Review an AI scanner finding as a lead to investigate, not as proof of a vulnerability. Start from the annotated line, open the complete alert, and trace the data from its source to the final sink. Then verify the trust boundary, input controls, authorization decision, required identity, reachable execution path, and likely impact in your deployment. Only after that should you fix, escalate, or document the result. A clean scan is not a security sign-off: automated tools recognize known patterns, while a human reviewer must determine whether business logic and permissions make the path exploitable.

The workflow below gives every pull request a consistent triage path, adds specialist review where the blast radius is high, and prevents untested AI-generated patches from entering production.

What does an AI scanner finding actually tell you?

An AI-generated finding is a hypothesis supported by a rule, pattern, or data-flow model. It may identify a real defect, a harmless code path, or a problem that is already mitigated elsewhere. The highlighted line is only the point at which the tool recognized a pattern; it is not the complete security argument.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

OWASP summarizes the division of labor this way: automated scanners find known patterns, while human review finds flaws that require context. A reviewer therefore has to establish whether attacker-controlled data can reach a sensitive operation, under which identity, with which permissions, and with what business consequence. Treating the alert as automatically exploitable creates unnecessary rework; dismissing it without tracing the path can leave a serious authorization or logic flaw in production.

How should I classify a finding before investigating it?

Record enough context that another reviewer can reproduce your reasoning and audit the final decision. Do this before changing code or dismissing the alert.

  1. Identify the detector. Note the scanner and rule name, and whether the result came from CodeQL, an AI Scan pull-request detector, an AI-application detector, software-composition analysis, secret scanning, infrastructure-as-code analysis, or another check.
  2. Capture location and scope. Record the repository, pull request, file, changed lines, surrounding function, severity, and any CWE or category supplied by the tool.
  3. Separate code findings from AI-application findings. A conventional code alert may concern injection, authorization, or unsafe deserialization. An AI-application detector may instead flag indirect prompt injection or another agent-specific risk; the mitigations and reviewers can differ.
  4. Check whether the finding is advisory. GitHub documents its AI Scan as an AI-based pull-request detector. Its findings are advisory and cannot currently be used directly as ruleset requirements, so a passing or failing annotation does not by itself enforce your merge policy.

On GitHub, AI Scan findings appear with an AI indicator in the pull request’s Conversation and Files changed tabs. Use that indicator to locate the alert, but open the full details before deciding what it means.

How do I read the evidence and trace the complete path?

Open the alert’s data-flow explanation, then inspect code outside the highlighted line. Your goal is to connect an input an attacker can influence to the operation that could cause harm, while accounting for validation, authorization, configuration, and tests along the way.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Start at the source

Find the actual source of the value: an HTTP parameter, request body, header, uploaded file, message-queue payload, webhook, database record, environment variable, or tool result. Determine whether an untrusted user can set it directly, influence it indirectly, or reach it only after another service transforms it. If the value originates from a trusted configuration file or an administrator-only control, document that boundary rather than assuming the scanner’s generic rule applies.

Follow every transformation

Read callers, wrappers, serializers, parsers, and helper functions between the source and the highlighted line. Look for canonicalization, allowlists, escaping, schema validation, type checks, encoding changes, and length limits. A sanitizer may be effective for one sink but ineffective for another; record exactly which operation it protects and whether the same value is later decoded or concatenated.

Inspect the sink and its side effects

Identify what the destination does with the value: execute a query, render markup, invoke a shell or template, deserialize an object, write a file, call an external service, alter a permission, or send a model prompt. Read the sink’s implementation and configuration, not just its function name. Then list the side effects if the operation succeeds, including data disclosure, account changes, code execution, or a denial of service.

Verify authorization and trust boundaries

Check the identity established at the endpoint, the resource-level authorization decision, tenant or organization checks, and any elevation performed by a service account. A valid session does not prove that the caller may access the specific object. Trace whether a background job, webhook consumer, or internal service crosses a trust boundary without rechecking permissions.

Use tests and configuration as evidence

Search for unit, integration, and end-to-end tests that exercise the path. Inspect feature flags, deployment manifests, reverse-proxy rules, database permissions, and runtime settings. A test that covers only a happy path does not demonstrate that malicious input is rejected, but a negative test or an explicit policy check can materially change the assessment.

How can I decide whether the alert is exploitable in this deployment?

Exploitability depends on reachability, required privileges, deployed configuration, and compensating controls. Evaluate those conditions explicitly instead of relying on the scanner’s default severity.

