Insurance companies are deploying AI faster than they can verify the evidence those systems use. A September 2026 report found no mentions of synthetic media, synthetic identity or voice cloning in 76 regulatory and investor filings from 49 insurers and reinsurers. Yet manipulated photos, documents, voices and identities can already enter claims, underwriting and customer-service workflows. The practical risk is not simply that an automated model makes a wrong prediction; it is that the model receives convincing but false evidence. Insurers need verification controls that assess provenance and authenticity, create an audit trail, and send uncertain or vulnerable cases to trained people instead of silently denying legitimate claims.

What is the insurance AI verification gap?

The verification gap is the distance between how quickly insurers automate decisions and how well they can establish that the underlying information is genuine. In practical terms, an insurer may have an automated claims triage model, an underwriting assistant or an agentic workflow, but no consistent control for determining whether a submitted image, document, voice or identity was generated or manipulated by AI.

The finding comes from The Speed of Trust: Building the Trust Intelligence Layer for Insurance in the Age of Agentic AI, released on September 3, 2026. Sabine VanderLinden, CEO of Alchemy Crew Ventures, independently authored the report after reviewing 76 annual reports, 10-K filings, proxy statements and statutory returns from 49 insurers and reinsurers, 31 industry studies and 16 interviews with claims and underwriting leaders in the United States and United Kingdom.

Its central warning is operational: “Insurance is automating decisions faster than it can verify the information behind them.” Verification therefore has to become a measurable part of the policyholder journey, not a late fraud check applied only after a loss has been paid.

What did the filing analysis find?

The filings show that public risk reporting is not yet keeping pace with generative-AI evidence threats. Across all 76 documents, the analysis found zero mentions of synthetic media, synthetic identity or voice cloning. Six of the 49 companies mentioned deepfakes, but those references treated them as cybersecurity issues rather than as evidence that could affect a claim or underwriting decision. None of the five top reinsurers examined mentioned deepfakes, synthetic media or AI-generated evidence in their latest annual reports.

Finding Scope and qualification
Synthetic media, synthetic identity and voice cloning Zero mentions across 76 filings reviewed for 49 insurers and reinsurers
Deepfake references Six companies; references framed as cybersecurity concerns, not claims or underwriting evidence
Top reinsurers None of the five analyzed mentioned deepfakes, synthetic media or AI-generated evidence in their latest reporting
Claims-professional confidence A March 2026 survey cited by Clearspeed found 98% saw AI editing tools driving more digital-media fraud; 32% were very confident they could identify a deepfake

The March 2026 survey result is reported by Clearspeed, and the release does not identify the original survey publisher. It should therefore be treated as a secondary attribution rather than an independently verified benchmark. It still illustrates the practical mismatch: professionals can recognize that manipulation is increasing while many remain unsure they can identify it reliably.

Why can AI-generated evidence affect a genuine customer?

Fraud controls can create a second harm when they treat every unusual submission as suspicious. The report cites the Coalition Against Insurance Fraud’s estimate that fraud represents roughly 10% of property-and-casualty losses and says the U.S. insurance system loses at least $308.6 billion annually to the broader trust deficit. At the same time, former Zurich Group Chief Claims Officer Ian Thompson is quoted saying that more than 90% of customers who make a claim are honest people whose service needs should simply be handled quickly.

That creates a double-friction problem. A minority of bad actors can use generated or altered evidence to obtain payment, while broad suspicion can force honest policyholders through extra documents, repeated interviews and longer waits. A useful verification design must do both of the following:

  • clear low-risk, well-supported interactions quickly; and
  • route ambiguous, inconsistent or high-impact cases to an appropriately trained human reviewer.

Verification is not the same as an automatic rejection. A signal that a photograph has been edited, a voice may be cloned or an identity cannot be corroborated should prompt proportionate review, preserve the customer’s opportunity to explain, and remain visible in the case record.

How is generative AI changing the evidence problem?

Older fraud controls often assumed that producing convincing evidence required time, specialist equipment or access to an authentic source. Generative tools reduce those barriers. A claimant’s photograph can be altered, a repair invoice can be synthesized, a voice can be cloned for an agent call, and identity attributes can be assembled into a plausible profile. The same workflow automation that speeds legitimate service can distribute false material at machine speed.

Claims evidence

Images of damage, receipts, medical paperwork and repair estimates can be inserted into automated intake systems before a human sees them. Metadata checks, duplicate-image detection and consistency tests can help, but none proves authenticity alone. A system should compare the submission with policy details, prior loss information, geolocation where lawfully available, and the chronology of the reported event, while recording which checks were run.

