An effective AI governance checklist is a repeatable operating process: inventory every AI use, assign accountable owners, assess potential harm, apply controls that match the risk, record decisions, and review the result when systems, data, vendors, or laws change. It should cover software your team builds, buys, embeds in other products, or uses informally—not just projects labelled “AI.”
Use the checklist as an internal management aid, not as a universal certification or a replacement for jurisdiction-specific legal advice. A practical program can align with NIST AI RMF, ISO/IEC 42001, the EU AI Act, UK AIME, or the UK AI Risk Management Toolkit, but the right combination depends on your locations, sector, role in the AI supply chain, and the impact of each use case.
How do I create an AI governance checklist for my business?
Start with one controlled register of AI use cases, then run each entry through six gates: scope and inventory, accountability and policy, risk treatment, data/testing/human oversight, vendors/records/incidents, and legal mapping/ongoing review. Give every answer an owner, evidence location, status, and next review date so the checklist drives work rather than becoming a static policy document.
- Discover. Gather declared projects, procurement records, application inventories, data-flow diagrams, and an employee reporting route for unapproved or informal AI use.
- Describe. Record the purpose, provider, model or feature, users, data, affected people, decision impact, and operating locations.
- Assign authority. Name an executive sponsor, a day-to-day owner, an owner for the vendor relationship, and people who can approve, restrict, pause, or stop the use.
- Assess risk. Identify plausible harms, who could experience them, likelihood, severity, evidence, mitigations, residual risk, and the rationale for approval.
- Implement controls. Set data permissions, testing, human review, security, logging, disclosures, incident handling, and change-control requirements proportionate to the use.
- Record and monitor. Keep the decision, version, evaluations, approvals, incidents, and monitoring results together. Reassess on a defined schedule and after specified triggers.
- Map obligations. Check the countries, sectors, contracts, and provider/deployer roles that apply, then verify current legal dates and duties before launch and whenever the environment changes.
The rest of this guide turns those gates into fields and operating steps your team can implement in a spreadsheet, ticketing system, GRC platform, or document workflow.

What should an AI inventory contain?
An inventory is the foundation: you cannot govern a use you cannot see. Include internally built models, purchased applications, AI features switched on inside ordinary software, experiments, and employee use of public tools. NIST describes risk management across design, development, use, and evaluation; the UK toolkit also covers procuring, operating, and delivering AI products.
Capture the use case, not just the product name
One vendor may support several materially different uses. Create a separate record when the purpose, data, affected population, decision, or location changes. A useful record contains:
- Purpose and workflow: what the system is intended to do, what process it sits in, and what it must not do.
- System identity: product or model name, provider or internal team, version, enabled features, interfaces, and deployment environment.
- People and authority: business owner, technical owner, vendor owner, users, administrators, reviewers, and escalation contacts.
- Data lifecycle: inputs, outputs, retention, training or feedback use, data locations, access permissions, and deletion process.
- Impact: decisions or recommendations influenced, people affected, reversibility, and whether an error can deny a benefit, create a safety issue, expose confidential information, or damage someone’s reputation.
- Geography and context: countries, states, sectors, employment or consumer context, and whether the organization acts as developer/provider, deployer/user, or both.
- Lifecycle status: proposed, pilot, approved, restricted, paused, retired, or awaiting reassessment.
Find informal and embedded use
Ask department heads, procurement, security, privacy, legal, HR, customer support, and engineering for their lists. Add a short employee declaration form for tools used outside procurement. Review browser, identity, API, expense, and software-asset records for AI features that may be enabled by default. Treat an undisclosed use as an inventory gap to close, not automatically as misconduct; the objective is visibility and safe correction.
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Set a minimum record before experimentation
Require a lightweight entry before a pilot receives production data or influences a consequential decision. At minimum, capture purpose, owner, provider, data classes, affected people, location, proposed safeguards, and an expiry or review date. A fuller assessment can follow for low-impact trials, but the initial record prevents “temporary” experiments from becoming permanent without a decision.
Who should own AI governance decisions?
Accountability must be explicit at both executive and operational levels. ISO/IEC 42001:2023 is a management-system standard for organizations that develop, provide, or use AI-based products or services; its value is the organization-wide system of responsibilities, operation, maintenance, and continual improvement. The checklist should translate that principle into named people and usable authority.
