failures and hostile instructions in retrieved notes.
  2. Define pass criteria: correct eligibility decision, exactly one authorized refund or a correct escalation, an audit record, and a truthful user message.
  3. Trace identity verification, retrieval IDs, policy checks, refund arguments, tool response, retries, handoff and final state.
  4. Score outcome success separately from trajectory: a refund issued after an invalid first argument is a process defect even if the final state is correct.
  5. Report cost and p95 latency per successful refund, plus unauthorized-action rate and duplicate-refund incidents.

When a test fails, the trace should identify whether the cause was stale retrieval, a schema validator, authorization, model selection, a tool outage or an unclear contract. That diagnosis is more actionable than lowering a single conversational score.

How do I troubleshoot a falling evaluation score?

Start with the failure category and version dimensions rather than immediately changing the model.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Outcome down, trajectory stable: inspect retrieval freshness, business rules, expected-state checks and rubric labeling.
  • Tool errors up: examine schema changes, authentication, rate limits, timeout handling and argument validation.
  • Safety regression: isolate the policy, permission or prompt-injection slice; pause rollout if a severe violation appears.
  • Latency or cost up: inspect retries, excess steps, route changes, context growth and slow dependencies.
  • Run-to-run variance up: compare randomness settings, tool nondeterminism, context ordering and race conditions.
  • Human handoffs up: determine whether the agent is correctly escalating difficult cases or failing routine ones.

Reproduce the failure with the stored trace, add a minimized case to the versioned set, apply the narrowest fix, and rerun both the affected slice and the full regression suite.

What is the practical rollout plan?

A staged rollout reduces risk while instrumentation and labels mature.

  1. Week 1—Contract: select the highest-value task families, define outcomes, forbidden actions, escalation rules, budgets and failure taxonomy.
  2. Week 2—Evidence: implement append-only traces, state verification, redaction, retention and access controls; label an initial golden set.
  3. Week 3—Scoring: add deterministic outcome and trajectory checks, a calibrated human rubric, repeated-run tests and cost/latency calculations.
  4. Week 4—Operations: wire CI regression, production sampling, dashboards, alerts, review queues and rollback procedures.
  5. After launch: review incidents, add representative cases, and rerun evaluations for every material system change.

Keep the first release narrow enough that a reviewer can explain every failure. Observability and a trustworthy contract create more value than a complex evaluator that cannot connect a score to a real outcome.

Frequently Asked Questions

Can I evaluate an agent when there is no single correct answer?

Yes. Define a rubric with observable requirements, unacceptable behaviors and partial-credit boundaries. Use multiple calibrated reviewers for a sample, report agreement, and reserve automated judges for triage rather than treating their preference score as ground truth.

Free tools Windows power users keep installed

How much production traffic should I sample for review?

There is no universal percentage. Stratify sampling so rare high-risk tasks, new routes, policy flags, judge disagreement and unusual costs are overrepresented; increase the sample until estimates for each important task family are stable enough for your release decisions.

Should evaluation traces include the model’s hidden chain of thought?

You can evaluate decisions without retaining private chain-of-thought text. Store observable prompts, retrieved evidence, tool arguments and results, guardrail outcomes, state changes and the final response, with redaction and access controls appropriate to the data.

When should an agent release be rolled back?

Roll back immediately for a severe unauthorized action, sensitive-data exposure or other hard safety-gate breach. For non-severe regressions, compare the change with the approved tolerance for success, latency and cost, then pause or revert when the affected task or risk slice exceeds that tolerance.
