Monitor an enterprise AI agent as a distributed, probabilistic system—not as a single API response. Instrument every run with OpenTelemetry-aligned traces and AI-native logs; capture model, retrieval, tool, identity, policy, latency, token, cost, and error data; continuously evaluate quality and safety on live traffic; and alert on drift as well as outages. The goal is to reconstruct what the agent did, why it did it, which permissions it used, and whether the task was completed correctly, while enforcing privacy, retention, residency, and access controls.
This guide presents an implementation model, the telemetry schema to collect, production metrics, evaluation and drift workflows, identity requirements, platform patterns, and a practical rollout sequence.
What makes production monitoring of AI agents different?
An agent can plan, call several tools, retrieve documents, ask another model for help, retry after an error, and stop with an answer that sounds plausible but is wrong. A conventional uptime check sees only whether an endpoint returned a response. Production monitoring must explain the entire decision path and judge the result against the user’s intended task.
Three properties drive the design:
- Distributed execution: one user request may cross models, retrieval systems, business APIs, policy services, and other agents.
- Probabilistic behavior: identical inputs can produce different plans, tool choices, or wording, so quality needs statistical baselines and representative evaluations.
- Delegated authority: the agent may act with permissions that affect customer records, payments, infrastructure, or confidential data. Logs must show both what happened and under whose authority.
Traditional logs, metrics, and traces remain necessary, but they need AI-specific fields and evaluation signals. Microsoft describes this as evolving logs, metrics, and traces to be AI-native because traditional observability alone does not expose agent quality, grounding, or policy behavior.

What should you capture for every agent run?
Define an event and retention contract before adding instrumentation. A run should be reconstructable from its root trace, while sensitive content is minimized, redacted, encrypted, and access-controlled according to your organization’s policy.
Stable identity and context
- Request identity context, including the initiating user, service principal, tenant, and authorization context.
- Agent identity and deployed agent version.
- Conversation ID and unique run ID.
- Timestamp with a consistent time standard.
- Workflow, environment, region, and release identifier.
For delegated actions, retain both the human identity that initiated the request and the agent identity that executed it. For autonomous actions, retain the unique agent identity. This dual-attribution model is documented by Google Cloud for agent telemetry and auditability.
Inputs, instructions, and outputs
- User input, with redaction or tokenization for sensitive fields.
- System and developer instructions, or a versioned reference to them when storing full text is prohibited.
- Model name, provider, deployment, parameter settings, and prompt or policy version.
- Final response and structured output, including validation status.
- Stop reason, refusal category, and any fallback model or template used.
Version references are important: without the prompt, policy, and model versions, a later investigator cannot distinguish a code regression from a changed instruction set.

Retrieval and knowledge provenance
- Retriever and index version.
- Query sent to the retriever and filters applied.
- Document, record, or chunk identifiers returned.
- Source locations, relevance scores when available, and access decisions.
- Context actually supplied to the model, subject to redaction.
Storing provenance lets an evaluator test groundedness and lets an operator determine whether a bad answer came from a stale document, an incorrect filter, or a generation error.
Tool calls and service dependencies
- Tool name and version.
- Arguments after sensitive-value masking.
- Permission or policy decision made before invocation.
- Start and end timestamps, timeout setting, retry count, and status.
- Tool output, error class, and downstream request identifier.
- Whether the call changed data, sent a message, or otherwise produced an external side effect.
Do not log secrets, access tokens, or unrestricted payloads by default. Store a redacted representation plus a secure reference when an investigation requires the original data.
Usage, timing, and failure data
- Input, output, and cached token counts.
- Per-call and per-run latency.
- Queue, model, retrieval, tool, and serialization time where measurable.
- Retries, timeouts, cancellation, rate-limit responses, and dependency errors.
- Estimated cost and the pricing or accounting version used to calculate it.
Keep calculation inputs with the cost record. Token prices and provider billing rules change, so a number without its accounting version is not auditable.
How do I trace multi-step and multi-agent workflows?
Represent one agent run as a distributed trace. Create a root span for the user-requested run and nested spans for every model generation, retrieval operation, tool invocation, policy check, and external service call. AWS describes a trace as the complete record of one unit of work, including a single agent run, while spans record timing, inputs, outputs, and attributes.

