To audit an autonomous AI agent, compare its approved purpose, identity, permissions, tools, and dependencies with what it actually did. Reconstruct sampled actions by correlating the agent’s traces with independent records from the identity provider, cloud platform, APIs, databases, or connected services; then test whether approvals, alerts, and interruption controls work. A final answer or agent-generated trace alone is not enough to establish that a consequential action was authorized or correctly recorded.
What should an audit establish?
An audit should let an organization answer four questions about a consequential run: who or what initiated it, which identity and authority the agent used, what actions it took, and what changed as a result. It should also establish whether the run followed the approved policy and whether operators could detect and contain unexpected behavior.
That means examining more than the model’s response. An agent may call tools, retrieve data, use memory, delegate work, or trigger actions in connected services. The audit boundary should include the orchestrator and any sub-agent or service that can act, change state, or affect the evidence trail. The Canadian Centre for Cyber Security recommends monitoring agent operations such as identity and privilege changes, tool calls, memory interactions, decisions, and actions. The UK government’s AI cyber security code calls for an audit trail and logging and analysis of system and user actions.

What should an AI agent log capture?
Logs need enough context and attribution to reconstruct decisions and state changes—not just the final output. For each sampled run, look for a connected chain of events with timestamps and identifiers that can be matched across the agent platform and downstream systems.
| Evidence area | What to capture or verify | Why it matters |
|---|---|---|
| Initiation and identity | Initiating user, task or trigger, agent identity and version, delegated principal, and relevant identity or privilege changes. | Shows who initiated the work and which identity actually operated the agent or its tools. |
| Context and decision | Relevant request or task context, memory interactions, policy or authorization decision, and any errors or cross-agent communication that affected the run. | Helps explain why an action was considered and whether the applicable rule was checked. |
| Tool and action | Tool or service used, action attempted, execution time, approval or preview when required, and the result returned. | Connects the agent’s stated plan to an attempted or completed operation. |
| Outcome and state | Success or failure, downstream state change, and enough contextual identifiers to locate the corresponding service record. | Shows whether the action took effect and what it changed. |
| Evidence integrity | Access, export, change, or deletion of the logs; relevant access records; and gaps or synchronization issues. | Helps determine whether the audit trail itself is accessible and trustworthy. |
The Singapore government’s addendum on generative AI and agentic AI describes a broad logging scope across models, databases and files, memory, agents, tools, MCP interactions, agent communications, and external actions. It identifies action, input and output, state change, error, timing, and contextual identifiers as useful evidence categories. The UK code likewise emphasizes logging and analysis of system and user activity.
For practical reconstruction, preserve a stable task, workflow, or transaction identifier on every event. Record the event type, originating component, executing principal, target resource, policy result, approval reference, start and completion times, and a link to the downstream record. Keep version identifiers for the model, prompt or policy configuration, tool definition, orchestrator, and relevant connector. If a run is retried, forked, handed to another agent, or resumed from memory, retain parent and child identifiers so the sequence is not mistaken for one uninterrupted call.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsRecord both attempted and completed actions. A denied deletion, a timed-out API call, and a successful deletion have different security meanings even if the final response says that the request failed. Capture error codes, retries, fallbacks, and tool unavailability when they influence the decision or operator response. Where an agent communicates with another agent, record the sender, recipient, message or reference, authorization context, and resulting action if that exchange affects state.
Do not interpret “capture context” as “retain every prompt and retrieved record forever.” Prompts and tool payloads can contain sensitive personal, commercial, or security information. Apply the organization’s privacy rules, retention requirements, access controls, and data-minimization practices. Where a full payload is unnecessary or inappropriate, retain a suitable redacted record, cryptographic reference, structured summary, or pointer to an access-controlled store that still supports attribution and reconstruction.
How do you audit an agent step by step?
Use a repeatable sequence: define the system boundary and approved authority, trace a sample of runs, corroborate them independently, exercise controls, then record findings and retest fixes. Keep the audit evidence tied to specific runs, identities, and downstream effects.
1. Scope the system and its dependencies
Start with a deployment inventory, not just a model name. Record the agent’s business purpose, accountable owner, users, autonomy level, model and provider version, connected tools and data sources, memory behavior, configuration, and relevant services. Include orchestrators and delegated agents if they can take actions or affect state. Identify the identities used by the agent, including delegated principals and service identities.
Microsoft’s guidance recommends inventorying models, tools, plugins, and data sources and assigning agents unique auditable identities. An inventory should be specific enough to distinguish deployments and versions; a generic entry such as “support agent” may not reveal which tool configuration or identity a sampled run used.
Document trust boundaries and dependencies as well. Note where prompts are assembled, where memory is stored, which databases or files can be retrieved, which external services can be called, and which component makes the final authorization decision. Identify logging destinations and whether they are controlled by the same administrator or service account that operates the agent. This makes it possible to test whether an agent can influence, suppress, or alter its own evidence.

