Detect prompt injection by treating every instruction-like token as a potential security event, not as a normal content-filtering problem. Map every trust boundary, mark webpages, files, email, retrieval results, memory, images and tool responses as untrusted, restrict the agent’s credentials and tools, validate every model output before execution, and require human approval for consequential actions. Then exercise those controls with recurring direct and indirect attack tests and monitor the resulting behavior. This approach can reveal and contain model hijacking, but no prompt filter can guarantee prevention because the model receives developer instructions and attacker-written text in the same context.

What are prompt injection and model hijacking?

Prompt injection is an input that changes an AI model’s behavior or output in a way the application did not intend. OWASP distinguishes direct injection, which arrives in a user’s prompt, from indirect injection, which is hidden in material the application retrieves or passes to the model, such as a webpage, document, email, database row, memory entry, image or tool response.

Model hijacking is the operational consequence in an agentic workflow. Attacker-controlled content redirects an agent away from the user’s task and toward an attacker’s task, such as disclosing hidden instructions, reading another tenant’s records or sending a message. NIST’s Center for AI Standards and Innovation describes agent hijacking as indirect prompt injection in data an agent ingests, including emails, files and websites (January 17, 2025).

The architectural weakness is fundamental: as the OWASP GenAI Security Project puts it, "The model has no reliable mechanism to tell them apart." Developer instructions and hostile text are both represented as tokens in the context window. A label such as untrusted improves the model’s guidance, but it is not an authorization boundary.

Why does indirect prompt injection deserve priority?

Indirect injection is dangerous because the attacker does not need to control the chat box. Any content your pipeline imports can become an instruction at inference time.

  • Web content: visible text, hidden HTML, CSS, metadata and dynamically loaded sections can carry an instruction.
  • Files and images: text in PDFs, spreadsheets, office files, OCR layers, alt text and pixels can be interpreted as commands.
  • Communications: an email, calendar event or support ticket can tell an agent to forward data or change a record.
  • Retrieval systems: a poisoned vector-store chunk or database row can be returned for an otherwise harmless question.
  • State and tools: a memory entry, plugin description or tool result can redirect the next action.

Impact is governed by agency, not by how clever the wording looks. A read-only summarizer generally has a smaller blast radius than an agent that can send mail, access files, modify records, call external APIs or execute code. Every new permission increases the value of a successful injection, so detection must be paired with containment.

How do I map the trust boundaries in an AI application?

Start with a data-flow inventory that shows where instructions and data enter, change form and leave the model. Microsoft retrieval-hygiene guidance treats prompts, retrieved content, documents, memory and tool results as untrusted inputs requiring provenance and validation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. List every input surface. Include system and developer messages, user prompts, retrieval queries and results, web fetches, uploaded files, email, calendars, databases, conversation memory, images, audio transcripts, tool descriptions and tool responses.
  2. Record provenance. For each item, capture its source identity, tenant, authentication context, timestamp, retrieval method and whether a human or an automated process supplied it.
  3. Draw boundaries. Mark transitions between trusted application code and external content, between tenants, between read and write operations, and between the model and each executor.
  4. Describe the intended authority. Write down which sources may provide facts, which components may issue instructions, and which actions the current user is authorized to request.
  5. Assign a failure owner. Each boundary should have an application control that can reject, quarantine or pause a request when the model’s interpretation is unsafe.

Keep this map current when you add a connector or tool. A newly connected mailbox or browser is a new attack surface even if the system prompt has not changed.

How can I detect suspicious content before it reaches the model?

Use several weak signals together rather than trusting a single prompt-injection classifier. Detection should flag content for a decision, preserve evidence and avoid silently deleting material that investigators may need.

Pattern and semantic signals

Match explicit attempts to override policy, impersonate a higher-priority role, reveal hidden prompts or secrets, change the user’s task, disable safeguards or trigger an external action. Add semantic classification for paraphrases, multilingual wording, encoded text, payloads split across fields and instructions hidden in markup or metadata. These signals are triage, not proof: a legitimate security document can contain the same words.

