Use AI to review coding-agent output, but never let the model be the final approver. A second model can inspect a large diff in repository context, run or interpret checks, and surface likely correctness, security, dependency, and maintainability problems quickly. It can also miss defects, invent findings, or recommend an insecure fix. A qualified human must understand the change, verify the evidence, and accept responsibility before merge. The dependable approach is layered: define behavior first, have an AI reviewer analyze the change, run deterministic and security checks, then perform accountable human and peer review.
Why should coding-agent output receive both AI and human review?
AI-generated code has the same obligation as hand-written code: it must satisfy the specification, pass the project’s quality gates, and be safe in its deployment context. AI review adds useful scale and repository awareness, while human review supplies intent, judgment, and accountability.
The UK Home Office standard, updated 20 March 2026, states: “AI-assisted outputs MUST be reviewed and approved by a human before reaching production.” The same standard says, “AI-assisted code MUST meet the same security expectations as human-written code.” GOV.UK guidance is equally direct: “You should only commit code changes that you understand.” Chromium’s policy requires authors to self-review and understand every code and documentation update, with or without AI tooling. OWASP AISVS AC.4.1 says to verify that AI-generated code always goes through code review by a qualified human engineer.
AI review is therefore a second line of analysis, not a substitute for ownership. A model can explain control flow or notice an untested branch, but it cannot reliably decide whether a product requirement is correct, whether a privacy trade-off is acceptable, or whether a business rule was intentionally changed.

What can an AI reviewer find in code produced by another AI?
An AI reviewer is most valuable when it receives the complete diff, relevant repository files, configuration, tests, and the stated acceptance criteria. With that context it can inspect more than syntax.
Behavior and correctness defects
- Unreachable branches, incorrect conditions, missing error handling, and state transitions that violate the stated behavior.
- Edge cases such as empty collections, null values, retries, time zones, race conditions, and partial failures.
- API contract mismatches, serialization errors, incorrect status codes, and assumptions that conflict with neighboring modules.
- Tests that assert implementation details but fail to exercise the acceptance criteria.
Security and privacy risks
- Missing authorization checks, insecure direct object references, injection opportunities, unsafe deserialization, and weak validation.
- Secrets embedded in source, logs, fixtures, or configuration; excessive logging of personal or authentication data.
- Insecure defaults in infrastructure files, permissive network rules, and missing transport or storage protections.
Dependency and supply-chain changes
- New packages, transitive dependencies, version changes, license implications, and unpinned artifacts.
- Use of a library API that is deprecated, unsupported, or inconsistent with the project’s approved versions.
Maintainability and operational impact
- Duplicated logic, unclear naming, oversized functions, brittle mocks, and changes that bypass established abstractions.
- Migration scripts that are not reversible, incompatible database changes, missing metrics, or behavior that will make incidents harder to diagnose.
Models can hallucinate a problem or suggest a fix that introduces a new vulnerability. Treat every finding as a hypothesis: reproduce it, compare it with trusted documentation and the specification, and reject it when the evidence does not support it.
How do I review AI-generated code step by step?
Start with a small, traceable change and a written definition of “done.” The following sequence keeps model feedback inside an accountable engineering process.
Quick wins for a faster PC:
Scan for outdated or missing drivers - takes under a minuteDriver Scan →Repair Windows errors before they cause bigger problemsFix Now →- Define intended behavior before editing. Write acceptance criteria, invariants, error behavior, compatibility requirements, and tests that should pass. Record security or privacy constraints and identify files that require elevated review.
- Constrain the agent’s task. Give the coding agent a narrowly scoped objective, the relevant repository conventions, and explicit limits on dependencies, generated files, and unrelated refactoring. Ask it to describe assumptions and list files it changed.
- Capture a clean diff and context. Keep the work on a branch, make commits small and specific, and include the issue or specification. Provide the AI reviewer with the diff, surrounding code, configuration, test results, and the acceptance criteria rather than an isolated snippet.
- Ask an AI reviewer for structured findings. Request separate sections for correctness, security, dependency and licensing impact, tests, performance, and maintainability. Require file-and-line references, the violated requirement, severity, confidence, and a minimal remediation. Ask it to state what it could not verify.
- Run deterministic checks. Execute unit, integration, regression, static-analysis, formatting, build, and type checks. Run secret scanning, software-composition or dependency checks, and infrastructure scanning where applicable. Record the tool versions and results in the pull request.
- Verify every model finding and fix. Reproduce the behavior with a test, a local run, or authoritative documentation. Inspect proposed patches for changed semantics, new attack paths, performance regressions, and unnecessary dependencies. A finding that cannot be demonstrated should be marked unconfirmed, not silently accepted.
- Perform human and peer approval. Read the entire diff yourself, including generated files and migrations. A qualified reviewer must be able to explain the change, its risks, and its test evidence. Require a second reviewer or a specialist for security-critical, privacy-sensitive, financial, authentication, or infrastructure code.
- Merge only through protected gates. Use branch protection to require passing checks, a qualified approval, and resolution of review threads. Do not allow the coding agent or the AI reviewer to approve its own changes.
- Retain useful traceability. Keep the prompt or task description, agent session or action log when policy permits, commit history, AI findings, test output, and approvals. This record supports incident analysis and shows why the change was accepted.
Which automated checks belong on an agent-generated pull request?
Model comments are probabilistic; automated checks provide repeatable evidence. Select checks according to the languages, deployment target, and risk of the change.
| Check | What it establishes | Typical response to failure |
|---|---|---|
| Unit tests | Local behavior and edge cases for changed components | Fix the implementation or add a regression test that captures the required behavior |
| Integration and contract tests | Compatibility among services, databases, queues, and external APIs | Reconcile schemas, status codes, retries, and migration order before approval |
| Regression tests | That existing user-visible behavior remains intact | Identify the changed assumption; do not waive the failure merely because new tests pass |
| Static analysis and type checks | Definite bugs, unsafe patterns, type mismatches, and quality-rule violations | Correct the issue or document a narrowly justified, reviewed suppression |
| Secret scanning | Credentials and sensitive tokens in source, history, fixtures, and logs | Revoke exposed credentials, remove them from all relevant history, and replace them with managed secrets |
| Dependency and license checks | Known vulnerabilities, version provenance, transitive packages, and license policy | Upgrade, remove, isolate, or explicitly approve the dependency with security and legal owners |
| SAST and infrastructure scanning | Application security patterns and unsafe cloud, container, or deployment settings | Apply a verified fix and repeat the scan; escalate unresolved high-severity findings |
| Build, formatting, and packaging checks | Reproducible compilation and deployable artifacts | Regenerate only the intended artifacts and inspect generated changes |
A green pipeline does not prove that the feature meets its business intent. It proves only what the checks cover, so the acceptance criteria and human reading remain necessary.
How should I triage findings from an AI code review?
Use evidence and impact rather than the model’s wording. Classify each comment as confirmed defect, likely risk requiring investigation, false positive, or out of scope.
Confirm the claim
Locate the cited code, reproduce the path, and compare the behavior with the requirement and trusted API or security documentation. For a security claim, demonstrate exploitability in a safe test or obtain a qualified security review. For a correctness claim, add or run a focused test.

