Monitor an AI agent by tracing its run and meaningful steps at workflow boundaries, exporting telemetry asynchronously, and capturing only the data operators need. OpenTelemetry-compatible spans can show model calls, retrieval, memory, tool use, policy checks, and downstream services while sampling, redaction, retention limits, and least-privilege access reduce operational and privacy risk. Pair those traces with quality and safety evaluations, behavioral baselines, and a human approval path for irreversible actions. This approach preserves the agent’s normal execution path while making latency, errors, decisions, tool activity, drift, and misuse visible.
Why are ordinary uptime and error dashboards not enough for AI agents?
Traditional service monitoring answers whether a process is reachable and whether requests fail. An agent can be available, return HTTP 200, and still choose an unsafe tool, follow an ungrounded answer, loop through unnecessary steps, or drift from the user’s approved goal. Microsoft Learn summarizes the problem directly: “Traditional observability practices no longer suffice,” and “Uptime and error rates are not good indicators of quality and reliability in AI systems.”
An agent is a probabilistic, distributed workflow. Its result depends on model sampling, retrieved context, memory state, tool responses, policy gates, permissions, and external services. Monitoring therefore has to connect operational telemetry with workflow behavior and outcome quality rather than treating the model call as a single opaque request.
- Operational signals: latency, throughput, errors, retries, timeouts, token use, and dependency timing.
- Workflow signals: number and sequence of tool calls, retrieval provenance, memory reads and writes, loop indicators, and whether each action succeeded.
- Quality signals: groundedness, factuality, task completion, tool-use correctness, and human-review outcomes.
- Safety signals: policy decisions, harmful-content indicators, suspicious destinations, privilege changes, prompt-injection indicators, and possible data exfiltration.
The result is an evidence trail that explains not only that a run went wrong, but where and why it went wrong.

What should one agent trace contain?
Create one trace for each user request, scheduled job, or other top-level agent run. Give every meaningful operation a child span so an operator can reconstruct the execution path without recording unrestricted hidden reasoning.
| Trace area | Capture | Why it matters |
|---|---|---|
| Run identity | Trace ID, run or session ID, start and end timestamps, status, tenant or workload classification | Joins events from a single execution and supports incident reconstruction. |
| Model invocation | Model and version, request status, latency, token counts, retry count, and a reference to input/output payloads when permitted | Shows model-specific latency, cost drivers, failures, and release changes. |
| Planning or decision step | Step type, selected action, policy result, confidence or evaluator fields your application already exposes, and parent span | Reveals how an agent moved from an objective to an action without requiring unrestricted chain-of-thought storage. |
| Retrieval | Retriever name and version, query identifier, source or document identifiers, ranking metadata, and latency | Allows groundedness checks and identifies stale, missing, or inappropriate sources. |
| Memory | Read/write operation, memory store, record identifiers or hashes, authorization result, and size or token metadata | Shows whether persistent context influenced a decision and whether writes were allowed. |
| Tool call | Tool name and version, sanitized arguments or an argument reference, permissions, destination, start/end time, result status, and error class | Supports tool-use correctness, security review, replay of safe metadata, and anomaly detection. |
| Policy and approval | Policy or rule identifier, decision, approver or gate status, timestamp, and reason code | Proves which control allowed, blocked, or paused an action. |
| External dependency | Service name, operation, status, latency, retry, and correlation ID | Separates agent behavior from failures in APIs, databases, queues, or other services. |
| Outcome | Completion state, user-visible result reference, evaluator scores, human-review result, and follow-up action | Connects technical execution to quality and safety outcomes. |
Keep a stable schema for these fields and version it as the workflow changes. OpenTelemetry semantic conventions are useful because they give different frameworks and backends a common shape while agent observability practices continue to evolve.
How do I instrument an agent without changing its workflow?
Instrument boundaries around existing operations rather than rewriting the planner or inserting blocking diagnostics into every prompt. The instrumentation should create spans, attach metadata, and hand records to an exporter; the agent should continue using the same model, tools, permissions, and control flow.
Quick wins for a faster PC:
Scan for outdated or missing drivers - takes under a minuteDriver Scan →Repair Windows errors before they cause bigger problemsFix Now →1. Map the execution boundaries
Draw the path of a normal run: request entry, planner or decision loop, model calls, retrieval, memory, tools, policy checks, queues, and downstream services. Mark asynchronous boundaries explicitly so a trace can follow work that continues after the original request handler returns.
2. Add a root trace and child spans
Create a root span for each run and propagate its trace context into child operations. Add spans around every model invocation, planner step, retrieval operation, memory read or write, tool call, policy check, and external dependency. Google Cloud describes this telemetry as the way to inspect otherwise nondeterministic decisions and tool selections; AWS CloudWatch Omni represents agent activity with OpenTelemetry traces and sessions for aggregate production views.
3. Export out of band
Send records to a collector or telemetry pipeline asynchronously where your reliability design permits it. Keep the application path independent of a monitoring backend: a collector outage should not cause an agent action to fail. Define bounded queues, an explicit drop policy, and a health signal for telemetry loss so operators know when visibility is incomplete.
4. Start with metadata
Begin with run ID, timestamps, model name, status, latency, token counts, tool name, and outcome. Add complete prompts, responses, or tool arguments only for approved workflows, targeted incidents, or evaluation runs. This gives operators useful timing and behavior data before sensitive payloads enter a telemetry system.
5. Make capture switchable
Use feature flags or collector policies for sampling rate, payload capture, redaction, and retention. A policy can increase capture for a failing release, a selected tenant, or a high-risk tool while leaving routine traffic at a lower level. Record which policy was active so later analysis does not confuse a sampling change with a behavior change.

