Assume every AI-generated or AI-modified change is untrusted until it passes the same—and, for security-critical paths, stronger—controls you use for human code. Label the change, review its threat model, run deterministic SAST, software-composition, secret, and infrastructure-as-code scans in the developer loop and in CI, verify every suggested dependency, and test abuse cases such as forged tokens and injection payloads. Then have a qualified human reproduce each finding, check reachability and exploitability, and rank it by impact, asset criticality, exposure, and evidence. AI can explain a confirmed finding or draft a fix, but it cannot be the reviewer or the approver.
That discipline matters because controlled studies have found security weaknesses in a substantial share of generated snippets and programs, while also showing that rates vary sharply by language, prompt, task, and measurement method. There is no single percentage that predicts the risk of your repository.
Why can functional AI-generated code still be insecure?
Code completion optimizes for a plausible implementation of the prompt, not for every security requirement that was never stated. A function may return the expected result while trusting user-controlled input, skipping an authorization check, logging a credential, or selecting a package that does not exist. Treat generated code as a new input to your secure-development process, regardless of whether an engineer edited it afterward.

A controlled IEEE Symposium on Security and Privacy study (2022) evaluated 1,689 Copilot programs across 89 scenarios and found that approximately 40% were vulnerable, with large differences by CWE, prompt, and domain. A 2024 ICSE user study of 25 participants found the assistant associated with more secure solutions on harder tasks, no measurable effect on easier tasks, and no disproportionate effect by vulnerability type. These results are not directly comparable, so use them to justify controls—not to estimate a universal prevalence rate.
“A developer who says ‘the AI wrote it’ is still responsible for the code they generate.” — OWASP Secure Coding with AI Cheat Sheet
OWASP’s AI Security Verification Standard (AISVS) likewise requires AI-generated code to receive review by a qualified human engineer; the AI agent does not count as that reviewer.

How do I inventory and label AI-generated changes?
Make provenance visible before scanning. A reviewer cannot apply the right scrutiny if generated files, prompts, or agent edits are indistinguishable from ordinary changes.
- Mark the scope. Record generated or materially AI-modified source files, tests, build scripts, configuration, infrastructure definitions, pull requests, and generated documentation that affects security behavior.
- Record provenance. Preserve the assistant or agent name, model and version, relevant extension or agent configuration, date, and whether the output was accepted verbatim, edited, or regenerated.
- Assign an owner. Name a human engineer accountable for the change, the threat-model review, scanner results, and final approval. Ownership remains with the repository team even when an external agent made the edit.
- Keep the audit trail. Store prompts or task descriptions when policy permits, review comments, tool output, suppressions, approvals, and the commit that fixed each confirmed issue. Remove secrets and prohibited data from those records.
Use a pull-request label or repository metadata field that survives merges into release branches. The label is a routing aid, not a risk score: a tiny generated test helper can be harmless, while a short generated authorization middleware can be critical.
What should I review before reading the generated syntax?
Start with the threat model and requirements. This prevents a reviewer from approving code that is cleanly written but violates an unstated security property.
Map trust boundaries and data
- Identify public endpoints, message queues, plug-ins, file uploads, webhooks, and other external interfaces.
- List sensitive data such as credentials, personal information, payment records, tokens, and signing keys, and where each can be stored, logged, or transmitted.
- Mark privileged operations, administrative functions, control-plane APIs, and service-to-service identities.
Write the required controls
State authentication, authorization, tenant isolation, validation, output encoding, rate limits, audit logging, retention, and failure behavior as testable requirements. Include abuse cases: an attacker supplying malformed input, replaying an expired token, changing an object identifier, racing two requests, or inducing an error path. A prompt that says “build an upload endpoint” does not establish which file types, destinations, quotas, or permissions are acceptable.