Use a consistent span hierarchy
- Run span: conversation ID, run ID, tenant, agent version, initiating identity, final status, and total duration.
- Planning or model spans: model deployment, prompt version, token counts, generation settings, and response metadata.
- Retrieval spans: query, index, filters, returned source IDs, and retrieval latency.
- Tool spans: tool identity, arguments hash or redacted arguments, authorization result, output status, retries, and side-effect classification.
- Child-agent spans: parent run ID, child agent identity and version, delegated objective, and returned result.
- Policy spans: guardrail checks, intervention decisions, and reason codes.
Propagate trace context across queues and asynchronous workers. When a tool or child agent cannot accept the original trace headers, include the run ID and parent span ID in a signed correlation record so the relationship remains verifiable.
What a useful trace should answer
- Which instruction caused the agent to choose a particular tool?
- Did retrieval provide an authoritative source, and did the model use it?
- Which permission check allowed or denied an action?
- How much time and spend came from each dependency?
- Where did a retry, timeout, or malformed result first appear?
- Was the final answer produced after a policy intervention or fallback?
Adopt OpenTelemetry GenAI semantic conventions where available so traces can move between collectors and analysis systems. Keep custom attributes namespaced and documented; changing field names between releases makes historical comparisons unreliable.
Which production metrics should I monitor?
Use separate dashboards for reliability, performance, spend, behavior, quality, and safety. Every metric should be filterable by agent version, model, tenant, workflow, tool, and region so an aggregate can be decomposed into the population that changed.
Reliability and performance
- Request volume and concurrency.
- Run success, failure, cancellation, and abandonment rates.
- Latency percentiles for complete runs and for each span type.
- Timeout, retry, rate-limit, and dependency-error rates.
- Queue time, model time, retrieval time, and tool time.
Track distributions rather than only averages. A stable mean can hide a severe tail-latency regression affecting a small but important group of customers.
The Tool Desk
Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →Outbyte PC Repair FREEClear out junk files and repair common Windows errorsFree Scan →Tokens, cost, and capacity
- Input, output, cached, and total tokens per run.
- Estimated cost per run, task, tenant, and completed outcome.
- Token growth caused by long conversations, retrieval context, or repeated tool results.
- Model fallback frequency and the capacity consumed by retries.
Cost per successful task is more actionable than cost per request because failed retries and abandoned runs still consume resources.
Tool and workflow behavior
- Calls per run, tool-selection frequency, and unusual tool sequences.
- Invalid arguments, schema-validation failures, and authorization denials.
- Side-effecting calls, compensating actions, and rollback outcomes.
- Child-agent fan-out, depth, and circular-call detection.
Baseline normal sequences for each workflow. A sudden increase in a rarely used administrative tool can indicate a prompt-injection attempt, an orchestration bug, or a permissions mistake even when the final response is successful.
How can I measure agent quality and safety continuously?
Operational health does not prove that an agent solved the user’s problem. Add evaluators that score live traces and compare them with offline datasets before releasing a new model, prompt, retrieval configuration, tool, or policy.

Quality dimensions
- Task adherence: whether the response followed the required workflow and constraints.
- Task completion: whether the requested business outcome was actually achieved, not merely described.
- Groundedness: whether claims are supported by the retrieved or approved sources.
- Intent resolution: whether the agent correctly understood the user’s goal and asked for clarification when necessary.
- Tool-call accuracy: whether it selected the right tool, supplied valid arguments, and interpreted the result correctly.
Combine automated scores with sampled human review and deterministic business checks. A model-based evaluator can judge relevance, but a database assertion may be the only reliable test that an order, ticket, or configuration change actually occurred.

Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minuteSafety and policy dimensions
- Policy compliance and prohibited-action detection.
- Sensitive-data exposure and unsafe handling of personal information.
- Indirect prompt-injection indicators in retrieved content or tool output.
- Data-exfiltration patterns, such as an unusual transfer from a protected system.
- Task misalignment, including actions outside the user’s stated objective.
- Content-risk categories relevant to the deployment.
Microsoft Foundry documents continuous production evaluation and runtime intervention points for these agent-specific risks. Interventions should be explicit events in the trace, with the rule, action, and resulting user-visible behavior recorded.
Build an evaluation dataset from reality
- Sample representative successful and failed production traces, removing or masking sensitive content.
- Label the intended task, expected constraints, acceptable tool sequence, and completion criteria.
- Include edge cases, refusals, ambiguous requests, policy blocks, and dependency failures.
- Version the dataset and evaluator definitions together.
- Run the set offline for every material change, then compare scores and failure categories with live evaluations.
A production evaluator should emit a score, confidence or rationale where supported, evaluator version, and trace reference. Alert on both average degradation and a newly appearing failure category.
How do I detect quality drift and set alerts?
Establish a baseline during a period that represents normal traffic. Record latency, cost, token volume, tool sequences, error classes, evaluator scores, refusal rates, and policy interventions. Compare later windows with that baseline by agent version, model, tenant, and workflow rather than relying on one global threshold.
Alert types
- Threshold alerts: a metric crosses a defined limit, such as a timeout or policy-violation rate.
- Distribution alerts: the shape of latency, token use, tool calls, or evaluator scores changes materially.
- Sequence alerts: a new or abnormal order of tools appears.
- Change-point alerts: behavior shifts immediately after a deployment, prompt, model, retrieval, or policy change.
- Security alerts: injection indicators, data-exfiltration patterns, privilege failures, or repeated blocked actions.
Attach run IDs and trace links to every alert. The on-call engineer should be able to open a representative failing trace, compare it with a healthy baseline, and see the release and policy versions involved.
Set release and incident thresholds separately
A release gate can require an evaluator score or error rate to remain within an approved range on an offline dataset. An incident threshold should be more conservative for live traffic and can trigger throttling, human approval, tool disablement, or rollback. Keep the two policies distinct so a small statistical change does not automatically stop a production workflow, while a safety violation still receives immediate intervention.
How do privacy, retention, and audit requirements change the design?
Agent telemetry can contain personal information, confidential documents, credentials accidentally pasted by users, and the full arguments for sensitive operations. Treat it as a governed data product rather than an unrestricted debug log.

Minimum controls
- Define which fields are required for operations, security, audit, and evaluation.
- Redact, hash, tokenize, or summarize content before it leaves the execution boundary.
- Encrypt data in transit and at rest, and separate encryption keys by environment or tenant where required.
- Use role-based access with least privilege; restrict raw prompts and tool payloads more tightly than aggregate metrics.
- Set retention periods by purpose, with deletion and legal-hold procedures.
- Document residency requirements and keep telemetry in approved regions.
- Record access to sensitive traces and review it.
When a raw payload is necessary for an investigation, use a short-lived, audited access path instead of broadening routine dashboard permissions. Data contracts should specify field owners, allowed values, redaction behavior, retention, and schema-change policy.
How do I implement enterprise agent monitoring step by step?
- Map the execution graph. List every model, retriever, tool, queue, policy service, child agent, and external dependency in each workflow.
- Define identifiers. Standardize agent, version, tenant, conversation, run, trace, span, model, tool, and release IDs.
- Write the telemetry contract. Specify required fields, sensitive fields, redaction rules, retention, residency, access roles, and ownership.
- Add OpenTelemetry instrumentation. Emit a root run trace and nested spans for generation, retrieval, tools, policies, and dependencies. Propagate context through asynchronous boundaries.
- Instrument identity and authorization. Record initiating human identity, agent identity, delegated permissions, policy decisions, and side-effect classification.
- Build the first dashboards. Start with run volume, success and error rates, latency percentiles, token and cost trends, tool failures, evaluator scores, and policy interventions.
- Create an offline evaluation set. Use representative tasks and sanitized production failures; version expected outcomes and evaluator definitions.
- Attach online evaluators. Score live traces for adherence, completion, groundedness, intent, tool accuracy, safety, and sensitive-data exposure.
- Establish baselines and alerts. Configure threshold, distribution, sequence, deployment-change, and security alerts with trace-linked notifications.
- Integrate the incident workflow. Give responders trace search, release comparison, evaluator details, audit records, and controls to pause tools, require approval, or roll back.
- Re-test every material change. Run offline evaluations and a controlled live comparison after prompt, model, retrieval, tool, policy, or orchestration changes.
What do major observability platform patterns provide?
Official documentation describes several viable patterns. Capabilities can change, so confirm current service behavior, region, retention, and pricing before procurement.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problems| Platform pattern | Documented capabilities |
|---|---|
| Microsoft Foundry control plane | End-to-end tracing, continuous evaluation on production traffic, agent-specific evaluators, thresholds and alerts, and runtime controls for personal-data risks, indirect prompt injection, prohibited actions, task misalignment, and content risks. |
| Amazon CloudWatch Omni | OpenTelemetry traces, sessions, topology, built-in or custom evaluators, datasets, experiments, and aggregate production monitoring across Bedrock AgentCore and other compute environments. |
| Google Cloud Gemini Enterprise Agent Platform | Online monitors for live traces with predefined or custom metrics; telemetry for agent identity, conversation ID, inputs, outputs, system instructions, and tool definitions; results in Cloud Logging and Cloud Monitoring. |
| Grafana Cloud Agent Observability | Conversation search, trace correlation, generation and tool telemetry, token and cost tracking, agent-version comparisons, online evaluations, guards, and offline experiments. |
| OpenSearch AI observability | OpenTelemetry and GenAI semantic conventions, automatic instrumentation for major model and agent frameworks, hierarchical traces, and production dashboards. |
These are patterns rather than interchangeable feature checklists. A platform may be strongest for a particular cloud, framework, identity system, or existing operations workflow.
How should I choose an AI agent observability platform?
Score candidates against the execution and governance requirements you established, not only the appearance of a dashboard.
- Portability: Can traces use OpenTelemetry and GenAI semantic conventions, and can you export them?
- Trace depth: Does one view connect models, retrieval, tools, child agents, queues, and policy decisions?
- Evaluation: Are online and offline evaluators available, versioned, and tied to release gates?
- Drift and alerting: Can the system detect metric, distribution, sequence, and quality changes?
- Runtime controls: Can it intervene on unsafe content, injection, prohibited actions, or misalignment?
- Identity and audit: Can it preserve human-plus-agent attribution and integrate with policy logs?
- Data governance: Are redaction, retention, residency, encryption, and role-based access configurable?
- Operations: Does it support searchable conversations, incident links, dashboards, and your on-call process?
- Coverage: Does it instrument the models, agent frameworks, and tools you actually run?
- Cost attribution: Can spend be assigned to runs, tasks, tenants, models, and successful outcomes?
What commonly fails, and how do I fix it?
Only endpoint metrics are collected
Symptom: uptime is green, but users report wrong answers or unsafe actions. Fix: add nested spans, retrieval provenance, tool arguments and results, evaluator scores, and policy events; then sample complete traces during incidents.
Logs contain prompts but not versions
Symptom: an old failure cannot be reproduced after a deployment. Fix: record immutable model, prompt, policy, retriever, tool, and agent-version identifiers on every run.
Tool calls are visible but authorization is not
Symptom: investigators see an external side effect but cannot determine why it was allowed. Fix: emit authorization input, decision, policy version, actor identities, and side-effect classification as linked spans.