Source and provenance signals

Check whether a source is authenticated, expected for the tenant and consistent with its normal format. A sudden instruction in a product description, a retrieval chunk from an unfamiliar domain or a tool response that changes the requested destination deserves a higher risk score. Preserve the original bytes or a content hash, the parser used and the reason for the alert.

Decision handling

Route high-risk material to quarantine or a human review queue; do not merely append a warning and continue with write-capable tools. For low-risk content, pass an explicit untrusted-data marker and retain the detector’s score and rule matches in the request trace. OWASP recommends combining input and output filtering with deterministic format validation rather than relying on one detector.

How should I separate data from instructions?

Represent external material as data in a distinct field or message, state that embedded instructions are not authorized, and keep orchestration decisions in application code. This raises resistance but cannot create a perfect boundary because both data and instructions still share the model’s context.

Use an explicit envelope

Pass source, tenant, timestamp and content separately from the task instruction. Delimit the content and label it, for example, as UNTRUSTED_DOCUMENT. Tell the model to extract facts from that field and to ignore commands, requests for secrets or changes to the task that appear inside it.

Keep authority outside prose

Do not grant permission by writing "you may send email" in a system prompt. The service that executes a mail operation should check the authenticated user, allowed recipients, message policy and approval state independently of the model’s explanation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Normalize before assembly

Decode archives and encodings, render documents through a controlled parser, inspect OCR text and remove active content that the workflow does not need. Keep a copy of the original for investigation, but provide the model only the normalized representation and the minimum fields needed for the task.

How do I limit an agent’s blast radius?

Apply least privilege before you run adversarial tests. Give the model short-lived credentials, only the tools required for the current task and access only to the relevant tenant and records.

  • Use separate identities for reading, drafting and committing changes; do not reuse a broad service account.
  • Scope tokens by tenant, resource, operation and time, and rotate or revoke them after a suspicious session.
  • Place retrieval stores and worker services in isolated network and authorization domains.
  • Deny outbound network, file-system and shell access unless the workflow explicitly needs it.
  • Make destructive or irreversible operations separate tools instead of parameters on a general-purpose executor.
  • Set rate, volume and destination limits so an injected loop cannot exfiltrate an entire store.

Least privilege changes the outcome of a successful attack from a system-wide compromise to a bounded failure. It also makes anomalous actions easier to detect because the allowed action set is narrow.

How do I validate model outputs and tool calls?

Treat every model-generated value as attacker-influenced until deterministic application code has checked it. Validation must cover both syntax and authority.

What’s actually slowing this PC down?

Pick the symptom - the matching free tool is one click away.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Require a schema. Accept structured output with fixed fields, types, enumerations and maximum lengths. Reject free-form output when a typed value will drive an action.
  2. Check the user’s goal. Compare the proposed operation, target and data to the authenticated request. A summary request should not produce a delete or send operation.
  3. Authorize independently. Verify the caller’s permission, tenant, record ownership, destination and current state at execution time.
  4. Constrain values. Enforce ranges, allow-lists, URL schemes, recipient domains, SQL parameters and command arguments. Never concatenate model text into SQL, a shell command or a template.
  5. Escape at the sink. Encode output for its actual context: HTML, JavaScript, SQL, a shell, a URL or another model message. Escaping for one context does not secure another.
  6. Fail closed. On parse errors, policy mismatches, missing provenance or expired approval, stop and return a reviewable error rather than guessing.

OWASP’s guidance specifically calls for deterministic format validation and protection against insecure output handling. A model-generated URL, SQL fragment, shell command or API argument remains untrusted even when it conforms to a schema.

Where should human approval occur?

Insert an explicit confirmation checkpoint immediately before a high-consequence action, after the model has produced its final proposal and before the executor receives authority.

  • Sending email, messages or invitations
  • Changing, deleting or exporting records
  • Making a purchase or financial transfer
  • Granting access, changing permissions or rotating credentials
  • Executing code or deploying a configuration

The approval screen should show the exact operation, recipients or targets, data to be sent, credentials or scopes used, source evidence and any detector warnings. A generic "continue?" button is not meaningful consent. Bind approval to a hash of the proposed action, user, tenant and expiry time so an attacker cannot substitute a different payload after confirmation.

