A smartwatch can show glucose readings and alerts from a compatible continuous glucose monitor (CGM), but it does not measure blood glucose itself. In the documented U.S. Dexcom G7 setup, an Apple Watch can mirror the iPhone app or receive data directly from the CGM transmitter after setup. The CGM—not the watch—is the measuring device.
What a smartwatch can—and cannot—do
The U.S. Food and Drug Administration says it has not authorized, cleared, or approved a smartwatch or smart ring that measures or estimates glucose on its own. Its February 21, 2024 safety communication distinguishes those products from watch apps that display readings from authorized devices such as CGMs, which use a sensor inserted under the skin. That distinction matters for treatment decisions. The FDA warns that inaccurate glucose measurements can contribute to errors in diabetes management, including taking an incorrect dose of insulin or another glucose-lowering medicine. Do not use a watch or ring claiming to measure glucose without a sensor as a replacement for an authorized CGM or other clinician-recommended method.
How a CGM sends readings to a watch
In the Dexcom G7 system described in FDA documentation, a small flexible sensor wire sits under the skin and measures glucose in interstitial fluid. The transmitter calculates estimated glucose values and sends them to a display device. The system requires a sensor and at least one display, such as a compatible phone app, G7 receiver, or—in supported Apple setups—the watch app. The G7 is a prescription CGM in the reviewed U.S. submission. For the G7, there are two documented ways to use an Apple Watch display:

These modes are described in the FDA review and Dexcom’s G7 watch instructions. They are not a general feature of every CGM and smartwatch pairing.

Set up G7 Direct to Watch
Exact app screens and supported devices can change. Follow the current Dexcom instructions for your model and app version; the FDA-reviewed workflow establishes that the iOS app is used to configure Direct to Watch, but it does not provide a complete current compatibility list.
- Check compatibility first. Confirm that your iPhone, Apple Watch, operating systems, and app versions are supported using Dexcom’s current compatibility information. The reviewed evidence confirms a compatible iPhone and Apple Watch are required, not a specific model list.
- Start the G7 sensor session. Use the iPhone G7 app as directed by Dexcom and wait for the session to start.
- Enable Direct to Watch in the iOS app. Complete the app’s setup and configuration prompts, then follow the instructions to connect the watch app to the transmitter.
- Check that the watch receives data and alerts. Keep Bluetooth and the device settings recommended by Dexcom enabled. Verify the watch display is updating rather than assuming a blank or stale screen is current.
If Direct to Watch is unavailable for your device combination, the standard mirrored mode may still work if supported, but it requires the iPhone nearby. The G7 receiver is another display option; FDA documentation describes it as a separate handheld device that can communicate with the transmitter and show readings, graphs, and alerts.

Choose the display arrangement that fits your needs
- For glanceable access: A compatible watch can put CGM readings and alerts on your wrist. It is an optional display, not a substitute for the sensor-based CGM.
- If you want to leave your phone behind: Direct to Watch is the documented G7 option that can receive readings and alerts without the iPhone staying close after setup. Confirm the current compatibility requirements before relying on it.
- If you want another display route: The G7 receiver is a separate handheld option. Consider whether you want access to the phone app, receiver, or both rather than relying on a single screen.
- When selecting a CGM: Compare supported devices, intended use, prescription requirements, and available displays with your healthcare professional and the manufacturer. A watch’s presence alone does not establish that a particular CGM supports it.
For example, the FDA announced U.S. authorization of Abbott’s Libre Duo 10 Day Continuous Dual Glucose Ketone Monitoring System on August 25, 2026, for people aged 2 and older living with diabetes. The FDA announcement says readings are sent wirelessly to a compatible smartphone; it does not establish smartwatch display support. A watch display and its alerts depend on working sensor, transmitter, wireless connection, device settings, and current software. FDA records describe two version-specific G7 software corrections: an older issue involving failure to alert after a transmitter failure, and a later issue that could delay glucose-value and alert processing. These notices do not mean every current version is affected; check the live notices and follow Dexcom’s correction and update instructions.

- The FDA record for a correction initiated April 14, 2026 and posted May 11, 2026 includes G7 watchOS app versions 2.9.0 through 2.11.2. The described issue could delay estimated glucose values and alert processing. Check the record for current status and instructions: FDA correction record.
- A separate FDA notice describes a failure-to-alert issue for specified older G7 app versions after transmitter failure, which could leave users without glucose data for an extended period. Review the notice and update as directed: FDA G7 app correction notice.
If readings or alerts stop, appear delayed, or seem inconsistent, do not treat an old watch value as current. Check the CGM’s own app or receiver, follow the device instructions for connection or sensor problems, and use your established backup monitoring and treatment plan. Contact your healthcare professional when you are unsure how to act on a reading or interruption.
Product specifications are system-specific
The FDA G7 submission describes a reportable range of 40–400 mg/dL, with values outside that range shown as LOW or HIGH, and a wear period of up to 10 days plus a 12-hour grace period.