2. Define the authority the agent is supposed to have
Obtain the approved task scope, action policy, and permission assignments. Map each permission to a business need, including access inherited through delegated identities. Classify actions by consequence and reversibility. Decide which actions need a preview, human approval, dual control, or emergency stop before execution.
Look closely at write and delete privileges, access to sensitive data, and tools that can create external or difficult-to-reverse effects. Identify newly added or unknown tools and treat them as needing review until their purpose and authorization are established. OWASP recommends action previews, explicit approval for high-impact or irreversible actions, audit trails, and interruption or rollback controls.
Write the authority in operational terms: which resources may be read, created, changed, or deleted; which values or destinations are allowed; what limits apply to volume, time, geography, or cost; and what conditions require escalation. Distinguish the authority of the initiating user from the authority delegated to the agent. A user may be allowed to request an activity without the agent being allowed to perform every step autonomously.
3. Select representative runs
Choose samples that cover both routine work and meaningful risk. Include normal in-scope activity and high-impact workflows, not only successful or neatly documented runs. For each sample, preserve the task or workflow identifier, the evidence references, the expected control behavior, and the observed result.
Sampling should reflect how the agent is actually used. Consider different user groups, tools, permission paths, delegated agents, and workflow outcomes. If the agent can change its configuration or acquire new tools, include runs from before and after relevant changes where records are available.
Include at least one run with a denial, error, retry, or partial completion. Failures often reveal whether approvals are checked before execution, whether fallback tools have broader privileges, and whether operators receive a usable alert. For high-impact workflows, select a sample large enough to reveal inconsistent enforcement rather than relying on a single demonstration run.

Follow the chain from the user request or automated trigger through the agent identity, delegated principal, relevant context, policy decision, tool invocation, approval, downstream action, result, and final state. Check whether timestamps and correlation identifiers let you join events across services. If a step is missing, record exactly which link in the chain cannot be established rather than inferring that it occurred.
The Singapore addendum’s event categories can help organize this trace: action, input or output, state change, error, timing, and context identifiers. Include memory interactions or agent-to-agent messages when they materially affect the action or help explain it.
Compare event times carefully. Clock skew, asynchronous queues, retries, and delayed exports can make a valid sequence look inconsistent. Verify the time source and timezone for each system, preserve original timestamps, and record ingestion time separately when possible. A correlation identifier should follow the workflow across the orchestrator and service boundary; if a platform generates a new identifier, retain the mapping rather than discarding the original.

5. Reconcile agent traces with independent records
Agent traces are useful for understanding the reported sequence, but they are not independent proof that a tool call executed as described. Compare them with available identity-provider, cloud, API, database, or SaaS audit records. Verify which principal performed the operation, when it happened, whether the principal had approved permissions, and whether the downstream state matches the reported outcome.