Underwriting information

Applicants can submit generated documents or identities that pass superficial formatting checks. Underwriting controls need provenance and corroboration, not merely a confidence score. Material discrepancies should trigger an explanation request or manual review rather than an unexplained adverse outcome.

Voice and conversational channels

As insurers use voice bots and call automation, a cloned voice can be used to impersonate a policyholder, beneficiary or employee. Voice analysis can provide a risk indicator, but it should be combined with independent authentication and transaction limits. A voice signal by itself should not authorize a payout or alter a policy.

Are insurers adopting AI faster than they can govern it?

Several 2026 industry studies point in the same direction: adoption and enthusiasm are high, while foundations, oversight and value measurement lag.

Source and population Reported result What it indicates
IAIS, 2025 Global Insurance Market Report AI-adoption data remains largely anecdotal and quickly outdated; supervisors cite cost reduction and better data analysis as main motives Regulatory visibility is behind the pace of deployment; privacy, cybersecurity and IT-risk governance are the most common controls
KPMG, September 2026 44% place themselves in the top quartile for AI transformation; 77% say failure to redesign enterprise architecture will undermine competitiveness within five years; 71% still focus AI on content generation or routine automation Perceived transformation is ahead of enterprise redesign
KPMG, September 2026 Only 11% report strong data foundations and governance; 29% run front-to-back processes through agents or automation; 92% report productivity or cost improvements; 25% use AI for new products; 11% have a very clear ROI view Efficiency gains are easier to report than controlled growth or measurable returns
Sapiens 2026 Insurance AI Maturity Index Based on 475 senior leaders in five regions, surveyed July–September 2026: 87% say agentic AI is important, while 16% have adopted it in production Importance, pilots and production capability are different maturity stages

Sapiens respondents identify data privacy and security (63%), regulatory and compliance concerns (59%) and integration with existing systems (51%) as leading barriers. Their reported reasons for human overrides are also instructive: compliance or policy requirements (46%), customer vulnerability or exceptional handling (43%), poor or missing data (42%) and lack of confidence in a model or rule (39%). Those are not edge cases; they are design requirements for any automated insurance process.

What is a Trust Intelligence Layer?

The report proposes a Trust Intelligence Layer as a continuous, regulator-ready risk indicator operating across the policyholder journey. It is intended to inform a decision, not make the decision; create an auditable record, not trigger an opaque denial; and work without demographic or historical knowledge of the individual being assessed.

In an implementation, the layer would collect evidence signals from several points—identity checks, media provenance, document consistency, interaction patterns and workflow context—then return a reasoned risk indication with confidence and missing-data flags. The case-management system would retain the inputs, model version, rules applied, reviewer actions and final outcome. A low-risk result could support straight-through processing. An uncertain or high-impact result would create a human-review task with service-level targets.

Controls the layer should include

  • Evidence authenticity: detection for manipulated media, synthetic identity, cloned voice and inconsistent provenance.
  • Governance and auditability: regulator-ready records that explain which signals affected routing.
  • Human escalation: mandatory review for low-confidence, vulnerable, exceptional or financially consequential cases.
  • Customer safeguards: a clear way to correct data, submit alternative evidence and challenge an adverse action.
  • Data minimization: collection limited to what is necessary, with retention and access controls appropriate to sensitive personal information.

How should an insurer build verification into an AI workflow?

Start with the evidence and decisions that matter most, then add controls in stages. The following sequence avoids treating a vendor score or pilot count as proof of readiness.

  1. Map the policyholder journey. List every point where a customer, intermediary or external system supplies information: quotation, identity proofing, first notice of loss, document upload, adjuster interaction, payment authorization and appeal.
  2. Classify the consequence. Mark which outputs can delay service, change price, restrict coverage or release money. High-impact decisions need stronger corroboration and a defined human owner.
  3. Define evidence requirements. For each input, specify acceptable provenance, freshness, corroborating sources and what happens when data is missing. Do not let a model infer that missing evidence is evidence of fraud.
  4. Test authenticity and consistency. Combine media-forensics signals with document, identity, policy and event checks. Measure false positives on known legitimate cases as well as detection on confirmed fraud.
  5. Set escalation rules. Route low confidence, contradictory evidence, customer vulnerability and policy exceptions to trained reviewers. Set response times so escalation does not become an indefinite queue.
  6. Log the decision path. Store the evidence presented, checks performed, model and rule versions, explanations shown to staff, overrides and final disposition. Make the record exportable for audits and complaints.
  7. Monitor outcomes. Track cycle time, leakage, referral rates, reversal rates, complaint outcomes, customer abandonment, reviewer workload and return on investment by product and channel.
  8. Revalidate after change. Re-test when a model, data source, vendor, policy rule or customer channel changes. Generative attacks and normal customer behavior both evolve.