Set severity by consequence
Prioritize authentication and authorization bypasses, data exposure, destructive migrations, remote-code execution paths, payment or safety logic, and failures that can corrupt durable state. A stylistic concern should not delay a release while a plausible access-control flaw is unresolved.
Review the proposed fix as a new change
AI-suggested remediations can weaken validation, hide errors, broaden permissions, or change an API contract. Compare the patch with the original acceptance tests, run the full relevant suite, and ask a human specialist to review security-sensitive fixes.
Close the loop
Mark why a finding was fixed, deferred with an owner and deadline, or rejected as unsupported. This prevents repeated model comments and leaves an auditable decision trail.
Are coding agents safe to merge?
They are safe to merge only when the same controls used for human code are satisfied and a qualified human accepts responsibility. “The agent wrote it” is not a risk classification or an approval.
- The change has a clear owner, linked requirement, and bounded scope.
- Acceptance tests and relevant automated checks pass on the exact commit proposed for merge.
- Dependencies, generated files, migrations, and configuration changes have been inspected.
- Security, privacy, and data-handling impacts have been assessed; high-risk findings are resolved or formally accepted by the right owner.
- A human reviewer understands the implementation and has authority to approve it.
- Branch protection prevents bypassing required checks and approvals.
Do not merge when the reviewer cannot explain a critical function, when tests are missing for the stated behavior, when a model-generated fix is accepted without verification, or when a secret, dependency, or infrastructure change remains unexplained.
What should reviewers inspect beyond the code diff?
The diff is necessary but not sufficient. Review the evidence around how the change was produced and validated.
- Agent rationale and assumptions: Check whether the implementation relied on an unstated product rule, guessed API behavior, or ignored an instruction.
- Tool and action logs: Look for commands run, files read or modified, network access, generated artifacts, and failed attempts that may have influenced the result.
- Test evidence: Confirm that tests ran after the final edit, cover the changed paths, and were not skipped, weakened, or made less meaningful.
- Dependency inventory: Compare lockfiles and manifests with the base branch; inspect new transitive packages and their provenance.
- Configuration and deployment effects: Examine environment variables, permissions, container images, infrastructure code, migrations, feature flags, and rollback plans.
- Data handling: Verify that prompts, source code, logs, and test data comply with your organization’s approved-tool and confidentiality rules.
How should a team govern AI-assisted code review?
Publish a policy that defines approved tools, permitted data, required human roles, retention of prompts and logs, and escalation paths. The policy should apply equally to code generated by an IDE assistant, a pull-request bot, or an autonomous coding agent.