Should I use OpenTelemetry for agent tracing?
OpenTelemetry is a strong default when you need portable traces across frameworks, clouds, or backends. Its semantic conventions reduce lock-in and make it easier to combine model, tool, retrieval, and service spans, but you still have to design the schema, privacy controls, dashboards, and evaluation pipeline.
| Decision axis | OpenTelemetry-first stack | Integrated cloud observability suite |
|---|---|---|
| Portability | Strong: common conventions and multiple backend choices. | Depends on export and integration support. |
| Setup effort | More collector, schema, dashboard, and evaluator assembly. | Faster path to managed dashboards, traces, evaluations, and fleet views. |
| Workflow coverage | Determined by the instrumentation libraries you select and maintain. | Often includes framework or platform integrations. |
| Privacy controls | Implemented in collectors and backend policies, plus application redaction. | Uses provider controls plus application-level redaction. |
| Evaluation | Add an evaluator and dataset pipeline that consume trace data. | Often available in the same monitoring surface. |
| Best fit | Teams avoiding lock-in or spanning clouds. | Teams prioritizing integrated operations and managed services. |
OpenTelemetry guidance published on 6 March 2025 describes agent telemetry as a feedback loop for continuous evaluation and improvement. Choose the integrated suite when its controls and integrations meet your requirements; choose an OpenTelemetry-first design when portability, independent backend choice, or multi-cloud operation is more important. In either case, keep application-level redaction and authorization under your control.
How can I keep telemetry from slowing an agent down?
There is no universal monitoring-overhead percentage: the official guidance reviewed for this topic does not publish a comparable benchmark for latency increase, cost per trace, or capture overhead. Measure your own workflow, but design for low disruption from the outset.
Windows Errors? Fix Them Before They Spread
Repair common Windows errors and clear accumulated junk for a smoother, more stable PC - no reinstall needed.Free scan · no reinstallOutdated Drivers Are Slowing You Down
One free scan finds every outdated or missing driver and matches the right update for your exact hardware.Free scan · exact hardware match- Prefer metadata first: IDs, timestamps, statuses, token counts, and references are smaller and safer than full payloads.
- Sample deliberately: retain a representative baseline, capture all failures or policy blocks where feasible, and temporarily increase sampling for an incident or release comparison.
- Export asynchronously: decouple collector and backend availability from the agent’s critical path, with bounded buffering and an observable drop policy.
- Filter at the collector: remove unnecessary fields before they cross a trust boundary or enter long-term storage.
- Use trace links instead of duplication: reference a payload or evaluation record rather than copying the same large text into every span.
- Measure two paths: compare end-to-end latency and completion outcomes with telemetry enabled and with the same workload under a controlled baseline.
Do not optimize away the spans that explain a safety decision or a destructive tool call merely because routine model spans are noisy. Reduce volume by sampling and field selection, not by removing the evidence needed to investigate high-impact actions.
What should I log from prompts, responses, and decisions?
Log enough to explain an outcome, evaluate quality, and investigate security events, but do not treat the telemetry store as a second unrestricted transcript database. Microsoft recommends data contracts that balance forensic reconstruction with privacy, residency, minimization, retention, and compliance.
Use a data contract
For each field, document its purpose, sensitivity class, owner, allowed destinations, retention period, and whether it is required, sampled, or incident-only. Include a schema version in every trace so an evaluator can interpret records produced by earlier agent releases.
Redact before export
Remove credentials, access tokens, personal information, secrets, and sensitive tool arguments before telemetry leaves the application boundary. Use structured redaction for known fields and a review process for new tools; a generic text filter should not be the only defense for a database or payment argument.
Record decisions without unrestricted chain of thought
Store the policy identifier, decision, reason code, selected tool, permissions, and relevant provenance. A concise decision record is generally more useful for audit and evaluation than retaining unrestricted internal reasoning text, which can contain personal or confidential information and is difficult to govern consistently.

