Review AI-generated code as if a human teammate submitted it, but assume neither the agent’s explanation nor a green test run proves the change is correct. A qualified human must understand the requested behavior, inspect the complete diff, run the repository’s normal checks, add independent tests for acceptance criteria and edge cases, and make the production approval decision. The same engineering, reliability, privacy, and security expectations apply whether an agent, a contractor, or an employee wrote the code.

This workflow keeps the useful speed of coding agents without outsourcing accountability. It also catches failures that an agent can accidentally hide: an unrelated configuration edit, a deleted regression test, a mock that never exercises the real path, an unsafe dependency, or a test that merely confirms the implementation’s assumptions.

What is the reviewer responsible for when an AI coding agent opens a pull request?

The reviewer is responsible for deciding whether the change satisfies the real task and is safe to merge. The agent can propose code, tests, a summary, and even a security explanation, but those outputs are evidence to examine—not approval. The UK Home Office engineering standard states that “AI-assisted outputs MUST be reviewed and approved by a human before reaching production” and that teams retain full accountability for AI-assisted code (Use AI – Engineering Guidance and Standards, updated 20 March 2026). That wording is a departmental standard, not a universal law, but it is a useful model for an explicit team policy.

Your review should answer four questions:

  • Does the patch implement the requested behavior, including error, boundary, and backwards-compatibility cases?
  • Did it change anything outside the authorized scope?
  • Do independent checks provide convincing evidence for the risk of this change?
  • Is there a clear human decision, with exceptions and follow-up work recorded?

Do not use generic claims about AI productivity or defect rates to decide how much to trust a particular patch. Review depth should follow the change’s consequences and evidence, not the authoring tool.

How do I restate the task before reading the implementation?

Translate the issue, ticket, or product request into observable acceptance criteria before you get anchored on the agent’s design. This prevents a plausible implementation from redefining the problem.

Write behavior, interfaces, and constraints

Record the inputs, outputs, side effects, supported clients, error semantics, performance or availability requirements, and security obligations. Note which public APIs, database schemas, messages, files, or deployment environments may change. If the request is ambiguous, resolve it with the owner before approval rather than letting the generated code choose silently.

Check the repository’s intended design

Read the relevant architecture notes, contribution guide, coding conventions, migration policy, and nearby implementations. Confirm naming, error handling, logging, feature-flag, and compatibility patterns. A patch can be functionally plausible and still violate a project convention that affects operations or future maintenance. GitHub’s guidance for reviewing AI-generated code emphasizes checking both functional behavior and fit with the project’s intent and conventions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Define the cases the implementation must not forget

  • Normal valid inputs and the expected successful result.
  • Empty, malformed, oversized, duplicated, stale, or out-of-order inputs.
  • Missing permissions, expired credentials, unavailable dependencies, and partial failures.
  • Retries, timeouts, cancellation, concurrency, and idempotency where applicable.
  • Existing data, older clients, downgrade or rollback behavior, and migration failure.
  • Privacy, tenant isolation, logging, and secret-handling requirements.

These criteria become the basis for tests that are independent of the agent’s explanation and for questions you ask when a line of code appears clever but unnecessary.

How do I inspect the complete AI-generated diff?

Start with the changed-file list, then read the full diff file by file. Never limit the review to the files mentioned in the pull-request summary or to the feature’s main source file.

Use a repeatable diff pass

  1. Confirm the base. Verify the pull request’s target branch and that the local checkout represents the exact revision being reviewed.
  2. Inventory every file. Classify source, tests, documentation, generated artifacts, configuration, CI workflows, deployment scripts, lockfiles, schema or migration files, and binary assets.
  3. Read surrounding code. Inspect callers, interfaces, error paths, data models, and existing tests, not just added lines. Ask what assumptions the new code makes about ordering, types, permissions, and lifecycle.
  4. Trace each requirement. For every acceptance criterion, identify the implementation path and a test or manual check that would fail if it were removed.
  5. Compare scope with authorization. Investigate unrelated formatting churn, renamed files, changed defaults, new telemetry, altered permissions, or edits to release configuration.
  6. Inspect removals as carefully as additions. A deleted test, relaxed assertion, disabled linter, or removed validation can be the most consequential line in the patch.

