Use AI output as an unverified draft, not as an authority. To fact-check it, split the response into individual claims, rank those claims by the harm a mistake could cause, and trace important statements to current, authoritative original sources. Open every citation, check that it really supports the wording, verify dates and context, and obtain qualified human review for specialist or consequential work.
This approach catches fabricated references, stale information, omitted exceptions and plausible-sounding errors without requiring you to distrust every sentence equally. Search tools and citations can speed up checking, but responsibility remains with the person who sends, publishes or acts on the answer.
What is an AI hallucination, and what does confabulation mean?
An AI hallucination is an incorrect or misleading output presented as if it were reliable. OpenAI uses the term for failures such as fabricated quotes, studies, citations or references to sources that do not exist, as well as answers delivered with unjustified confidence. The National Institute of Standards and Technology (NIST) uses the broader term confabulation for content that is erroneous, false, inconsistent with an earlier answer or divergent from the prompt.
These labels describe an output failure, not a claim that the system is deliberately lying. A model can produce a fluent paragraph because the wording is statistically likely, even when the underlying statement is unsupported. A citation can look perfectly formatted while pointing to a nonexistent paper, a real document that says something else, or an outdated version.

There is no universal, trustworthy hallucination percentage that can be applied to every model, task and setting. Error rates vary with the subject, prompt, language, retrieval setup and evaluation method, so a fixed percentage would give everyday users false reassurance.
Why can an AI answer sound confident when it is wrong?
Generative models predict a likely sequence of words from patterns in their training data; they do not automatically retrieve and verify facts before writing. Google Search Central describes this as prediction rather than fact retrieval. The result can be polished prose whose confidence reflects the model’s language patterns, not the strength of its evidence.
Several characteristics make mistakes hard to notice:

- Fluency: grammatical, detailed language can make an unsupported claim feel researched.
- Pattern completion: when a prompt resembles familiar material, the model may fill gaps with a plausible but invented name, date or quotation.
- Mixed certainty: one response can combine well-established facts with guesses without clearly separating them.
- Context drift: a true statement from one country, year, edition or definition may be applied to a different situation.
- Internal inconsistency: a later paragraph may contradict an earlier one while maintaining the same confident tone.
Polishing the prose does not validate it. Microsoft describes validation as checking output after generation and before acting on, sharing or relying on it; clearer wording is not a substitute for that check.
What should I verify first in an AI-generated answer?
Start with claims that could change a decision or cause harm. Do not spend equal effort on a harmless transition sentence and a legal deadline. Mark the following items before you begin detailed checking.

British Columbia’s output-review guidance specifically calls out names, places, organisations, dates, numbers, laws, policies, procedures, quotations and references. Treat that list as a practical scanning checklist, then add anything unique to your field.
How do I fact-check an AI answer step by step?
The following workflow works for an email, briefing, spreadsheet explanation or draft report. Keep the original response, your edits and the evidence trail together so another person can see what changed.

Define the task and its risk
Write down what the answer will be used for, who will rely on it and what happens if a key statement is wrong. A draft for brainstorming has a different review threshold from a patient instruction, a contract summary or a public announcement. For legal, medical, financial, safety, employment or policy decisions, arrange human or expert review before asking the model to draft.

Request a bounded draft
Tell the model the jurisdiction, date range, audience, permitted sources and output format. Ask it to state assumptions, identify uncertainty and distinguish facts from recommendations. A bounded request reduces ambiguity, but it does not make the answer true.
Draft a 300-word summary for [jurisdiction] as of [date]. Separate verified facts, assumptions and recommendations. For each factual claim, provide the original source title and a link. If you cannot verify a claim, mark it unknown rather than guessing. -
Atomize the response into claims
Copy the answer into a working document and give each factual assertion an identifier. Split compound sentences: a sentence that contains a name, a date and a statistic may need three separate checks. Mark every number, quotation, named person or organisation, legal or policy statement, recommendation and citation. Do not let a long paragraph hide several independent assertions.

Trace each important claim to the original
Open the cited link rather than trusting the title or the model’s description. Confirm that the page or document exists, is the intended work and contains the information attributed to it. Prefer legislation, regulators, official statistics, standards bodies, original research and first-party documentation. A news summary or an AI-generated bibliography can help you locate a source, but it is not the source itself.

