AI-generated code and tests are drafts, not evidence that a change is safe. Re-run the original test suite, inspect the complete diff (including tests and lockfiles), and compare the implementation with the requirements and threat model. Then add independent negative, boundary, malformed-input, expired-token, concurrency, and authorization tests; run static analysis, secret scanning, and dependency audits; and have a qualified reviewer reproduce security findings before merge. A suite produced by the same agent that wrote the code cannot independently validate it: OWASP warns that “a passing test suite generated by the same agent that produced the code provides no independent assurance.” OWASP Secure Coding with AI Cheat Sheet provides the baseline controls, while published studies show both high test failure rates and real vulnerabilities in generated snippets.
Why can AI-generated code pass tests and still be wrong?
Tests only prove the behavior they actually assert. An assistant can make a test pass by changing production behavior, weakening an assertion, deleting a failing case, or replacing a real dependency with a mock that never exercises the risky path. Passing also says nothing about requirements that no test covers, such as authorization, token expiry, race conditions, or safe handling of malformed input.
Independent evidence is especially important because the generator and its tests share the same blind spots. OWASP puts it plainly: “100% passing means nothing if the tests assert broken behavior.” The person who accepts the output remains accountable for it, as stated in OWASP SAMM guidance on AI-assisted development.

| Finding | What it means for your review | Source |
|---|---|---|
| 29.5% of Python snippets and 24.2% of JavaScript snippets in a 733-snippet GitHub-project sample contained security weaknesses across 43 CWE categories. | Security defects are common enough that generated code needs a deliberate security review, not just a style review. | Fu et al., ACM TOSEM, 2025 |
| Up to 55.5% of identified issues were fixed when Copilot Chat received static-analysis warning messages. | Give the assistant precise, independently generated findings if you use it to propose a repair; still verify the patch yourself. | Fu et al., ACM TOSEM, 2025 |
| With an existing Python suite, 45.28% of generated tests passed; 54.72% were failing, broken, or empty. | A green subset does not establish that the generated tests are complete or useful. | TU Delft/ACM AST study, 2024 |
| Without an existing suite, 92.45% of generated tests were failing, broken, or empty. | Provide a known-good harness and requirements, then independently check every generated test. | TU Delft/ACM AST study, 2024 |
A 25-person ICSE user study found that Copilot accompanied more secure solutions on harder problems, had no effect on easier ones, and showed no disproportionate effect by vulnerability type. That result argues against both blanket optimism and blanket rejection: the task, controls, and review process matter. IEEE/ACM ICSE study, 2024

How do I spot that an AI agent weakened or fabricated a test?
Start with the test diff, not the green build badge. Compare the branch with the last trusted revision and look for changes that reduce the test’s ability to fail when behavior is wrong.
Check for deleted tests and reduced assertions
- Flag any deleted or skipped test unless the change request explains the removal and a replacement exists.
- Compare assertion counts and strength.
assertEquals(expected, actual)changed toassertNotNull(actual)is a classic weakening. - Look for broad exception handling such as
try/except Exception: passor a test that accepts any status from an HTTP call. - Check that a test still reaches the production code. An early return, an unconditional mock, or a fixture that never loads the target can make a test appear green while testing nothing.
OWASP recommends human review of every AI-generated test modification and CI rules that flag test deletions or assertion-count reductions. Treat an unexplained reduction as a merge blocker.
Look for mocks that bypass the behavior under test
Mocks are useful when a dependency is slow or external, but they can hide authorization checks, serialization errors, transaction boundaries, and retry behavior. Compare the mocked call with the real contract: method name, arguments, return shape, exceptions, and side effects. Keep at least one integration-level test that uses the real adapter or a faithful test server for each security-sensitive boundary.

Detect tests that assert the bug
Read the expected value in plain language. A generated test may encode an insecure result—for example, accepting an unsanitized string in HTML, allowing a user to read another tenant’s record, or treating an expired token as valid. Ask, “Would this outcome be acceptable under the requirement and threat model?” before trusting the assertion.
Check that failure is possible
Temporarily introduce a controlled defect (such as changing a comparison operator or removing an authorization branch) in a disposable branch. A meaningful test should fail. Mutation testing tools can automate this check; when they are unavailable, a small manual mutation of critical paths is still informative.

