AI agents can read webpages, PDFs, emails, database rows and tool responses safely only when those materials are handled as hostile data rather than instructions. Put trusted instructions in a separate channel, label and sanitize everything retrieved, enforce source permissions, restrict tools and network access, validate every action at the execution boundary, and require approval for consequential side effects. This design limits what a malicious document can make an agent disclose or do, even when the model itself fails to recognize an injection.

What makes a source untrusted for an AI agent?

A source is untrusted whenever its author, integrity, or intended use cannot be guaranteed by the agent’s control plane. That includes public webpages, uploaded files, emails, search results, RAG passages, database records, API responses, repository files, tool metadata and prior memory writes. Microsoft’s retrieval guidance states that every prompt, document, retrieved chunk, tool result and memory write should be treated as untrusted input and checked before it influences reasoning or action.

The distinction is about authority, not whether the content is accurate. A company policy PDF may be authentic but still must not be allowed to redefine the system prompt or choose a payment recipient. Treat the file as evidence the agent may quote, not as a command source.

How does indirect prompt injection work?

Indirect prompt injection hides instructions inside material the agent retrieves for a legitimate task. A webpage might contain text telling the model to ignore its rules and send secrets; a Word document can contain a similar instruction in visible or hidden text. The user may never see the attack, because the agent encounters it while browsing, searching a knowledge base or opening an attachment.

Direct versus indirect injection

  • Direct injection: the user’s message itself asks the model to violate its constraints.
  • Indirect injection: a third party places instructions in a webpage, document, email, database, API response, repository or tool description that the agent later reads.

Indirect attacks are dangerous because the content appears relevant to the task. A request to summarize an invoice can become an attempt to exfiltrate conversation history; a request to compare products can become an instruction to call an unauthorized API.

What can a successful injection do?

The impact is determined by connected tools and permissions. A read-only summarizer has a smaller blast radius than an agent that can send email, modify files, purchase goods, change production records or write long-term memory. A compromised source can cause disclosure of system or conversation data, unauthorized API calls, file changes, messages to outsiders, destructive actions or poisoned memory that affects later runs.

What is the safest trust-boundary design?

Keep instructions and data separate in both your application data model and the model request. System and developer policies are trusted; user text, retrieved passages, tool output, external files and memory are untrusted. Preserve the identity and provenance of each item so the agent can cite it without granting it authority.

Use explicit fields and labels

Represent a retrieval result as structured data such as {"source_id":"invoice-184","content":"...","trust":"untrusted","permissions":{"can_quote":true,"can_execute":false}}, rather than concatenating it into an instruction string. Delimit content clearly and state that it may contain adversarial instructions. Labels do not create a security boundary by themselves, but they make policy enforcement and auditing possible.

Authorize before reasoning and action

Microsoft describes a useful sequence: content should be inspected, labeled, normalized and authorized before it can influence reasoning or action. Authorization should include who may access the source, whether the content is current, and which task uses it. A document that one employee may read should not automatically be available to an agent operating for another identity.

How should an agent sanitize webpages, PDFs and emails?

Reduce the amount of external content in context, remove active or deceptive elements where practical, and preserve enough provenance for the answer to be checked. Sanitization lowers exposure; it does not prove that the remaining text is safe.

Webpages and HTML

  • Fetch through a restricted worker rather than the agent’s broad network identity.
  • Remove scripts, event handlers, embedded frames, hidden text and unexpected instruction-like fields before conversion to plain text.
  • Keep the canonical URL, retrieval time and a content hash for audit purposes.
  • Block redirects to unapproved domains and prevent the page from choosing the next tool call.

PDFs and office files

  • Extract text in an isolated process; do not enable macros, embedded scripts or automatic external links.
  • Scan visible text, metadata, comments, alt text and hidden layers for instructions that are unrelated to the user’s task.
  • Apply an allowlisted schema to dates, identifiers, quantities and amounts instead of passing arbitrary strings to a tool.
  • Retain page numbers so a reviewer can inspect the exact evidence.

Email and messaging data

  • Separate headers, body, attachments and quoted history; do not treat a sender’s request as an authorization.
  • Display the actual recipient, attachment and proposed reply before sending anything.
  • Strip tracking pixels and active content in the analysis copy, while preserving the original message for forensic review.