OWASP’s Secure Coding with AI Cheat Sheet warns that reviewers can become anchored on the requested feature and miss unrelated or sensitive modifications. Treat unexpected scope as a review blocker until the author explains it.

Files that deserve deliberate attention

  • Lockfiles and manifests: identify newly introduced, upgraded, or downgraded packages; check transitive effects, license policy, maintenance, and known vulnerabilities.
  • CI and release configuration: look for skipped jobs, weaker permissions, exposed variables, changed deployment targets, or steps that run untrusted code with elevated credentials.
  • Infrastructure and environment files: verify network exposure, storage access, retention, region, and production defaults.
  • Database and schema changes: check migration ordering, rollback or forward-fix plans, indexing, data backfill, and behavior while old and new application versions coexist.
  • Tests: compare assertions and fixtures with the previous version; a shorter test is not automatically a better test.
  • Generated or vendored files: confirm whether they should be committed and whether regeneration is reproducible.

How do I run tests independently of the coding agent?

Run the repository’s normal build, test, lint, type-check, and static-analysis commands yourself or in a trusted CI environment. A green suite establishes only that those checks passed on the exercised paths; it does not establish that the acceptance criteria were complete or that the tests are meaningful.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Establish a baseline

Where practical, run the same checks on the target branch and record existing warnings, flaky tests, coverage behavior, and known failures. Compare the proposed revision with that baseline. A new warning hidden among pre-existing output, a changed test count, or a disabled check may otherwise go unnoticed.

Use the project’s real execution paths

  1. Install dependencies from the reviewed lockfile in a clean, isolated environment.
  2. Compile or build with production-representative settings, not only an editor or development shortcut.
  3. Run unit and component tests, then integration tests that exercise real boundaries such as a database, queue, filesystem, or HTTP contract where those boundaries matter.
  4. Run end-to-end or smoke tests for user-critical workflows when the change crosses several services or interfaces.
  5. Run linting, type checking, formatting verification, and static analysis; investigate new warnings rather than treating them as cosmetic.
  6. Repeat relevant checks after a clean rebuild or fresh environment if caching could mask a missing dependency or generated file.

GitHub recommends tests and static analysis as early review checks, while OWASP SAMM’s guidance for AI-assisted development stresses that people must define what should be tested and verify that generated tests reflect the intended behavior.

Add tests from requirements, not from implementation

For every acceptance criterion, write or run at least one assertion that would fail if the behavior were absent. Add negative and boundary cases where the requirement makes them relevant. If the agent implemented a parser, for example, test malformed input, limits, and ambiguity rather than only the examples embedded in its code. If it changed authorization, test both an allowed request and each important denied role or tenant.

Prefer tests that observe externally meaningful behavior. A unit test that asserts an internal helper was called can complement, but should not replace, a test that verifies the resulting state, response, event, or side effect. Keep test data realistic enough to expose encoding, time-zone, transaction, and serialization errors.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How should I review tests written or edited by the agent?

Agent-authored tests are part of the change and require the same skepticism as production code. They can be useful, but they may also encode the generated implementation, omit failure paths, or make a defect appear fixed by weakening the test.

Look for weakened coverage

  • A failing test disappears without a documented requirement change.
  • An exact assertion becomes a broad truthiness, non-null, snapshot, or status-code check.
  • Expected exceptions are caught and ignored, or a test passes when the operation silently does nothing.
  • Mocks replace the behavior under test, so the test verifies a configured mock rather than a real integration.
  • Fixtures contain only happy-path values even though the feature handles untrusted or variable input.
  • The test asserts a private function call or generated data structure instead of the user-visible requirement.
  • A test reproduces the implementation’s algorithm, allowing the same mistaken assumption on both sides.