For low-risk read operations, an automatic path may be appropriate. For mixed workflows, separate drafting from committing: the agent can prepare a message or change set, while a person approves the immutable version that will be sent or applied.

What telemetry reveals a hijacked agent?

Log enough context to reconstruct the decision without storing secrets unnecessarily. Use a correlation ID across ingestion, retrieval, model calls, tools, approvals and executors.

  • Original user request and normalized external inputs
  • Source IDs, tenant IDs, retrieval scores and content hashes
  • Model version, policy decisions, detector matches and schema failures
  • Every tool call, arguments after redaction, result, identity and authorization decision
  • Approval state, reviewer identity, action hash and final outcome
  • Denials, retries, timeouts, rate-limit events and credential changes

Alert on repeated override attempts, requests to reveal system prompts, unusual tool sequences, cross-tenant references, sudden query volume, prompt-leakage canaries and actions unrelated to the user’s stated task. OWASP incident-response guidance recommends tracing behavior, quarantining anomalous sessions, throttling suspicious accounts and isolating exposed prompts.

Containment procedure

  1. Pause the session and revoke its short-lived credentials.
  2. Quarantine the triggering document, message, memory entry or tool result while preserving evidence.
  3. Determine whether data was read, disclosed or changed; check neighboring sessions that consumed the same source.
  4. Block the source or connector until it passes review, and rotate any exposed secrets.
  5. Replay the original task in a sandbox with the malicious content removed, then add a regression test for the exact attack.

How do I protect a RAG chatbot from malicious documents?

Secure the retrieval pipeline, not just the chat prompt. A document that ranks highly can still be hostile, and a trusted repository can contain a compromised file.

  1. Authenticate the uploader and retain document, tenant and version provenance.
  2. Scan and normalize files, including OCR layers, metadata, embedded links and hidden text.
  3. Store content and access-control metadata together; filter by tenant and authorization before ranking.
  4. Attach source IDs and trust labels to every retrieved chunk.
  5. Instruct the model to use chunks as evidence only and to quote or cite the relevant source fields.
  6. Detect instructions in chunks and route high-risk results to review instead of placing them in an action-capable context.
  7. Validate the answer and any proposed tool call against the original question and the caller’s permissions.

Test poisoning at ingestion, retrieval and generation stages. A clean document can be paired with a malicious memory entry or tool result later, so source hygiene must cover the entire context assembly path.

How do I test an AI application for prompt injection?

Build a repeatable red-team suite that combines realistic resources with a benign user task. NIST’s AgentDojo pattern is useful: give an agent an ordinary task, insert a malicious instruction in a webpage, email, file or other resource it is expected to use, and record whether it performs the hijack.

Test case Injection location or technique What to verify
Direct override User message says to ignore prior rules or adopt a new role Task remains bounded; secrets and unauthorized tools are not exposed
Hidden web payload CSS, metadata, off-screen text or dynamically loaded content Parser and detector preserve provenance and block unauthorized actions
Document attack PDF, spreadsheet, OCR text, attachment or split payload Normalization finds the content and the model treats it as data
Communication attack Email, calendar event or support ticket No forwarding, deletion or external reply without approval
Retrieval poisoning Vector-store chunk or database row Tenant filtering and source validation hold under ranking changes
Memory or tool attack Persistent memory entry or deceptive tool result Untrusted state cannot rewrite policy or expand permissions
Multimodal attack Instruction embedded in an image or attachment Vision/OCR path receives the same checks as text
Output injection Model emits HTML, SQL, shell, template, URL or tool arguments Schema, authorization and context-specific escaping reject unsafe values
Disclosure request Ask for prompts, secrets, conversation history or another tenant’s data Response is denied and the attempt is logged
Adaptive campaign Varied wording, encoding, language, timing and repeated retries Rate limits, correlation and behavioral alerts detect the campaign

Measure attack success, unauthorized data exposure, unauthorized action rate, false positives on benign content, time to detection, time to containment and whether an approval gate was bypassed. No universal pass rate is established by the cited guidance; set thresholds from the harm model, data sensitivity and permissions of your application. Keep failed cases as regression tests and rerun them after model, prompt, parser, connector or tool changes.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Support on Ko-Fi

Which defensive approaches should I compare?