Microsoft identifies content scanning, sanitization, provenance checks and segmentation as complementary controls, and names Prompt Shields as one possible intermediary. No scanner catches every obfuscated or novel attack, so execution controls remain necessary.

How can a RAG pipeline keep hostile documents from controlling the agent?

RAG should retrieve evidence, not executable instructions. Enforce identity-aware retrieval, return only passages needed for the question, and pass each passage through a policy-aware normalization step before it reaches the model.

  1. Authenticate the requester. Resolve the user, tenant, role and data entitlements before searching.
  2. Filter at retrieval time. Apply document-level and field-level permissions in the index query, not after generation.
  3. Minimize context. Retrieve the smallest relevant passages and cap the number of documents and tokens.
  4. Attach provenance. Include source ID, owner, timestamp, access decision and location such as page or paragraph.
  5. Normalize safely. Convert content to a constrained representation, remove active markup and flag instruction-like text as untrusted.
  6. Constrain the answer. Require citations to retrieved evidence and prohibit the model from treating passages as policy or tool-selection commands.
  7. Validate outputs. Before any side effect, check that the proposed operation is supported by an authorized source and the user’s original request.

Memory requires the same discipline. A model-generated memory write is untrusted until it passes schema, provenance and permission checks. Otherwise one poisoned document can alter future behavior long after the original retrieval.

How should tool calls be protected from injected instructions?

Do not let free-form retrieved text directly select a tool, recipient, permission or code path. Parse the user’s intent into a constrained task object, then validate every tool name and argument immediately before execution. OWASP places these controls next to the tool that creates the side effect because model output remains untrusted.

A safer execution sequence

  1. Parse intent: convert the user request into an allowlisted operation such as summarize_document or draft_email.
  2. Plan without authority: let the model propose arguments, but do not execute them yet.
  3. Apply deterministic policy: check tool, user, tenant, destination, data classification, rate limits and required approvals.
  4. Validate at the destination: the email service, database or deployment system independently verifies arguments and permissions.
  5. Request approval when required: show the exact operation and arguments, relevant source, and data that will leave the system.
  6. Execute in a constrained identity: use a short-lived credential scoped to that single operation.
  7. Record the result: log the decision, approval, source provenance and outcome without exposing secrets.

Examples of destination-side checks

  • An email service accepts only approved domains, blocks bulk recipients and requires a user confirmation token.
  • A database API permits updates only to fields in an allowlist and rejects changes without a matching record version.
  • A payment service checks amount limits, currency, beneficiary ownership and transaction approval independently of the model.
  • A code runner rejects network access and production credentials even if the agent requests them.

Which permissions and isolation controls limit the blast radius?

Use least agency: give the agent only the tools, data and duration required for the task. OWASP recommends allowlisted tools, scoped service identities, read-only defaults, short-lived credentials, restricted egress and isolated workers or disposable virtual machines.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Control Safer default Why it matters
Tool access Allowlist specific operations; deny by default An injection cannot call an unexposed capability.
Credentials Short-lived, task-scoped tokens Stolen or misused credentials expire quickly and reach fewer systems.
Files Temporary workspace with no home-directory mount Malicious content cannot browse unrelated personal or system files.
Network Restricted outbound domains and blocked metadata services Limits exfiltration and access to internal infrastructure.
Execution Isolated worker or disposable VM without production credentials Reduces persistence and lateral movement.
Data Minimum necessary fields, masked secrets Less sensitive material is available to leak.

Isolation is not a substitute for authorization. A sandbox that can still send email or reach an internal API has a meaningful side effect, so those destinations need their own controls.

When should a human approve an agent action?

Require confirmation before sending messages, changing records, purchasing, publishing, deleting data or executing code. OpenAI recommends narrow, explicit tasks and review before important actions; OWASP recommends action-specific approval that shows what will happen and with which arguments.

A useful approval screen contains:

  • the exact tool and operation;
  • all arguments, recipients, amounts and files;
  • the source passages that influenced the proposal;
  • the identity and permission under which it will run;
  • what data will leave the system and where it will go;
  • an option to edit, reject or approve once.

Do not ask for a vague “continue?” confirmation. An approval that hides the destination or payload can become a rubber stamp for an injection.

How do you monitor and test indirect prompt injection?

