When an AI agent appears stuck, do not immediately retry it. First freeze the run and inspect whether its state is changing, whether the same tool and arguments recur, whether token use is climbing, and whether elapsed time is abnormal. A single warning sign is inconclusive; the combination of repeated actions and no meaningful state progress is the strongest evidence of a loop. Capture the execution trace, identify the first divergence, then fix the termination condition, tool choice, state update, verifier, or retry policy that caused the behavior. Finally, add hard runtime limits and turn the failure into a regression test.

What does a stuck or looping AI agent look like?

A stuck agent is not merely slow. It is an agent whose execution no longer moves the task toward a valid completion state. It may call the same tool with identical arguments, alternate between two calls, retry after an unchanged result, or consume tokens while producing no new external effect.

Camunda notes that no single health indicator proves an agent is stuck. A credible diagnosis combines several observations:

Signal What to inspect Why it matters
State stagnation The task state, files, records, or workflow status are unchanged across iterations. An action that does not change the state cannot advance the task.
Repeated calls The same tool and nearly identical normalized arguments recur. Identical inputs usually produce the same observation, so repeating them adds no information.
Growing usage Tokens, model calls, or retries rise without a corresponding state transition. Cost can increase even when the agent is not making progress.
Abnormal duration Wall-clock time is disproportionate to comparable successful runs. Latency helps distinguish ordinary work from an execution that has lost its path to completion.
Missing stop reason The run ends only when a platform timeout or user cancellation occurs. The agent lacks a deliberate completion or escalation path.

One slow API call, one transient error, or one repeated-looking request is not enough to label a loop. Look for the pattern across consecutive iterations and compare observations with the state the agent believes it has changed.

How can I tell whether the agent is making progress?

Define progress as an observable state transition, not as another model response. For each iteration, record the state before the action, the selected action, the tool result, and the state after the action. If the “after” state is equivalent to the “before” state and the next action is unchanged, the retry is not productive.

Use a progress ledger

  • Goal: the user’s requested outcome in a testable form.
  • Current state: the facts, files, records, permissions, and workflow status available to the agent.
  • Action: the tool name and exact arguments selected for this iteration.
  • Observation: the structured result, including what changed and what did not.
  • Next-state predicate: the condition that must be true before another action is useful.
  • Completion predicate: the condition that ends the run successfully.

A progress ledger makes a silent failure visible. For example, “write configuration” is not progress if the write was denied and the subsequent read still shows the old configuration. The agent needs an explicit observation such as write_succeeded: false and a policy that chooses another strategy or escalates instead of issuing the same write again.

Detect cycles, not just identical calls

Normalize arguments before comparing them: sort keys in JSON objects, remove request IDs and timestamps, and canonicalize equivalent paths. Hash the normalized tool name plus arguments. Then inspect both exact repeats and short cycles such as read → update → read → update where the update never changes the read result. A cycle detector should also compare the returned state, because two different calls can still be semantically redundant.

What should an execution trace contain?

Start debugging from a complete trace rather than launching another retry. Databricks describes a trace as a structured record of the complete execution, including intermediate language-model calls, retrievals, tool invocations, latency, and errors. JetBrains and Google Cloud likewise emphasize iteration details, state changes, token usage, and tool/API exchanges.

Trace field Minimum useful detail Debugging question it answers
User goal Original request plus any constraints and expected output. What was the agent actually trying to finish?
Model calls Prompt or message version, model identifier, response, timestamp, latency, and token counts. Did a prompt change, context overflow, or budget affect behavior?
Tool spans Tool name, exact arguments, normalized signature, start/end time, result, and error. Which action repeated, and did its output change?
State and memory Relevant state snapshots or diffs before and after each action. Did the external or internal state move?
Retries Attempt number, retry reason, backoff, and policy that authorized it. Was a transient failure treated as deterministic?
Verification Verifier input, output, score, and failure explanation. Can the success check ever return true?
Stop data Stop reason, final state, escalation reason, and last successful action. Why did the run end, and what should happen next?

Keep sensitive values protected, but do not replace every argument with a redacted placeholder. A debugger needs enough structure to distinguish a genuine new request from the same request with a changed record ID. Store a trace ID that links model, retrieval, memory, verifier, and tool spans so a single iteration can be reconstructed end to end.

How do I triage a runaway run safely?

Use this order so you preserve evidence before changing the conditions that produced the failure.

  1. Freeze the run. Cancel or pause an unsafe execution, especially one that can write data, send messages, place orders, or modify infrastructure. Save the trace, current external state, and any queued actions.
  2. Check for a pending human step. In an interactive environment, look for an unanswered question, approval request, authentication prompt, or command waiting for input. VS Code recommends checking these conditions, along with running commands, before assuming the model is looping.
  3. Find the earliest divergence. Review recent tool calls, terminal output, changed files, and verifier results. The first incorrect tool selection or stale observation is usually more useful than the final repeated call.
  4. Group actions by normalized signature. Count identical calls, alternating cycles, and retries whose outputs are unchanged. Record the first iteration at which the repeat threshold was crossed.
  5. Compare observations with state. If a tool returns the same result and the state is unchanged, stop retrying it. If the tool was inappropriate for the task, correct routing and tool documentation.
  6. Test the success condition. Ask whether the verifier can ever return success with the available data. A vague goal or impossible verifier leaves the model without a valid stopping signal.
  7. Inspect permissions and side effects. Check denied writes, read-after-write consistency, stale caches, transaction rollbacks, idempotency keys, and asynchronous jobs that have not completed. An agent may repeat an action because the effect occurred but is not visible yet, or because it never occurred at all.