Design adversarial checks

Write at least a few tests the agent did not author. Vary input order, omit optional fields, use boundary lengths and numeric limits, repeat requests, interrupt dependencies, and exercise unauthorized identities. For stateful code, run operations in different sequences and verify recovery after a partial failure. For concurrent code, use controlled scheduling or repeated runs to expose races rather than relying on one timing-sensitive example.

Check test determinism and isolation

Confirm that tests clean up data, do not depend on another test’s order, use controlled clocks and randomness where necessary, and fail for the right reason. A test that passes only with a warm cache, a developer’s local secret, or a particular timezone is not reliable evidence. If a test is intentionally slow, document why and keep a focused faster check for ordinary pull requests.

Which security checks belong in an AI-assisted code review?

Choose security verification in proportion to the change, but do not treat AI-generated security explanations as a substitute for testing or skilled review. The Australian Cyber Security Centre’s September 2026 Guidelines for software development recommends combining peer and code review, positive and negative unit or integration tests, static application security testing (SAST), dynamic application security testing (DAST), and software composition analysis (SCA). It also says AI findings should supplement traditional testing and be validated by appropriately skilled personnel.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Review the security-sensitive behavior directly

  • Authentication: verify token validation, expiry, audience, issuer, session invalidation, and failure behavior.
  • Authorization: test least privilege, object-level access, tenant boundaries, administrative paths, and default-deny behavior.
  • Input and output handling: check validation, canonicalization, injection defenses, encoding, file paths, redirects, and content-type assumptions.
  • Secrets and personal data: search source, history, logs, fixtures, build output, and error messages for exposed credentials or sensitive records.
  • Dependencies and services: inspect package provenance, versions, licenses, maintenance, network calls, and permissions.
  • Failure and abuse paths: examine rate limits, resource exhaustion, retry storms, replay, deserialization, and denial-of-service behavior.

Apply layered verification

NIST IR 8397: Guidelines on Minimum Standards for Developer Verification of Software, published in October 2021, lists eleven broadly applicable techniques. Relevant examples include threat modeling, automated tests, static code scanning, hardcoded-secret detection, built-in protections, black-box and code-based structural tests, historical tests, fuzzing, web-application scanning where applicable, and checks of included libraries, packages, and services. Select the techniques that match the attack surface instead of running every tool indiscriminately.

For a public API, that may mean authorization tests, schema-fuzzing, SAST, dependency analysis, secret detection, and DAST against a safe environment. For a local refactor with no new input or privilege boundary, focused behavioral tests and static analysis may be sufficient. Record what you did not run and why when the omission could affect the approval decision.

How much review depth does an AI-generated change need?

There is no universal score that converts lines of generated code into an approval level. Use the following axes to choose evidence and reviewer expertise. This is a practical decision framework, not a mandated standard.

  1. Restate the request. Write the acceptance criteria, interfaces, constraints, edge cases, and security requirements in the pull request or review notes.
  2. Check the repository context. Read architecture and contribution guidance, identify the normal commands, and confirm the target branch and deployment model.
  3. Inspect the entire diff. Review every changed file, including removals, tests, lockfiles, CI, infrastructure, generated output, and documentation.
  4. Map requirements to behavior. Follow each criterion through the implementation and identify the observable evidence that proves it.
  5. Review tests as production code. Look for deleted cases, weakened assertions, over-mocking, implementation-coupled expectations, nondeterminism, and missing negative paths.
  6. Run the baseline and proposed checks. Build cleanly; run unit, integration, end-to-end, lint, type, static, and relevant security checks; compare with the target branch.
  7. Add independent verification. Create tests or manual checks from the requirements, especially boundaries, authorization failures, malformed input, retries, and partial outages.
  8. Recheck dependencies and sensitive paths. Validate package and service choices, licensing and maintenance policy, secrets, permissions, data handling, and error behavior.
  9. Review operational readiness. Confirm logs and metrics are useful without leaking data, migrations are safe, rollout and rollback are understood, and alerts cover likely failure modes.

