Treat every AI-generated security finding as an unverified lead, not a vulnerability verdict. Before changing code, preserve the agent’s context and raw output, reconstruct the component’s intended behavior and trust boundaries, inspect the exact data flow, and independently replay the claimed effect. A qualified human should then classify the result as VERIFIED, FLAGGED, or REJECTED. If a fix is justified, make the smallest change in an isolated branch or test environment, inspect the complete diff, rerun the original scanner and relevant tests, add adversarial cases, and check that no new alerts, syntax errors, regressions, deleted tests, or weakened assertions were introduced.

Why should an AI security finding be treated as a hypothesis?

AI systems can produce fluent explanations from incomplete context. Their confidence, detail, or ability to generate a proof-of-concept does not establish that an exploitable condition exists. Acceptance requires source-grounded evidence and a reproducible security effect.

Automated tools also lack reliable knowledge of your product’s intended behavior. An endpoint may be intentionally public, a data field may be non-sensitive, or an attacker may already need a privilege that makes the alleged escalation impossible. GitHub Security Lab has documented false positives caused by missing threat modeling; understanding intended use is part of verification, not an optional review step.

What should I record before accepting or rejecting the finding?

Freeze the finding as an auditable record without adopting the agent’s conclusion. The summary is not a substitute for the underlying artifacts.

  • Copy the complete prompt and relevant conversation context, including repository or tool instructions.
  • Record the agent, model or tool version, configuration, enabled tools, timestamp, and repository revision or commit.
  • Save exact file paths, line ranges, tool output, logs, screenshots, traces, payloads, and any generated proof-of-concept.
  • Capture the claimed severity, attack prerequisites, affected identities or assets, and the impact the agent asserts.
  • Hash or otherwise identify large artifacts so a later reviewer can confirm they were not changed.
  • Redact credentials and personal data in copies, while retaining enough provenance to reproduce the observation in a controlled environment.

OWASP’s Application Security Verification guidance requires evidence artifacts and provenance; unsupported or unverified observations should remain separate from the main findings list until a verifier accepts them.

How do I reconstruct the scope and threat model?

Describe what the component is supposed to do before judging whether its behavior is insecure. Write down the security boundary in terms a reviewer can test.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Map the component

  • Identify the service, library, job, endpoint, configuration file, or deployment unit involved.
  • List every entry point and the call chain from the untrusted input to the alleged sink or sensitive operation.
  • Identify data owners, tenants, roles, authentication requirements, and authorization checks.
  • Record where trust changes: browser to API, API to worker, service to database, or user-controlled file to parser.

Define a realistic attacker

State whether the attacker is unauthenticated, a normal user, a tenant administrator, a compromised service, or someone with local access. Include prerequisites such as a valid account, a known object identifier, network placement, or a feature flag. If the agent assumes a capability the attacker cannot obtain, its impact claim may not apply.

Check intended use

Read API contracts, authorization policy, data classification, comments, and product documentation. A publicly readable health endpoint, an intentionally shared identifier, or an administrative action restricted to a documented role is not automatically a vulnerability. Record the design decision and the evidence supporting it.

What evidence is strong enough to support an AI security claim?

Require a reviewer to connect the claim to exact code and an observable effect. Labels such as “possible IDOR,” “unsafe input,” or “critical exposure” are triage hints, not proof.

Minimum source-grounded package

  • Exact paths and line numbers, tied to the revision that was analyzed.
  • A concise data- or control-flow explanation showing where attacker-controlled values enter and what they influence.
  • The violated security requirement, such as an authorization rule, confidentiality property, integrity invariant, or safe parsing requirement.
  • A realistic attack sequence with prerequisites, requests or inputs, expected result, and attacker gain.
  • Evidence that the behavior occurs in the deployed configuration or a faithful test configuration.

What does not qualify by itself?

  • A model’s chain-of-thought or an explanation that cannot be reproduced from visible artifacts.
  • A static pattern match without proof that the value reaches a security-sensitive operation.
  • A test that only checks the agent’s expected response, without an independent observation of the effect.
  • A severity label copied from a taxonomy without demonstrating the required impact and attack complexity.

How can I independently replay the finding?

Use a fresh verifier context containing the finding record, evidence, and target context, but not the discovering agent’s hidden reasoning. The verifier should be able to reach the same conclusion from the artifacts alone.

  1. Prepare an isolated target. Use a disposable branch, local container, staging system, or test tenant. Replay only against assets you are authorized to test; never send an unverified exploit to production.
  2. Pin the revision and configuration. Record the commit, dependencies, feature flags, identity setup, and network conditions. A different build can invalidate both a positive and a negative result.
  3. Build a minimal harness. Reproduce the smallest request, input, fixture, or sequence needed to exercise the alleged path. Remove unrelated agent-generated code from the experiment.
  4. Observe an independent effect. Confirm a captured callback, authenticated response, database or log side effect, file change, privilege transition, or other signal directly. Do not treat the agent’s assertion that the exploit “worked” as the observation.
  5. Repeat with controls. Run a benign input, an unauthorized identity, and boundary values. Compare the results so the effect cannot be explained by a fixture, mock, cache, or test harness error.
  6. Save the run. Keep commands, inputs, outputs, timestamps, environment details, and failure logs with the finding record.

