Build one living risk record for every AI use case, not a one-time questionnaire. Start by assigning an accountable owner and documenting the system’s purpose, users, data, integrations, lifecycle stage and affected people. Then map threats, potential harms and legal obligations; score likelihood and impact with evidence; record controls and residual risk; obtain an explicit approval or rejection; and monitor for drift, incidents, control failures and material changes.

A practical way to make the process usable is to organize it with the NIST AI Risk Management Framework’s Govern, Map, Measure and Manage functions, then connect each use case to the NIST security Risk Management Framework (RMF): Prepare, Categorize, Select, Implement, Assess, Authorize and Monitor. The result is a decision record that security, IT, privacy, legal and business leaders can operate after deployment—not a document that becomes stale when the model changes.

What is an AI risk assessment, and why should security and IT own part of it?

An AI risk assessment is a documented, repeatable decision process for understanding what an AI system does, what could go wrong, who could be harmed and whether the remaining risk is acceptable. It covers a model supplied by a vendor, an embedded feature in an existing application, an internally built workflow and an AI capability connected to company data or tools.

Security and IT participation matters because many serious failures occur outside the model itself. Identity and access settings, exposed APIs, cloud configuration, retrieval stores, plugins, secrets, logging, backups and supplier changes can turn a technically accurate model into an unsafe service. A useful assessment therefore joins technical testing with privacy, fairness, safety, reliability, accessibility, human-oversight and regulatory analysis.

The assessment should produce three operational outputs:

  • A current inventory record with a named owner and system boundary.
  • A risk register that links each threat or harm to evidence, controls, treatment and a due date.
  • An authorization decision that states approved uses, prohibited uses, conditions, residual risk and the next review trigger.

Which frameworks give an assessment a workable structure?

Use one framework as the spine and map other requirements into it. NIST’s AI framework is voluntary and non-sector-specific; it can be applied by organizations that design, develop, deploy or use AI. ISO guidance is useful when an organization needs a management-system view, while the security RMF supplies a disciplined authorization and monitoring cycle.

Framework or guidance Publication or update How to use it
NIST AI RMF 1.0 January 26, 2023 Organize work under Govern, Map, Measure and Manage. It is voluntary and intended to improve trustworthy AI across design, development, use and evaluation.
NIST AI RMF and Generative AI Profile Generative AI Profile released July 26, 2024 Add generative-AI risks and actions, such as prompt injection, data leakage, provenance and misuse, to the core assessment.
ISO/IEC 23894:2023 February 6, 2023 Apply its AI-specific risk-management guidance to organizations developing, producing, deploying or using AI products, systems and services.
UK AI Risk Management Toolkit September 8, 2026 Use the guidance and workbook with a multidisciplinary team when a practical workshop format is helpful.

For organizations aligning AI governance with a management system, the NIST-to-ISO crosswalk maps inventory, roles, competence, awareness, communication and leadership accountability to management-system requirements: NIST AI RMF to ISO/IEC 42001 Crosswalk.

How do you build the assessment from inventory to authorization?

Run the following sequence for each distinct use case. A single application may need several records if different workflows have different data, users, decisions or risk levels.

1. Inventory and scope the use case

Create one record for every model, vendor feature, automated workflow or embedded capability. Record the business purpose in plain language and define what is outside the system boundary. Capture:

  • Accountable business owner, technical owner and service provider.
  • Model name, version, deployment location and lifecycle stage.
  • User groups, affected people, geographic scope and whether decisions are advisory or automated.
  • Data classes entering, leaving or stored by the system, including personal, confidential and regulated data.
  • Connected applications, retrieval stores, APIs, plugins, cloud services and downstream consumers.
  • Your role: developer, deployer, procurer or user.

Give the record a stable system ID. Without an inventory, unapproved “shadow AI” cannot be assigned an owner, tested or retired.

2. Establish governance and accountability

Define who may approve, pause, remediate or retire the system. Include security, IT operations, privacy, legal, procurement, data science where applicable and the business sponsor. Document escalation paths for an incident, a failed control test or a material change.

Leadership must explicitly accept residual risk; an assessment team should not silently accept it on the business’s behalf. Set the review cadence, evidence-retention period and authority for emergency suspension. Competence, awareness, communication and leadership accountability are management-system concerns as well as project tasks.

3. Map intended use, misuse, harms and obligations

Describe the expected user journey and foreseeable misuse before selecting a score. Consider confidentiality, integrity, availability, privacy, fairness, safety, reliability, explainability, accessibility and meaningful human oversight. Identify who may be disadvantaged by an error and what remedy is available.

Map dependencies such as training data, retrieval content, APIs, plugins, infrastructure, vendors and downstream users. Flag sector rules, contractual duties and jurisdiction-specific requirements before deciding whether a risk is tolerable. A system used for internal drafting may have a different impact from one that influences employment, credit, health or safety decisions even when both use the same model.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

4. Measure risk with evidence and stated uncertainty

Use a repeatable scale with written definitions. For every risk, record likelihood, impact, confidence in the evidence, affected populations, current control effectiveness and residual risk. Attach test dates, assumptions, evidence links and unresolved uncertainty to the score.