Assign four complementary roles
- Executive sponsor: provides budget, resolves cross-functional conflicts, accepts or rejects significant residual risk, and receives periodic reporting.
- Use-case owner: accountable for the business purpose, user instructions, outcomes, controls, and review schedule for one AI use.
- Control owners: named people for data protection, security, testing, human oversight, procurement, records, and incident response. One person may hold several roles in a small business, but the responsibilities still need to be written.
- Independent challenge or approval: a privacy, security, legal, risk, or ethics reviewer who can question evidence and require changes before launch. For higher-impact uses, avoid having the person measured on delivery be the only approver.
Write decision rights and escalation routes
State who may approve a low-risk pilot, who must approve a high-impact deployment, who can impose restrictions, and who can pause or disable a system immediately. Define an escalation route for suspected discrimination, unsafe output, privacy exposure, security compromise, vendor failure, or unexplained performance change. Include out-of-hours contacts when the system supports safety, essential services, or time-sensitive decisions.
Train people by role
Give users practical instructions on permitted data, verification, disclosure, recordkeeping, and reporting. Train reviewers to challenge outputs rather than rubber-stamp them, administrators to manage access and logs, developers to document tests and limitations, and managers to recognize when a new purpose requires reassessment. Record completion, refresher timing, and exceptions for contractors and temporary staff.
What should an AI policy include?
A policy sets the organization-wide boundaries; the checklist proves that a particular use follows them. Keep the policy short enough to be used and link it to detailed procedures, approval forms, and incident playbooks.

Permitted, prohibited, and conditional uses
Define approved categories such as drafting, summarization, coding assistance, or internal search only when the data and review conditions are met. Prohibit uses your organization cannot safely support, such as entering restricted information into an unapproved public service or allowing an unreviewed output to make a consequential decision. For conditional uses, specify the required reviewer, disclosure, test evidence, retention, and approval level.
Data and confidentiality rules
Classify data that may be entered, retained, or returned. Explain whether prompts and outputs may be used by a provider for training, where data is stored, how access is revoked, and how records are deleted. Require least-privilege access, approved accounts, and a route to report accidental disclosure. Align these rules with existing privacy, security, records, employment, and sector policies rather than creating contradictory exceptions.
Human responsibility and transparency
State when a qualified person must review, correct, or override an output and what information that person needs to do so. Tell users when an interaction or content is AI-generated if the context or applicable rule requires disclosure. Explain how affected people can ask questions, challenge an outcome, or request human consideration where your process provides that right.
Enforcement and exceptions
Describe monitoring, consequences for bypassing controls, and a documented exception process with an expiry date, compensating controls, and an approving authority. Emergency suspension should not depend on a lengthy policy exception.
How do I assess AI risk and choose proportionate controls?
Risk assessment should connect a plausible harm to a specific control and an accountable person. Record the affected people, likelihood, severity, evidence, mitigation, residual risk, and approval rationale. Do not assume that every obligation described for high-risk systems under the EU AI Act applies to every AI use; classification and organizational role determine what is relevant.
Describe harms in concrete terms
- Accuracy and reliability: fabricated facts, missed detections, unstable results, or performance that degrades outside the test data.
- Fairness and discrimination: unequal error rates, proxy effects, inaccessible interfaces, or recommendations that disadvantage a protected or vulnerable group.
- Safety: physical, medical, financial, or operational harm caused by an incorrect or poorly timed output.
- Privacy and confidentiality: unnecessary collection, re-identification, sensitive inference, unauthorized retention, or disclosure through prompts, outputs, logs, or support channels.
- Security and abuse: prompt injection, data exfiltration, model theft, malicious content, compromised integrations, or excessive automated action.
- Human and organizational effects: opaque decisions, automation bias, loss of professional judgment, worker surveillance, reputational damage, or inability to contest an outcome.
Score likelihood and severity without false precision
Use a scale your team can explain, such as low, medium, and high, or a numeric scale with written definitions. Severity should reflect the worst credible outcome and reversibility; likelihood should reflect exposure, safeguards, and evidence, not optimism. Note uncertainty and the assumptions behind the rating. A low-confidence “low” score is a reason for more evidence, not reassurance.
