Do not treat an AI answer as verified evidence. Treat it as a bundle of claims: identify the claims that matter, open the original sources, compare them with an independent authoritative source, test assumptions and edge cases, and keep a person responsible for the decision. If a material claim cannot be confirmed, omit it or label it clearly as unverified.
A fluent or confident response is not a reliability score. Guidance from the OpenAI Help Center, the Technical University of Munich, the UK House of Commons Library and other public bodies warns that AI can produce incorrect facts, invented quotations or citations, incomplete explanations and confident answers to ambiguous questions.
How do I know if an AI answer is accurate?
Accuracy is established claim by claim, not by how polished the response sounds or which model produced it. Use a repeatable workflow that connects each consequential statement to evidence suitable for the decision you are about to make.
- Define the intended use. Decide whether the answer is for learning, publication, a purchase, code, a health or legal decision, a financial action, a safety procedure or an operational change.
- Split the response into claims. Separate facts, numbers, dates, names, quotations, recommendations, legal rules, causal explanations and predictions.
- Trace important claims to original evidence. Ask for sources, then open the documents, data, standards, laws or studies yourself.
- Check what the evidence actually says. Match the wording, date, geography, population, definitions and level of certainty.
- Corroborate consequential claims independently. Prefer a second source that did not simply copy the first.
- Test context and edge cases. Look for missing assumptions, exceptions, affected groups, bias and conditions under which the answer fails.
- Apply a risk-based stop rule. Obtain expert review or stop using the claim when an error could cause serious or irreversible harm.
This process follows the UK House of Commons Library’s practical principle that AI is “an assistant, not an authority.”

What should I verify first in an AI response?
Start with claims that could change what you do or what other people believe. Verification time is limited, so triage the response before researching every sentence.
Mark high-impact claim types
- Numbers and dates: prices, rates, limits, deadlines, release dates, measurements and statistics.
- Identities and quotations: names, job titles, organizations, attributed words and alleged eyewitness accounts.
- Rules and obligations: laws, regulations, policies, contracts, licensing terms and compliance requirements.
- Recommendations: instructions to buy, install, publish, vote, invest, diagnose, disclose or change a system.
- Causal statements: assertions that one action will cause a result, prevent a risk or explain an event.
- Current or contested information: breaking news, active incidents, rapidly changing products and disputed findings.
Separate fact from interpretation
Rewrite a long answer as short, testable statements. “The service supports encrypted backups in every plan” is a factual product claim. “It is the best choice for a small team” is a recommendation that depends on criteria. “The outage was caused by a database migration” is a causal claim requiring stronger evidence than a simple timestamp.
Record the exact wording before you check it. This prevents a source that supports a weaker statement from being used to validate a stronger one.
How do I trace an AI claim to a real source?
Ask the system to identify the source for each important claim, but regard the response as a lead rather than proof. Open the cited item yourself and confirm that it exists and supports the same statement.
Use the original record whenever possible
For a regulation, read the issuing government text. For a technical behavior, read the vendor’s current documentation or the relevant standard. For a statistic, inspect the dataset and its methodology. For a study, read the paper rather than relying on a summary. For an event, consult an official record and reputable reporting that identifies its evidence.
Microsoft’s validation guidance asks whether an answer accurately reflects the underlying files or data and whether key statements can be traced to a reliable source. Its practical rule is simple: if a statement cannot be traced, treat it as unconfirmed until you verify it another way.

Check that the citation is real and relevant
- Does the page, paper, dataset or document actually exist?
- Does the cited passage contain the claimed fact, rather than merely a related topic?
- Is the source about the same country, jurisdiction, product version, population or time period?
- Does the source support the AI’s certainty, or does it present a tentative result that the answer made definitive?
- Has the source been superseded, corrected, withdrawn or updated?
A genuine URL can still be irrelevant, outdated or misrepresented. The OSCE puts this plainly: even when an AI service provides a link to an original source, go to the resource and check whether the stated fact is actually there.

