Design approval and rollback as one control system, not as an emergency add-on. First classify every action by potential impact, likelihood of harm, and reversibility. Require a trained, authorized person to approve high-impact or hard-to-reverse actions before execution; technically prevent the AI from bypassing that gate; continuously monitor what happens; and provide tested pause, override, rollback, shutdown, and manual-fallback paths. The Canadian cyber guidance, UK National Cyber Security Centre (NCSC), Australian National AI Centre, European Commission, EU AI Act materials, and NIST guidance all support this risk-based approach. The exact legal duties depend on your system, sector, and jurisdiction, so treat the workflow below as an engineering and governance pattern to adapt with legal and operational owners.
What makes an AI action high-impact?
An action is high-impact when a mistaken, unauthorized, or poorly timed execution could seriously affect people, money, access, safety, essential services, regulated records, or the organization’s ability to operate. Impact is only one dimension: a low-probability action may still require approval if it is irreversible, difficult to detect, or impossible to contain quickly.
Inventory actions rather than judging the AI application as a whole. An assistant that only drafts text may be low risk, while the same system becomes high risk when it can send messages, change permissions, issue payments, delete records, alter production systems, or make decisions about people.
Recommended Free Tools
| Action category | Typical examples | Default control |
|---|---|---|
| Low impact and reversible | Formatting a report, creating a draft, querying non-sensitive data | Run within documented limits; log the activity and sample it for quality |
| Moderate impact or bounded external effect | Updating a non-critical ticket, changing a reversible configuration, sending an internal notification | Predefined limits, monitoring, and human-on-the-loop intervention |
| High impact or hard to reverse | Deleting critical records, changing privileged access, making network egress changes, resetting systems, releasing funds, sending an external legal or safety notice | Human approval before execution, with a tested recovery or compensating-action plan |
| Prohibited | Actions barred by law, policy, segregation-of-duties rules, or safety constraints | Block technically; do not offer an approval route unless policy owners explicitly change the prohibition |
The Canadian guidance specifically identifies system resets, network egress, and deletion of critical records as examples that may need prior approval. Your own classification should also consider affected people, data sensitivity, blast radius, time pressure, detectability of failure, and whether a safe alternative exists.

How should I classify actions before choosing an approval gate?
Create an action register with one row for every tool call or external effect the AI can trigger. Classify the action outside the agent, using accountable policy owners; the agent must not decide whether its own approval requirement applies.
- Describe the effect. Record the verb, target resource, actor identity, required privileges, data involved, and whether the effect is external or internal.
- Estimate impact. Ask what happens if the action is wrong, repeated, delayed, exposed, or applied to the wrong target. Consider financial, safety, privacy, security, legal, reputational, and service-continuity consequences.
- Assess likelihood and exposure. Include model error, prompt injection, compromised tools, stale data, ambiguous instructions, and human misunderstanding. Note how many records, users, systems, or jurisdictions could be affected.
- Assess reversibility. Distinguish a true restore from a compensating action. A database snapshot may restore data, but it may not retract an email, undo a disclosure, reverse a market transaction, or erase copies already made.
- Set the control class. Mark the action as allowed within limits, approval-required, or prohibited. Record the policy owner, review date, and conditions that would raise or lower the class.
Use explicit thresholds where your environment supports them, such as named systems, transaction limits, data classes, or time windows. Do not invent a universal risk score or rollback-time target: official guidance provides qualitative factors, not a single numerical formula.
When should a human approve an AI action?
Require approval before execution when the potential consequence is significant, reversal is incomplete or slow, the action affects a person’s rights or access, or policy requires a named decision-maker. Prior approval is the safest default for high-impact actions; monitoring after the fact is not an equivalent control when harm can occur immediately.
Human-in-the-loop
A person reviews and approves each proposed action before it runs. Use this for high-impact, privileged, externally visible, or difficult-to-reverse effects. It introduces delay and reviewer workload, so design queue priorities and escalation for urgent cases rather than silently bypassing the gate.
Human-on-the-loop
The system acts within an authorized envelope while a person monitors and can intervene. This can suit moderate-risk, rapidly repeated actions only when monitoring is reliable, alerts arrive quickly, and intervention can stop the consequence before the damage grows. It is not a substitute for prior approval where policy classifies the action as approval-required.
Human-out-of-the-loop
The system acts autonomously without human review. This may be acceptable for low-impact, reversible work after an organizational risk decision, but it is inappropriate for actions that require prior approval. Document why autonomy is safe, what limits apply, and how the system will be stopped if assumptions change.