Match controls to the decision impact
For a low-impact drafting aid, controls may include approved data, user verification, access limits, and periodic sampling. A system influencing employment, credit, eligibility, health, safety, or access to essential services needs stronger evidence, qualified human review, traceable logs, a challenge route, and explicit approval. The checklist should state why the selected review level is proportionate and what residual risk remains after controls.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteUse a risk-treatment record
For each material risk, record:
- The harm, affected population, and triggering condition.
- Likelihood, severity, confidence, and evidence.
- Preventive controls, detective monitoring, and response actions.
- Control owner, due date, test method, and pass criteria.
- Residual risk, acceptance authority, and any restrictions or expiry.
Which data, testing, and human-oversight checks belong on the checklist?
Testing must reflect the conditions and populations in which the system will operate. The checklist should require evidence that data sources and permissions are appropriate, outputs are measured for relevant quality and fairness dimensions, and a human can understand and challenge results when needed.

Data checks
- Document source, provenance, license or permission, collection purpose, and retention.
- Identify personal, confidential, regulated, or sensitive data and confirm minimization.
- Check representativeness and known gaps for affected populations and operating contexts.
- Separate development, test, and production data where appropriate; protect test data with the same care as live data.
- Define how prompts, outputs, feedback, and logs are stored, accessed, corrected, and deleted.
Testing checks
- Test normal, edge, adversarial, and failure conditions relevant to the workflow.
- Measure accuracy, reliability, latency, security, and relevant fairness outcomes; document the metric, sample, threshold, and limitations.
- Test language, accessibility, and population differences that matter in the deployment context.
- Repeat key tests after model, prompt, data, integration, or vendor changes.
- Record who ran the test, the version tested, results, unresolved defects, and launch decision.
Human-review checks
Name the reviewer role and define when review is mandatory, what evidence the reviewer sees, and how disagreement is recorded. The reviewer must have enough competence, time, and authority to reject or override an output; a nominal click-through is not meaningful oversight. Provide a manual fallback for outages, uncertainty, or unsafe results.
Disclosure and contestability checks
Decide whether users, customers, workers, or other affected people need notice that AI is involved. Provide an accessible way to ask for an explanation, correction, or human review when appropriate. Keep evidence that disclosures were shown and that challenges were handled.
How should I govern AI vendors and embedded features?
Procurement is part of governance, not a one-time security questionnaire. Review the service before purchase, during onboarding, and after material changes. The UK AI Risk Management Toolkit is designed for teams designing, procuring, operating, or delivering AI products and includes practical workbook resources.

Ask for evidence that matches your use
- System purpose, model or feature description, versioning, known limitations, and intended operating conditions.
- Data handling: collection, training use, retention, location, subprocessors, deletion, and customer isolation.
- Security controls, access management, incident notification, testing summaries, and independent assurance where available.
- Performance, fairness, robustness, and monitoring information relevant to your users and geography.
- Change notices, support commitments, audit or information rights, portability, exit assistance, and service termination behavior.
Put operational duties in the contract
Define permitted purpose, data restrictions, confidentiality, security responsibilities, breach notification, cooperation with investigations, records access, and notice before material model or feature changes. Allocate responsibilities between provider and your organization; a provider’s general compliance statement does not remove your duties as a deployer or user.
Control embedded and free-tier features
Maintain an approved-software list and configuration baseline. Disable optional AI features until their data use, access, and retention are understood. Review updates to ordinary productivity, customer-service, recruitment, analytics, and developer tools because an embedded feature can change the risk profile of an existing workflow.
What records and incident process make the checklist operational?
A governance decision is only defensible if someone can reconstruct what was approved, which version was used, what evidence existed, and what happened afterward. Keep records in a controlled location with access, retention, and version history.
Minimum decision record
- Use-case description, owner, provider, model or feature version, purpose, and locations.
- Data map, affected people, decision impact, risk assessment, controls, residual risk, and approval rationale.
- Testing plans and results, human-review design, disclosures, user training, and vendor evidence.
- Launch restrictions, monitoring metrics, review date, change triggers, and named approvers.
Logs for important actions
For systems that influence significant decisions or automated actions, retain enough event information to investigate: timestamp, system and version, input or reference, output, human action, overrides, errors, and relevant configuration. Minimize sensitive content and set a retention period that fits legal and operational needs. Logging is useful only if clocks, identifiers, permissions, and retrieval procedures are reliable.