Quality scores look stable while users lose trust
Symptom: an aggregate evaluator hides failures in one tenant, workflow, language, or model version. Fix: segment metrics, maintain representative slices, add human review, and alert on new failure categories as well as averages.
Telemetry creates a new data-exposure risk
Symptom: raw prompts and tool payloads are copied into broad-access logs. Fix: minimize fields, redact before export, separate raw content from metrics, restrict access, and enforce retention and residency contracts.
What should the ongoing operating review include?
Review monitoring as part of each release and incident cycle. Confirm that traces still cover new tools and agents, schemas remain compatible, evaluators match the current task definition, and dashboards are segmented by the dimensions operators use. Sample successful as well as failed runs to detect silent quality degradation. Reconcile estimated spend with provider billing or internal accounting, and periodically test that retention deletion, access logging, and emergency intervention controls work as designed.

Frequently Asked Questions
Can OpenTelemetry monitor agents built with different frameworks?
Yes, OpenTelemetry provides a vendor-neutral foundation, while GenAI semantic conventions give common names to model and agent attributes. Framework-specific instrumentation or custom spans may still be needed for planners, tools, and child agents that are not covered automatically.
How much of a user conversation should be stored in telemetry?
Store the minimum content needed for operations, security, audit, and evaluation. Prefer redacted text, hashes, structured labels, or secure references; keep unrestricted raw conversations in a separately governed store with narrower access and a defined retention period.
What is the fastest way to investigate a bad agent action?
Search by run ID or conversation ID, open the root trace, and follow the first abnormal span. Verify the initiating identity, policy decision, retrieved sources, tool arguments, downstream result, model and prompt versions, and any retry or fallback before deciding whether to disable a tool or roll back.
Should every production trace be evaluated by another language model?
No. Use deterministic checks for schemas, permissions, transaction outcomes, and required fields, and reserve model-based evaluators for qualities such as groundedness or adherence. Sample human reviews to calibrate evaluators and detect systematic scoring errors.
