Red-team an AI model by testing it with realistic, adversarial prompts and workflows that could expose harmful, insecure, or policy-violating behavior before users encounter it. A useful assessment goes beyond trying clever jailbreaks: define what is in scope, map likely harms, test the complete application in a sandbox, record reproducible findings, mitigate them, and retest before release.

What does it mean to red-team an AI model?

Red teaming is a structured attempt to discover how an AI system can fail under adversarial or high-risk conditions. OpenAI describes it as using adversarial test cases to uncover unsafe, insecure, or policy-violating behavior before deployment. Unlike an ordinary capability evaluation, which may ask whether a system performs a task correctly, a red-team assessment asks how it behaves when instructions, inputs, context, or tools push it toward a harmful or unreliable outcome.

The target should usually be the system people will actually use, not just a bare model endpoint. That can include the model, system instructions, retrieval and memory, connected tools, safety filters, user interface, and downstream actions. A model may refuse a dangerous request in isolation yet fail when a harmful instruction is embedded in a retrieved document or when a tool can take an external action.

Red teaming is not a guarantee of safety. It is a way to find and document weaknesses, prioritize them by risk, and provide evidence for a deployment decision. A clean result means the tested cases did not reveal a failure under the conditions examined; it does not prove that no other failure exists.

How do I define the scope and authorization?

Write down what you are permitted to test and what the assessment must cover before sending adversarial inputs. This prevents tests from reaching systems or people outside the agreed boundary and makes results meaningful when the model or application changes.

Set the boundary

  • System: Identify the model, application, deployment environment, and interfaces under test.
  • Components: Include retrieval, memory, tools, filters, user interface, and downstream actions if they are part of the user experience.
  • Users and contexts: Specify relevant user groups, tasks, and deployment geography. Risks and expected behavior can depend on who uses the system and where.
  • Actions and data: List what the system can read, change, send, or execute, and identify sensitive information it might encounter.
  • Authorization: Test only systems and assets you own or are expressly authorized to assess. Define excluded targets and stop conditions.

Record the tested configuration

Record the model and application versions, system instructions or policy version, tool permissions, and safety configuration. Without this snapshot, a finding may not reproduce after a prompt, model, or permission change, and a test result can be mistakenly applied to a configuration it never examined.

Which harms and reliability failures should I test?

Start with risks that could plausibly affect your application and its users, then prioritize them by potential impact and likelihood. Do not treat a generic list as a substitute for examining what the system can do, what information it can access, and how its outputs are used.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Potential harm categories

  • Dangerous or illegal assistance: Does the system provide actionable help that could enable harm or unlawful activity?
  • Hate, harassment, and sexual content involving minors: Does it generate, endorse, or assist with disallowed abusive or exploitative content?
  • Self-harm: Does it encourage self-harm or respond in a way that increases danger?
  • Privacy leakage and data exfiltration: Can a user extract sensitive data from the model, retrieved context, memory, or tools?
  • Prompt injection: Can untrusted user input, a retrieved document, or a tool result override intended instructions or redirect the system?
  • Insecure output handling and excessive agency: Can generated content trigger unsafe downstream behavior, or can the system take actions beyond what the task requires?
  • Unreliable answers: Does it hallucinate, fabricate citations, follow instructions inconsistently, or express more certainty than the evidence supports?
  • Fairness and accessibility: Does behavior become harmful or materially worse for particular groups, languages, or accessibility needs?

NIST’s AI Risk Management Framework identifies trustworthy-AI characteristics including validity and reliability, safety, security and resilience, accountability and transparency, explainability and interpretability, privacy enhancement, and fairness with harmful bias managed. These dimensions can conflict or fail independently: a system that gives accurate answers can still expose private data, and a system that refuses harmful requests can still provide unreliable factual claims. Define thresholds appropriate to the application rather than assuming one passing result covers every dimension.

How do I build a useful red-team test matrix?

For every prioritized harm, vary the conditions under which it might occur. The goal is to test a family of failure modes rather than find one dramatic prompt that may not represent realistic use.

Vary the attack and context

Include direct and indirect requests, role-play, spelling changes, obfuscated or encoded text, conflicting instructions, multi-turn escalation, long context, multilingual prompts, and instructions embedded in retrieved documents or tool outputs. Include benign near-neighbors—requests that resemble a harmful case but should receive ordinary assistance—to see whether safeguards are both effective and appropriately scoped.