For claims involving external callbacks or data exfiltration, use a controlled endpoint and synthetic data. Out-of-band confirmation is especially valuable because it separates the claimed behavior from the agent’s interpretation of an HTTP response or log line.

How do I calibrate severity and classify the result?

Severity follows demonstrated impact under the real threat model, not the most alarming label in the agent’s report. Compare what was observed with the requirements for the severity scheme your organization uses.

Use three explicit statuses

Status Meaning Queue treatment
VERIFIED Source, prerequisites, and security effect are independently reproduced. Enter remediation with evidence, affected versions, and an owner.
FLAGGED There is a plausible concern, but reproduction, intent, scope, or impact remains uncertain. Assign a follow-up owner; do not present it as a confirmed vulnerability.
REJECTED The behavior is intentional, unreachable under the threat model, not reproducible, or unsupported by the evidence. Keep the record and reason for auditability, but remove it from the remediation queue.

Unsupported Critical or High ratings should be downgraded or flagged until the required impact is demonstrated. A finding can be technically real but low impact, or severe in theory but unreachable by any realistic attacker; document which condition applies.

How should I prepare an AI-proposed fix safely?

Apply a narrowly scoped change in an isolated branch or disposable environment. The objective is to remove the verified condition without silently changing unrelated behavior.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Create a branch from the exact revision used for verification and write the security invariant the fix must enforce.
  2. Prefer the smallest change that restores that invariant, such as a server-side authorization check, strict parser setting, output encoding at the correct sink, or safe API usage.
  3. Review the complete diff, not only the lines the agent describes. Include lockfiles, dependency manifests, migrations, configuration, generated files, tests, and documentation.
  4. Inspect every new package or script. An apparently routine dependency can expand the attack surface or introduce supply-chain risk.
  5. Check that the change does not move a check to a client, rely on a user-controlled value, broaden an allow-list, or disable logging and monitoring needed for detection.
  6. Reject test edits that delete coverage, weaken assertions, skip cases, replace the unit under test with a mock, or change expected output merely to make the suite pass.

OWASP guidance specifically warns against trusting routine-looking AI changes, suggested packages, or modified tests without examining what was removed and what is now asserted.

How do I verify that the fix really works?

A fix is verified only when the original finding disappears under independent checks and the change does not create a new defect.

  1. Re-run the original scanner or detection rule against the fixed revision and retain its output. If the rule still fires, investigate whether the finding is unresolved or the rule is stale.
  2. Run the relevant unit, integration, regression, and security tests with the same configuration used for reproduction.
  3. Add negative tests that exercise unauthorized identities, malformed input, cross-tenant identifiers, boundary values, and alternate code paths.
  4. Compare test counts, assertion counts, skipped tests, snapshots, and coverage reports with the pre-fix baseline. A green suite with fewer assertions is not evidence of a successful fix.
  5. Run static analysis, dependency checks, secret scanning, and applicable dynamic or black-box tests. A clean SAST result does not prove that authorization or business-logic flaws are absent.
  6. Inspect logs and responses for accidental information disclosure, changed status codes, cache behavior, or error messages that reveal more than before.
  7. Review the final diff for syntax errors, generated-file drift, unrelated refactors, new alerts, performance regressions, and behavior outside the requested scope.

GitHub describes automated findings as advisory. Its Autofix guidance likewise calls for rerunning the relevant tools and tests and checking for deleted tests, weakened assertions, and unintended changes.

Which verification approach should I use: manual review, conventional scanners, or AI assistance?

These approaches complement one another. Choose based on the evidence you need, then combine them for security-critical changes rather than treating any single method as authoritative.

Approach Evidence quality Independence Reproducibility Threat-model coverage False-positive handling Regression detection Auditability Typical cost and latency
Manual review Can connect design intent, source, and business impact; depends on reviewer skill and recorded artifacts. High when a separate reviewer examines the evidence. High if the reviewer builds a repeatable test; variable for reasoning-only conclusions. Strong for authorization and business logic when the reviewer understands the product. Good when intent and reachable paths are examined; inconsistent without a checklist. Requires explicit tests and diff review. Strong when decisions and evidence are logged. Highest human time; suitable for high-risk paths.
Conventional scanners Usually provide rule matches, traces, and machine-readable output; may miss design context. Independent of an AI discovering agent. High for deterministic rules and repeatable scans. Good for covered patterns; limited for business logic and undocumented flows. Handled through rule tuning and human triage. Good when run before and after a change. Strong through versioned reports and rule IDs. Low to moderate runtime cost; easy to gate in CI.
AI-assisted verification Can summarize flows and generate tests, but narrative quality is not evidence. Weak if the same agent controls discovery and verification; stronger with a fresh verifier and human review. Requires an independent harness and observable effect. Can expose overlooked paths, but may invent assumptions or miss implicit boundaries. Needs explicit intent and reachability checks. Can suggest adversarial cases; deterministic suites must decide pass or fail. Depends on preserved prompts, tool output, versions, and provenance. Fast for triage and test generation; review time rises with unexplained or broad changes.