How can insurers measure whether verification is working?

A mature program measures customer and financial outcomes together. A lower paid-loss figure alone may indicate more denials rather than better detection, while a faster claims cycle may hide increased leakage. A balanced dashboard should include:

  • Value realization: benefits compared with implementation, integration, monitoring and reviewer costs—not only the number of pilots launched.

Results should be segmented by product, geography, channel and customer circumstance where lawful and appropriate. That makes it easier to detect whether a control that appears accurate overall is creating disproportionate friction for a particular group.

What does the 2030 agent-to-agent scenario mean?

The report envisions a material share of insurance interactions becoming agent-to-agent by 2030: a customer’s AI agent could transact with an insurer’s AI agent at machine speed. In that environment, a workflow can execute perfectly while relying on false human-provided information. Verification therefore moves upstream. Insurers will need to authenticate the origin, authority and integrity of data before an agent is allowed to quote, amend coverage, submit a claim or authorize payment.

Agent-to-agent processing also raises permission questions. An insurer should know what an external agent is authorized to do, which person or organization it represents, how long that authorization lasts, and when a human must confirm a consequential action. These controls complement, rather than replace, evidence-authenticity checks.

What are the most common implementation mistakes?

  • Confusing a deepfake detector with proof. Detection tools produce probabilistic signals and can miss new generation methods. Use multiple corroborating controls.
  • Automating the denial. A risk score should route work and explain uncertainty; it should not silently reject a claim.
  • Measuring pilots instead of outcomes. Count production coverage, customer cycle time, leakage and ROI, not demonstrations.
  • Ignoring data foundations. Inconsistent policy, claims and identity data makes even a sophisticated model unreliable.
  • Leaving reviewers out of design. Claims and underwriting staff know which exceptions matter and need usable explanations, not an unexplained score.
  • Failing to plan for appeals. Customers need a way to provide alternative evidence and obtain a timely human decision.
  • Over-collecting sensitive information. More data can increase privacy and security exposure without improving verification.

What should an insurer ask before putting an AI verifier into production?

Procurement and risk teams should require evidence that a proposed capability works in the insurer’s own channels and can be governed after launch.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Which media, identity and provenance threats does it detect, and which does it not detect?
  • What independent test set, geographic scope and date support the performance claims?
  • How does the system handle uncertainty, missing data, accessibility needs and legitimate exceptions?
  • Can staff see an actionable explanation and can the customer challenge the result?
  • What records are retained, for how long, and can they be produced to a regulator or ombudsman?
  • How are model updates, drift, incidents and vendor changes approved and documented?
  • What production integrations exist today, rather than in a proof of concept?
  • Which metrics will determine continuation, rollback or expansion?

What is the practical conclusion for insurance leaders?

AI adoption is not the same as trustworthy automation. The 2026 evidence shows a sector moving toward agentic workflows while public filings rarely acknowledge synthetic evidence and only a small minority report strong data foundations, governance or a clear return on investment. Insurers that add verification as an auditable operating layer can protect losses without making honest customers prove their innocence repeatedly. The safest design treats authenticity signals as decision support, preserves human judgment for exceptions, and measures success by both fraud outcomes and the speed and fairness of legitimate service.

Frequently Asked Questions

Can an insurer legally reject a claim because an image might be AI-generated?

A suspicion signal is not automatically a lawful or sufficient reason to reject a claim. The insurer must apply the policy, applicable claims and consumer-protection rules, explain the evidence issue, allow the customer to respond and retain a reviewable record of the decision.

How can a policyholder protect evidence submitted to an insurer?

Keep the original files, capture dates and device details where available, and avoid repeatedly exporting or editing the same image. Submit requested supporting records through the insurer’s authenticated channel and ask how an unclear or corrupted file can be replaced.

Do watermarks prove that an insurance photograph is authentic?

No. Watermarks can be removed, copied or added after creation. They may help establish provenance when combined with secure capture, metadata and independent corroboration, but they should not be treated as conclusive proof.

What is the difference between an AI pilot and production maturity in insurance?

A pilot demonstrates that a use case can work under selected conditions. Production maturity requires integrated workflows, monitoring, data-quality controls, human escalation, audit records, incident response and measured customer and financial outcomes over time.