Review high-risk boundaries twice
Authentication, authorization, session management, cryptography, parsers, and external interfaces deserve a security-team review or two-person approval. Require the reviewer to trace data from its source to every security-sensitive sink rather than relying on comments or the model’s explanation.
Which security scans belong in the developer inner loop?
Run fast, deterministic checks while the code is being written so defects are corrected before a pull request. Use several scanners because each observes a different failure mode.
Static application security testing (SAST)
Configure rules for dangerous sinks and missing controls: SQL or command construction, dynamic evaluation, unsafe deserialization, path handling, cryptography APIs, authorization checks, and unvalidated input. Add organization-specific rules for defects that recur in generated code, and make findings point to a secure reference pattern.
Software-composition analysis (SCA)
Scan manifests, lockfiles, vendored libraries, container layers, and generated import statements for known vulnerabilities and license-policy violations. Resolve the package and version actually selected by the build; a clean source file does not make a vulnerable transitive dependency safe.
Secret detection
Inspect source, test fixtures, notebooks, CI files, container recipes, and generated documentation for API keys, passwords, private keys, and tokens. When a real credential is found, revoke or rotate it immediately, then remove it from history according to your incident procedure.
Infrastructure-as-code scanning
Scan cloud templates, Kubernetes manifests, CI configuration, and deployment scripts for public storage, excessive permissions, unencrypted data, unsafe network exposure, and missing logging. Generated infrastructure can create a production vulnerability even when application code is correct.
Representative integrations include Semgrep, Trivy, Checkmarx, GitHub Code Security, and Snyk. Select tools by coverage and workflow fit, not by a single severity count.

What must run on every pull request and build?
For every pull request containing AI-generated code, enforce deterministic SAST, SCA, secret, and IaC checks. OWASP AISVS also calls for interactive, dynamic, and application testing where appropriate; use IAST or DAST in environments that can exercise the deployed behavior.

| SCA | Detect vulnerable direct and transitive dependencies | Verify the locked dependency graph and container contents | Block known critical vulnerabilities in reachable production components |
| Secret scanning | Prevent credentials from entering the repository | Scan generated artifacts and release configuration | Block any unapproved live credential; rotate exposed secrets |
| IaC scanning | Catch insecure cloud and deployment changes | Validate the rendered configuration for the target environment | Block public exposure or excessive privilege without documented approval |
| IAST or DAST | Exercise changed endpoints when a test environment is available | Probe deployed behavior and authentication boundaries | Gate confirmed exploitable release defects |
Keep the checks reproducible: pin scanner versions and rule sets, retain results, and fail closed when a required scanner cannot run. OWASP AISVS gives CVSS 9.0 or an equivalent organizational threshold as an example for a merge-block policy; choose and publish your own threshold.
How do I verify an AI-suggested dependency before installing it?
Assume a package name, version, or import suggested by an assistant may be hallucinated, typosquatted, abandoned, or vulnerable. Verify it before the package manager downloads anything.
- Confirm identity. Check that the package exists in the intended registry and that its name exactly matches the documented project. Compare spelling, scope, repository links, and official ownership indicators.
- Inspect history. Review maintainers, publication dates, release cadence, download patterns, issue activity, and whether the project changed ownership unexpectedly. A recently published package with a familiar name deserves extra scrutiny.
- Check advisories. Query your SCA database and the package’s advisory history for the proposed version and its transitive dependencies.
- Constrain installation. Pin an approved version or digest, use a lockfile, and install through an internal proxy or allowlist where possible. A minimum-age policy can reduce exposure to newly uploaded malicious packages.
- Review behavior. Inspect install scripts, network access, filesystem writes, and requested permissions. Test the dependency in a sandbox before it reaches a privileged build or production environment.
Do not “fix” an uncertain package by silently substituting a similarly named library. Ask the owning team to select and approve the dependency, then record why it is needed.
How should I test AI-generated security behavior?
A generated test suite often follows the happy path that the prompt described. Add adversarial cases designed independently of the implementation and have a separate reviewer write or approve tests for security-critical behavior.

Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →- Input abuse: empty, oversized, malformed, truncated, and unexpected-type values; encoding changes and duplicate parameters.
- Injection: SQL, command, template, expression, path, and header payloads appropriate to each sink.
- Identity and authorization: expired, forged, replayed, or wrong-audience tokens; cross-tenant object identifiers; privilege changes during a session; and direct requests to administrative routes.
- Concurrency: simultaneous updates, duplicate submissions, race conditions around balances or permissions, and retries after partial failure.
- Boundary values: minimum and maximum lengths, numeric overflow, time-zone transitions, clock skew, and empty collections.
- Failure paths: upstream timeouts, parser errors, unavailable key services, and logging or redaction behavior when an exception occurs.
Run these tests against the same authentication, network, storage, and configuration conditions used in deployment. A passing test suite proves only that those cases passed; it does not prove that untested paths are secure.
How do I validate scanner findings before triage?
Do not rank a finding from its rule name alone. A qualified reviewer should reproduce it and establish whether an attacker can reach the vulnerable operation under realistic conditions.

