Do not treat an AI-generated report as evidence until a human reviewer has tested it. Start by defining the decision it will support and the consequence of an error. Then extract every material claim into a review table, open and verify each source, independently reproduce important calculations, test assumptions and consistency, and escalate consequential findings to a qualified subject-matter reviewer. Keep a complete record of the model or system, prompt, source files, tests, exceptions and approval decision. This process catches fabricated citations, omitted context, unstable outputs and plausible-looking arithmetic before they influence a health, financial, legal, employment, safety, policy or security decision.
What does an audit of an AI-generated report actually involve?
An audit is a structured challenge to the report’s claims, evidence, methods and controls—not a spellcheck or a general read-through. The report should be treated as an unverified draft, even when it is fluent, detailed or confident. A useful audit asks whether each important conclusion is supported by authentic, relevant and current evidence; whether the calculations can be reproduced; whether the assumptions fit the decision; and whether the process is safe, fair, explainable and properly governed.
Before checking a sentence, write down the intended decision, decision owner, deadline, affected people and maximum acceptable error. A report that is adequate for brainstorming may be unacceptable for approving a loan, changing a medical treatment, dismissing an employee or selecting a safety control.

Set an explicit disposition
- Approve: material claims pass the required tests and limitations are acceptable.
- Approve with conditions: use is restricted by stated caveats, additional monitoring or a lower-risk decision.
- Return for correction: errors are fixable, but the report cannot yet support action.
- Reject: evidence is fabricated, inaccessible, materially wrong or unfit for the decision.
How do I triage the report before spending time on every detail?
Use consequence and uncertainty to determine review depth. High-consequence decisions require qualified human review even when the report appears well sourced. Unknown model identity, inaccessible evidence, unexplained discrepancies and sensitive personal data are escalation triggers.

Do not let a high volume of citations create false reassurance. Ten weak or duplicated sources do not substitute for one authoritative source that directly supports the claim.
How do I turn a long report into auditable claims?
Break the report into statements that could individually change the decision. Include factual assertions, quantified results, causal explanations, forecasts, recommendations and statements about the AI system itself. Separate a claim from its interpretation: “sales fell 12%” is testable data; “the campaign failed” is an inference that needs a defined comparison and time period.
- Copy the report into a controlled version and assign a report identifier and date.
- Highlight every material claim, number, citation, assumption and recommendation.
- Give each item a unique claim ID. Combine duplicates only when they have exactly the same meaning and evidence.
- Record the source named by the report, publication date, version, jurisdiction, definitions and stated limitations.
- Assign an initial confidence and risk rating, then update it after testing.
Use a worksheet with these fields:

How do I detect fabricated citations and confabulated support?
NIST calls confidently presented but erroneous or false content “confabulation” and warns that generative systems can fabricate citations or reasoning. A citation is not validated until you open the original material and match the exact passage to the exact claim.
Verify the source itself
- Open the cited publication, dataset, regulation, standard or web page independently of the report.
- Confirm that the title, author or issuing body, date, edition and identifier actually exist.
- Locate the cited passage, table or data series. Read the surrounding context, footnotes and definitions.
- Check whether the source supports the report’s wording, or only a narrower statement.
- Record geography, population, time period, version and scope limits. A real source can still be misapplied.
Watch for citations that resolve to a different title, a dead page, a search-result snippet, a secondary summary or a document published after the report’s stated retrieval date. A reference that cannot be accessed is unverified, not automatically false; mark it unresolved and prevent it from carrying a decisive conclusion.

Check for citation laundering
AI reports may repeat a claim through several references that all trace back to one unsourced assertion. Map the dependency: identify the primary evidence, then distinguish independent corroboration from copies, press releases and commentary. If the report says a source “proves” causation but the source reports only correlation, downgrade or reject that conclusion.
How do I test evidence, provenance and fitness for purpose?
Ask whether the evidence is suitable for the decision, not merely whether it exists. The National Telecommunications and Information Administration says AI audits should evaluate claims about an AI system’s fitness for purpose, performance, processes and controls. Apply those same questions to the report.
- Fitness: Does the evidence measure the outcome the decision requires, using a relevant population and timeframe?
- Data quality: Are inputs complete, accurate, current and consistently defined? Are missing values, exclusions, duplicates and transformations disclosed?
- Provenance: Can you trace data from collection through storage, retrieval, transformation and inclusion in the report?
- Independence: Could the source or evaluator benefit from the conclusion? Are conflicts disclosed?
- Methods: Does the report explain sampling, search, model use, coding, weighting and limitations well enough to reproduce the work?
- Controls: Who approved the prompt, data access, model configuration, output review and release?
Source quality is multidimensional. An official dataset can be outdated; a recent study can have a narrow sample; a large survey can use a definition that does not match your policy. Record the mismatch instead of silently “correcting” it.

Every number capable of changing the decision needs an independent check. Obtain the underlying data where possible, recreate the formula in a separate spreadsheet or script, and compare the result with the report. Do not merely recalculate using the report’s rounded intermediate values.

