To find out whether your information was included in a healthcare provider’s cyberattack, check the provider’s authentic breach notice and contact the provider through an official channel. The U.S. Department of Health and Human Services (HHS) Office for Civil Rights (OCR) breach portal can show whether an organization reported a qualifying large breach, but it does not let you search for your name or confirm whether your records were affected.
How to check whether your information was involved
- Look for a notice. Search your mail and email for a letter or message from the provider or a breach-response company acting on its behalf. A logo, caller ID, or unsolicited message alone does not establish that a notice is genuine.
- Verify the contact details independently. Go to the provider’s official website yourself or call a number you already know is official. Ask to be connected with its privacy office or the team handling the incident. If the notice gives an incident-specific number or reference number, use it only after you have verified the notice.
- Ask direct questions. Ask whether your information was among the affected records; which categories of information were involved; what steps you should take; and, if monitoring is offered, who is eligible, what it covers, how long it lasts, and the enrollment deadline.
- Check the OCR portal for context. Search the provider’s name in the HHS OCR breach portal. Treat a listing as organization-level information, not proof that your own records were involved. If there is no listing, that does not establish that no incident occurred or that you were unaffected.
- Follow the instructions in the verified notice. Take the actions that apply to the information it says was involved. Do not assume every healthcare breach includes Social Security numbers, financial-account details, or information that can be used for identity theft.
What the HHS breach portal can—and cannot—tell you
OCR’s public HIPAA breach listing covers reported incidents affecting 500 or more individuals. Its records include organization-level details such as the entity name, state, number of people affected, submission date, breach type, and location of compromised information. The portal is useful for checking whether a provider reported a large incident; it is not a patient-by-patient lookup.
Incidents affecting fewer than 500 people may not appear on that public list. OCR says it may investigate smaller breaches depending on its resources and priorities. The list therefore is not a complete record of every healthcare incident, and absence from it cannot answer whether your information was exposed.
How HIPAA breach notices are supposed to work
Under the HIPAA Breach Notification Rule, covered entities generally must notify affected individuals after a breach of unsecured protected health information (PHI). Individual notice must be provided without unreasonable delay and no later than 60 days after discovery. The 60-day limit is an outside deadline, not a promise that every notice will arrive on day 60.

Notice to an individual is generally in writing by first-class mail, or by email if the person agreed to receive electronic notices. If contact information is inadequate for 10 or more people, substitute notice can include a website posting for at least 90 days or notice through major local media, along with a toll-free number available for at least 90 days. If contact information is inadequate for fewer than 10 people, alternate written, telephone, or other notice is permitted.
A notice should explain the breach, the types of information involved, steps individuals should take, actions taken to investigate and mitigate the breach and prevent recurrence, and how to contact the organization. If a notice leaves out something you need to know, contact the provider through a verified channel and ask for clarification.

Why the provider’s reporting deadline may differ from your notice
The deadline for an organization to report a breach to the HHS Secretary is separate from its duty to notify affected people. For breaches affecting at least 500 people, the report is due without unreasonable delay and within 60 calendar days after discovery. For breaches affecting fewer than 500 people, the report is due within 60 days after the end of the calendar year in which the breach was discovered. These are entity reporting deadlines; they do not tell you when a particular person will receive notice.

This guidance describes the U.S. HIPAA process. Not every provider is a HIPAA covered entity, not every incident triggers HIPAA notice, and state laws or other federal rules may apply separately. Health apps and other services may fall under different requirements, so the HIPAA portal and timelines should not be treated as rules for every health-related service or for providers outside the United States.