Investigate missing, duplicate, or inconsistent events; unexplained identity changes; and access to logging or monitoring systems by the agent. A trace that says an action succeeded while the service record shows no execution is a discrepancy to resolve, not a successful audit result. Likewise, a downstream change with no corresponding agent or approval evidence may indicate a logging gap or an unaccounted execution path.
Use independent records to distinguish an intended call from an executed call. An API gateway may show a request that a database rejected; a cloud record may show the service identity while the agent trace names only a user; a SaaS audit log may show a state change after the orchestration trace ended. Reconcile these differences and document which record is authoritative for each fact. Preserve the original records and the analyst’s reconciliation notes so another reviewer can reproduce the conclusion.
6. Exercise controls with real test cases
Documentation establishes what a control is supposed to do; an audit should also check whether it works. In a controlled environment or otherwise approved test process, record expected and observed results for cases such as:
- An action that is within the approved scope.
- An out-of-scope request that should be blocked.
- A high-impact action that should pause for approval before execution.
- An unavailable or compromised tool, to assess how the agent and operators respond.
- An interruption during a run, to establish whether the operator can stop or contain it.
Also test a denied approval, an expired approval, a changed permission during execution, and a retry after a transient failure when those conditions are realistic for the deployment. Verify that a preview accurately describes the pending action and that approval is bound to the specific task, resource, and version rather than being reusable for an unrelated request. Check whether rollback is available and whether it actually restores the intended state; where rollback is impossible, verify that escalation and containment are defined.

For each test, record the evidence identifiers, control owner, and any remediation. Confirm that anomaly detection and incident response can use the available records to reconstruct what happened. These are practical audit procedures derived from guidance on monitoring, logging, approval, and interruption—not a single mandated test suite.
7. Check log integrity, access, and retention
Determine who can read, export, change, or delete logs, and whether access to the logs is itself recorded. Check how the organization identifies missing events or unauthorized changes, and whether event timing and identifiers synchronize across sources. Consider whether the agent can access or alter the monitoring systems meant to observe it.
Review separation of duties: the person or service that operates an agent should not be able to silently rewrite evidence of its actions. Examine administrative roles, export paths, backups, retention locks, and alerting for deletion or unexpected gaps. Test a controlled query or export to confirm that records remain searchable and that redaction does not remove the identifiers needed for correlation.
Set retention and redaction based on applicable law, contracts, incident-response needs, and the sensitivity of prompts or retrieved data. The guidance cited here supports comprehensive audit trails and monitoring but does not establish one retention period or log schema for every organization. Document the rationale for the specific deployment instead of treating a chosen duration as a universal requirement.