How do I evaluate the evidence behind a citation?
Read beyond the headline, search snippet or abstract. Evidence quality depends on fit: the material must answer your question under comparable conditions.
Match scope and conditions
Check the source’s date, geography, population, sample size, definitions, assumptions and measurement method. A national average may not describe a particular city; a rule in one jurisdiction may not apply in another; a benchmark on one hardware configuration may not predict your workload.
Match certainty to evidence
Look for words such as “may,” “associated with,” “in this sample” or “under these conditions.” An AI may turn a correlation into a cause, a preliminary result into a settled conclusion or a possibility into a guarantee. Preserve the source’s limits in your own wording.
Inspect quotations and numbers
Search for the complete quotation in the original document and check who said it, when and in what context. Recalculate percentages, units and totals when practical. Confirm whether a figure is a one-time observation, a recurring allowance, a forecast or a promotional value.

| Comparison criterion | What to ask |
|---|---|
| Authority | Is this a primary record, official statistic, standard, original study or issuing organization? |
| Independence | Did the second source collect or analyze evidence independently, or repeat the same press release? |
| Recency | Does its publication or update date fit the question and the period the AI described? |
| Scope | Do geography, population, product edition, definitions and conditions match? |
| Methods | Does it explain how the claim was measured, calculated or selected? |
| Reviewer expertise | Has a qualified person or institution examined the evidence? |
| Error consequence | How much corroboration is justified by the harm or irreversibility of being wrong? |
The University of Illinois recommends sourcing the claim, investigating the tool, finding a second tool or source, tracing to original evidence and testing the edges. The Government of Canada likewise advises reviewing generated content against trusted sources or an expert.
Resolve disagreement instead of averaging it
When credible sources differ, identify why. They may use different dates, definitions, jurisdictions, populations or methods. Do not average incompatible figures or choose the answer that sounds most confident. State the disagreement and the condition under which each source applies.
How can I check context, omissions and edge cases?
An answer can contain individually true statements and still mislead by leaving out a condition or affected group. Microsoft’s four-part check provides a practical audit.
| Check | Questions to ask | Typical failure |
|---|---|---|
| Source | Does the response match the underlying material? | Blended sources, invented details or an overstated conclusion. |
| Verified | Which names, dates, numbers and recommendations need independent confirmation? | A plausible detail is accepted without evidence. |
| Context | What assumptions, caveats, dependencies, risks or exceptions are missing? | A rule is presented without its jurisdiction or limitation. |
| Resilient | Would the answer hold for another audience, changed conditions or an alternate interpretation? | A small wording change produces a different conclusion. |
Ask whose perspective is absent
British Columbia guidance specifically calls for checking names, dates, numbers, statistics, laws, policies, quotations, citations and links. It also warns that generated text can omit perspectives or reproduce bias. Ask who bears the risk, whose experiences are missing and whether the source’s sample or incentives exclude them.
Test boundary conditions
Try the cases at the edges: the smallest and largest input, an unusual location, an older version, a different user group, a missing field or a conflicting requirement. If the answer depends on an unstated assumption, make that assumption explicit before relying on it.
What prompts help reveal a weak AI answer?
Use follow-up prompts to expose uncertainty, not to make the model sound more certain. Prompting can improve the audit trail, but it cannot replace external evidence.

- Ask, “List every factual claim in your previous answer, including numbers, dates, names, quotations and recommendations.”
- Ask, “For each claim, provide the original source, the relevant passage and the date and jurisdiction it covers.”
- Ask, “Which parts are uncertain, inferred or based on an assumption rather than directly supported?”
- Ask, “What evidence would disprove this conclusion, and what edge cases could make it fail?”
- Rephrase the original question with a different ordering, audience or set of constraints and compare the material conclusions.
If small wording changes produce materially different answers, lower your trust and investigate more deeply. A model’s ability to produce a neat explanation is not evidence that its underlying claims are stable.

