Code-review delays are a flow problem, not simply a matter of asking maintainers to work faster. Measure when a change becomes ready, when a reviewer first provides substantive feedback, how long discussion lasts, and what happens after approval. Then compare those timings with change size, review iterations, reviewer capacity, and escaped defects. This separates a queue caused by ownership or capacity from one caused by oversized changes, specialist work, automation, or platform limits.

The most useful starting pair is time to first substantive review and total review-stage time. Use medians and distributions rather than averages, segment the results by type of work, and set a response expectation with a clear reassignment path. Reduce delay by making changes independently reviewable, routing specialist work deliberately, automating deterministic checks, and preserving quality guardrails.

What a code-review bottleneck looks like

A pull request can be “stuck” in several different ways. It may wait unassigned in a queue, wait for a particular specialist, spend days in back-and-forth discussion, or be technically approved while a merge queue is constrained. Treating all of those as one number leads to the wrong fix.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Model review as a sequence of states:

  1. Ready: the author has finished the change and required automated checks are available.
  2. Assigned: an owner or reviewer has been identified.
  3. First substantive review: a non-author provides meaningful comments, requested changes, or approval.
  4. Discussion and revision: the author responds and may upload additional commits or patchsets.
  5. Approved: required decisions are complete.
  6. Merged or closed: the change leaves the review system.

The delay between states matters. A project with a long assignment wait needs better ownership and capacity coverage; a project with short assignment waits but many revision rounds needs clearer requirements, smaller diffs, or earlier design discussion.

Metrics that expose the queue

Time to first substantive review

Measure from the moment a change is ready or assigned to the first meaningful comment or approval from someone other than the author. GitLab’s review-time analytics uses the first non-author comment as the start of review and exposes review time alongside author, approvers, comments, commits, and line changes. A bot acknowledgement, automatic status update, or trivial “looks queued” note should not count as substantive feedback.

Report the median and a percentile or tail view. The median describes the typical contributor experience; the slowest reviews reveal whether a small number of requests are consuming the queue. Keep the event definition stable so a policy change does not appear to improve performance merely because the timestamp changed.

Total review-stage time and time to merge

Track the interval from ready or assignment through approval and merge, and keep the approval-to-merge portion visible. LFX analytics distinguishes merge lead time, median time to review, and median time to close. A pull request can receive prompt comments yet wait for a release branch, a merge train, or a required second approver. Combining those stages hides the operational constraint.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use medians for the normal experience and inspect long-tail outliers individually. A single average can rise because of one abandoned migration while the ordinary feature queue is healthy.

Pull-request size

Record lines changed, files touched, and a size band for every review. GitLab’s code-review guidance treats approximately 200 changed lines as a practical target for a merge request. Size is not a quality score: a 200-line generated update may be harder to inspect than a carefully designed 400-line change. It is a signal that helps explain why review time, discussion length, or revision count rises.

Keep additions and deletions separate where the platform provides them. Also mark migrations, vendored updates, generated files, and emergency fixes, because their size has a different relationship to human review effort.

Review iterations and patchsets

Count review cycles or patchsets, not just the number of comments. LFX defines patchsets per review and review efficiency as review analytics measures. Repeated rounds can indicate unclear acceptance criteria, inconsistent style expectations, late test failures, or unresolved design disagreement. Pair the count with the reason for another round so maintainers do not try to reduce iterations by suppressing useful feedback.

Reviewer-load distribution

Report reviews completed per person and by specialty. A total such as “the project completed 80 reviews” says little if 70 were handled by two maintainers. Concentration creates a queue when either maintainer is unavailable and increases burnout and succession risk.

Show both volume and waiting work: assigned reviews, unassigned reviews, specialist requests, and the age of the oldest item. Map primary and backup reviewers for critical areas. Few approvers may reflect genuine expertise requirements, but it should be an explicit risk rather than an accidental dependency.

Quality and contributor guardrails

Place speed metrics beside failed changes, incidents, escaped defects, security findings, follow-up fixes, and developer feedback. GitLab warns that activity measures alone can reward the wrong behavior and advises focusing on value, quality, safety, and delays instead of visible counts. A faster approval rate without quality guardrails can mean rubber-stamping.

Use the same time window and segment definitions for speed and quality. If a faster process is introduced for urgent fixes, compare those changes with other urgent fixes rather than with ordinary feature work.

Build a review data set you can trust

Capture stable events

Export or record, for each pull request or merge request:

  • Creation time and the time it became ready for review.
  • Assignment time, reviewer identity, specialty, and any reassignment.
  • First non-author substantive comment or approval.
  • Every author revision, patchset, approval, requested-change event, and withdrawal.
  • Merge, close, or abandonment time.
  • Lines added and removed, files touched, labels, repository area, and work type.
  • Automated-check start, completion, and failure events.
  • Post-merge defects, incidents, security findings, and corrective changes when they can be linked reliably.