8. Report findings so they can be fixed
For each finding, state the approved expectation, observed evidence, risk, affected action or data, control owner, and remediation. Prioritize findings that prevent reliable attribution of consequential actions; show privileges beyond approved scope; lack approval evidence; break cross-system correlation; permit undetected log changes; or leave operators unable to stop or contain a run. Retest the remediation and repeat the review after material system changes.
Describe severity in terms of plausible consequence and evidentiary uncertainty. A missing trace for a harmless read is different from an un attributable deletion, but both may indicate a systemic gap. Identify compensating controls, deadlines, and the evidence required to close the finding. Keep unresolved assumptions visible rather than presenting a reconstructed sequence as certain when independent records are incomplete.
How do you judge whether the audit approach is adequate?
Assess the evidence and controls against the deployment’s risk rather than checking whether a platform has a feature called “audit log.” A useful approach covers both what the agent reports and what independent systems can verify.
- Event coverage: Can the organization account for agent decisions, tools, approvals, delegated activity, errors, and downstream state changes relevant to the workflow?
- Attribution: Can a reviewer connect each consequential action to an initiating task, agent identity, and executing principal?
- Independent corroboration: Are there platform or service records that can confirm whether an operation actually occurred?
- Integrity and access: Can the organization detect missing or altered records, and are access to logs and monitoring systems controlled and recorded?
- Privacy and retention: Does the evidence preserve what is needed for audit and incident response without collecting or retaining unnecessary sensitive payloads?
- Operational response: Can alerts reach responsible operators, and can they investigate, pause, or contain a run?
- Portability: Can evidence be correlated across the model, orchestration layer, cloud, and connected services used by the deployment?
When comparing implementation approaches, examine coverage of agent and downstream events, identity and delegated-action correlation, independent corroboration, tamper resistance, access control, privacy and minimization, retention and export, alerting, incident workflow, and portability across model, orchestration, cloud, and SaaS services. A centralized platform may simplify operations, while native service records may provide stronger evidence of execution; assess how the two are joined and what happens when one is unavailable.
A platform’s own trace view can help operators understand a run. Underlying service audit records are valuable for corroborating whether an action executed, under which identity, and with what result. These evidence sources serve different purposes; one should not be assumed to replace the other.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. When should an organization repeat the audit?
Agent auditability is an ongoing control, not a one-time sign-off. Monitor for anomalous behavior and configuration or permission drift, and repeat the review after a material change to the model, tools, prompts, data sources, permissions, or connected services.
Microsoft’s guidance emphasizes inventory and lifecycle governance, least privilege, unique auditable identities, and accessible post-execution records of actions, tools, and outcomes. The Canadian Centre for Cyber Security recommends continuous monitoring and auditing, including anomaly detection and independent review. For professional audit contexts, the Financial Reporting Council lists its Generative and Agentic AI Guidance as published on 30 March 2026; it addresses audit-quality risks, mitigations, and professional judgment within its scope.
Trigger an additional review after a new connector, model or provider version, memory store, prompt policy, delegated identity, or autonomy setting is introduced. Review permission changes and unusual tool use between formal audits. If an incident occurs, preserve relevant records before routine retention processes remove them, then feed lessons into the authority matrix, logging design, and control tests.

Frequently Asked Questions
Can an AI agent’s own explanation prove what it did?
No. A generated explanation can help a reviewer understand a run, but it may omit events or fail to match what a connected service executed. Corroborate important claims against the service or platform records that exist for that workflow.
Who should own an autonomous-agent audit?
The accountable business owner should be identifiable, while security, platform, governance, and internal audit teams contribute according to their responsibilities. Separate the person responsible for approving the agent’s authority from the evidence reviewer where practical, especially for consequential workflows.
Does auditing an AI agent require saving every prompt and tool payload?
Not necessarily. The evidence must be sufficient to attribute and reconstruct relevant actions, but sensitive payloads should be minimized, redacted, access-controlled, and retained according to applicable requirements and incident needs.
Is there a standard AI agent audit checklist or required log format?
The guidance discussed here does not establish one universal checklist, event schema, or retention period for all deployments. Organizations need to document a risk-based approach that fits their jurisdiction, sector, data, and system role.
Should delegated sub-agents and MCP tools be included?
Yes, when they can retrieve data, call tools, communicate decisions, or change state. Include them in the inventory and preserve parent-child identifiers, identities, authorization decisions, inputs or outputs, errors, and downstream effects so the delegation chain can be reconstructed.
What if the agent trace and a service audit log disagree?
Treat the mismatch as a finding to investigate. Compare timestamps, correlation identifiers, principals, retries, and service outcomes; determine which record is authoritative for the fact at issue, preserve both records, and document any remaining uncertainty.
How can an organization test an emergency stop safely?
Use an approved controlled environment or test process. Start a representative run, invoke the interruption mechanism, verify that further actions are prevented or contained, check the downstream state, and confirm that the stop event and operator response are logged.
How often should permissions and logs be reviewed?
Review continuously for anomalies and drift, and perform a fuller audit after material changes to models, tools, prompts, data sources, permissions, memory, or connected services. The appropriate cadence depends on the deployment’s risk and obligations.
The Bottom Line
An auditable agent is one whose authority is explicit, consequential actions are attributable, traces are corroborated by independent system records, and operators can detect, approve, interrupt, and contain its behavior. Revisit those controls whenever the agent or its dependencies change.
