Do not dismiss a cybersecurity alert just because it looks familiar, has a low severity, or turns out to involve an approved activity. First establish what happened, which person or system was involved, whether the activity was authorized, and what other evidence supports or contradicts the alert. If malicious activity is supported, escalate and contain it; if the activity was real but authorized, record it as benign or expected; if the activity did not happen or the detector was demonstrably wrong, document why it is a false positive. When the evidence is incomplete, keep investigating rather than closing the alert.

What is the difference between an event, an alert, and an incident?

An event is an action recorded by a system. An alert is a notification triggered by an event, and the event may or may not indicate a threat. An incident is a group of correlated alerts that people or automation judge likely to represent a genuine threat. Microsoft Security uses these distinctions in its incident-response guidance.

This matters because an alert is a prompt to investigate, not a final finding. One unusual login, file detection, or process event may be harmless on its own; related evidence can change its significance. Conversely, an alert that appears suspicious may be based on activity that never occurred or on a detector that interpreted the evidence incorrectly.

  • False positive: The system alerted, but the suspected activity did not actually occur or the detection was wrong.
  • False negative: Malicious behavior occurred without an alert.
  • True negative: Benign activity generated no alert.

In day-to-day triage, there is an important distinction between a false positive and a benign true positive. If a real login, scan, deployment, or test triggered an alert but was authorized, the activity happened; it is benign or expected, not a false alarm. Microsoft Defender documentation makes this distinction explicit.

How can I tell whether a security alert is real?

There is no single clue that proves an alert is genuine. Build a case from the observed activity, the asset and user involved, authorization, and evidence from other systems; treat uncertainty as unresolved rather than as proof of safety.

Preserve the alert and its context

Before changing, suppressing, or closing anything, record the alert text and detector, timestamp, severity, affected user and device, relevant IP address, file or URL, and any automated response already taken. Keep related evidence available for review. Deleting or losing the original context can make it harder to establish what the detector actually saw.

Severity is useful for prioritization, but it is not a verdict. A low-severity alert can be important when it involves a privileged identity, a critical system, or part of a larger sequence. A high-severity alert still needs investigation to establish whether the activity was malicious, authorized, or misidentified.

Establish what happened, when, and to whom

Start with five questions recommended by the UK Government Cyber Security Hub’s triage guidance:

  1. What was observed?
  2. When did it happen?
  3. Who is affected?
  4. Which systems or data are involved?
  5. Is this a known threat type?

These questions establish the nature, scope, and possible impact of the alert. The Hub advises treating every unusual event as a potential cybersecurity incident until it has been assessed. That does not mean every alert is an incident; it means an analyst should not assume it is harmless before checking.

Resolve the alert to a real asset and business role

Identify the actual user, endpoint, server, identity, application, or service behind the alert. Then determine what that asset does, whether it supports a critical or essential function, and whether the account or process normally belongs there. An unfamiliar process on a sensitive server deserves a different priority from the same process in a planned test environment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Asset resolution also prevents a common triage mistake: treating a technical label as though it explains the business context. A hostname, service account, or IP address may not make its owner or purpose obvious. NCSC and NHS incident-management guidance emphasize resolving assets, considering criticality, prioritizing work, and recording triage actions.

Verify whether the activity was authorized

Ask the relevant owner whether the action was planned. Check for documented maintenance, software deployment, vulnerability scanning, penetration testing, red-team work, or administrative changes. Confirm that the timing, account, system, and activity match the authorization rather than relying on a general statement that “a test was happening.”

Authorization changes the classification, but it does not make the evidence disappear. A real, approved scan that triggers a detection is benign activity or an informational expected event. If an alert claims a scan occurred but logs and the responsible owner show that it did not, the issue may instead be a false positive. Microsoft’s guidance separates benign true positives from false alarms on this basis.

Correlate the alert with other evidence

Check whether independent telemetry supports the alert. Depending on the event, compare endpoint, identity, email, DNS, firewall, proxy, cloud, and authentication records. Look for related alerts involving the same person or device, known indicators of compromise, unusual timing or locations, privilege changes, persistence, or lateral movement.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Correlation can expose a sequence that no single alert makes clear. For example, an identity alert and an endpoint alert involving the same account may have greater significance together than either has alone. NCSC guidance recommends enriching alerts with network knowledge and correlating them through incident management; Microsoft distinguishes incidents from individual alerts partly through this grouping of related evidence.

Determine scope and possible impact

Find out whether the activity is limited to one account or device or appears across multiple systems, accounts, or data sets. Consider the sensitivity of the information involved, the criticality of affected services, and the plausible business impact. This assessment helps determine urgency; a confirmed event affecting a critical function should not be handled like an isolated, unverified event on a low-impact asset.

Scope may remain uncertain during early triage. Record what is known, what has not been checked, and which evidence or owner confirmation is still needed. Do not convert an incomplete investigation into a clean bill of health merely because the first affected device is the only one identified so far.

Which signs make an alert more concerning?

Concern rises when the alert is supported by independent evidence, involves high-impact assets or behavior, or fits a broader suspicious sequence. No individual signal below is a universal proof, but several matching signals make prompt escalation more important.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • The alert matches a known malicious indicator, malware family, phishing technique, or compromised-account pattern.
  • The same user, identity, or device has related alerts in different telemetry sources.
  • The activity involves a privileged account, critical system, sensitive data, credential theft, ransomware-like behavior, or attempts to tamper with security controls.
  • The user or system owner cannot verify the action, or the time, location, process, or command line is inconsistent with normal behavior.
  • The alert appears to be one step in a sequence whose combined meaning is more serious than any single event.

