Build human oversight into the transaction workflow: decide which actions an AI may recommend or execute, which require approval, and when it must pause or escalate. Give reviewers enough information, expertise, time, and authority to challenge the system, then monitor whether the controls work after launch. A human click on an approval screen is not meaningful oversight by itself.
What does human-in-the-loop mean for a financial transaction?
Human-in-the-loop (HITL) means a person reviews, approves, challenges, or can override an AI output or decision before it is used. The Financial Conduct Authority (FCA) uses that definition in its glossary. For a transaction, the important question is not simply whether a person saw an AI result; it is whether the person could understand the proposed action and affect what happened next.
That distinction matters because an AI system may participate at several points: interpreting a customer request, recommending a payment, checking it against policy, authorizing execution, or monitoring a transaction after it is sent. A system that merely suggests a transfer has a different role from an agent that can initiate and execute it. Map those steps separately, and distinguish advice from an action that moves money, changes account access, or otherwise affects a customer.

The FCA’s October 2026 guidance says firms should decide which work AI supports, automates, abstains from, or escalates. The Financial Stability Board (FSB) describes several possible oversight forms and factors for choosing among them. Both point toward controls proportionate to the impact and risk of the use case, not a universal rule.

Where in the transaction lifecycle should a person intervene?
Start by tracing a transaction from the initial request to execution and any later challenge or remediation. Place controls at the points where a mistake could cause material harm—and, for transfers that may become final, put intervention before execution wherever practicable.
- Request or instruction: Record who or what initiated the transaction, what the customer or authorized agent asked for, and what information the AI receives. Identify ambiguity, missing data, or instructions that need clarification.
- Recommendation: Establish whether the AI is interpreting the request, selecting a recipient, choosing a transaction type, or proposing an amount. Label recommendations clearly so reviewers do not mistake them for completed actions.
- Authorization: Identify who or what is allowed to authorize the action. A control that reviews a recommendation is not equivalent to a control that prevents an unauthorized execution.
- Execution and settlement: Mark the point money moves or another consequential change takes effect. Ask whether the action can be paused or reversed, and what would happen if a concern emerges immediately before or after execution.
- Challenge and remediation: Define how an affected customer can contest an AI-enabled outcome, how the case is reviewed, and who can correct the error or provide redress.
At each stage, assess potential consumer harm, financial loss, compliance consequences, market effects, reversibility, and the speed at which a problem must be detected. The FCA recommends beginning with the consumer need and target market and assessing consumer outcomes. The IMF’s April 2026 note on agentic AI and payments emphasizes that correcting errors after legal finality can be difficult or impossible. That makes pre-execution controls particularly important for consequential transfers.
Which oversight pattern fits each kind of transaction?
Choose a control based on transaction impact, the AI’s autonomy, the time available to intervene, reversibility, the information a reviewer can see, operational capacity, customer recourse, and applicable legal or supervisory requirements. Patterns can be combined; a firm might allow routine low-risk actions within defined limits while requiring approval for a more consequential transaction.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problems| Control pattern | What the person does | When it may fit | Main limitation |
|---|---|---|---|
| Human-in-the-loop | Approves or rejects each covered action before it proceeds. | When the consequence of error warrants a transaction-level decision and there is enough time for review. | Creates review latency and staffing needs; a nominal approval is not effective if the reviewer cannot assess or change the action. |
| Human-on-the-loop | Monitors AI activity and intervenes when a threshold or exception is reached. | When routine actions can be automated and monitoring can detect a problem early enough for a person to act. | Monitoring is not a safeguard if alerts arrive too late, are overlooked, or cannot stop the action in time. |
| Human-in-command | Sets the system’s permitted autonomy, guardrails, and overall governance. | As an organization-level control over what a system is allowed to do. | Does not replace transaction-level approval or intervention where the particular action warrants it. |
| Abstain and escalate | Routes a case to a person when policy limits, uncertainty, anomalies, or missing information prevent a safe automated decision. | When the system should not act without clarification or a human decision. | Needs a functioning route for review; otherwise cases may be delayed or left unresolved. |
| Graduated constraint or kill switch | Pauses an individual action, reduces an agent’s authority, returns a workflow to manual handling, or halts the AI operation. | When an autonomous workflow needs a way to contain a problem and move to a fallback. | Operators need clear authority and a fallback that has been tested, not just a control that exists in theory. |
| Contestability | Allows an affected person to challenge an AI-enabled outcome and seek review or redress. | As a route for correcting or disputing outcomes that affect customers. | It is a recourse mechanism; it does not by itself prevent a harmful transaction before execution. |
The FSB’s June 2026 report is a consultation report, not a binding rule. It describes approval of every decision, monitoring with human intervention, human-in-command governance, kill switches, and contestability as oversight mechanisms. The FCA guidance also identifies abstention, redirection, and escalation as choices firms should consider when deciding how AI should be used.
How should you set limits on an AI’s transaction authority?
Write an action policy for each class of transaction. State what the AI may recommend, what it may execute within limits, what requires prior approval, and what it must refuse or escalate. Make the limits specific enough to enforce and monitor, but do not treat any particular amount as universally safe.