Which cases do generated tests usually miss?
Generated tests tend to follow the happy path visible in a function signature or nearby examples. Write adversarial cases by hand and keep them independent of the implementation’s internal structure.
Invalid and malformed input
- Empty, null, overlong, truncated, and incorrectly encoded values.
- Unexpected JSON types, duplicate keys, unknown fields, and deeply nested objects.
- Path traversal sequences, control characters, dangerous HTML, and shell metacharacters at every output boundary.
- Values outside numeric, date, and collection limits.
Authentication and authorization
- No credentials, malformed credentials, revoked credentials, and expired access or refresh tokens.
- A valid user requesting another user’s object, another tenant’s data, or an administrator-only operation.
- Method and content-type changes that might bypass middleware.
- Direct access to an endpoint that the normal UI would hide.
Concurrency and state transitions
- Two updates to the same record at once, duplicate requests, retries after partial failure, and out-of-order messages.
- Resource exhaustion: large uploads, many pagination requests, slow clients, and repeated login attempts.
- Transitions that should be one-way, such as consuming a token twice or approving an already cancelled operation.
Cryptography and randomness
Verify that secrets use a cryptographically secure random source, keys are generated and stored correctly, nonces are unique where required, and comparisons are appropriate for secrets. A test that merely checks that a token is non-empty will not detect predictable randomness or token reuse.
How can I identify security bugs in AI-generated code before merging?
Review in layers: understand the data flow, run independent scanners, then reproduce every finding. The ACM study found real-project issues including CWE-330 (insufficiently random values), CWE-94 (code injection), and CWE-79 (cross-site scripting), demonstrating why a test-only review is insufficient. Fu et al., ACM TOSEM, 2025
Trace trust boundaries and sensitive data
- Mark where untrusted input enters, where privilege is decided, where data is deserialized, and where output reaches a browser, shell, database, template, or code interpreter.
- Confirm authentication happens before data access and authorization is checked against the object or tenant, not just the route.
- Check validation at the boundary and context-appropriate output encoding; escaping for HTML does not make a value safe for a shell command or SQL query.
- Verify error responses do not disclose tokens, credentials, stack traces, or internal paths.
Run static analysis and secret scanning
Run your language’s SAST rules, secret scanner, and lint checks on the complete diff and the built artifact. Feed scanner findings to an assistant only as a proposed repair input; the scanner, not the assistant, remains the independent detector. Keep high-confidence findings as CI gates.
Audit dependencies and provenance
AI can suggest obsolete, hallucinated, or typosquatted packages. Before installation, verify the package name, maintainer and repository provenance, release history, license, and version. Run the ecosystem’s audit command—npm audit, pip audit, govulncheck, or cargo audit—and cross-check advisories in the NVD, OSV, and the GitHub Advisory Database. Pin versions through normal lockfile and update processes; do not copy an unverified package name from a chat response.

Inspect the assistant’s context and permissions
Find out which files, terminal output, prompts, and environment variables the assistant can send to its provider. Exclude secrets, production data, and sensitive directories. Treat generated plugins, package-install commands, and MCP servers as untrusted until verified. Limit write, shell, network, and deployment permissions, and require approval for actions outside the requested scope. OWASP SAMM identifies excessive agency and prompt injection as risks in AI-assisted development.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsWhat is the safest workflow for fixing a failing test or vulnerability?
Use a frozen, reproducible sequence so the assistant cannot silently redefine success.
- Freeze the diff. Create a branch or commit containing the generated changes. Save the exact failing test, scanner output, dependency lockfile, runtime version, and command used.
- Reproduce independently. Run the original suite and the smallest reproducer in a clean environment. Confirm whether the failure is deterministic and record the expected behavior from requirements, API contracts, and the threat model.
- Restore evidence. Bring back deleted tests and original assertions before requesting a fix. Document why each test exists and mark any intentional behavior change for review.
- Write a hand-authored regression test. Make it fail against the current code. Include the malicious, negative, boundary, expired-token, concurrency, or authorization case that exposes the defect.
- Request a minimal patch. Give the assistant explicit requirements, threat-model assumptions, allowed files, and prohibited changes. Ask it to explain the root cause and produce the smallest patch; do not authorize broad refactoring while repairing a security issue.
- Review every changed line. Check production code, tests, fixtures, configuration, generated files, and lockfiles. Ensure the patch fixes the root cause rather than suppressing the symptom.
- Run layered verification. Execute unit and integration tests, then mutation or property-based tests where useful, SAST, secret scanning, and dependency audits. Re-run the original reproducer and the new regression test.
- Reproduce security findings manually. Use a safe test environment to demonstrate the exploit before and after the patch. Verify that the vulnerable data flow is closed and that alternate routes, encodings, and race paths do not remain.
- Obtain qualified approval. Require a human reviewer with relevant expertise for authentication, authorization, cryptography, parsers, interpreters, payment logic, external interfaces, and deployment changes. OWASP SAMM recommends human-directed review and deterministic guardrails for high-risk areas.
- Keep the gates. Fail CI on unexplained test deletion, reduced assertion counts, known vulnerable dependencies, exposed secrets, and high-confidence static-analysis findings.
How should I ask an AI assistant for a repair without creating a second bug?
Constrain the task so the model cannot trade correctness for a green build. A useful request includes:
- The exact failing command, stack trace, and smallest reproducible input.
- The intended behavior, non-negotiable security properties, and assumptions about users, tenants, tokens, and trust boundaries.
- The files it may change and files it must not change, with a request for a minimal diff.
- Required tests: one regression test plus negative, boundary, malformed-input, authorization, and concurrency cases relevant to the defect.
- A requirement to explain why the existing test missed the issue and to list residual risks.
- A prohibition on deleting, skipping, weakening, or broadly mocking tests to obtain a pass.
After the response, ask a second reviewer—or a separate toolchain—to critique the patch against the same requirements. Do not treat a model’s self-review as independent assurance.
Which checks should be independent, deterministic, and reproducible?
Choose controls that differ from the agent that wrote the code and can run consistently in CI.