For example, if the system answers questions using retrieved documents, test both a direct attempt to override its instructions and a document that contains such an instruction. If it has tools, test whether tool outputs or user-provided content can redirect a tool call. Keep the examples appropriate to your authorization and safety controls; the point is to examine behavior without causing real-world side effects.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Include reliability checks

  • Factuality: Does the system invent facts or continue confidently when evidence is missing?
  • Calibration: Does the answer communicate uncertainty when the available context is incomplete or ambiguous?
  • Citation correctness: Do cited sources support the claims attributed to them, and are citations fabricated?
  • Refusal consistency: Does the system apply its stated boundary consistently across wording, language, and conversation turns?
  • Instruction following: Does it follow legitimate instructions without letting untrusted content take precedence?

For each test, define the expected behavior and pass/fail rule before running it. A refusal-only rubric can miss dangerous tool use or false claims; a factuality-only rubric can miss a harmful answer that happens to be factually correct.

How do I test the complete system safely?

Use a sandbox that reproduces the important parts of the real workflow while isolating credentials, data, and external actions. Test the model together with retrieval, memory, tools, filters, interface, and downstream systems where applicable; a model-only test cannot establish how the integrated product behaves.

  1. Prepare an isolated environment. Use test credentials and test data. Disable or safely simulate actions that could send messages, alter production records, spend money, or affect real users.
  2. Confirm access boundaries. Check that the test configuration has only the permissions needed for the assessment. Do not use sensitive production data unless the authorization and safeguards explicitly allow it.
  3. Exercise realistic workflows. Run prompts through the same retrieval, memory, tools, filters, and interface paths that matter to users, including the transitions between components.
  4. Validate outputs and actions. Inspect both what the model says and what the surrounding system does with it. An apparently harmless response can still be mishandled by an integration.
  5. Stop on unsafe side effects. If a test escapes the sandbox or produces an unexpected real-world action, stop the run, contain the effect, and follow the organization’s incident procedure.

Canadian Centre for Cyber Security and partner-agency guidance on agentic systems recommends sandboxing, capability elicitation, red teaming for loopholes and unintended behavior, monitoring, and output validation. Its guidance also says organizations should use agentic AI only for low-risk and non-sensitive tasks. Treat this as a caution about granting autonomous capabilities, not as proof that sandboxing alone makes an agent safe.

How should I measure and review the results?

Begin with manual review to understand what the system is doing, then automate repeatable checks and retain spot checks. Microsoft recommends measurement sets, documented outputs, clear frequency and severity metrics, iterative manual review, and automation with spot checks. It also calls for repeating measurements after mitigations.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Set rules before the test run

For each case, state what counts as a pass, a failure, or an inconclusive result. Define severity and frequency in terms that fit the system: for example, whether a harmful answer is merely present, provides actionable detail, reaches sensitive data, or causes a tool to act. Keep these categories stable across runs so results can be compared.

Review more than a single output

Some failures depend on repeated attempts or conversation history. Record how often a behavior occurs across repeats and preserve the exact prompt sequence, not just the final turn. Human reviewers should assess ambiguous results and record their rationale; an automated score without review can conceal a mistaken classification or miss the practical significance of an answer.

Separate a model failure from an application or integration failure. A model may generate an unsafe output, while a separate issue may arise because the application executed an unvalidated output, exposed data through retrieval, or granted a tool unnecessary permission. That distinction helps identify the control that addresses the actual cause.

What should a useful finding contain?

A finding should let an independent reviewer understand the risk and reproduce the behavior without guessing about the setup. Capture enough information to connect the observed output to the tested system version and conditions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Test ID, date, and UTC timezone
  • Tester and authorization
  • Model and application versions
  • System and developer instructions, tool permissions, and relevant safety configuration
  • Dataset or retrieved context involved in the test
  • Exact prompt sequence and raw output
  • Expected behavior and evaluator decision, including rationale
  • Harm category, affected population, severity, confidence, and frequency across repeats
  • Exploit preconditions and whether tools or sensitive data were reached
  • Mitigation owner, retest result, residual risk, and release decision

Preserve relevant context securely. A report may itself contain sensitive prompts, outputs, or data exposure details; limit access and retention according to the organization’s handling rules.

How do I prioritize and fix a red-team finding?

Prioritize findings by plausible impact and likelihood, considering the affected user, exploit preconditions, reproducibility, exposure of sensitive data, and whether the system can take consequential actions. A high-impact failure with straightforward reproduction generally deserves attention before a low-impact edge case, but context matters: even a less frequent failure can be serious if the consequences are severe.

Choose a control that addresses the cause

Use the least disruptive control that adequately reduces the risk. Depending on the failure, options include revising system policy or prompts, filtering retrieved content, validating outputs, applying content filters or rate limits, reducing tool permissions, using structured outputs, requiring human approval, and making actions reversible.