Do not present an organizational scoring convention as a universal probability. Framework sources do not prescribe one numeric likelihood or impact scale; your organization must calibrate bands to its loss tolerance, incident history and regulatory exposure. Re-score when better evidence changes confidence, even if the model has not changed.

5. Select and implement controls

Choose controls that address the specific threat and verify that they operate in the real deployment. Security controls should cover confidentiality, integrity and availability, as well as vulnerability management, identity, secrets, isolation, logging, monitoring, backup, recovery, incident response and suppliers.

Joint guidance from NSA AISC, CISA, FBI, ASD ACSC, CCCS, NCSC-NZ and NCSC-UK emphasizes secure deployment that improves confidentiality, integrity and availability, mitigates known vulnerabilities and enables protection, detection and response to malicious activity. The guidance is available at Joint Guidance on Deploying AI Systems Securely.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

6. Assess independently and authorize explicitly

Have an assessor who was not responsible for the build decision review the evidence and test whether controls operate as claimed. The decision record should state the approved use, prohibited use, conditions, compensating controls, residual risks, expiry or review date and named authorizing official.

NIST SP 800-37 describes categorization, control selection, implementation, assessment, authorization and continuous monitoring with explicit accountability: NIST SP 800-37. Treat “approved with conditions,” “needs remediation” and “reject or retire” as different outcomes; a conditional approval must list the condition, owner and deadline.

7. Monitor, change-control and retire

Define operating metrics before launch. Useful measures include incident count and severity, model or data drift, error and override rates, measurable false positives and negatives, access anomalies, data-quality indicators, control-test results, vendor changes and unresolved findings.

Trigger reassessment after a model, data source, prompt, integration, owner, user population, threat, incident or legal requirement changes. At retirement, revoke access, disable integrations, preserve required records and verify deletion or retention obligations. A system is not retired until credentials, scheduled jobs, cached data and downstream dependencies are addressed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What should be in an AI risk register?

Use a row for each material threat or harm, while keeping system-level facts in the same record so a reviewer can understand context without searching another repository.

Field What to record
System ID and use case Stable identifier, business purpose and workflow boundary.
Owner and role Business owner, technical owner, provider and whether your organization develops, deploys, procures or uses the system.
Model and lifecycle Provider, model and version, deployment location, launch date, current stage and planned retirement trigger.
Users and affected people User groups, decision subjects, geography, vulnerable populations and human decision points.
Data and integrations Data classes, retention, retrieval stores, APIs, plugins, cloud services and downstream systems.
Threat or harm Security threat, privacy issue, fairness or safety harm, reliability failure, misuse case or legal obligation.
Scoring evidence Likelihood, impact, evidence confidence, assumptions, affected population and test date.
Controls Existing safeguards, control owner, effectiveness result and the date each control was tested.
Treatment and residual risk Mitigation, transfer, acceptance, avoidance or retirement; remaining risk after treatment.
Decision and follow-up Approver, approval conditions, due date, monitoring metric, next review and retirement trigger.

How should you score likelihood and impact?

Choose bands that people can apply consistently, then test calibration on past incidents or realistic scenarios. A simple five-band model can work when definitions are explicit: likelihood from rare to almost certain, and impact from negligible to severe. Define impact separately for confidentiality, integrity, availability, privacy, safety, fairness, financial loss, legal exposure and service disruption rather than hiding all consequences in one vague number.

Record confidence beside the score. “High impact, low confidence” is not the same decision as “high impact, high confidence”: the first may require targeted testing before launch, while the second may already exceed the approval threshold. Document who may accept each residual-risk band, and never let a low-confidence estimate automatically lower priority.

Use the score to choose treatment, not to create false precision. A risk with a modest numerical rating may still be unacceptable when law, contract or policy prohibits the use, and a high rating may be conditionally acceptable when strong compensating controls are independently verified.

Control area Questions and evidence
Identity and access Are users, service accounts and administrators separated? Is least privilege enforced for models, data, tools and retrieval stores? Review access logs and removal tests.
Secrets and interfaces Are API keys in a managed secret store, rotated and scoped? Are endpoints authenticated, rate-limited and protected against replay or abuse?
Data protection Are sensitive inputs minimized, encrypted in transit and at rest, retained for a defined period and excluded from provider training where contractually required?
Isolation and supply chain Are tenants, workloads, plugins and tools isolated? Review dependency inventories, vulnerability scans, update procedures and supplier change notices.
Integrity and provenance Can the team identify model, code, data and prompt versions? Are tampering, poisoned data and unauthorized model changes detectable?
Logging and detection Are prompts, outputs, tool calls, administrator actions and policy decisions logged with appropriate redaction? Can anomalous access and abuse generate an alert?
Resilience Are backups, restore tests, capacity limits, rate controls and a safe degraded mode documented? Can the service be isolated without losing required records?
Incident response Do playbooks cover data leakage, malicious prompts, compromised credentials, harmful outputs, supplier incidents and notification duties?

What additional checks are needed for generative AI?