Do not delete the trace after cancelling the run. Preserve the external state and the exact stop point so you can reproduce the failure without risking another uncontrolled execution.

Why do agents loop, and how do I fix each cause?

There is no explicit termination condition

An instruction such as “keep working until it is perfect” does not define completion. Add a machine-checkable completion predicate and a bounded failure path. Microsoft’s Agent Framework guidance is direct: autonomous loops should always be bounded because completion can fail and a model can stall.

Write the predicate in terms of observable state: required fields exist, a test command returns success, a record has a particular status, or a verifier reaches a specified threshold. If the predicate is false after the allowed attempts, return a blocker and escalate instead of silently continuing.

The policy is blind to repeated actions

Maintain a per-run history of normalized tool signatures. After a small number of unsuccessful identical actions, force a stop or replan. AWS specifically recommends termination after three identical repeated actions as an example guardrail. Treat that number as a starting policy to tune against the workload, not a universal law.

When a repeat is detected, pass the agent a structured event containing the repeated signature, attempt count, last result, and available alternatives. A useful response is “the last three calls produced no state change; explain the blocker, choose a different tool, or ask for help,” not another natural-language warning with no enforcement.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The agent selects the wrong tool

Incorrect tool selection is a common loop cause. Improve each tool description with its intended use, exclusions, argument schema, side effects, expected result, and a short positive and negative example. Make mutually exclusive tools explicit: for example, distinguish a read-only lookup from a mutation and identify which one supports a given resource type.

Verify the selected tool in traces. If the model repeatedly uses a search tool where a transaction-status tool is required, changing the prompt alone is unlikely to be reliable; fix the routing description, available tool set, or planner policy.

The state never updates or the observation is ambiguous

Return structured observations that report both the attempted operation and its effect. Include fields such as changed, version_before, version_after, permission_denied, and pending where they apply. Make the next-step policy depend on those fields.

The agent retries without new information

Separate transient failures from deterministic failures. Retry network timeouts, rate limits, and temporary service-unavailable responses with a finite count and exponential backoff. For schema errors, permission denials, invalid arguments, and reproducible verifier failures, change the strategy or escalate; another identical request cannot repair a deterministic problem.

The prompt says “never give up”

Replace persistence slogans with explicit stop rules. A practical instruction is: “After three unsuccessful identical actions, explain the blocker and ask for help. Stop immediately on permission denial or an invalid argument. Stop when the completion predicate is true.” Enforcement must live in the runtime as well as the prompt, because a model can ignore or misinterpret prose.

The verifier can never succeed

Run the verifier independently on known-good and known-bad states. Check that it reads the same version of the data the agent modified, that its required fields are attainable, and that failure messages identify the missing condition. If the verifier is opaque, the model may keep changing unrelated parts of the task in search of a signal it cannot obtain.

Which runtime guardrails should every autonomous agent have?

Use multiple independent bounds. A single iteration limit will not protect against one very slow call, while a time limit alone will not contain rapid token consumption. Microsoft’s loop-agent examples use a completion evaluator and configurable maximum iterations.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Guardrail What it limits When it should fire
Maximum iterations Total planning and action cycles. The agent has not completed within a workload-specific step budget.
Wall-clock deadline Elapsed real time, including waiting on tools. The run exceeds the service-level deadline or user patience threshold.
Token budget Input and output tokens for the run or session. Usage grows without a proportional state transition.
Per-tool retry limit Attempts for one failing operation. The same transient operation has exceeded its retry policy.
Repeated-action threshold Identical or cycle-equivalent calls. The normalized signature repeats without new state.
Human escalation condition Risky or ambiguous work. Permission denial, conflicting instructions, irreversible side effect, or exhausted budget.

AWS documents example conversational-agent budgets of 5,000–10,000 tokens per session and 10–15 reasoning steps. Those are documented examples to tune against your workload, not universal defaults. Start with limits derived from successful traces, then leave headroom for normal variance.

Return a structured guardrail result

When any bound trips, return a predictable object rather than a generic timeout. It should contain:

How should I choose an observability approach?

Whether you use an in-house trace store or a managed platform, evaluate the system on the same four axes.

Axis Questions to ask
Trace completeness Does it capture model, retrieval, tool, memory, state, and verifier spans with a shared run ID?
Detection controls Can it hash repeated calls, detect short cycles, enforce step and token limits, and alert on abnormal latency?
Debug workflow Can engineers query traces, replay a run safely, annotate failures, build evaluation datasets, and roll back a prompt or tool version?
Operational fit What are the cloud integrations, data-residency options, access controls, cost, and latency overhead?