  • Attacker control: Can an external user, lower-privileged account, compromised integration, or untrusted pull request supply the value?
  • Reachability: Is the vulnerable function called in production, or is it dead code, test-only code, behind a disabled feature flag, or restricted to an unreachable route?
  • Identity and permissions: What account executes the operation? Does it have access to other tenants, secrets, file systems, cloud APIs, or administrative actions?
  • Impact: If the path is abused, what confidentiality, integrity, availability, safety, or compliance consequence follows?
  • Compensating controls: Do a gateway, policy engine, database permission, sandbox, network rule, rate limit, or monitoring control prevent or limit exploitation?
  • Severity fit: Does the impact and likelihood in this deployment justify the tool’s severity, or should the team adjust it with a written rationale?

For an AI agent, add three questions: can untrusted content influence the agent’s instructions (indirect prompt injection), can the agent reveal sensitive information, and can it take actions beyond what the user intended (excessive agency)? Review retrieved documents, tool outputs, memory, and approval gates as possible attack inputs, not only the user prompt.

  • Authentication, session creation, token handling, password reset, or account recovery
  • Authorization, role mapping, tenant isolation, impersonation, or permission checks
  • Cryptography, key management, certificate validation, or security-sensitive randomness
  • Input parsing, deserialization, template processing, command execution, or file uploads
  • New public endpoints, webhooks, third-party integrations, or changes to CORS
  • Data-store access, query construction, migrations that alter permissions, or sensitive-data handling
  • Infrastructure and CI changes that affect public exposure, IAM scope, encryption, logging, actions or images, secrets, or privileged-token execution

Infrastructure and workflow edits deserve the same scrutiny as application code. Check for newly public services, widened IAM permissions, disabled encryption or audit logging, unpinned actions or container images, secrets placed in workflow variables, and privileged tokens exposed while running untrusted pull-request code.

How should I review AI-generated code in the pull request?

Apply at least the scrutiny you would give code from an unknown contributor, even when the change was suggested by an assistant or accepted from an automated patch. A human who understands the deployment must approve security-relevant code and be accountable for its maintainability.

Check whether the author can explain it

Ask the author to describe the data flow, authorization assumptions, failure behavior, and reason for each new dependency. Code that no responsible maintainer can explain should not merge until it is rewritten or fully understood.

Look for recurring generated-code weaknesses

  • Missing object-level authorization or checks performed only on the user interface
  • Validation applied to one endpoint but omitted from alternate callers
  • Weak, deprecated, or incorrectly configured cryptography
  • Queries built by string concatenation or unsafe interpolation
  • Unfamiliar, hallucinated, abandoned, or unnecessarily broad dependencies
  • Exception handling that leaks secrets or silently bypasses a security control
  • Edits to agent instruction files, repository hooks, or automation that change what future tools can execute

OWASP’s secure-coding guidance states that every AI-assisted change should be reviewed, approved, and attributable to a developer responsible for its security and maintainability. An AI review comment is evidence to consider, not an approval identity.

What is the practical pre-merge sequence?

Use the same sequence for a scanner alert, an AI-suggested fix, or a finding raised during manual review. Keep the final disposition attached to the pull request or issue so later maintainers can see why the decision was made.

  1. Classify the finding. Record the detector, rule, location, changed lines, severity, category or CWE, and whether it is a code, dependency, secret, infrastructure, or AI-application result.
  2. Read the evidence. Open the alert details and follow the data-flow explanation. Inspect surrounding code, callers, sanitizers, authorization checks, configuration, tests, and the destination reached by the data. Never decide from the highlighted line alone.
  3. Test reachability and impact. Establish whether attacker-controlled input reaches the sink, what identity and permissions are required, what data or action is exposed, and whether compensating controls apply. For agents, assess indirect prompt injection, sensitive-information disclosure, and excessive agency.
  4. Choose a documented outcome. Fix a valid issue; request specialist review for a security-sensitive or trust-boundary change; or dismiss and document a false positive or accepted risk. A dismissal should include the technical reason, named owner, and follow-up issue or review date when residual risk remains.
  5. Validate the fix. Review the complete patch for regressions, run normal unit, integration, and security tests, and inspect dependency and configuration changes. If an automated assistant proposes a patch, assess it, modify it when necessary, commit and push the tested version, then confirm that the alert closes after a re-scan.
  6. Apply the repository policy. Configure code-scanning checks, branch protection, required reviews, and severity thresholds that match your risk tolerance. Because GitHub AI Scan findings are advisory, use those repository controls when a finding must block merging.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Support on Ko-Fi

How do I validate an AI-generated fix before merging?

A patch is successful only when it removes the vulnerable path without creating a new one. Re-run the detector after the change, but treat a closed alert as one piece of evidence rather than a complete security test.

Review the whole diff

Check adjacent files, new dependencies, generated artifacts, migrations, feature flags, and configuration. Confirm that the fix is applied to every entry point and not only to the line named by the alert. Ensure error paths fail closed and that logging does not expose the input or secret the patch is meant to protect.