| Axis | Good control | Warning sign |
|---|---|---|
| Independence | A separate test author, scanner, or review process challenges the generated output. | The same prompt, model, and fixtures generate both code and its only tests. |
| Coverage | Negative, boundary, authorization, malformed-input, and concurrency cases are explicit. | Only happy-path examples and nominal status codes are checked. |
| Determinism | Versioned rules and repeatable commands run in CI. | A one-off chat judgment is the only acceptance criterion. |
| Reproducibility | A reviewer can recreate the failure and verify the fix from a clean checkout. | The result depends on an undocumented prompt, local state, or unavailable service. |
| Scope control | File, shell, network, package-install, and deployment permissions are least-privilege. | The agent can modify unrelated code or deploy without approval. |
| Freshness | Dependency and vulnerability databases are updated on a defined schedule. | Audits use stale data or an unpinned dependency graph. |
| Accountability | A named, qualified reviewer approves high-risk changes. | “The AI said it was secure” substitutes for ownership. |
What does real-world evidence say about AI-generated vulnerabilities and repairs?
Evidence from project code shows that the risk is measurable, not hypothetical. In a DevGPT study, manual review found 56 vulnerabilities in 48 files among 2,315 C, C++, and C# snippets. In a September 2025 experiment, repair prompts led GPT-4.1 to fix 42 cases, GPT-5 to fix 44, and Claude Opus 4.1 to fix 43. These figures describe that study’s dataset and experiment; they are not a guarantee for your codebase. Empirical Software Engineering study
The same pattern appears in test generation: having an existing suite dramatically improved the proportion of usable tests in the TU Delft/ACM AST study, but it still left more than half failing, broken, or empty. A harness and baseline help; neither replaces review.
What should a pre-merge checklist contain?
- Full diff reviewed, including tests, fixtures, configuration, generated files, and lockfiles.
- No unexplained test deletion, skipped test, assertion weakening, or mock that bypasses real behavior.
- Hand-written regression test for every fixed defect.
- Adversarial coverage for malformed input, boundaries, expired credentials, authorization, and relevant races.
- Language-appropriate unit and integration tests, plus mutation or property-based tests when they add signal.
- SAST, secret scanning, and dependency audit completed with current databases.
- Every new dependency’s name, version, provenance, license, and advisory status verified.
- Assistant context excludes secrets and sensitive directories; permissions are least-privilege.
- Each security finding reproduced independently, with the threat-model decision documented.
- Qualified human approval recorded for high-risk components.
What if the failure is flaky or only appears in CI?
Capture the exact environment before changing code: operating-system image, language and dependency versions, timezone, locale, random seed, parallelism, database state, and network behavior. Run the smallest test repeatedly with a fixed seed, then with randomized seeds, and compare serial versus parallel execution. Flakiness often exposes an unawaited task, shared mutable fixture, time assumption, or race that a generated happy-path test never exercises.
Do not “fix” a flaky security test by increasing sleeps or retries without identifying the synchronization problem. Preserve a deterministic regression test for the underlying race or state error, and keep a bounded stress or repeated-run job when concurrency is part of the risk.
When should an AI-generated patch be rejected outright?
Reject or return it for redesign when it deletes evidence, broadens privileges, adds an unverified dependency, disables a scanner, hard-codes a secret, suppresses an exception, changes cryptographic primitives without analysis, or cannot be reproduced from a clean checkout. Also reject a patch whose only justification is that all generated tests pass. A smaller, slower, human-reviewed change is safer than a broad refactor whose behavior no one can explain.

Frequently Asked Questions
Can I trust AI-generated tests if they pass in continuous integration?
Not by themselves. Verify that the tests can fail against a deliberate defect, cover the security requirements you care about, and were not generated by the same change that weakened the implementation. Keep an independent scanner or reviewer in the acceptance path.
How do I prove that a generated dependency is legitimate?
Resolve the exact package in the official registry, inspect its repository and release history, confirm the maintainer and license, and compare the requested version with current advisory databases. Install only after the name and provenance match your approved dependency policy, then commit the lockfile.
What should I do if an AI assistant has seen a secret in a prompt or terminal output?
Assume the secret is exposed: revoke or rotate it, inspect provider retention and access controls, and determine what logs or repositories may contain it. Add the relevant paths to context exclusions and prevent future prompts from including credentials or production data.
Who is qualified to approve an AI-generated authentication or cryptography change?
Use a reviewer who understands the relevant protocol, threat model, and language security properties, rather than relying on general code ownership alone. For high-impact systems, require two-person review or a security sign-off and retain the reasoning with the change record.
How can I tell whether a security fix removed the root cause or only hid the warning?
Reproduce the original exploit, trace the input through every reachable path, and test alternate encodings, routes, roles, and race timings. The scanner warning should disappear for a reason you can explain, while the regression test fails on the pre-fix revision and passes on the fixed one.