Protect storage and access
Encrypt telemetry in transit and at rest. Separate operator access from application permissions, use least privilege, and log access to sensitive traces. Set retention and data-residency rules by data class and incident need rather than keeping every payload indefinitely.
The Australian Office of the Information Commissioner notes that privacy obligations apply both to personal information entered into AI systems and to personal information in generated outputs. Tell users when AI is being used and provide human review when outputs affect people.
How should operational, quality, and safety monitoring work together?
Keep the signal families distinct in dashboards and alert policies, then join them through the trace and run IDs. A low-latency run is not automatically a good run, and a correct answer can still violate a permission or privacy policy.

Operational dashboard
Track latency by operation and end-to-end run, throughput, error and timeout rates, retries, token counts, queue time, and dependency latency. Break down these measures by model version, tool, tenant or workload class, and release so a regression has a plausible comparison group.
Workflow dashboard
Show tool-call volume and success rate, common tool sequences, retrieval-source coverage, memory access patterns, loop indicators, and incomplete runs. An unusual increase in calls or a new destination may be a safety issue even when every individual API request succeeds.
Quality dashboard
Run evaluators for groundedness, factuality, task completion, tool-use correctness, and other task-specific criteria. Keep fixed datasets for release comparisons and record evaluator version, dataset version, and threshold with each result. AWS documentation describes a loop that combines traces, sessions, evaluators, datasets, experiments, and production monitoring.
Safety dashboard
Monitor policy decisions, harmful-content indicators, privilege changes, access to sensitive stores, suspicious destinations, suspected prompt injection, and data-exfiltration patterns. Include human-review outcomes so false positives and misses can improve the policy and evaluator rather than silently accumulating.

Useful deviation signals
- Unusual tool volume, repeated failures, or a loop that exceeds the normal step pattern.
- A new external destination, data store, or permission not present in the approved workflow.
- Privilege changes, access outside the task’s data class, or a policy result that conflicts with the requested action.
- Goal drift, such as an action sequence that no longer matches the approved intent.
- Prompt-injection indicators, retrieval from an unexpected source, or patterns consistent with data exfiltration.
- A sustained change in groundedness, factuality, completion rate, or human-review outcomes after a model, prompt, tool, or data release.
The Canadian Centre for Cyber Security recommends behavioral baselines, anomaly detection, monitoring of internal processes, and a pause or alert mechanism when intent and behavior diverge. Treat an alert as a control signal: route the run to review, restrict the next tool, or pause execution according to the risk level.
When should a human approve or pause an agent?
Require a policy gate or explicit approval before actions that are irreversible, financial, privacy-sensitive, security-sensitive, or capable of changing permissions or external systems. The gate should evaluate the intended action and its arguments, not merely whether the model produced a plausible explanation.
- Create an approval span containing the proposed tool, sanitized arguments or a secure reference, target, requested permissions, and policy result.
- Pause execution before the tool runs; do not let a timeout silently become approval.
- Show the reviewer the relevant provenance, prior actions, and expected impact without exposing unnecessary personal data.
- Record approve, reject, modify, or expire decisions with the reviewer identity and timestamp.
- Resume with a new child span that links the decision to the resulting tool outcome.
Maintain time-stamped records of what the agent did, accessed, and changed. The Canadian federal guidance calls this auditability and warns that automation drift can move behavior away from approved intent over time.

