AI-generated code should be treated as an untrusted draft, not as production-ready software. Test it in layers: first confirm that it solves the requested problem, then run deterministic build and security checks, inspect the complete diff, attack the generated tests with negative cases, verify every dependency and execution boundary, and require a qualified human to reproduce important findings and approve the release. A green test suite from the same agent that wrote the code is not independent evidence. The workflow below combines ordinary engineering review with threat modeling, supply-chain checks, adversarial testing, and documented accountability.

What does it mean to review AI-generated code like a human?

Human-style review asks more than whether the program runs. It checks intent, architecture, security, maintainability, operational impact, and whether the evidence for correctness is independent of the generator. Generated code can be syntactically correct while solving the wrong problem, exposing sensitive data, relying on an unmaintained package, or encoding a faulty assumption in both implementation and tests.

Use the same acceptance bar you would use for code written by a colleague, with extra attention to plausible-looking errors and missing context. The person approving a change remains responsible for production behavior; the use of an AI tool does not transfer that responsibility.

How do I establish what the code is supposed to do?

Start with the requested behavior and scope before judging implementation details. A reviewer cannot identify a correct solution if the acceptance criteria are ambiguous or only a fragment of the change is visible.

  1. Read the issue, acceptance criteria, design or architecture notes, and relevant operational requirements.
  2. Inspect the complete diff, including generated files, lockfiles, configuration, migrations, tests, and deleted lines. Do not review only the file the AI edited most recently.
  3. Write down the intended inputs, outputs, error behavior, data ownership, performance expectations, and compatibility constraints.
  4. Compare the change with local conventions for naming, layering, logging, retries, authorization, and feature flags.
  5. Ask whether the change solves the requested problem at the right boundary. A technically elegant implementation is still wrong if it changes an API contract, bypasses an existing service, or expands scope without approval.

GitHub’s review guidance places context and intent after initial functional checks, but in practice you should capture the intended behavior before interpreting a surprising test result. This prevents a passing test for an unintended behavior from becoming the definition of success.

Which deterministic checks should run before manual approval?

Run reproducible checks in a clean, controlled environment and retain their results with the change. No single check proves safety; the value comes from independent layers that fail for different reasons.

  • Build and compile: compile all targets, generate artifacts, and verify that production and test configurations use the same relevant flags.
  • Unit and integration tests: exercise both isolated logic and real interfaces such as databases, queues, identity providers, and serializers.
  • End-to-end and regression tests: verify user-visible flows and previously fixed defects, not just the new happy path.
  • Linting and static analysis: run language linters, type checks, code-quality rules, and static application security testing.
  • Dependency and license checks: inspect direct and transitive packages, versions, licenses, advisories, and whether the lockfile changed unexpectedly.
  • Secret scanning: check source, tests, fixtures, generated files, history, and configuration for credentials, tokens, private keys, and connection strings.
  • Infrastructure validation: lint Dockerfiles, deployment manifests, infrastructure code, CI workflows, and release scripts as executable code.

GitHub specifically identifies CodeQL, Dependabot, and code-quality checks as useful parts of this process. Treat their alerts as evidence to investigate, not as an automatic verdict. A clean report means only that the configured checks found nothing within their scope.

How should I inspect the diff for human-level quality?

After automated checks, read the diff line by line as a maintainer who will need to debug and change it six months from now. Generated code often contains unnecessary abstraction, duplicated logic, broad exception handling, or names that hide a security-sensitive operation.

Check correctness and edge behavior

  • Trace normal, empty, malformed, oversized, duplicated, delayed, and out-of-order inputs.
  • Verify boundary values such as zero, maximum lengths, time-zone transitions, integer limits, pagination edges, and expired credentials.
  • Confirm that failures return the documented status or error type and do not leak stack traces, secrets, or internal identifiers.
  • Check retries, timeouts, cancellation, idempotency, and concurrency. An AI-generated loop can duplicate writes or retry a non-idempotent operation indefinitely.

Check maintainability and architecture

  • Prefer the smallest design that satisfies the requirement. Reject a change that is harder to understand or refactor than a short, explicit rewrite.
  • Look for duplicated validation, inconsistent naming, dead branches, magic constants, and comments that describe what the code used to do.
  • Confirm that logging, metrics, tracing, and feature-flag behavior match the surrounding system and avoid recording personal or secret data.
  • Verify backward compatibility for public APIs, database schemas, message formats, and client versions.