Use those signals to guide what to verify next, not to make unsupported conclusions. A known indicator or unexpected location is a reason to investigate and correlate, not a substitute for checking the affected asset, authorization, and surrounding activity.

What evidence supports a benign or false-positive classification?

Classify activity as benign or a false positive only when evidence supports that specific outcome. A familiar alert or a plausible explanation is not enough by itself.

Evidence for authorized, benign activity

Useful evidence includes a documented maintenance window, approved deployment, vulnerability scan, penetration test, or other security exercise that matches the alert’s time, target, and activity. Confirmation should come from an owner or record with responsibility for the work, not just from an assumption that someone probably ran it.

Repeated alerts can also be benign when investigation confirms the same expected behavior from a known application, user group, or machine role. Microsoft recommends checking repetition, configuration, and alert definitions before deciding whether to exclude or tune a detection. Repetition alone is not proof: repeated malicious behavior can also generate repeated alerts.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Evidence for a false positive

A false-positive finding needs evidence that the alerted activity did not occur or that the detector misidentified a file, process, URL, or behavior. For instance, if the alert names a process but independent records do not support the suspected action and examination shows the detector misclassified it, document those findings. The important distinction is between “we have not found evidence yet” and “we have evidence the detected activity was absent or misidentified.”

If neither the malicious explanation nor the benign explanation is established, leave the result unresolved and gather more information. A missing log, unavailable owner, or inconclusive scan is a limit on what is known, not proof that the alert was wrong.

How should I classify the alert and decide what to do next?

Use the classification that best matches the evidence, then take an action proportionate to the potential risk. Microsoft, MITRE, NCSC, and NHS guidance support distinguishing confirmed malicious activity, authorized activity, false alarms, and cases that still need investigation.

Finding Classification Typical next step
Malicious behavior occurred and was detected. True positive Escalate, contain, determine scope, and remediate.
Activity occurred and was authorized or simulated. Benign true positive or informational expected activity Record the authorization, monitor as appropriate, and consider careful tuning if the alert is repetitive.
The alerted activity did not occur or the detection is demonstrably wrong. False positive Document the supporting evidence, then suppress narrowly or tune the detector where supported.
Evidence is incomplete or contradictory. Unresolved; continue investigating Preserve evidence, gather logs and owner confirmation, and avoid premature closure.

For a supported malicious finding, escalation and containment should follow the organization’s incident process. For an authorized event, retain enough information to explain why the action was permitted and whether the alert remains useful. For a false positive, record what established that the detection was wrong before changing how future alerts are handled.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Support on Ko-Fi

When should I suppress or tune a detection?

Suppress or tune only after classifying the activity and understanding what the change will hide. An exception that removes an entire detection family, asset class, or user group from view can conceal a future attack that resembles the benign activity.

Prefer a narrow, evidence-based change tied to the known application, activity, or context. Check how the rule behaves against expected activity and whether genuine incidents remain distinguishable from false alarms. NCSC recommends testing and tuning alerts for that reason; Microsoft’s workflow likewise places investigation and classification before suppression or tuning.

Repeated alerts may indicate that a detector’s configuration or definition needs review, but frequency alone does not justify a broad exclusion. Keep a record of the rationale, scope, and owner for any tuning decision, and revisit the decision if the application, system role, or expected activity changes.

What should I avoid while triaging an alert?

Several shortcuts can turn a manageable alert into a visibility or response problem:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Do not close it solely because it is low severity. Severity does not establish whether activity is malicious or whether the affected asset matters.
  • Do not dismiss it because it is familiar or noisy. Repeated alerts require validation; attackers can use behavior that resembles routine activity.
  • Do not label real authorized activity a false positive. It is a benign true positive or expected activity if it happened.
  • Do not create a broad suppression rule without testing its effect. Overly broad exceptions may remove visibility into genuine incidents.
  • Do not treat missing evidence as evidence of absence. If logs or owner confirmation are unavailable, document the gap and keep the case unresolved when needed.

How can a team make alert triage more reliable?

A reliable process makes it possible to reconstruct why an alert was escalated, classified, suppressed, or left open. It should combine evidence gathering with clear ownership and a reviewable record, rather than depending on an analyst’s memory or an informal “looks fine.”

  • Record the alert details, relevant evidence, affected assets, analyst reasoning, actions taken, and follow-up owner.
  • Maintain enough asset and identity context to resolve technical alerts to users, services, and business criticality.
  • Correlate relevant telemetry instead of assessing each notification in isolation.
  • Use consistent labels for true positives, benign true positives or expected activity, false positives, and unresolved cases.
  • Review repeated alerts for configuration or detection-definition issues, then test narrow tuning changes.
  • Keep a clear escalation path for alerts involving critical assets, sensitive data, or credible malicious activity.

When assessing an alert-management process or tool, useful criteria include evidence enrichment and log breadth; asset and identity resolution; cross-source correlation; classification vocabulary; severity and criticality handling; audit trails and analyst notes; suppression scope and testing; automation with human review; and support for playbooks or standard operating procedures. These criteria reflect the operational concerns emphasized in NCSC, NHS, and Microsoft guidance.

Frequently Asked Questions

Can a true positive be harmless?

Yes. “True positive” describes whether the detector caught activity that actually happened; it does not by itself say the activity was malicious. An approved test or deployment can be a true detection of benign behavior.

Does a false positive mean the security tool is broken?

Not necessarily. A detector can be useful overall and still misidentify a particular file, process, URL, or behavior; the case should be documented so the definition or configuration can be reviewed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Is there a standard acceptable false-positive rate for security alerts?

There is no universal rate or suppression threshold established for every organization. Teams need to calculate their own rates from alert and investigation records and interpret them alongside severity, asset criticality, and whether genuine incidents remain visible.