Do not assume that adding a stronger instruction is always sufficient. If an integration gives the model broad permissions, reducing those permissions may address the underlying exposure more directly. If retrieved content can steer the system, retrieval controls and treatment of untrusted content may matter alongside model instructions. Match the mitigation to the component where the failure originates.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Retest the original case and its neighbors

After a mitigation, rerun the exact failing test and related variants, including benign near-neighbors. Confirm that the original behavior changed, that the control works in the integrated workflow, and that the fix did not create a new failure such as blocking legitimate requests or breaking reliable tool use. Record the retest result and any residual risk rather than treating a code or prompt change as evidence of success.

How do I decide whether the system is ready to deploy?

Use documented release criteria tied to the application’s risks, not a general claim that the model is “safe.” The decision should account for the findings, their severity and reproducibility, the safeguards in place, and the consequences if a failure still occurs.

Before release, confirm that high-priority findings have an owner and disposition, required mitigations have been retested, and residual risks have been reviewed by the people responsible for the deployment. Keep a release record that identifies the tested versions and the decision made. A pass on one test matrix is evidence about that configuration and those cases, not a permanent certification.

How do I keep red teaming useful after launch?

Make testing part of the system lifecycle. Reassess after changes to the model, prompts, data, tools, or policies, and establish monitoring, user feedback channels, incident response, and rollback or shutdown procedures for production operation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

UK Department for Science, Innovation and Technology guidance published September 8, 2026 recommends independent red teaming under stress conditions, incident procedures, and tested bypass or deactivation plans. Independence can make it easier to expose assumptions missed by the development team; stress conditions can reveal failures that routine use does not. Ensure that anyone conducting tests still has clear authorization and a safe way to report findings.

For release and ongoing operations, decide who can pause or roll back the system, what signals trigger escalation, and how incidents are recorded. Test the bypass or deactivation plan rather than assuming it will work during an urgent event. Run regression tests on important prior findings whenever a material change could alter system behavior.

Which tools or frameworks can help?

Tools can generate or organize test cases and help inspect outputs, but they do not replace scope definition, domain expertise, or human judgment. OpenAI identifies Promptfoo as an open-source framework for generating adversarial test cases and inspecting target behavior. Microsoft also discusses safety evaluations and guardrails in Azure. Choose an approach based on coverage, realism, evaluator quality, measurement rigor, operational safety, and how well it fits release and monitoring processes.

Selection axis What to check
Coverage Does the approach address the harm categories and attack variations relevant to the application?
Realism Can it test the full application and its tools, or only a model endpoint?
Evaluator quality Are reviewers qualified for the subject matter, and is review independent enough to challenge assumptions?
Measurement rigor Can you use predefined rubrics, repeat tests consistently, automate suitable checks, and perform spot checks?
Operational safety Can tests run with sandboxing, least privilege, and rollback or shutdown controls?
Lifecycle integration Can findings feed into release gates, monitoring, and incident response?

Frameworks provide structure, not proof of safety. A tool’s ability to generate many prompts does not establish that the prompts represent your users, that severity is measured consistently, or that a finding has been fixed. Keep the test set and evaluation rules connected to the actual deployment context.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What does a complete red-team cycle look like?

A defensible cycle connects authorization, tests, findings, mitigation, and release operations rather than treating prompt attacks as a one-off exercise.

  1. Authorize and scope: document the target, boundaries, configurations, users, and actions in scope.
  2. Prioritize harms: identify application-specific risks and set impact and likelihood priorities.
  3. Design test variations: cover realistic prompt, context, language, tool, and reliability variants with pass/fail rules.
  4. Run safely: exercise the integrated system in an isolated environment with limited permissions.
  5. Review and record: manually inspect outputs, measure repeatability, and preserve evidence for reproduction.
  6. Mitigate and retest: apply controls for the actual cause and rerun the original and neighboring cases.
  7. Make and maintain the release decision: document residual risk, monitor operation, and retest after relevant changes.

Frequently Asked Questions

Can I red-team a model without a dedicated security team?

Yes, if you have authorization, a clearly bounded target, a safe test environment, and reviewers capable of judging the risks in your application. For high-impact systems or sensitive data, involve relevant security, privacy, legal, and domain specialists rather than treating a prompt checklist as a substitute for expertise.

How often should I repeat AI red-team tests?

Repeat relevant tests after changes that could affect behavior, including model, prompt, data, tool, or policy changes. The appropriate schedule between changes depends on the system’s risk and operational context; establish it as part of release and monitoring procedures.

Are jailbreak tests enough to establish that an AI system is safe?

No. Jailbreak attempts cover only part of the risk surface. An assessment also needs to examine reliability, privacy, tools and downstream actions, fairness, and the behavior of the integrated application.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Should every red-team finding block a release?

Not automatically. The release decision depends on the finding’s severity, likelihood, affected users, available safeguards, and residual risk; record who accepted or mitigated each material risk and why.