Check currency and context
Record the publication or update date, jurisdiction, edition, definitions, exceptions and effective date. Look for a replacement, amendment or archived version. A statement that was accurate last year may no longer describe the current rule; a national statistic may not apply to a local population; and a technical instruction may depend on a particular software version.

Confirm consequential claims independently
For a claim that could change a decision, use a second authoritative source or ask a qualified subject-matter expert. Independent confirmation is especially important when a source is ambiguous, the claim is novel or contested, or the model has made a recommendation rather than merely summarising evidence. If two credible sources disagree, preserve both positions and escalate instead of selecting the one that sounds more certain.

Record the evidence
Maintain a simple claim ledger with the claim, source URL, source title, publication or update date, exact supporting passage, reviewer and status. Note qualifications such as “applies only to residents of…” or “estimate, not a measured count.” This record lets a colleague reproduce your check and makes later corrections possible when a source changes.

Edit, disclose and take responsibility
Remove unsupported statements, narrow wording that overreaches the evidence and preserve meaningful uncertainty. Follow your employer’s rules for disclosing AI assistance; some workplaces require a note in the document or a record in the project file. Do not pass along a citation you have not opened and checked, and do not imply that the tool, rather than you or your organisation, accepted the responsibility for the final work.

How do I check whether an AI citation really supports the claim?
Validate the relationship between the wording and the evidence, not just the existence of a link. A real source can still be irrelevant, outdated or quoted out of context.

- Resolve it: open the URL, confirm the domain and make sure the document is available rather than a search-result snippet.
- Identify it: check the title, authoring organisation, version and publication or update date against the citation.
- Locate the passage: use the document’s search function for the name, number or phrase in the claim. Read surrounding paragraphs, footnotes and definitions.
- Compare scope: check whether the source covers the same country, population, product edition, time period and legal status as the answer.
- Test the wording: distinguish what the source states from what the AI inferred. “May” is not “must,” a recommendation is not a requirement, and an estimate is not a measured fact.
- Check status: look for amendments, errata, withdrawn papers, superseded standards or newer official guidance.
Keep the exact supporting passage in your ledger. If you cannot find support after a reasonable search, label the claim unsupported and remove or rewrite it. Never treat a high citation count as a measure of accuracy.
What prompt can I use to make an AI answer easier to verify?
Ask for structure that exposes uncertainty and evidence. Prompts cannot prevent hallucinations, but they can make omissions and assumptions easier to see.
Answer only within [scope and date]. Create a table with these columns: claim, confidence reason, original source, source date, relevant qualification, and what remains unknown. Do not invent citations. If no authoritative source is available, write “not verified.” Separate description of evidence from your recommendation.For an existing response, Microsoft suggests a validation request such as: “Cross-check this response against the source material and highlight mismatches, unsupported claims, and missing qualifications.” Supply the actual source material when confidentiality rules permit. Then perform the checks yourself; asking the same model to approve its own answer is not independent confirmation.

Can search or retrieval features make AI answers reliable?
Search and retrieval can improve freshness and make sources easier to inspect, but they do not transfer responsibility to the system. OpenAI notes that search can provide more up-to-date, cited answers and that data-analysis tools can improve calculations while still advising users to verify important information directly.
Retrieval systems may fail to find the best document, select a secondary summary, misread a table or omit a relevant exception. The generated explanation can also overstate what the retrieved passage proves. Use retrieval to shorten the path to evidence, then apply the same claim-by-claim checks: open the source, compare scope and wording, and confirm important facts independently.
How should I protect confidential information while checking AI output?
Do not paste classified, sensitive, personal, proprietary or otherwise restricted information into an AI service unless your organisation has explicitly approved that handling. UK civil-service guidance warns that prompts can reveal confidential intent, and Canadian federal guidance says generated content is not authoritative and should be checked against trusted sources or by an expert.
- Use a redacted example and remove names, account numbers, unique identifiers and confidential figures.
- Submit the minimum information needed to perform the task; replace real values with clearly labelled placeholders.
- Use an approved enterprise environment and understand its retention, access and training settings before uploading documents.
- Keep the evidence ledger in the organisation’s approved storage, with access limited to people who need it.
- Remember that privacy controls reduce exposure; they do not validate the resulting answer.
When should I ask a human or subject-matter expert to review the answer?
Escalate when an error could affect health, safety, rights, money, legal compliance, employment, public communication or a specialised technical system. Escalation is also appropriate when sources disagree, the claim is novel or contested, or you lack enough domain knowledge to recognise a mistake.
- Clinical or safety use: a qualified professional must assess the person’s or system’s actual circumstances.
- Legal and policy use: an authorised reviewer should interpret the applicable rule, exceptions and jurisdiction.
- Financial or employment decisions: an accountable decision-maker should verify assumptions, eligibility and consequences.
- Public-facing material: an editor or communications owner should check names, quotations, statistics and reputational risk.
- Specialised engineering or security: an experienced operator should test instructions in a safe environment before production use.
The reviewer does not merely proofread. They decide whether the evidence supports the conclusion and whether the organisation is willing to act on it. The user remains the decision-maker; AI can organise evidence but cannot accept professional or organisational accountability.