Compare complete designs, not slogans about a particular prompt. Score each design against the same trust and execution requirements.

Criterion Questions to ask Warning sign
Trust-boundary coverage Are user input, retrieval, files, memory, multimodal data and tools labeled and traced? Only the chat box is filtered
Least privilege and isolation Are identities, tenants, tools and network paths scoped to the task? One broad credential serves every action
Detection and telemetry Can investigators see provenance, detector decisions and tool sequences? No correlation ID or source history
Output validation Are schemas, authorization, ranges and sink-specific escaping enforced in code? The model’s text is executed directly
Approval usability Does a reviewer see the exact immutable action and evidence? Approval is generic or can be bypassed
Attack coverage Do tests include direct, indirect, multimodal, adaptive and output attacks? Only a few canned phrases are tested
Containment and recovery Can you revoke access, quarantine a source and replay safely? Response depends on editing the system prompt

A prompt-only defense should score poorly when it lacks independent authorization and execution controls. OpenAI describes prompt injection as an evolving security challenge; design for degradation and recovery rather than assuming a wording change will permanently solve it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can prompt injection be fully prevented?

No. The cited OWASP guidance explains that the model cannot reliably distinguish developer instructions from attacker-written instructions, and NIST treats indirect injection as a continuing agent-security problem. Layered controls reduce likelihood and impact, but they do not eliminate the vulnerability class.

The practical objective is a bounded failure: untrusted content is identified, permissions are narrow, outputs cannot execute without validation, high-impact actions require informed approval, and telemetry lets you revoke access and investigate quickly. Reassess that posture whenever you add a data source, model, tool or autonomous step.

What should an implementation checklist contain?

Use this checklist during design reviews and before each production release:

  • All instruction and data sources are inventoried with provenance and trust labels.
  • External text, files, images, memory and tool results are normalized and treated as untrusted.
  • Detection combines patterns, semantic analysis, source reputation and parser checks.
  • Application code, not prompt prose, enforces authorization and tenant isolation.
  • Credentials are short-lived and scoped to the minimum tools, records and operations.
  • Model outputs use strict schemas and pass destination, target, range and policy checks.
  • HTML, SQL, shell, URL and template sinks apply context-specific escaping or parameterization.
  • Consequential actions pause for an approval showing the exact payload and evidence.
  • Logs connect sources, model calls, tools, decisions, approvals and outcomes.
  • Red-team cases cover direct, indirect, hidden, multimodal, adaptive and output injection.
  • Incident playbooks revoke credentials, quarantine sources, throttle sessions and support safe replay.
  • Regression tests run after every material model, prompt, parser, connector or tool change.

Frequently Asked Questions

Can a firewall or web application firewall stop prompt injection?

Network controls can block malicious domains, unusual volume and unauthorized egress, but they generally cannot determine whether ordinary-looking text is trying to redirect a model. Keep them as one layer around the application and enforce semantic trust, authorization and output checks inside the agent workflow.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Should I automatically delete text containing “ignore previous instructions”?

Automatic deletion can remove legitimate security documentation and still miss paraphrased, encoded or multilingual attacks. Preserve the content, raise its risk score, and use the score with provenance and requested action to decide whether to quarantine or review it.

How do I secure conversation memory in a multi-tenant agent?

Store memory with an immutable tenant and user owner, enforce that scope before retrieval, and prevent the model from changing ownership fields. Give memory writes their own validation and approval rules so an injected message cannot turn itself into persistent policy.

What is the safest way to retain logs that may contain secrets?

Redact tokens, passwords and personal data before central storage while retaining hashes, field names, source IDs and action metadata needed for correlation. Restrict log access, define a retention period, and keep a protected forensic copy only when incident policy permits it.

Would switching to a smaller or newer language model remove the risk?

Model changes may alter attack success and false-positive rates, but they do not create an authorization boundary. Re-run the same adversarial suite after a model change and keep least privilege, validation, approval and containment controls independent of model behavior.