Incident workflow
- Report: provide a simple route for users, customers, workers, and vendors to flag harmful, inaccurate, biased, unsafe, insecure, or privacy-invasive behavior.
- Triage: classify impact, affected people, urgency, ongoing exposure, and whether the system should be paused.
- Contain: disable a feature, revoke access, isolate data, switch to a manual process, or restrict outputs while facts are gathered.
- Investigate: preserve relevant logs and versions, identify root and contributing causes, and check whether similar cases exist.
- Notify and remediate: meet applicable notification duties, correct decisions or data where necessary, and communicate with affected people appropriately.
- Learn: document corrective actions, owners, due dates, residual risk, and changes to tests, training, contracts, or policy.
Which AI governance framework should a business use?
These instruments serve different purposes. You can use one as the backbone and borrow controls from the others, but no framework automatically proves compliance in every jurisdiction or sector.

Choose by the decision you need to make
- Need a flexible starting structure? Use NIST AI RMF concepts and tailor the language to your size and sector. NIST describes the framework as intended for voluntary use.
- Need a formal management system? Map processes, responsibilities, records, internal review, and continual improvement to ISO/IEC 42001:2023. Certification questions should be handled with ISO or an accredited provider.
- Operate in or serve the EU? Classify each use under the AI Act, determine whether you are a provider, deployer, or both, and verify the current transition dates and category-specific obligations.
- Need a small-business self-assessment? AIME can expose organizational gaps and create an action list, but UK guidance says completing it does not represent compliance and it does not replace the underlying frameworks.
- Need implementation worksheets? The UK toolkit offers practical material for procurement and operations; adapt it to your own approvals, records, and legal requirements.
How do I map geography, sector, and legal role?
Legal scope is a separate checklist gate because the same tool can create different duties in different contexts. List every country and state where the organization, provider, user, affected person, data, or decision is connected. Add sector rules, employment or consumer obligations, contracts, and internal risk requirements.
Determine your role
For each system, decide whether your organization develops or provides it, deploys or uses it, integrates another provider’s model, or performs more than one role. Record the conclusion and the facts supporting it. Role affects which documentation, oversight, transparency, monitoring, and incident duties may apply.
Check EU AI Act status carefully
If EU activity is in scope, categorize the use and review the European Commission’s current overview immediately before approval and publication. The implementation schedule is staggered and has been affected by amendments; do not replace it with one universal start date. Confirm the exact obligation for the system category, provider/deployer role, and transition period.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Keep voluntary guidance distinct from law
NIST AI RMF is voluntary. A framework can improve practice and help organize evidence, but adopting it does not by itself satisfy a law, contract, regulator, or certification requirement. Record which obligations are legal, which are contractual, and which are voluntary controls chosen by the business.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. When should an AI governance checklist be reviewed?
Set both a routine cadence and event-driven triggers. ISO/IEC 42001 emphasizes continual improvement, and NIST places risk management throughout the AI lifecycle; a checklist that is reviewed only at launch will miss changing behavior and obligations.
Use event triggers
- Model, prompt, feature, integration, or vendor change.
- New data source, new retention behavior, or a change in permissions.
- New purpose, user group, geography, sector, or affected population.
- Incident, near miss, complaint, unexpected output, or material performance drift.
- Change in law, regulator guidance, contract, organizational ownership, or risk appetite.
- Expansion from recommendation or drafting into automated action or consequential decision support.
Set a routine schedule
Choose a frequency based on impact and change rate, then put the date and accountable reviewer in the register. At each review, confirm the inventory record, owner, vendor status, data map, tests, incidents, metrics, open risks, training, and legal analysis. Close, renew, restrict, or retire uses that no longer meet their approval conditions.
Track useful indicators
Report measures that reveal control performance, not vanity totals: inventory coverage, overdue reviews, unresolved high residual risks, test failures, override rates, incident age, training completion, vendor changes assessed, and uses operating outside approved purpose. Explain the denominator and period so leaders can interpret trends.

First 30 days: establish visibility
- Name an executive sponsor and an operational coordinator.
- Publish a temporary rule for sensitive data and unapproved public AI tools.
- Collect use cases from every team, including informal and embedded uses.
- Classify each use by purpose, affected people, decision impact, geography, and role.
- Pause uses with unknown data handling or unacceptable immediate risk while facts are gathered.
Days 31–60: add controls
- Approve low-impact uses with standard data, verification, access, and disclosure controls.