Store timestamps in a consistent timezone and preserve the original event time. Define how reopened requests, draft-to-ready transitions, rebases, force-pushes, and abandoned requests are treated before calculating a baseline.

Define “substantive” before reporting

Write a short rule that reviewers and analysts can apply consistently. For example, count a comment that identifies a defect, asks for a design or behavior change, explains a risk, or records an approval decision. Exclude bot messages, author comments, and generic notifications. If a platform’s built-in analytics uses a different definition, report that definition rather than silently mixing it with a local one.

Establish a four-to-six-week baseline

For four to six weeks, collect medians and distributions for first-review time, total review time, size, iteration count, reviewer-load concentration, and quality outcomes. Do not change several policies during the baseline. Sample both the fastest and slowest reviews and record the apparent cause: ownership, capacity, size, automation, specialist routing, disagreement, or a platform constraint.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The baseline is not a target to optimize blindly. It is a reference that lets you test one intervention against a comparable segment.

Diagnose the cause before changing policy

Separate waiting from active review

Start with a timeline for representative requests. Measure:

  • Assignment wait: ready to assignment.
  • Reviewer wait: assignment to first substantive review.
  • Discussion time: first review to final approval.
  • Merge wait: approval to merge or closure.

GitLab recommends sampling merge requests, opening the underlying issues, and speaking with developers and reviewers to identify causes such as unclear ownership, large changes, limited capacity, late automation, or specialist-review needs. A dashboard shows where to look; conversations explain why the event occurred.

Segment instead of averaging everything

Compare size bands, labels, repository domains, contributor experience, urgency, and normal work against migrations or emergency fixes. Also separate changes that need security, database, infrastructure, or API specialists. An acceptable overall median can conceal a severe queue in one specialty.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use enough history in each segment to avoid reacting to a single unusual request. Keep the segment definition when comparing before and after an intervention.

Map ownership and backup coverage

List the files or components that require specialist approval, the primary approvers, and the backups who can act when the primary is unavailable. If a request repeatedly waits for one person, the issue is a capacity or succession gap, not an author-performance problem. Define when a request can be reassigned and who has authority to make that decision.

Check platform constraints

At high volume, the hosting service itself can become part of the queue. GitHub documents a limit of 1,000 open pull requests against one branch, a rate limit of one merged pull request per minute, and diff limits. If a repository approaches those limits, investigate branch strategy, merge-queue configuration, repository decomposition, or infrastructure capacity before adding pressure to reviewers.

Interventions that reduce review delay without lowering quality

Set a response expectation and an escape route

Publish who owns the review queue, what counts as an urgent request, when a reminder is appropriate, and how reassignment works. GitLab’s handbook defines review-response SLO as “the time when review is provided” minus “the time when the review is assigned to the reviewer.” Its target for GitLab team-member merge requests is less than two business days. A volunteer project should adapt the target to actual availability rather than copy the number as a promise it cannot keep.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

An SLO needs an escape route. Ask a reviewer who cannot meet the expectation to help find a replacement, and allow the author or triager to reassign after the published interval. Do not turn the SLO into a public ranking of individuals; its purpose is to make ownership and escalation predictable.

Make changes smaller and independently reviewable

Use the approximately 200-line target as a prompt to split work, not as an automatic rejection rule. Separate refactoring, formatting, generated output, and behavior changes when they can be reviewed independently. For larger features, use stacked changes: each layer should build, test, and explain its own purpose while depending on the previous layer.

Minimize the number of required reviewers to the people needed for the risk and domain. Smaller diffs reduce the context a reviewer must hold and make feedback-based commits easier to inspect. Explain intentional exceptions, such as a coordinated schema migration, in the request so reviewers can adjust expectations.

Route specialist work deliberately

When a change spans domains, request an expert for each affected area and state the focus of that request. “Please check backward compatibility of the public API” is more actionable than “please review.” For a long feature series, identify maintainers who can retain context across the sequence instead of restarting the explanation for every patchset.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not add reviewers merely to increase the approval count. Extra observers can create conflicting requests and extend discussion without adding risk coverage.

Automate deterministic checks before human review

Run formatting, linting, tests, security checks, and other repeatable validation before requesting maintainer attention. GitLab recommends enforcing code style through automation and requesting maintainer review when tests pass. Automation should remove routine comment traffic; humans should decide design, risk, compatibility, and domain behavior.

Make failures easy to act on. A check should identify the rule, affected file, and remediation. Avoid a workflow in which reviewers spend their limited time reporting failures that a pre-review job could have caught.

Use reciprocity and courteous escalation

Volunteer projects depend on reciprocal effort. LLVM’s code-review policy says, “If you help out, everybody will be more willing to do the same for you; goodwill is our currency.” Make reviewing visible in contributor onboarding and rotate responsibility where practical.

What’s actually slowing this PC down?

Pick the symptom - the matching free tool is one click away.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

LLVM recommends a periodic ping every couple of days for urgent work and about weekly for non-urgent work. Treat a reminder as queue management, not blame. Include what is needed, the current risk, and the reassignment path; avoid repeated messages that add notification volume without new information.