Generative systems require tests beyond conventional application security because users can influence instructions and outputs in unpredictable ways. Add prompt-injection and jailbreak testing, sensitive-data leakage checks, output validation, retrieval and tool-permission reviews, model and data provenance, abuse monitoring, red-team evidence and safe fallback paths.

Constrain tools by allow-list, identity and purpose; require confirmation for consequential actions; and treat retrieved documents as untrusted input. Validate output format and content before it reaches a database, customer, code repository or decision workflow. Keep a human escalation path when the model is uncertain, refuses incorrectly or produces a potentially harmful result.

For general-purpose providers with systemic risk, European Commission guidance calls for standardized evaluation, adversarial testing, systemic-risk mitigation, serious-incident reporting and cybersecurity safeguards. Consult the applicable obligations at European Commission guidance for general-purpose AI providers and determine whether your organization is a provider, deployer or user before assigning duties.

How can you compare systems and make an approval decision?

Evaluate alternatives against the same evidence requirements rather than choosing the system with the best demonstration. Compare regulatory exposure; security and privacy risk; reliability and performance evidence; explainability and human oversight; resilience and recoverability; vendor transparency; data and model provenance; integration effort; operating cost; control maturity; and residual risk after treatment.

Decision Use when Record
Acceptable with conditions Residual risk is within tolerance only if named safeguards, limits or review dates remain in force. Conditions, owners, evidence required and the date approval expires or is reviewed.
Needs remediation A material control gap can be corrected before use or before expanding the user population. Specific finding, treatment plan, due date and who can authorize temporary operation, if any.
Reject or retire Risk exceeds tolerance, a required control cannot be implemented, or the use is prohibited. Reason, affected assets, shutdown steps, record-retention duties and any replacement process.

Do not merge these outcomes into a single “risk rating.” An approval is an accountable decision with scope and conditions; it is not a claim that the system is safe in every context.

How do you keep the assessment current after deployment?

Assign a review owner and make change management part of normal operations. A scheduled review is necessary but not sufficient: event-based reassessment catches risk between calendar dates.

  • Review metrics and open findings at an agreed cadence based on impact and exposure.
  • Require notice before model, provider, prompt, data, integration, infrastructure or ownership changes.
  • Re-test controls after upgrades, incidents, major configuration changes or newly disclosed vulnerabilities.
  • Sample outputs and user overrides for quality, fairness, privacy and unsafe behavior; retain enough evidence to reproduce a disputed decision.
  • Reconfirm contracts, data-retention terms, geographic processing and legal obligations when the provider or user population changes.
  • Use a kill switch or isolation procedure that has been exercised, not merely documented.

Drift can be technical or organizational. A stable model may become riskier when data quality declines, a new population is added, a plugin gains write access or staff begin using outputs for a decision the authorization never covered.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What commonly makes an AI assessment fail?

Starting with a generic questionnaire

Long forms produce superficial answers when the team has not defined the use case. Begin with the system boundary, data flows, decisions and affected people; ask only questions that can change treatment or authorization.

Assigning risk to the model alone

A model card cannot describe your permissions, prompts, retrieval corpus, integrations or incident process. Assess the complete socio-technical system and name owners for dependencies.

Using unexplained numbers

A score without definitions, evidence confidence and an acceptance threshold creates the appearance of rigor. Publish the band definitions and retain the assumptions behind every rating.

Approving without operating conditions

“Approved” should not mean unrestricted use. State prohibited uses, human-review requirements, data limits, logging expectations, expiry dates and the authority to pause service.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Treating monitoring as a dashboard project

Metrics matter only when a threshold triggers an owner, an investigation and a decision. Link each metric to an escalation path and test that path during an exercise.

What should the final decision record contain?

Keep the signed record short enough to use and detailed enough to audit. It should identify the system and owner, summarize material harms and threats, link evidence and test results, list controls and residual risks, state the authorized scope and restrictions, name the authorizing official, set the next review date and define events that force reassessment or retirement.

This record is the handoff between project delivery and operations. Store it where security, IT, privacy and business reviewers can find the current version, while preserving prior decisions so changes and accountability remain traceable.

Frequently Asked Questions

Who should sign off an AI risk assessment?

The named authorizing official should have authority over the business risk and resources needed for treatment. Security, IT, privacy, legal and procurement should provide evidence or concurrence within their remit, but they should not be treated as the silent owner of residual business risk.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How often should an AI risk assessment be reviewed?

Set a risk-based calendar cadence, then review immediately after a material model, data, prompt, integration, ownership, user, threat, incident or legal change. High-impact systems generally need more frequent evidence checks than low-impact internal assistance, even when both use the same provider.

Can an AI vendor’s certification replace our assessment?

No. Supplier attestations can support evidence about the provider’s controls, but they do not cover your data flows, permissions, prompts, users, downstream decisions or local legal duties. Treat vendor material as one input and test the deployed configuration yourself.

What evidence is useful when the model is a black box?

Use behavior tests, input and output samples, error and override measurements, access and tool-call logs, provenance records, incident exercises and documented human-review procedures. Limited explainability should increase monitoring and decision restrictions rather than being hidden by a confident score.