Which verification method should I use for different kinds of work?
Compare methods by traceability, currency, coverage, domain fit, privacy, auditability and effort rather than by the number of citations produced.

Most weak reviews fail because they check presentation instead of evidence or stop at the first plausible source.
Trusting a confident tone
Fix: ignore style during the first pass. Highlight claims and require a source or an explicit “not verified” label for each material statement.

Checking only the links that look suspicious
Fix: inspect every citation attached to a consequential claim. Fabricated references are only one failure mode; a genuine but loosely related source can be just as misleading.
Reading the abstract or headline but not the qualification
Fix: read the surrounding passage, definitions, footnotes and exceptions. Copy the exact supporting text into the evidence record.
Assuming a current-looking page applies everywhere
Fix: record jurisdiction, effective date, edition and population. State those limits in the final text instead of silently generalising.
Asking the model to verify itself
Fix: use a separate reviewer, an original source or an independent authoritative publication. A second response from the same model is not independent evidence.

Leaving unsupported recommendations in a summary
Fix: separate evidence from advice, name the decision-maker and state what additional information would change the recommendation.
How can I make fact-checking a repeatable team process?
Turn the workflow into a small, visible control rather than an individual habit. At intake, record the intended use, risk level, owner and deadline. During drafting, require claim identifiers and source fields. Before release, have a reviewer sign off on critical claims and unresolved uncertainty.
- Intake: define audience, jurisdiction, cutoff date, confidentiality level and consequences of error.
- Draft: use a bounded prompt and retain the unedited AI response.
- Review: complete the claim ledger, open sources and obtain specialist review where required.
- Approval: record who accepted the evidence, what remains uncertain and when the material should be revisited.
- Maintenance: monitor sources that can change, such as policies, software documentation, regulations and statistics.
This process also helps with corrections. If a source changes, you can identify the affected claim instead of rereading an entire report from scratch.

Send or publish it only after you can explain which claims matter, where their evidence comes from, what qualifications apply and who reviewed the risky parts. OpenAI’s Help Center puts the principle plainly: “Use ChatGPT as a first draft, not a final source. Please check important information.” The UK Government similarly advises users to treat outputs with caution and challenge them using their own judgement and knowledge.
That standard is achievable in everyday work: use AI for speed and organisation, use authoritative sources for evidence, and use accountable people for decisions.
Frequently Asked Questions
Can I rely on an AI-generated bibliography if every item has a DOI or publisher page?
No. Persistent identifiers show that a record may exist, but they do not prove that the cited work contains the claimed result. Open the paper or publisher record, check the relevant passage and confirm that the model has not attributed a different study’s finding to it.
Is asking an AI to say “I don’t know” enough to stop hallucinations?
No. An uncertainty instruction can make caution more visible, but the model may still express an unsupported statement confidently. Treat the instruction as a formatting aid and continue with source and context checks.
What should I do when two authoritative sources disagree?
Do not average the claims or choose the newer-looking sentence automatically. Record the definitions, dates and jurisdictions behind each source, explain the conflict to the decision-maker and obtain specialist advice when the outcome matters.
Should I correct an AI draft or start over after finding one serious error?
Reassess the whole answer rather than fixing only the flagged sentence. A serious error can indicate that the model misunderstood the task or used an unreliable source, so re-check the surrounding claims and regenerate with tighter scope if necessary.
How can I preserve an audit trail for a shared document?
Give each material claim an identifier and store the source URL, title, date, supporting passage, reviewer and status alongside the draft version. Keep a change log when a source, qualification or conclusion changes so later readers can reconstruct the decision.