- Reproduce. Confirm the tool’s file, line, data flow, dependency version, or configuration and create a minimal proof or failing test.
- Trace reachability. Follow the value from an external or privileged source to the sink. Determine whether dead code, feature flags, validation, or a safe wrapper prevents execution.
- Establish preconditions. Record required network position, account role, privileges, tenant, physical access, timing, and configuration. Unusual local access lowers exploitability but does not erase impact.
- Assess impact. Determine whether exploitation enables remote code execution, authentication or authorization bypass, sensitive-data exposure, integrity loss, or service disruption.
- Resolve the record. Fix confirmed issues, suppress false positives with a reason and expiry, and link the regression test or commit. Reopen the finding if later configuration makes the path reachable.
For critical components, require security-team or two-person validation even when the scanner labels the result informational. Conversely, do not block a release solely because a high-severity rule points to unreachable code; document the analysis and monitor for architectural changes.
How should I prioritize confirmed vulnerabilities?
Use a documented scorecard rather than severity labels alone. A practical order is impact, exploitability and reachability, asset criticality, exposure and prevalence, then evidence and remediation confidence.
Free tools Windows power users keep installed
One-click scans. No signup required.
| Factor | Questions to answer | Higher priority when… |
|---|---|---|
| Impact | What can an attacker change, read, execute, or stop? | The issue enables remote code execution, authentication or authorization bypass, sensitive-data exposure, integrity loss, or service disruption. |
| Exploitability and reachability | Can an external request reach the sink, and what preconditions are needed? | The path is internet-facing and a reproducible exploit needs little privilege or preparation. |
| Asset criticality | Which system and data are affected? | The component handles production identity, payments, safety functions, or a control plane rather than a low-value development utility. |
| Exposure and prevalence | How many services, repositories, or generated files share the defect? | The issue appears in a shared library or repeated generated pattern, allowing one fix to remove many instances. |
| Evidence and remediation confidence | How certain is the finding and can the fix be regression-tested? | A known CVE, confirmed exploit, or reproducible proof exists and the corrective change is clear. |
Set an explicit merge-block threshold. Critical findings should block merges by default; an organization may use CVSS 9.0 or an equivalent threshold, as OWASP AISVS illustrates. Exceptions require authorized human approval, a compensating control, an owner, and an expiration date. Recalculate priority when exposure, reachability, or asset role changes.
Which vulnerability patterns appear repeatedly in generated code?
Use these patterns to focus review and to create custom rules, secure examples, and adversarial tests.
| Pattern | What to inspect | Useful control |
|---|---|---|
| SQL or command string concatenation | User data inserted into query or shell text | Parameterized APIs, strict allowlists, and sink-focused SAST |
| Dynamic evaluation | Runtime execution of generated strings or expressions | Remove evaluation, or isolate and constrain it with a narrowly defined parser |
| Weak cryptography | Home-grown algorithms, obsolete hashes, hard-coded keys, or incorrect nonce use | Approved libraries, key management, and security review |
| Missing authorization | Authentication checked but object or action permissions omitted | Central policy enforcement and cross-tenant tests |
| Hardcoded credentials | Secrets in source, fixtures, examples, or CI files | Secret scanning, revocation, and managed secret storage |
| Unsafe deserialization | Untrusted serialized objects or polymorphic types | Safe formats, type allowlists, and parser isolation |
| Path traversal and unvalidated input | File paths, URLs, headers, and structured payloads copied into privileged operations | Canonicalization, validation, bounded resources, and adversarial tests |
What extra risks do AI agents introduce?
An autonomous agent can alter more than application source. Prompt injection may redirect it, poisoned instruction files may change its priorities, and excessive permissions can turn a small mistake into a repository or cloud compromise.
- Review agent instructions, configuration files, tool definitions, MCP servers, and tool responses as code.
- Sandbox agents and grant only the filesystem, network, and repository permissions required for the task.
- Use scoped, short-lived credentials; never provide broad production keys to an interactive agent.
- Allowlist tools and dependencies, and require human approval for irreversible actions such as publishing packages, changing access policies, deleting data, or deploying.
- Log agent actions and preserve enough context to investigate a prompt-injection or credential-exposure incident.
Hallucinated dependencies and copied secrets are supply-chain risks even when the agent never executes the resulting code. Apply the same provenance and scanner gates to agent-generated infrastructure and automation as to application files.