NISTIR 8397 recommends a baseline that spans threat modeling, automated testing, static code scanning, fuzzing, web-application scanners where applicable, and dependency or service review. AI assistance is most useful when it accelerates those activities without replacing their independent evidence.

What merge gates and separation of duties should be required?

Make the verification status and evidence prerequisites for merging, not optional comments in a pull request.

  • Require a qualified human to review every AI-proposed security fix.
  • Use two-person review or security-team sign-off for security-critical files, authorization code, cryptographic code, identity boundaries, and deployment policy.
  • Block merges for Critical findings unless an authorized human approves a written exception that records residual risk and an expiry or review date.
  • Require the finding ID, reproduction result, scanner rerun, test results, and final diff review in the change record.
  • Keep the discovering agent from authoring or controlling the evidence used to verify its own claim.
  • Run deterministic policy checks independently of the AI, so a model cannot bypass a gate by changing its explanation.

These controls implement the principle stated by OWASP SAMM: AI tools may check their own output, but that does not replace independent verification; people review every AI-proposed fix and verify it by rerunning the tool and relevant tests.

How can repeated AI findings become lasting guardrails?

When the same verified issue appears repeatedly, encode the requirement in a control that does not depend on model interpretation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Turn a recurring unsafe API pattern into a custom static-analysis rule with a documented rationale and approved exceptions.
  • Add policy-as-code checks for authentication, authorization, encryption settings, network exposure, or dependency provenance.
  • Keep regression tests that demonstrate both the rejected attack and legitimate behavior.
  • Add pipeline gates that fail on new violations and report the exact evidence or rule ID.
  • Version the rule, test it against known true and false positives, and assign an owner for updates.
  • Link the control to the original requirement and verification record so auditors can trace why it exists.

OWASP SAMM recommends that these checks remain independent of the AI and traceable to evidence. An agent can propose a rule or test, but the pipeline should enforce the accepted policy deterministically.

What are the most common verification failures?

  • Accepting fluent prose as proof: demand source locations, prerequisites, and an observed effect.
  • Letting the discovering agent verify itself: use a separate context, reviewer, or harness with no hidden reasoning dependency.
  • Skipping design intent: confirm whether the behavior is documented, public by design, or outside the attacker’s reachable boundary.
  • Replaying against production: use authorized staging, disposable environments, synthetic data, and controlled callbacks.
  • Equating a clean SAST result with security: test authorization, business logic, workflows, and runtime behavior separately.
  • Trusting a green AI-generated test suite: compare assertions and test inventory, and look for deleted or mocked-away coverage.
  • Fixing more than the finding requires: inspect the full diff for unrelated refactors, new dependencies, and changed configuration.
  • Closing a ticket without provenance: retain rejected and flagged evidence, the decision maker, and the reason for the status.

What is a practical verification checklist?

Use this checklist in the finding record or pull request. Every unchecked item is a reason to keep the status FLAGGED rather than VERIFIED.

  • Finding context, tool version, repository revision, timestamps, and raw artifacts are preserved.
  • Component, entry points, trust boundaries, identities, and attacker prerequisites are documented.
  • Exact source locations and the violated security requirement are identified.
  • The attack sequence and expected attacker gain are realistic and authorized for testing.
  • An independent reviewer or verifier can reproduce the claim without hidden agent reasoning.
  • An out-of-band signal confirms the claimed security effect.
  • Design intent and severity are checked against the actual threat model.
  • The result is classified VERIFIED, FLAGGED, or REJECTED with a recorded rationale.
  • The fix is isolated, minimal, and reviewed across source, dependencies, configuration, generated files, and tests.
  • The original scanner, relevant tests, negative cases, and broader security checks pass.
  • No tests were deleted, assertions weakened, syntax errors introduced, or unrelated behavior changed.
  • Required human, two-person, or security-team approvals are present before merge.
  • Recurring verified issues have a deterministic rule, test, policy check, or pipeline gate.

Frequently Asked Questions

What should I do when an AI finding cannot be reproduced?

Keep the complete record and mark it FLAGGED or REJECTED with the exact environmental differences and failed reproduction steps. Do not silently delete it; a later build, feature flag, or missing dependency may explain the discrepancy.

Can the same engineer who ran the AI agent approve the fix?

They can provide context, but a separate qualified reviewer should make the acceptance decision for security-sensitive changes. Independent review reduces confirmation bias and exposes assumptions hidden in the original prompt or setup.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How should exploit demonstrations handle secrets and personal data?

Use synthetic accounts, non-production records, and a controlled callback or sink. Redact credentials from tickets and logs, rotate any secret that was exposed during testing, and preserve only the minimum data needed to prove the effect.

What if the scanner stops reporting the issue but the behavior still looks unsafe?

Treat the clean scan as one signal, not closure. Review the authorization and business rules manually, exercise alternate entry points and identities, and add a regression test that expresses the security requirement rather than relying on the scanner’s pattern.