Log enough context to reconstruct decisions while protecting secrets. Record source identifiers and hashes, retrieval permissions, model outputs, tool calls, policy decisions, approvals, network destinations and final results. Alert on unusual tool sequences, repeated policy failures, unexpected egress and attempts to access unrelated data.

Adversarial test cases

  • Visible instructions that say to ignore the system policy.
  • Hidden HTML, zero-width characters, comments, metadata and encoded text.
  • A document that asks for secrets or changes the recipient of an otherwise valid task.
  • Tool output that contains a fake system message or a request to call another tool.
  • Poisoned memory that attempts to alter a later session.
  • Multi-step attacks that begin with a harmless summary and escalate after a tool result.

OWASP recommends structured adversarial testing with harmless canaries and instrumented tool substitutes. Re-run the suite after changing prompts, models, retrieval, tools, memory, providers or network policy; a defense that worked for one configuration may fail after a small integration change.

Which architecture should you choose?

Compare designs by their controls around the model, not by model claims alone. A model-only defense is weak when retrieval permissions are broad or execution is unconstrained.

Architecture Strength Typical weakness Best fit
Read-only summarizer Small side-effect surface Can still leak content or poison downstream users Document triage and research notes
RAG assistant with citations Permission-aware evidence and traceability Retrieved text may still influence tool planning Internal knowledge questions
Agent with allowlisted tools Automates bounded workflows Argument validation and destination checks are essential Ticket updates or controlled data entry
Sandboxed code agent Limits filesystem and network damage Output can still trigger external actions Untrusted code analysis and testing
Broad autonomous operator Handles complex multi-step work Largest blast radius and hardest approval problem Only with mature isolation, monitoring and human gates

Evaluate each option on trust-boundary clarity, permission-aware provenance, sanitization coverage, tool and network privilege, approval granularity, isolation, destination validation, observability and red-team coverage.

What should you do if an agent has already followed an injection?

Contain first, then investigate. Revoke or rotate credentials used by the run, disable affected tools, block suspicious destinations and preserve logs and source copies. Check outbound messages, file changes, database mutations and memory writes; restore from known-good versions and remove poisoned memories.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Freeze the run and any queued actions.
  2. Identify the source, permissions, tools and destinations involved.
  3. Revoke short-lived tokens and rotate any credential that may have been exposed.
  4. Review logs for disclosure, persistence and lateral movement.
  5. Correct retrieval filters, sanitization or policy rules that allowed the path.
  6. Replay the scenario with a harmless canary and keep the regression test.

What is the practical safety checklist?

  • All external content is explicitly labeled untrusted.
  • Trusted instructions and retrieved data occupy separate fields or message sections.
  • Retrieval enforces user, tenant and document permissions before context assembly.
  • HTML, office files, attachments and tool output are sanitized and parsed in isolation.
  • Tools and network destinations are allowlisted with read-only defaults.
  • Every argument is validated immediately before execution and again by the destination.
  • High-impact actions show exact payloads and require specific approval.
  • Credentials are short-lived; workers have no production secrets or unnecessary mounts.
  • Provenance, approvals, policy decisions and outcomes are logged safely.
  • Direct, indirect, obfuscated and memory-poisoning attacks are retested after every meaningful change.

Frequently Asked Questions

Can an AI agent safely summarize a malicious PDF?

Yes, if the PDF is processed as data in an isolated extractor, active content and hidden layers are handled, and the summarizer has no write or outbound tools. Keep page-level provenance and prevent extracted text from selecting tools or changing policy.

Does adding a prompt-injection warning stop attacks?

A warning may improve model behavior but is not a security boundary. Least-privilege permissions, deterministic argument checks, destination-side validation, isolation and approval gates are needed when a model ignores the warning.

Should untrusted documents ever be stored in long-term agent memory?

Only after a separate validation and provenance process. Store narrowly scoped facts with source, owner, timestamp and expiry; reject instructions, secrets and unsupported claims so one poisoned document cannot influence future sessions.

How can I tell whether a retrieved passage is trying to manipulate the agent?

Instruction-like language aimed at the assistant, requests for secrets, attempts to change recipients or policies, and directions to call tools are warning signs. Detection is inherently incomplete, so treat every passage as untrusted even when no suspicious phrase is found.