Depending on the use case, the policy can define controls for amount, recipient, transaction type, confidence or anomaly signals, transaction velocity, and aggregate exposure. The relevant combination depends on how the system works and the harm a failure could cause. For example, a limit on a single payment may not control repeated payments that create a large aggregate exposure; a recipient constraint may be more important than an amount limit for a different workflow.
Set measurable thresholds where the system or monitoring team can apply them, and specify the action each threshold triggers: allow, request approval, pause, abstain, or escalate. The IMF recommends thresholds and dashboards for high-risk or high-value agentic transactions, but the cited material does not establish a universal monetary threshold. Define limits for the institution, transaction class, and jurisdiction rather than copying a number from another context.
What makes a transaction approval meaningful?
A reviewer must be able to understand the proposed action, assess relevant evidence, and change the outcome. If the person lacks the information, competence, time, or authority to challenge the system, the approval step risks becoming a rubber stamp.
Give the reviewer decision-relevant information
- The transaction’s amount, recipient, type, and current status.
- The AI’s proposed action and a useful explanation of why it is proposing it.
- Relevant supporting information, including uncertainty, warnings, anomalies, or missing data.
- The consequence of approving, rejecting, pausing, or escalating the action.
The display should help a person check the decision rather than simply repeat the AI’s conclusion. Where the decision depends on data a reviewer cannot inspect or understand, make that limitation visible and provide a route to pause or escalate.
Give the reviewer real authority and preparation
Allow the reviewer to reject, modify, pause, or escalate the action, and make clear who can exercise each option. Train staff on the system’s purpose, limitations, intervention routes, and the risk of automation bias: the tendency to defer to an automated output or accept it without sufficient challenge. The FCA’s October 2026 guidance recommends clear authority and escalation routes and appropriate training; the Bank of England’s October 2022 discussion paper discusses the risk of automation bias and rubber-stamping.

Record what the person did: reviewed, approved, changed, rejected, or escalated. An approval rate or a fast review time alone cannot show that oversight is effective. Assess whether reviewers can identify problematic cases and whether they use the available controls when they find one.
How do you design escalation, override, and recovery?
Plan for a concern to arise while a transaction is pending, during execution, or after the action has taken effect. Specify who takes the decision, what happens to affected transactions, and how the workflow returns to a safe state.
- Define the trigger. Identify the policy breach, uncertainty, anomaly, reviewer concern, or operational issue that requires intervention.
- Assign ownership. Name the role or team that receives the escalation and who has authority to approve a release, change the action, or halt the workflow.
- Control pending activity. Decide whether queued transactions are paused, canceled, or routed for manual handling while the concern is investigated.
- Choose the narrowest effective intervention. Pause a transaction, constrain the agent’s authority, revert the workflow to manual processing, or disable the system, depending on the scope and urgency of the risk.
- Capture evidence and recover. Preserve the relevant action and review records, follow the defined recovery process, and determine whether customer remediation or a broader incident response is needed.
Make fallback paths usable in practice. The Australian National AI Centre’s May 2026 guidance recommends human override points and alternative pathways for critical functions. The FSB consultation report describes a kill switch as a mechanism enabling human intervention to halt or constrain AI operations. A manual fallback should therefore be tested as part of the workflow, rather than assumed to work when needed.
How should you monitor oversight after launch?
Monitor what the system does and what happens to customers, not just model performance. Use an audit trail and explicit thresholds with named owners and actions, then review whether the people and processes around the model remain capable of intervening.
- Transaction outcomes: Track execution errors, false approvals, false declines, and complaints relevant to the use case.
- Intervention activity: Examine approvals, overrides, pauses, abstentions, and escalations. Changes in these patterns may warrant investigation; a rate on its own does not prove that a control is succeeding or failing.
- Performance and fairness: Check for drift and assess differences in outcomes across relevant groups.
- Reviewer capability: Use spot checks, staff feedback, and training reviews to see whether reviewers can recognize and act on problems.
- Threshold response: For every alert threshold, define who responds and what action follows, and preserve the record of that response.
The FCA’s October 2026 examples include dashboards, spot checks, monitoring groups, staff feedback, and training reviews. Its guidance also emphasizes monitoring outcomes and impacts, not solely technical measures. Reassess the control design when the transaction workflow, the system’s autonomy, the affected consumers, or the observed risks change.