- Run deeper assessments for consequential or externally facing uses.
- Review vendor terms, security, privacy, retention, change notices, and exit options.
- Define human-review, logging, incident, and manual-fallback procedures.
- Train users and record completion.
Days 61–90: make it repeatable
- Set review dates and event triggers in the register.
- Test representative and edge cases; record defects and residual risk.
- Run an incident tabletop exercise and verify pause authority.
- Map applicable jurisdictions, sectors, contracts, and provider/deployer roles.
- Report open risks and overdue actions to the sponsor, then improve the checklist from what the exercise revealed.
This staged approach is not a legal safe harbor. It is a practical way to create evidence, authority, and feedback before the AI portfolio becomes too large to understand.
What common checklist mistakes should I avoid?
- Listing products instead of uses: split materially different purposes and populations into separate records.
- Relying on a policy without owners: every control and decision needs a named person and escalation route.
- Using one risk score for everything: tie review depth to impact, reversibility, exposure, and evidence.
- Testing only average accuracy: include edge cases, affected populations, security abuse, and operational failure.
- Treating human review as a checkbox: verify competence, information, time, authority, and override behavior.
- Ignoring vendor change: require notices, reassessment triggers, and a way to restrict or exit a changed service.
- Keeping no usable logs: balance investigation needs with data minimization, retention, and access controls.
- Calling a framework “compliance”: distinguish voluntary guidance, self-assessment, management standards, contracts, and law.
- Hard-coding old dates: recheck the EU AI Act schedule, NIST revisions, and updated UK guidance immediately before relying on them.
- Leaving informal use outside scope: provide a safe reporting route and bring discovered uses into the same process.
What is the finished checklist structure?
Use the following sections as the register’s top-level tabs or workflow stages. Add evidence links, owner, status, due date, and review date to each item.
A. Inventory and scope
- AI systems, features, vendors, and planned uses identified.
- Purpose, workflow, decision influence, provider, version, users, and owner recorded.
- Input, retained, and returned data classified, with affected people and locations listed.
B. Accountability and policy
- Executive sponsor, operational owner, control owners, approvers, and pause authority named.
- Permitted, prohibited, conditional, and exception rules documented.
- Role-appropriate employee and contractor training assigned and tracked.
C. Risk assessment and treatment
- Harms, affected people, likelihood, severity, evidence, mitigations, and residual risk recorded.
- Approval level and review depth matched to impact and reversibility.
- Each control has an owner, test, due date, and acceptance decision.
D. Data, testing, and human oversight
- Sources, permissions, retention, representativeness, security, and deletion checked.
- Relevant accuracy, reliability, security, fairness, accessibility, and robustness tests completed.
- Human reviewers can understand, challenge, override, and document decisions where needed.
- Disclosure and contestability requirements decided and implemented.
E. Vendors, records, and incidents
- Vendor documentation, security, privacy, data handling, contracts, and change notices reviewed.
- Purpose, version, limitations, evaluations, approvals, monitoring, and logs retained.
- Report, triage, contain, investigate, notify, remediate, and learn procedures tested.
F. Legal mapping and ongoing review
- Countries, states, sectors, contracts, and provider/deployer roles assessed.
- Applicable category-based duties and transparency requirements verified.
- Routine review date and event triggers recorded; open risks reported and closed or accepted by authority.
When every section has an owner, evidence, and a next action, the checklist becomes a management system rather than a compliance document that sits unused.
Frequently Asked Questions
Can a spreadsheet be enough for AI governance?
Yes, for a small and stable portfolio if it has access control, version history, required fields, evidence links, owners, due dates, and a reliable backup. Move to a workflow or GRC system when approvals, audit trails, integrations, or the number of use cases make manual tracking error-prone.
How do we govern employee use of public generative-AI tools?
Provide an approved-tool list, prohibit sensitive data in unapproved services, require business accounts where retention and administration are understood, and offer a quick reporting route for accidental disclosure. Review aggregated usage and recurring questions so training and policy reflect real behavior rather than assumptions.
Do we need an AI ethics committee?
Not necessarily. A small business can use an accountable sponsor plus privacy, security, legal, technical, and business reviewers who join according to the use case; a standing committee becomes useful when volume or impact makes ad-hoc review inconsistent.
How should we handle an AI system that is already in production without records?