There is no universal pass percentage for arbitrary AI answers. NIST defines accuracy as closeness to true or accepted values and reliability as the ability to perform as required under stated conditions; both depend on the intended use and those conditions.
| Risk level | Examples | Minimum sensible response |
|---|---|---|
| High | Health, law, finance, safety, security, employment, public communications, current events or personal data. | Verify every material claim with primary evidence and obtain qualified human review. Do not act when a suitable source cannot be established. |
| Medium | Production code, business operations, purchases, technical configuration or advice that could create cost or downtime. | Check documentation, test in a safe environment and have an experienced person review consequential changes. |
| Low | Brainstorming, non-consequential summaries or early drafting. | Verify before publication or reuse; keep uncertain details out of the final version. |
The more harmful or irreversible the error, the more independent evidence and expertise you need. If you cannot establish a suitable source, omit the claim or label it explicitly unverified rather than filling the gap with plausible wording.
What do accuracy and reliability mean in practice?
Accuracy is not a permanent property of a model. A response may be correct for one version, date, audience and set of conditions and wrong for another. Reliability asks whether the system performs as required under stated conditions; it does not mean every response is true.
Confidence, fluent prose and a high level of detail are presentation characteristics. They are not measurements of truth. The Technical University of Munich explains that likely or confident wording is not the same as verified wording, while the OpenAI Help Center advises using ChatGPT as a first draft, not a final source.

What are the most common verification mistakes?
| Mistake | Why it fails | Correction |
|---|---|---|
| Trusting tone or formatting | Confidence is generated language, not a reliability score. | Extract and test the claims independently. |
| Clicking a citation without reading it | The link may be fabricated, irrelevant or misrepresented. | Open the original and match the exact passage and scope. |
| Using one secondary article | Many pages may repeat the same unsupported assertion. | Trace to primary evidence and seek an independent source. |
| Checking only the headline | Important limitations often appear in methods, footnotes or legal text. | Read the relevant section, definitions and conditions. |
| Ignoring dates and jurisdictions | Rules, prices, products and events change or vary by location. | Record the date, edition, region and governing authority. |
| Asking the same model to “confirm” itself | The system can repeat the same unsupported claim more confidently. | Use external evidence and an appropriately qualified reviewer. |
| Forcing a single answer when sources conflict | Differences may reflect definitions or methods rather than one error. | Explain the disagreement and state which condition applies. |
How should I document a checked AI answer?
Keep a short verification record when the output will be published, reused or used in a consequential decision. Documentation makes later review possible when a source changes.

- Save the original prompt, response and date.
- List each material claim in your own words.
- Record the source, passage, publication or update date, geography and version.
- Note corroborating sources and any disagreement.
- Write the assumptions, exclusions and unresolved uncertainty.
- Identify the reviewer and the decision or publication that the check supports.
For team workflows, require a reviewer to sign off high-risk claims rather than treating an automated “fact checked” label as approval.
What does a complete final check look like?
Before you publish, send instructions, change code or act on an AI response, run this checklist:
- Have you defined the decision and the consequences of being wrong?
- Did you break the answer into individual, testable claims?
- Did you verify every important number, date, name, quotation, rule, citation and recommendation?
- Did you open the original evidence instead of relying on a link or summary?
- Do source date, geography, version, population and definitions match your use?
- Does the evidence support the same certainty expressed in the answer?
- Did you obtain an independent authoritative source for consequential claims?
- Did you test assumptions, omissions, bias and boundary cases?
- Did you ask a qualified reviewer when the subject is specialized or contested?
- Have you removed or clearly labeled claims that remain unverified?
The human user remains responsible for what an AI-generated answer is used to publish or do. Validation is the step between receiving an answer and trusting it.

Can an AI tool verify its own answer?
It can list assumptions, expose possible failure modes and suggest sources, but that is still a second generated response. Use those suggestions to plan checking; rely on documents, data, tests and qualified reviewers outside the model for the final decision.
What should I do when an AI citation does not exist?
Treat the associated claim as unverified immediately. Search for the underlying primary record using distinctive wording or identifiers, and if you cannot find suitable evidence, remove the claim or disclose that it could not be confirmed.
Can automated fact-checking replace a human reviewer?
Automated systems can flag dates, duplicate claims, broken links and contradictions efficiently. They generally cannot judge whether evidence fits a specialized context or whether an omitted risk changes the decision, so human review remains necessary for consequential use.
How do I handle an AI answer that is partly right?
Keep only the claims that pass source and context checks, and rewrite the answer so unsupported inferences do not inherit the credibility of verified facts. Mark the boundary between established evidence and interpretation.
