For an urgent hospital update, call the hospital or use its designated urgent-contact route—not a portal message or ordinary email. A portal may offer authenticated access, but that does not mean staff will read a message promptly. For non-urgent sensitive details, use the hospital’s approved secure channel; email is not automatically forbidden, but safeguards and local rules matter.

Which channel should you use?

Situation Practical choice Why
Medical emergency or a change needing immediate action Call emergency services or the hospital’s designated urgent line. The VA says not to send urgent needs through its online messages, and NHS guidance advises considering whether delay could increase clinical risk. VA guidance; NHS England guidance.
Important update that needs a quick response but is not an emergency Call the hospital or care team through its published contact route and ask when to expect a reply. Message response times are service-specific. The VA, for example, sets a three-business-day response expectation for non-urgent secure messages; that is not a general hospital standard. VA guidance.
Non-urgent, sensitive health details Use the hospital’s authenticated portal or another channel it explicitly approves for confidential information. A login requirement can restrict access, but actual protections depend on the service and its configuration. ONC guidance; NHS England guidance.
Routine, non-urgent coordination where the provider invited email Email may be appropriate with reasonable safeguards; verify the address and keep sensitive details to what is necessary. HHS says providers may communicate with patients by email if they apply reasonable safeguards. HHS guidance.

Why urgency and privacy are separate questions

A secure channel can still be too slow for a time-sensitive update. Portal messaging and email are asynchronous: you send a message and wait for someone to see and respond to it. Neither should be treated as an emergency channel unless the hospital explicitly directs you to use it for that purpose.

The VA’s guidance is one concrete example: it tells patients not to send an online message for an emergency or urgent need, and says care teams should respond to non-urgent secure messages within three business days. That expectation applies to the VA service described, not to every hospital portal. VA: communicating with your health care team online.

For a message that is important but not immediately dangerous, use the contact method the hospital publishes for a prompt response. If you choose a portal, check its stated response window; if the timing is unsuitable or no window is given, call and ask how the team wants the update handled.

What makes a portal or email more private?

Patient portals

A portal commonly requires a username and password, adding an authenticated access step before someone can view health information. The Office of the National Coordinator for Health Information Technology (ONC) notes that safeguards vary by organization, so “portal” by itself is not a guarantee of a particular level of security. Follow the hospital’s instructions and use only its approved portal or messaging service. ONC: portal and Blue Button safety.

Email

HIPAA does not categorically ban a provider from communicating with a patient by email. HHS says covered providers may do so when they apply reasonable safeguards; patients may also initiate email communication. For unencrypted email, safeguards can include confirming the recipient’s address and limiting the information sent. You can ask the provider about another reasonable confidential method if you prefer not to use email. HHS: email communication with patients under HIPAA.

Know which response deadlines do—and do not—apply

Response targets for portal messages are set by the individual service. The VA’s three-business-day expectation is specific to its non-urgent secure-message program and operating schedule (Monday–Friday, 8 a.m.–5 p.m., excluding federal holidays); it is not a universal response time for hospital portals. VA guidance.

Likewise, HIPAA’s 30-calendar-day outer limit for acting on a qualifying request for access to medical records is about access to records—not a deadline for answering a portal message or responding to an urgent update. HHS describes circumstances in which an extension may apply. HHS: right of access under HIPAA.

Rules and procedures depend on where you receive care

The HHS guidance concerns U.S. HIPAA obligations, while NHS England’s guidance is for England’s health and care context. Legal requirements, approved channels and response procedures depend on the jurisdiction and the institution. Use the hospital’s own instructions for your care.