What should I do when the agent’s summary conflicts with the code?

Trust the executable evidence and the complete diff over the summary. Ask the author to explain the discrepancy, but do not merge while an unreviewed behavior remains. A summary that claims “only a refactor” while the diff changes authorization, dependencies, or deployment settings is a scope and security issue, even if every visible feature test passes.

If a test was deleted or weakened

Restore the original test unless the requirement genuinely changed, document the reason for the change, and add a regression test for the corrected behavior. Require the patch to demonstrate the bug fix without removing protection for neighboring cases.

If checks pass only with mocks

Add a test at the smallest real boundary that matters, such as a database transaction, HTTP contract, queue interaction, or filesystem operation. Keep unit tests for fast feedback, but do not present them as evidence that an integration behavior works.

If a dependency appears without a clear need

Ask why the package is required, what built-in or existing project capability was rejected, and how its version, provenance, license, maintenance, and vulnerability status were assessed. Remove it or obtain an explicit architectural decision when the justification is weak.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If the patch is too broad to reason about

Split the work into smaller pull requests or require a design review before approving. Smaller scope improves traceability; it does not eliminate the need to inspect the remaining files.

How do I preserve traceability for AI-assisted changes?

Use the same commit and pull-request system as other engineering work, with a concise record that AI assistance was used and that a human reviewed, validated, and approved the result. The record should help a future maintainer reconstruct the decision without becoming a ceremonial declaration.

Include:

  • The requested behavior and the final scope reviewed.
  • The agent’s role, such as generating an initial implementation, tests, or refactoring suggestions.
  • Commands, environments, test suites, scans, and manual checks that were run.
  • Known failures, accepted risks, deferred work, and any checks not run.
  • The names or roles of human reviewers and the approval decision.

The Home Office standard provides examples of a commit marker and a pull-request note. Adapt the format to your repository policy, and avoid placing secrets, customer data, or unreviewed agent transcripts in the record. Traceability is valuable because it shows not that an agent was used, but how responsibility was exercised.

What does a strong approval look like?

A strong approval is a bounded engineering judgment: the reviewer can explain what changed, why it meets the acceptance criteria, which evidence was collected, what remains uncertain, and why the residual risk is acceptable for this release. It is not a statement that the agent is trustworthy or that every possible defect has been eliminated.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Before approving, confirm that the diff, tests, dependencies, security-sensitive behavior, and operational effects have each had an appropriate check. If any material question cannot be answered, keep the pull request open, narrow its scope, add evidence, or escalate to a reviewer with the needed domain expertise.

Frequently Asked Questions

Can an AI coding agent approve its own pull request?

No. An agent may run checks and summarize results, but approval must come from a human with authority to accept the change. Automated merge rules can enforce mechanical conditions; they do not replace a human decision about intent, scope, and residual risk.

Should I disclose AI assistance in a commit or pull request?

Follow your organization’s policy, and make the record useful rather than performative. Note what the agent contributed, what a person reviewed, which checks ran, and any exceptions so that later maintainers can understand the provenance and validation of the change.

How can I test an AI-generated database migration safely?

Run it against a production-like copy with representative volume, verify behavior while old and new application versions coexist, and test interruption, retry, rollback or forward-fix, indexes, constraints, and backfill performance. Keep a recovery procedure and confirm that sensitive data is not copied into development logs or fixtures.

What if the repository already has flaky or failing tests?

Separate baseline failures from regressions by running the target revision and the proposed revision in comparable environments. Document pre-existing failures, add focused checks for the new behavior, and do not describe an unchanged red suite as evidence that the AI-generated patch is safe.