What should a human-oversight policy document?
A usable policy connects the transaction decision to the person and control responsible for it. It should be specific enough that staff can apply it consistently and that an auditor can reconstruct what happened.
- The transaction classes covered and the AI system’s role at each lifecycle stage.
- Permitted actions, limits, approval conditions, abstention triggers, and escalation routes.
- Reviewer roles, required information and training, and authority to reject, change, pause, or escalate.
- What happens to pending activity during review and how the manual fallback works.
- What is logged, who owns monitoring, what thresholds trigger action, and how control effectiveness is reviewed.
- How a customer can contest an outcome and how correction or remediation is handled.
Keep the record of an individual transaction distinct from the governance record that defines the system’s allowed autonomy. The first should show what the AI proposed and what the human did; the second should show who set the limits, why they were chosen, and how they are maintained.

That depends on the jurisdiction, the institution, the transaction, and the use of AI; the sources cited here do not create one universal legal requirement for human approval of every AI-supported transaction. Treat legal obligations separately from regulatory guidance, consultation material, and policy analysis.
The FCA material concerns UK financial services. The Central Bank of the UAE’s Rulebook chapter 7 addresses human oversight and consumer protection in the UAE. The FSB’s June 2026 document is consultation material, while the IMF’s April 2026 note is policy analysis. The Australian National AI Centre and UK Government materials are guidance. The Bank of England’s October 2022 discussion paper discusses automation bias and refers to UK GDPR and legislative proposals current at that time; its discussion should not be treated as a statement of present law.

For a specific deployment, identify the rules that apply to the regulated entity and transaction, confirm their current status with qualified legal or compliance advisers, and document how the chosen controls meet those obligations. A general design pattern can inform that work but cannot substitute for a jurisdiction-specific assessment.

How can you tell whether the design is ready?
Before enabling live transaction authority, walk through the workflow with realistic failure scenarios. The goal is to verify that a problem can be detected and that an authorized person can act before harm becomes irreversible.
- Can a reviewer distinguish an AI recommendation from an executed transaction?
- Can they see enough evidence and uncertainty to challenge the proposal?
- Can they reject, modify, pause, or escalate it without needing permission they do not have?
- Does the escalation reach someone with authority while the action can still be stopped?
- Do queued transactions behave as intended during an investigation or system shutdown?
- Can operations use the manual fallback, and are customer challenge and remediation routes clear?
- Can the organization reconstruct the AI action, human decision, and resulting transaction from its records?
If a control fails one of these checks, reduce the AI’s permitted autonomy or add a workable control before relying on it. Oversight should be a functioning part of the transaction design, not a label attached to an approval screen.
Frequently Asked Questions
Is a human approval screen enough to count as oversight?
No. The FCA’s glossary definition includes review, approval, challenge, or override, while its guidance emphasizes reviewer authority, information, and training. A screen that invites a click but leaves the person unable to assess or change the action does not provide those capabilities.
Quick wins for a faster PC:
Scan for outdated or missing drivers - takes under a minuteDriver Scan →Repair Windows errors before they cause bigger problemsFix Now →Should an AI payment agent be allowed to approve its own exceptions?
An exception that exceeds the agent’s authority or triggers an escalation policy should go to the designated human or control function, not be silently reclassified by the agent. Define that boundary in the policy and preserve the reason an exception was routed or cleared.
What is the difference between human-in-the-loop and human-in-command?
Human-in-the-loop concerns a person’s role in a particular decision or action; human-in-command concerns people setting the system’s overall autonomy and guardrails. Organization-level governance alone does not decide whether a specific payment needs prior human approval.
Do regulators require human approval for every AI-driven financial transaction?
The cited materials do not establish a universal requirement covering every transaction or jurisdiction. Requirements depend on the applicable law and supervisory regime, so firms should verify current obligations for their own entity and use case.