Ask a second reviewer for changes involving unfamiliar domains or irreversible operations. Independent eyes are especially valuable when the authoring agent also produced the tests and security explanation.

How do I test whether AI-generated tests are trustworthy?

Assume generated tests may be incomplete or coupled to the implementation. OWASP warns that an AI agent can make CI green by deleting failing tests, weakening assertions, mocking the unit under test, or asserting buggy behavior. A passing suite created by the same agent that produced the code therefore provides no independent assurance.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Compare the test set with the acceptance criteria and the failure modes you identified during scope review.
  2. Search the diff for deleted tests, reduced assertion counts, broad snapshots, and mocks that replace the dependency or function whose behavior is being tested.
  3. Read assertions for specificity. A test that checks only a non-null result or a status code may pass while the payload, authorization decision, or side effect is wrong.
  4. Add negative and adversarial cases: invalid inputs, malformed payloads, expired or replayed tokens, unauthorized users, boundary values, concurrent requests, partial outages, and abuse attempts.
  5. Use mutation testing, fault injection, or deliberate test-breaking changes where practical. A useful test should fail when the relevant behavior is removed or inverted.
  6. Run a small set of hand-written tests from a different author or toolchain for high-risk paths.

Keep test data realistic enough to expose encoding, time, locale, and permission problems, while ensuring that fixtures contain no production secrets. Tests should verify behavior through a stable interface rather than preserving incidental implementation details.

How do I verify dependencies and prevent supply-chain mistakes?

Review every new or changed package as a production dependency, even when an AI tool presents it as a routine import. Confirm that the package exists in the intended registry, is maintained, has a compatible license, and actually provides the API being used.

  • Check the exact package name, version, publisher or maintainer, release history, and integrity information.
  • Look for typosquatting and “slopsquatting,” in which an AI invents a plausible package name that an attacker can register.
  • Prefer an existing approved dependency or standard-library capability when it meets the requirement.
  • Review transitive dependencies and install or build scripts for network access, shell execution, downloads, and credential use.
  • Confirm that license obligations, export restrictions, and support expectations fit the project.
  • Record why the dependency is needed and how it will be updated or removed.

Do not allow a package into production solely because an AI assistant suggested it. Dependency alerts and automated scanners are valuable filters, but a human must decide whether a finding affects this application and whether the selected version is appropriate.

When does AI-generated code require a deeper security review?

Use risk-based escalation rather than treating every line equally. Require a security specialist or domain expert for changes touching authentication, authorization, cryptography, input parsing, deserialization, file uploads, public endpoints, new integrations, data stores, CORS, or network exposure.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Apply the same escalation to CI workflows, Dockerfiles, infrastructure manifests, release scripts, lockfile changes, and code that handles credentials or personal data. These files can alter the execution environment even when application tests pass.

Use a threat-modeling pass

Identify assets, trust boundaries, actors, entry points, and abuse goals. Ask what happens if an attacker controls a request, a file, a dependency, a build variable, or a downstream response. Confirm authentication and authorization independently; never infer permission from the presence of a valid session alone.

Use multiple verification techniques

NIST’s 2021 guidance lists eleven complementary techniques:

  1. Threat modeling.
  2. Automated testing.
  3. Static code scanning.
  4. Heuristic secret checks.
  5. Built-in security protections.
  6. Black-box tests.
  7. Structural tests.
  8. Historical tests based on previously observed failures.
  9. Fuzzing.
  10. Web-application scanners.
  11. Verification of included libraries and services.

OWASP’s DevSecOps guidance summarizes the division of labor well: automated scanners find known patterns, while human review finds flaws that require context. Use scanners to widen coverage and humans to determine exploitability, impact, and an acceptable fix.

How do I review build, agent, and deployment boundaries?

Inspect any change that can execute commands or reach a network. Package scripts, CI configuration, container definitions, deployment files, and release automation are production code, not harmless metadata.

