A needle exchange program—more commonly called a syringe services program (SSP)—helps people obtain sterile syringes, dispose of used equipment safely, and access prevention and care services. A supervised injection or consumption site adds a monitored place to use drugs a person has already obtained, with trained staff ready to respond to an overdose. The key difference is sterile-equipment access versus supervised consumption; programs can overlap in other services.
What is a needle exchange program?
The current public-health term used by the CDC is syringe services program. An SSP is a community-based prevention program that can provide sterile syringes and safe disposal, along with services such as testing, vaccination, naloxone, and connections to infectious-disease care or substance-use treatment. What is offered depends on the program. The term “exchange” does not establish that every program requires a one-for-one return of used syringes; local operating rules vary. What is a supervised injection or consumption site?
A supervised site is a designated location where people consume drugs they obtained elsewhere while trained staff monitor for overdose and can respond. Sites may supervise different forms of consumption; some focus specifically on injection. They may also provide sterile supplies, safer-use information, and referrals to health or social services. The U.S. Department of Justice’s August 30, 2024 memorandum describes programs where people consume illicit drugs in a hygienic environment under trained staff supervision; it does not establish the current legal or operating status of every local program. So the distinction is not simply “needles versus no needles.” Either model may offer supplies and referrals. The defining feature of a supervised site is the monitored place to consume and the immediate response available there.

What does the evidence say?
SSPs and infectious-disease prevention
The CDC says SSPs are associated with an estimated 50% reduction in HIV and hepatitis C incidence. “Associated” matters: this is not a guarantee for an individual participant or proof that every program produces the same result. In separate 2024 National HIV Behavioral Surveillance data, 57% of 9,237 participants across 19 U.S. cities reported obtaining sterile syringes from an SSP in the prior 12 months. City-level estimates ranged from 3% to 85%, so the overall figure is descriptive of those surveyed—not a rate for every U.S. community. Supervised sites and overdose outcomes
On-site overdose response and population-level overdose mortality are different outcomes. A 2026 systematic review examined six studies, all from Canada, covering evidence from 2016 through 2024. Provincial-level quasi-experimental analyses generally found no significant association between supervised consumption sites and population-level mortality; some smaller urban analyses suggested protective associations, but results were inconsistent. A 2021 systematic review included 22 studies, 16 focused on one Vancouver facility. The included studies mostly associated facilities with reduced overdose morbidity or mortality, improved treatment access, and no increase or reductions in crime or public nuisance. Because outcome measures differed, the review did not pool results into a single quantitative estimate. A 2019 review likewise emphasized that on-site and community outcomes, bundled services, association and causation, and effectiveness and cost-effectiveness are not interchangeable questions. Neither service is available in every community, and names, service offerings, and rules can differ. Check a local health department or harm-reduction directory for locations, hours, eligibility, and services. In the United States, verify current local operation and legal status with local public-health authorities rather than applying a single national description to every jurisdiction.