Databricks calls tracing the foundation for agent observability and quality. AWS describes an example trace with 177 spans and approximately 85,590 milliseconds of average latency as evidence of a loop. Its CloudWatch example shows 266.9K total tokens with a 0% error rate, demonstrating that an agent can loop without conventional errors. These figures illustrate what to look for in telemetry; they are not performance targets for every system.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Support on Ko-Fi

How do I verify that a loop fix really worked?

Do not validate a fix by watching one successful run. Databricks describes a repeatable quality cycle: trace executions, identify an issue, curate representative failures into an evaluation or golden dataset, write a scorer, change the agent, rerun the evaluation, and monitor production.

Build a regression case from the failure

  • Record the original user goal and relevant environment state.
  • Store the exact tool sequence that looped, including normalized arguments and outputs.
  • Define the expected state transition after each important action.
  • Set a maximum permitted repeat count and an explicit escalation message.
  • Include the permission, stale-read, verifier, or routing condition that triggered the loop.

Compare before and after

Metric What improvement looks like Caution
Completion rate More runs satisfy the completion predicate without unsafe side effects. A higher rate is not an improvement if failures are hidden by premature success.
Repeated-call count Fewer identical or cycle-equivalent calls per run. Some legitimate polling may repeat; pair this metric with state change.
Step count Fewer actions for the same successful task. Do not optimize away necessary verification.
Latency Shorter tail latency and fewer deadline stops. Separate tool latency from model and queue latency.
Token use Lower usage for equivalent outcomes. A short, incorrect answer is not a successful optimization.
Unintended side effects No duplicate writes, messages, or irreversible operations. Track this independently; it is a safety metric, not just a cost metric.

Run the regression against the same prompt, tool versions, permissions, and representative data that produced the incident. Then add nearby cases: a transient timeout, a denied write, a stale read, an impossible verifier, and a valid task that requires several legitimate iterations.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What should production monitoring alert on?

Alert on combinations rather than isolated thresholds. For example, a high token count alone may describe a difficult but successful task; high token count plus unchanged state and repeated signatures is a stronger incident signal.

  • Runs crossing the repeated-action threshold.
  • Runs ending at a deadline or maximum-iteration guardrail.
  • Rapid growth in tokens or tool calls without state-version changes.
  • Repeated permission denials, invalid-argument errors, or verifier failures.
  • Abnormal latency compared with successful runs for the same workflow.
  • Duplicate or conflicting external side effects.

Sample traces for routine analysis, but retain every trace that triggers a guardrail or affects an external system. Restrict access to sensitive prompts and tool arguments, and set retention according to your privacy and security requirements.

What is the fastest emergency checklist?

  1. Pause the run and prevent further external actions.
  2. Save the trace, current state, pending jobs, and last successful action.
  3. Check for an unanswered approval, question, authentication request, or running command.
  4. Identify repeated signatures and the first unchanged state.
  5. Inspect tool choice, permissions, read-after-write behavior, and verifier reachability.
  6. Apply a bounded fix: correct routing, expose state changes, change the retry strategy, or add a completion predicate.
  7. Replay in a sandbox with maximum iterations, time, tokens, and retries enforced.
  8. Convert the incident into an evaluation case before restoring production traffic.

What should the agent tell the user when it cannot continue?

Give a specific blocker rather than “something went wrong.” State what was attempted, what remained unchanged, which safety limit stopped the run, and what the user can provide or approve. For example: “The write was denied three times, so I stopped without further attempts. The last confirmed state is version 12; approve access or update the target path.” This preserves trust and gives a human a useful next action without exposing internal chain-of-thought.

Frequently Asked Questions

Can a zero-error run still be stuck?

Yes. An agent can receive successful HTTP responses while repeatedly performing an operation that changes nothing. Compare state versions, normalized tool signatures, token growth, and completion status instead of relying on error rates alone.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Should I increase the token or iteration limit when an agent loops?

Usually not as a first fix. A larger budget can make a faulty loop more expensive and slower; first determine whether the tool, state update, verifier, or stop policy is preventing progress, then tune limits from successful traces.

How do I distinguish legitimate polling from a loop?

Polling is legitimate when each observation can reveal a new state and the policy uses a bounded interval, timeout, and job status. Treat it as a loop when the result and state version remain unchanged beyond the allowed polling window or when the agent repeats a mutation instead of checking status.

What data should I redact from agent traces?

Remove secrets, authentication tokens, personal data, and sensitive payloads while retaining tool names, argument structure, identifiers sufficient to correlate calls, state versions, timings, and result categories. Use consistent hashing or reversible access-controlled references when exact values are unnecessary for routine debugging.

How can I stop duplicate side effects during retries?

Use idempotency keys or transaction identifiers for writes, record the server-confirmed result, and make retries query status before issuing the operation again. If the system cannot provide idempotency, require human approval for repeats that could create an irreversible or duplicated effect.