  • List new shell commands, downloads, third-party actions, registries, and outbound hosts.
  • Check whether a build step can read secrets, modify artifacts, publish packages, or alter deployment state.
  • Pin action and image versions where policy requires it, and verify their provenance.
  • Run agents with least privilege and keep them away from production credentials, signing keys, and unrestricted network access.
  • Confirm that generated patches cannot silently modify approval rules, branch protection, test selection, or release gates.

Use disposable environments for exploratory agent runs. A successful local build is not evidence that the same commands are safe in a privileged CI runner.

How should I reproduce and triage findings?

AI-assisted testing can produce false positives, duplicate alerts, and hallucinated vulnerabilities. Humans define the scope, constrain the tool, reproduce the behavior, judge exploitability and impact, and decide what to report.

  1. Capture the exact commit, configuration, input, tool version, and environment that produced the finding.
  2. Reproduce it independently, preferably without the generating agent and with a minimal test case.
  3. Determine whether the behavior is reachable in the deployed configuration and whether controls elsewhere prevent exploitation.
  4. Classify severity using the project’s risk model, document affected assets and evidence, and assign an owner and due date.
  5. Retest the fix and run regression checks before closing the finding.

Do not dismiss a reproducible issue because an automated tool described it poorly. Conversely, do not block a release on an unverified claim when the code path is unreachable; document the reasoning and any follow-up needed.

What evidence should accompany approval?

Make review traceable so another engineer can understand why the change was accepted. Mark AI-assisted changes in the commit or pull request, retain the relevant prompts or agent logs when policy permits, and record which human reviewed the implementation, tests, security findings, and deployment impact.

Review area Evidence to retain Approval question
Requirements and architecture Acceptance criteria, design notes, scope decision, complete diff Does this change solve the intended problem without unjustified expansion?
Functional behavior Build output, unit, integration, end-to-end, and regression results Do normal and failure paths meet the contract?
Test independence Negative cases, mutation or fault results, review of mocks and deleted tests Would these tests detect a plausible implementation bug?
Security and supply chain Threat model, scanner reports, dependency and secret checks, reproductions Are exploitable paths addressed and dependencies legitimate?
Operations and release CI and deployment validation, rollback plan, monitoring changes Can the change be released, observed, and reversed safely?
Accountability Named human approver, review date, unresolved-risk record Has a qualified person accepted the remaining risk?

A qualified human should approve before production, even when every automated gate is green. If risk remains, make it explicit and obtain the owner’s acceptance rather than hiding it behind a passing pipeline.

What is a practical end-to-end workflow?

For routine changes, use this sequence and scale the depth to the impact of the code:

  1. Frame the change: record behavior, scope, interfaces, data, and risk areas.
  2. Run deterministic gates: build, tests, lint, static analysis, dependency checks, secret scanning, and infrastructure validation.
  3. Read the complete diff: assess correctness, architecture, readability, error handling, observability, and edge cases.
  4. Attack the evidence: inspect test independence and add negative, boundary, concurrency, and abuse cases.
  5. Escalate sensitive paths: involve security or domain specialists for identity, parsing, cryptography, public exposure, and release machinery.
  6. Verify execution boundaries: review scripts, actions, containers, downloads, network access, and credential permissions.
  7. Reproduce findings: separate exploitable defects from false positives and document the decision.
  8. Approve and monitor: record human sign-off, deploy with an appropriate rollback plan, and watch logs and metrics for unexpected behavior.

This process is not an argument against using AI. It is a way to capture its speed while preserving the engineering judgment required for production software.

Frequently Asked Questions

Can I trust an AI-generated test suite if all checks pass?

Not by itself. Tests produced by the same agent as the implementation can share its assumptions, so obtain independent cases or review and rewrite the highest-risk tests before approval.

Should AI assistance be disclosed in a pull request?

Yes. A short disclosure improves traceability and tells reviewers to examine provenance, generated files, and test independence. Store additional agent logs only when your security and privacy policy allows it.

What should I do when an automated scanner reports a vulnerability that I cannot reproduce?

Record the exact commit, configuration, input, and scanner version, then reproduce in an isolated environment. If the path is unreachable, document that conclusion and any compensating control instead of deleting the alert without an audit trail.

Does human approval still matter for a small one-line AI change?

Yes, although the depth can be proportional to risk. A one-line change in an authorization check, CI workflow, or deployment manifest may deserve more scrutiny than a larger isolated refactor.