How do I choose security checks and tools?
Compare the complete workflow, not marketing severity totals. A useful evaluation records whether a tool covers the languages and frameworks you use, operates in the IDE and CI, explains data flow, supports reachability, handles false positives, produces auditable results, fits your deployment model, and has a sustainable cost.

No scanner replaces human review of security-critical logic. Use AI-assisted explanations or remediation suggestions only after a deterministic finding exists; a human must understand the change, test it, and approve it.
What governance keeps AI-assisted development safe over time?
Publish an AI-development policy that is specific enough to enforce. It should name approved tools and models, prohibited data, ownership, review gates, logging, retention, permission limits, and incident contacts.
- Require the provenance label and named human owner for generated changes.
- Define which repositories may use agents and which environments are off-limits.
- Specify minimum SAST, SCA, secret, IaC, and adversarial-test gates, plus the merge-block threshold and exception process.
- Require secure-team or two-person review for authentication, authorization, session, cryptographic, parser, and external-interface changes.
- Feed confirmed recurring defects into custom static-analysis rules and approved secure reference patterns.
- Review the policy after a serious incident, a major tool or model change, or a change in data-handling requirements.
For vulnerability disclosure, establish intake, severity assessment, coordinated communication, tracking, and remediation ownership. NIST SP 800-216 (2023) describes a federal framework for accepting, assessing, and managing disclosure reports that private programs can adapt.
What does a practical end-to-end workflow look like?
Use this sequence for each AI-generated pull request.
- Label generated and materially modified artifacts and record the tool and model version.
- Map trust boundaries, sensitive data, privileged operations, requirements, and abuse cases before reviewing implementation details.
- Run IDE SAST, SCA, secret, and IaC checks; correct obvious defects while context is fresh.
- Verify every new dependency’s identity, history, advisories, permissions, and pinned version before installation.
- Add independently designed adversarial tests for input handling, identity, authorization, concurrency, boundaries, and failure paths.
- Run deterministic scanners and required dynamic checks on the pull request and exact build commit.
- Reproduce findings, determine reachability and attacker preconditions, and discard only documented false positives.
- Score confirmed issues by impact, exploitability, asset criticality, prevalence, and evidence; block or approve according to policy.
- Have the qualified human owner and, for high-risk components, the security reviewer approve the change.
- Record the fix, regression test, exception expiry, and any new rule or secure pattern needed to prevent recurrence.
Frequently Asked Questions
Can a vulnerability that requires an administrator account be ignored?
No. Record the privilege requirement as an exploitability factor, then weigh it against impact and the value of the affected asset. An administrative path can still be serious if credentials are commonly compromised or if it controls production identity or data.
What should we do when two scanners disagree about the same code path?
Compare the data-flow assumptions, rule versions, and runtime configuration, then reproduce the behavior in a minimal test. Keep the finding that is reachable and evidenced, and document why the other result was suppressed rather than choosing the more alarming label.
Are generated comments and documentation part of the security review?
Review them when they influence operators, deployment settings, permissions, or future prompts. A misleading comment can cause a later engineer or agent to disable a control, so verify that each security claim matches executable behavior.
Should agent access tokens ever be reused between tasks?
Prefer scoped, short-lived credentials issued for one task or environment. Reuse expands the blast radius of prompt injection, poisoned instructions, or accidental disclosure and makes audit trails harder to interpret.
How can a team reduce the same AI mistake across many repositories?
Confirm the defect, add a regression test, encode the pattern in a custom static-analysis rule, and publish a secure reference implementation. Track affected shared components so one corrective change removes repeated instances instead of closing findings one file at a time.