Exercise both safe and hostile cases

Add or run tests for malformed input, boundary lengths, alternate encodings, unauthorized identities, cross-tenant object IDs, expired sessions, and failure responses. For an agent, include hostile retrieved content and tool output that attempts to override instructions or obtain protected data.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Confirm operational behavior

Run the repository’s standard unit, integration, and security suites, then inspect dependency and infrastructure checks. Verify that monitoring still records rejected attempts, that rate limits and timeouts remain effective, and that deployment configuration does not re-enable the original exposure.

What are GitHub AI Scan’s current coverage and enforcement limits?

GitHub documents AI Scan as a pull-request detector rather than a full-repository scanner. It runs on pull requests, does not provide full-repository scans, and excludes pull requests from forks and Dependabot. Those boundaries mean a clean AI Scan result says nothing about untouched legacy code or excluded pull requests.

GitHub also documents AI applications scanning as a separate detector with public-preview coverage focused on indirect prompt injection. Availability and coverage can change, so teams should verify the current product documentation when designing controls. Neither detector replaces SAST, software-composition analysis, secret scanning, infrastructure checks, or human review.

How should I compare scanners or review workflows?

Choose a workflow based on the evidence and controls your repository needs, not on an AI label alone. The following criteria expose gaps that a feature checklist can hide.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Comparison area Questions to answer Why it affects review
Language and framework coverage Are the languages, frameworks, generated files, and build systems in this repository analyzed? A rule that cannot parse a framework’s routing or authorization model may miss the highest-risk path.
Pull-request and repository scope Does the tool inspect changed files only, the full repository, forks, and dependency updates? Scope determines what a clean result actually covers.
Reachability evidence Does the alert show sources, sanitizers, callers, and sinks, or only a pattern match? Data-flow context helps reviewers test exploitability instead of guessing.
False-positive workflow Can reviewers comment, link an issue, assign an owner, dismiss with a reason, and re-run after a fix? Traceable dispositions prevent repeated debates and silent risk acceptance.
Rule customization Can the team add custom rules or turn recurring review findings into checks? Organization-specific authorization and data-handling rules are rarely covered by defaults.
Adjacent security checks Are dependency, secret, IaC, container, and AI-agent risks covered or integrated? A code-only scanner leaves important attack surfaces unexamined.
Merge integration Can severity thresholds, required reviews, and branch protection enforce the policy? Advisory comments require separate repository controls if they must block a merge.
Auditability and cost Can you retain approvals, explanations, ownership, licensing, and operational effort? Security decisions need a durable record and a sustainable process.

What should reviewers check immediately before approving?

  • The finding’s source, sink, trust boundary, required identity, and production reachability are written down.
  • Authorization, validation, parsing, and error paths were inspected outside the highlighted line.
  • Severity reflects actual impact and compensating controls in this deployment.
  • A trained reviewer approved authentication, authorization, cryptography, parsing, uploads, public endpoints, integrations, data stores, CORS, or infrastructure changes when applicable.
  • AI-generated code and dependencies are understood by an accountable maintainer.
  • Agent changes were tested against indirect prompt injection, sensitive-information disclosure, and excessive agency where relevant.
  • The complete patch passed appropriate unit, integration, security, dependency, and configuration checks.
  • A valid issue was fixed, or a false positive or accepted risk has a reason, owner, and follow-up.
  • Branch protection and required checks enforce the repository’s actual merge policy rather than relying on an advisory annotation.

What is the safest rule for merging an AI scanner result?

Merge only after a human has established what the alert means in the deployed system, verified the complete fix, and recorded responsibility for the decision. Scanner output accelerates discovery; it does not replace contextual security review or the repository’s approval controls.

Frequently Asked Questions

Can a clean AI scan prove that a pull request is secure?

No. A clean result covers only the detector’s supported rules and scope, and it may exclude untouched code, forked pull requests, or dependency updates. Business-logic flaws, missing authorization, and deployment mistakes still require human review and other security checks.

Who should approve a pull request that changes authorization?

A maintainer who understands the affected data and a security champion or trained security reviewer should approve it when the change crosses a trust boundary. The approval should identify the assumptions tested, not merely acknowledge that an automated check passed.

What should I do when a scanner finding is a false positive?

Document why the source is unreachable, trusted, or protected by a specific control, then dismiss it through the review system with an owner and any follow-up issue or review date. Revisit the decision if the endpoint, configuration, identity model, or data flow changes.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How are AI-agent findings different from ordinary code alerts?

An agent can be manipulated through content it retrieves or receives from tools, not only through a direct user request. Reviewers must therefore test instruction boundaries, data exposure, and the limits on actions the agent may take, including whether human approval is required for high-impact operations.