Pair speed with quality

Review the speed metric and its guardrails together. Track failed changes, incidents, escaped defects, security findings, follow-up fixes, and contributor feedback alongside review-stage time. If speed improves while these signals worsen, roll back or redesign the intervention. If both improve, examine which part of the flow changed so the useful practice can be documented.

Compare interventions by the decision they change

Intervention Primary delay addressed Capacity effect Quality protection Operational requirement
Response expectation with reassignment Assignment and reviewer waiting Reveals and redistributes hidden queue work Preserves required approvals and escalation Published owner, deadline, and backup path
Smaller or stacked changes Discussion and revision time Reduces context per review; does not create more reviewers Independent tests and explicit exceptions Author guidance and branch or stack coordination
Specialist routing Waiting for domain expertise Uses expertise where risk requires it Named focus for each specialist Ownership map and backup coverage
Pre-review automation Routine comment and rework time Shifts repeatable work to machines Tests, lint, formatting, and security checks Reliable checks with actionable failures
Reciprocal review and courteous pings Unassigned or forgotten work Improves participation without adding formal headcount Human review remains required for risk decisions Contribution norms and reminder cadence
Platform or queue changes Merge wait caused by service limits May require repository or infrastructure changes Preserves branch protection and merge checks Monitoring of documented platform limits

Evaluate each option on six axes: whether it reduces waiting or discussion, whether it spreads load or merely pressures the same maintainers, what catches shallow review, how it affects contributor uncertainty, what automation or coordination it costs, and whether it can be tried and reversed safely.

Run a repeatable improvement loop

  1. Baseline: collect four to six weeks of medians and distributions for first-review time, total review time, size, iterations, load concentration, and quality outcomes.
  2. Sample: inspect the fastest and slowest reviews and classify causes as ownership, capacity, size, automation, specialist routing, disagreement, or platform constraint.
  3. Select one intervention: match the remedy to the cause, such as a backup-reviewer rota for capacity or a size limit with stacked changes for oversized diffs.
  4. Re-measure the affected segment: compare the same work type, size band, and domain rather than only the project-wide total.
  5. Decide and document: keep the intervention when review time improves without deterioration in quality or contributor experience; revise or remove it when it does not change a decision.

Retire metrics that no longer influence a decision. Avoid a single composite score that hides trade-offs between speed, quality, load, and experience. A small set of well-defined measures is more useful than a large dashboard nobody acts on.

Common measurement and policy mistakes

Optimizing averages

An average can improve while a specialty queue becomes unusable. Always show the median, a tail view, and segment results by domain and work type.

Counting activity as value

Comments, approvals, and closed requests are activity measures. They do not prove that a change was safe or useful. Pair them with defects, incidents, security findings, follow-up fixes, and contributor feedback.

Applying a size rule mechanically

A line-count target is a diagnostic signal, not a substitute for engineering judgment. Generated files, migrations, and emergency changes need explicit handling and may require a different review plan.

Adding reviewers to solve a queue

More reviewers can create contradictory feedback and extra rounds. Route only the expertise the change needs, name each reviewer’s focus, and maintain backups for critical areas.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Using an SLO without reassignment

A deadline that offers no replacement path turns a measurement into frustration. Publish who can reassign, when they do it, and how urgent requests are identified.

Ignoring infrastructure limits

When open-request or merge-rate limits are reached, asking humans to review faster cannot remove the platform bottleneck. Check branch queues, merge processing, and documented diff constraints.

Implementation checklist for maintainers

  • Define ready, assigned, substantive review, approved, merged, and abandoned states.
  • Record first non-author substantive review, review iterations, size, files, labels, specialty, and reviewer identity.
  • Publish medians and tail distributions for first-review and total review-stage time.
  • Separate assignment wait, reviewer wait, discussion time, and merge wait.
  • Segment by size, domain, contributor experience, urgency, and exceptional work.
  • Map primary and backup reviewers for critical components.
  • Set a response expectation appropriate to volunteer availability and document reassignment.
  • Prefer independently reviewable or stacked changes; use approximately 200 changed lines as a practical target rather than an inflexible gate.
  • Run deterministic checks before human review.
  • Use reciprocal reviewing and courteous, predictable reminders.
  • Keep failed changes, incidents, escaped defects, security findings, follow-up fixes, and contributor feedback beside speed metrics.
  • Test one intervention at a time, re-measure its affected segment, and remove metrics that no longer guide action.

Bottom line

Measure code review as a flow: identify where a request waits, how much reviewer capacity is available, and whether faster movement preserves quality. Start with first substantive review time and total review-stage time, then add size, iterations, load distribution, and quality signals. Diagnose the specific queue before changing policy, set a realistic response and reassignment path, keep changes independently reviewable, automate deterministic checks, and treat specialist coverage and platform limits as operational constraints. This approach reduces delay without turning review into a race to approve.