What should an approval request contain?
The reviewer needs enough context to make an informed decision, not merely an “Approve” button. The following fields are an implementation pattern derived from oversight requirements; no cited authority prescribes one universal screen.
- Proposed action: the exact operation, parameters, target, scope, and requested time.
- Reason and instruction: the user request, triggering event, policy basis, and business purpose.
- Expected effects: resources changed, people notified, permissions granted, data exposed, and downstream systems called.
- Evidence: source records, tool outputs, validation checks, and relevant timestamps. Preserve links or identifiers so the reviewer can inspect originals.
- Uncertainty: ambiguities, missing data, conflicting instructions, model confidence where meaningful, and assumptions that could change the decision.
- Recovery: whether the action is reversible, the restore point or compensating action available, estimated recovery owner, and known residual effects.
- Decision controls: approve, reject, request changes, or escalate; require a reason for rejection or override.
Show the smallest scope that accomplishes the task. A request to change one account should not present a broad wildcard permission. If the agent revises the action after approval, treat the revision as a new request unless policy explicitly defines which changes are immaterial.
How do I assign approvers and accountability?
Name a role or person with the competence, training, time, and authority to intervene. The EU AI Act’s oversight language emphasizes these capabilities, and NCSC guidance recommends named responsibility for agentic AI where unintended activity could have significant consequences.
Define the authority boundary
Specify which actions the approver may authorize, which require a second approver, and which are prohibited. Separate request, approval, execution, and audit duties when fraud, conflict of interest, or privileged access is a concern.
Provide coverage and escalation
Document primary and backup approvers, service hours, maximum queue age, and the escalation path for an unavailable reviewer. For emergency work, define an emergency authority and require retrospective review; do not make “no one was available” an implicit bypass.
Train for the actual interface
Training should cover action classes, common model and tool failures, prompt-injection symptoms, evidence quality, privacy handling, rejection reasons, pause controls, rollback, and shutdown. Exercise these skills rather than relying on a policy document alone.

How can I enforce the approval gate technically?
Make approval a machine-enforced prerequisite, not a convention in a chat channel. The control plane should validate that the approval is authentic, current, scoped to the exact action, and issued by an authorized role before execution.
- Intercept the tool call. Route every external effect through a policy enforcement point. Do not allow a direct credential or alternate tool path that skips it.
- Evaluate policy outside the model. Match action type, target, data class, actor, environment, and risk class against centrally managed rules.
- Create an immutable request. Bind the request to a precise payload, resource identifiers, scope, version, and expiry time. If any material field changes, invalidate the approval.
- Authenticate and authorize the approver. Verify identity, role, separation-of-duties requirements, and any required second approval.
- Expire stale decisions. A decision made for an earlier system state, price, recipient, or incident should not remain valid indefinitely.
- Execute with least privilege. Issue narrowly scoped, short-lived credentials or capabilities only after approval.
- Log the full chain. Retain request, evidence references, policy result, approver, decision, execution result, timestamps, and any subsequent override or recovery.
- Fail closed for uncertainty. If the policy service, identity check, or approval record is unavailable, pause the action and route it to the defined manual process.
These implementation details operationalize NCSC advice that approvals should be guaranteed and gated. They are engineering recommendations, not a complete regulatory checklist.
How should monitoring and intervention work after deployment?
Monitoring must detect both bad outcomes and attempts to defeat the control. The NCSC recommends real-time monitoring, alerts, and the ability to stop agents; the European Commission describes monitoring operation and action on identified risks or serious incidents for deployers of high-risk AI systems.

- Record model, prompt or event, tool version, policy version, input data references, approval identity, and execution outcome so an investigator can reconstruct events.
- Give operators a prominent pause control that prevents new actions while preserving evidence. Define who may use it and what services remain available.
- Provide an override only to an authorized role, with a reason, scope, expiry, and post-event review.
- Test that alerts reach a person who can act; an alert in an unattended dashboard is not an intervention capability.
How do I design rollback when an AI action fails?
Separate stopping future activity from repairing state already changed. Pause or shut down the agent first when continued execution could enlarge the incident. Then restore, compensate, or remediate according to the action’s actual reversibility.
| Failure type | Immediate response | Recovery pattern |
|---|---|---|
| Queued but not executed | Cancel the queue and revoke pending credentials | Review the request and preserve the audit record |
| Partially executed reversible change | Pause further actions and identify the exact completed scope | Run an idempotent, tested restore or compensating transaction; verify the resulting state |
| Irreversible external effect | Stop related activity and contain access or disclosure | Use compensating communication, correction, cancellation, notification, or remediation; involve incident and legal teams as required |
| Unknown execution state | Assume neither success nor failure; prevent duplicate retries | Reconcile authoritative systems, then continue or recover under an incident procedure |
| System-wide unsafe behavior | Use the kill or shutdown control | Switch to the independent fallback, preserve records, investigate, and authorize a controlled restart or retirement |
Make rollback testable
Use versioned configuration, snapshots where appropriate, idempotent operations, transaction boundaries, and reconciliation checks. A rollback script that has never been run may fail because dependencies, credentials, external recipients, or data schemas changed. Test partial execution, duplicate requests, unavailable backups, corrupted state, and recovery during peak load.
Define shutdown criteria
Write objective triggers for pausing or retiring the system: repeated out-of-policy actions, inability to verify approvals, unexplained access changes, material data loss, unsafe output patterns, or loss of monitoring. Name the role that can invoke shutdown and the role that can authorize restart. Australian implementation guidance recommends defining shutdown criteria, responsible roles, record preservation, and impact planning.