- Copy the original inputs, units, dates and denominators into a controlled calculation file.
- Write the formula in plain language and then implement it independently. State whether the result is a sum, rate, percentage change, weighted average, forecast or model estimate.
- Recalculate from unrounded inputs and preserve the calculation steps and software version.
- Check dimensional consistency: dollars versus thousands of dollars, monthly versus annual rates, people versus households, and percentage points versus percent change.
- Test denominators and population definitions. A rate can change substantially when exclusions or a different baseline are used.
- Run reasonable alternative assumptions and record how much the result moves.
- Compare the reproduced value with every table, chart, narrative statement and recommendation in the report.
Investigate any mismatch, including a difference caused by rounding. A seemingly minor arithmetic error can reverse a ranking or push a threshold-based decision over its limit. The Australian Auditing and Assurance Standards Board warns reviewers to examine inputs, assumptions, logic, outputs and evidence authenticity rather than over-rely on AI-generated information.
Use a sensitivity record
For each consequential estimate, list the base assumption, plausible low and high alternatives, resulting range and decision threshold. If a recommendation changes under a reasonable alternative, present the result as conditional rather than definitive.
How do I test whether the report is grounded and internally consistent?
Groundedness means that generated claims can be traced to reliable source material and that the citations are accurate and relevant. AWS recommends measuring this property rather than assuming it from fluent prose.

- For each paragraph containing a factual claim, point to the exact source passage or data row.
- Mark claims that rely on general model knowledge, inference or an unstated assumption.
- Check that the conclusion does not exceed the strength, precision or population of its evidence.
- Compare summaries with full documents; summaries can omit qualifiers, exceptions and competing findings.
Run stability checks
Ask the system the same question in several phrasings, regenerate key sections when the system permits it, and compare report versions. Look for changing numbers, different definitions, omitted caveats or contradictory recommendations. Instability is a warning that the conclusion may depend on wording or sampling rather than evidence. Automated comparison can flag differences at scale, but a domain expert must judge whether a difference is material.

Check internal logic
Search for a denominator that changes between sections, a chart that disagrees with its caption, a recommendation that conflicts with a stated limitation, or a conclusion that reverses the direction of a reported effect. Treat unresolved contradictions as blocking issues when they affect the decision.
When must a qualified human reviewer intervene?
Human review is mandatory when consequences are serious or the evidence is weak. Escalate when the report affects health, safety, finances, legal rights, employment, public policy or security; when personal or confidential data is involved; when the model or source data is unknown; when evidence cannot be accessed; or when reviewers find unexplained discrepancies.
The reviewer should have subject-matter knowledge sufficient to challenge definitions, assumptions and practical implications—not merely the ability to edit prose. Define an override path: who can reject the AI output, who resolves disagreements, what evidence is required to change a disposition, and when the decision must be revisited. Financial Reporting Council guidance published March 30, 2026, emphasizes audit-quality risks, mitigation and professional judgment for generative and agentic AI.
Which trustworthiness criteria should I apply?
Use the NIST AI Risk Management Framework 1.0, published January 26, 2023, as a voluntary, use-case-agnostic set of review axes. More than 240 organizations contributed to its development. Apply only the criteria relevant to the decision, but document why an axis was omitted.

How should I document approval, rejection and future rechecks?
Preserve an immutable package that lets another reviewer reconstruct what happened. Keep the report version, model or system identifier, model settings where available, exact prompt, system instructions, source files, retrieval dates, access permissions, claim-check table, independent calculations, sensitivity results, reviewer names and qualifications, exceptions, unresolved issues and final approval or rejection.
Record the conditions of approval: permitted use, audience, date range, confidence limits, monitoring owner and expiry or review date. State what changes trigger a new audit, such as a source revision, model update, prompt change, data refresh, new jurisdiction or an incident. Define who can override the output and require the reason and evidence for every override.
What is a practical final-release checklist?
- The decision, error consequence and scope are written down.
- All material claims have owners, sources, dates, jurisdictions, confidence and dispositions.
- Every citation was opened and matched to the claim; inaccessible evidence is marked unresolved.
- Data provenance, definitions, missing values, transformations and conflicts are documented.
- Material calculations were independently reproduced with units, denominators and unrounded inputs checked.
- Reasonable alternative assumptions and decision thresholds were tested.
- Summaries, tables, charts and conclusions agree.
- Groundedness and stability checks found no unexplained material variation.
- Privacy, security, safety, explainability and fairness risks were assessed where relevant.
- A qualified human reviewer approved, conditioned, returned or rejected the report, with an override path and revisit date.
Frequently Asked Questions
Can I use an AI report if its original sources are behind a paywall or login?
Only with a documented limitation. Ask the publisher, data owner or a permitted independent reviewer to verify the exact passage; until that happens, mark the claim unresolved and exclude it from any decision that depends on it.

Use the least data necessary, follow your organization’s retention and access controls, and avoid pasting sensitive material into an unapproved service. Keep a record of where prompts, uploaded files and generated logs are stored and who can access them.
How often should an approved AI-generated report be re-audited?
Set a review date tied to the decision’s lifespan and trigger an earlier review after a model, prompt, source, dataset, jurisdiction or business-rule change. Reopen the audit after an incident or a material discrepancy, even if the scheduled date has not arrived.
Can automated citation or arithmetic checks replace a human reviewer?
No. Automation is useful for scale, such as detecting broken links, inconsistent totals or changed wording, but it cannot reliably judge context, legal meaning, fairness or whether evidence is fit for the decision. A qualified reviewer remains responsible for consequential approval.