Set role separation
The person who requested the change should self-review it, but sensitive changes should also receive independent peer or specialist review. An AI reviewer may comment and propose patches; it should not satisfy the human approval requirement.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesProtect sensitive inputs
Do not send secrets, regulated personal data, proprietary credentials, or restricted source to an unapproved service. Use redaction, access controls, retention limits, and an approved enterprise configuration where required. Document what data leaves the repository boundary.
Require reproducibility
Pin tool and dependency versions where practical, retain the exact commit and check results, and make it possible to reconstruct which prompt or agent session produced a material change. Small commits make both model analysis and human review more reliable.
Measure process quality without invented certainty
Track escaped defects, reverted changes, review latency, false-positive rates, and coverage of required checks for your own repositories. There is no single authoritative statistic showing how often agent output is reviewed or how much safer AI-assisted review is in every production setting; local evidence is more useful than a universal percentage.
When should AI review be limited or escalated?
Use AI for analysis, summarization, and test suggestions, but require elevated human review when the change can cause disproportionate harm.
- Authentication, authorization, cryptography, identity, secrets, or audit logging.
- Personal, health, financial, safety-critical, or regulated data processing.
- Payment calculations, entitlement decisions, destructive operations, or irreversible migrations.
- Network boundaries, infrastructure-as-code, container privileges, CI/CD credentials, or production access.
- Large refactors where the intended behavior is difficult to express in tests, or generated code that cannot be meaningfully understood by the owning team.
For these cases, route findings to the security, privacy, reliability, or domain owner specified by your policy. If the team cannot establish what the code does, stop the merge and simplify or rewrite the change.

- Acceptance criteria and non-functional requirements are linked.
- The diff is small, focused, and free of unexplained generated or unrelated files.
- The author has read and understands every changed code path.
- An AI review used repository context and reported assumptions, limits, and file-level findings.
- Every AI finding is confirmed, rejected with a reason, or assigned for follow-up.
- Unit, integration, regression, static-analysis, type, build, and formatting checks passed where applicable.
- Secret, dependency, license, SAST, and infrastructure scans passed or have documented, authorized exceptions.
- New dependencies, permissions, migrations, configuration, and data flows have human review.
- Security-critical or regulated changes have specialist approval.
- Agent prompts or session logs are retained when policy or incident traceability requires them.
- Branch protection shows the required approvals and checks on the exact merge commit.
How do AI code-review tools and workflows differ?
Compare a workflow by the evidence it can produce, not by how many comments it generates. The important axes are:
| Capability | Questions to ask |
|---|---|
| Repository context and execution | Can the reviewer inspect related files, configuration, history, and test output, or does it see only a pasted diff? |
| Actionable findings | Does each comment include a location, impact, confidence, reproduction path, and a minimal fix? How are false positives tracked? |
| Security coverage | Does the workflow complement SAST, dependency, secret, and infrastructure scanners rather than claiming to replace them? |
| Traceability | Can the team retain prompts, agent actions, commits, findings, approvals, and the exact tool version? |
| Data controls | What repositories and logs are sent to the service, how long are they retained, and is the tool approved for the data involved? |
| Governance integration | Does it respect branch protection, required reviewers, CODEOWNERS-style ownership, and escalation for sensitive files? |
| Small-diff support | Can it review incremental commits and identify changes in dependencies, generated files, and migrations? |
OpenAI research has reported better review quality when models receive repository access and execution abilities, but that result is not a universal production-safety guarantee. Validate any workflow against your own defect and false-positive data.

“The AI reviewer said it was safe.”
Replace the assertion with evidence: run the relevant tests and scans, reproduce important claims, and obtain human approval. A model’s confidence is not a security sign-off.
“The tests pass, so the change is correct.”
Check whether the tests encode the actual requirement and include negative and boundary cases. Add a failing test for the missing behavior before changing the implementation.
“The model found a vulnerability, so we applied its patch.”
Review the patch independently, especially for authorization, validation, cryptography, and error handling. Confirm the vulnerability and run security-focused tests; an unverified fix can conceal the original issue.
“The diff is too large to understand.”
Stop and split the work into behavior-preserving refactors and focused functional commits. Ask the agent to explain each commit separately, then review the resulting smaller diffs.

What is the final rule for reviewing coding-agent output?
Use AI to make review broader and faster, then use deterministic checks and qualified humans to make the decision dependable. The merge gate should be: a clear specification, a focused and traceable diff, reproducible test and security evidence, verified AI findings, and a human who understands the code and accepts responsibility.
Frequently Asked Questions
Can an AI coding agent review its own code?
It can perform a useful first pass, but self-review has no independence and cannot satisfy a human approval policy. Treat the agent’s report as an input, then have a different qualified person verify the change and evidence.
Should I keep prompts and coding-agent logs for every pull request?
Follow your organization’s retention and confidentiality policy rather than keeping everything by default. Retain prompts, action logs, and tool versions when they explain a material decision, support regulated traceability, or will be needed for incident analysis; redact secrets and restricted data.
What if an AI reviewer and a human reviewer disagree?
The human should resolve the disagreement by checking the specification, running a focused experiment, and consulting authoritative technical or security documentation. Record the reasoning, and escalate to the domain owner when the disagreement concerns a high-impact risk.
Does adding an AI reviewer reduce the need for peer review?
No. AI can increase the number of issues surfaced, but it does not provide accountable judgment or organizational context. Peer review remains necessary for design trade-offs, operational risk, and changes that affect other teams or users.