What fallback keeps a critical service running?
For every critical function, document an alternative pathway that does not depend on the AI system or the same failing component. This may be a manual procedure, a constrained legacy workflow, a second independently operated service, or a preapproved emergency process.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- Identify the minimum service that must continue and the maximum tolerable interruption for your organization.
- Keep current runbooks, contact lists, credentials, forms, and data access needed for the fallback.
- Assign a human owner and train them before an incident.
- Reconcile work performed during fallback with the AI system before resuming automation.
- Plan for retirement: preserve required records, notify affected parties, revoke integrations, and move the function to its replacement.
Australia’s National AI Centre specifically recommends alternative pathways for critical functions when an AI system malfunctions or is retired. The fallback should be safer, not merely faster; do not transfer an unreviewed AI decision into a manual queue without the information needed to check it.
How should incidents, records, and learning be managed?
Create an incident procedure before launch. It should define severity, triage, containment, notification, evidence preservation, recovery authority, and post-incident review. NIST’s Generative AI Profile supports documented incident roles and response procedures, while European requirements may impose monitoring and serious-incident obligations for systems classified as high risk.
After an event, compare the proposed action, approved action, executed action, and observed effect. Determine whether the failure came from the model, data, tool, policy, identity, reviewer, monitoring, or recovery path. Update the action register, gate conditions, training, tests, and fallback when the evidence warrants it. Preserve records needed for accountability and applicable legal duties.
What does the EU legal guidance change?
It changes the compliance analysis, not the basic engineering principle. The European Commission says deployers of high-risk AI systems must monitor operation, act on identified risks or serious incidents, and assign human oversight to sufficiently equipped people. EU AI Act Recital 73 discusses oversight constraints that the system cannot override and personnel with the competence, training, and authority to intervene.
Those materials do not mean every consequential AI action everywhere is legally classified as high risk. Applicability depends on the system’s classification, purpose, deployment context, provider or deployer role, and jurisdiction. Have legal and compliance owners map the workflow to the rules that actually apply before relying on a generic “human approval” label.

What should I rehearse before going live?
Run scenario-based exercises with the real interface, identities, alerts, and recovery tools. At minimum, test:
- An approver rejects a valid-looking request because evidence is incomplete.
- The AI changes a material parameter after approval.
- A tool partially succeeds and then times out.
- The approval service or identity provider is unavailable.
- A prompt injection attempts to trigger a prohibited action.
- Monitoring detects an out-of-scope action and an operator pauses the agent.
- Rollback cannot restore an external effect and compensating action is required.
- The AI is shut down and the critical function moves to its manual or alternate path.
Measure whether people can understand the request, make a decision within the available time, stop execution, reconstruct what happened, and keep the service operating. Revise the design when any of those outcomes depends on an undocumented assumption.
Frequently Asked Questions
Can an AI agent approve its own high-risk action?
No. Approval policy should be determined and enforced by accountable designers or operations, outside the agent. The agent may prepare evidence or request approval, but it must not decide that the gate does not apply.
What if an action cannot be rolled back completely?
Stop related execution, contain further exposure, and use compensating actions such as correction, cancellation, notification, access revocation, or affected-party remediation. Record the residual impact; calling a compensating action a restore can hide an important limitation.
How often should approval rules be reviewed?
Review them whenever the agent gains a tool, privilege, data source, or new use case, and after incidents, near misses, major system changes, or changes in applicable law. Keep a named owner and a review date in the action register rather than relying on an informal calendar reminder.
Should every AI output require a human review?
Not necessarily. Review the external effect and its consequence profile, not simply whether a model generated text. Low-impact, reversible work can run within limits when monitoring and fallback are credible; high-impact or hard-to-reverse actions need prior approval.