What is a practical rollout plan?
Roll out in stages so visibility improves without turning every production run into a full-payload experiment.
- Map the workflow: list run, model, planning, retrieval, memory, tool, policy, queue, and external-service boundaries, including high-risk actions.
- Define the contract: choose semantic field names, sensitivity classes, retention, residency, access roles, and a schema version.
- Add minimal traces: propagate trace IDs and capture metadata for timing, status, model, tokens, tools, and outcomes.
- Configure protection first: enable sampling, redaction, encryption, retention, and access controls before payload capture.
- Build operational views: dashboard latency, errors, retries, tokens, tool activity, dependency health, and run completion.
- Add evaluators: score groundedness, factuality, safety, task completion, and tool correctness on fixed datasets; compare releases with the evaluator and dataset versions recorded.
- Establish baselines: observe normal distributions for steps, tools, permissions, tokens, and outcomes before enabling strict anomaly alerts.
- Protect high-risk actions: add pause or approval gates and verify that denial, timeout, and telemetry-loss paths fail safely.
- Review incidents: use traces and human outcomes to tune sampling, redaction, policies, evaluators, and the workflow as its environment changes.
What commonly goes wrong, and how can I fix it?
Only model calls are visible
Symptom: a dashboard shows prompt latency but cannot explain a wrong tool choice. Fix: add child spans for planning, retrieval, memory, policy, tools, and downstream services, then propagate the same trace context across those boundaries.
Every prompt and response is stored
Symptom: telemetry becomes a high-risk copy of application data. Fix: switch to metadata and references, redact before export, restrict payload capture to approved incidents or evaluations, and enforce retention by data class.
Alerts fire before a baseline exists
Symptom: operators ignore alerts because normal variation looks like an attack. Fix: observe distributions for tools, permissions, tokens, latency, and outcomes, then alert on sustained or high-impact deviations.

Quality is judged by success codes
Symptom: technically successful runs produce ungrounded or unsafe answers. Fix: add groundedness, factuality, safety, task, and tool-use evaluators, and compare releases on fixed datasets with human-review outcomes.
How do I know the monitoring design is working?
You should be able to answer four questions for a selected run: what the agent was asked to do, what it accessed and decided, what it changed externally, and whether the result met quality and safety requirements. Test those answers with a controlled incident exercise, a privacy review, and a release comparison rather than relying on dashboard appearance.
- Can an operator find the complete run from one trace or session identifier?
- Can the team distinguish model, tool, retrieval, memory, policy, and dependency latency?
- Can an evaluator reproduce a quality or safety score from the permitted evidence?
- Can a reviewer stop a high-risk action before execution and prove what happened afterward?
- Can the organization delete, restrict, or locate telemetry according to retention and residency rules?
- Can the system show when sampling, redaction, collector failure, or schema changes limit the evidence?
A design that meets these checks gives engineers useful diagnostics without forcing the agent to wait for a dashboard, and gives security and compliance teams a defensible record of behavior.
Frequently Asked Questions
Does agent tracing require storing the model’s full chain of thought?
No. Store concise decision and policy records, selected tools, permissions, provenance, outcomes, and evaluator results. Full prompts, responses, or internal reasoning should be restricted to approved cases because they can contain personal, confidential, or security-sensitive information.
How can I compare two agent releases fairly?
Keep the task dataset, evaluator versions, sampling policy, and scoring thresholds fixed for the comparison, and record the model, prompt, tool, and retrieval versions in each run. Compare quality and safety outcomes alongside latency, token use, retries, and tool behavior rather than using a single score.
What happens when telemetry is missing during an incident?
Mark the run as having incomplete evidence and preserve the telemetry-loss signal, collector status, and active sampling policy. Do not infer that an unrecorded tool call did not occur; use application audit logs and approval records to fill the gap.
How should teams handle schema changes in agent telemetry?
Version the schema and keep a migration or interpretation rule for evaluators and dashboards. Add new fields compatibly, document changed meanings, and run a short overlap period so alerts do not treat a schema transition as behavioral drift.
