Assess a connected medical device as a complete clinical system, not as isolated firmware. Define the device’s intended function and operating environment; map every interface, data flow, dependency and trust boundary; model threats across deployment, interoperability, supply chain, maintenance and retirement; then rate how feasible exploitation is and what harm could result for safety, effectiveness, availability, integrity, confidentiality and clinical operations. Verify each control in the real care environment, document residual risk and maintain evidence throughout the product lifecycle.

This approach aligns with the U.S. Food and Drug Administration’s February 2026 premarket cybersecurity guidance, its postmarket expectations, IEC 81001-5-1:2021 for secure health-software lifecycle processes, and the general assessment structure in NIST SP 800-30 Rev. 1. None of those references turns a generic scan or a standards claim into proof that a device is safe.

What is the correct unit of analysis for a connected medical device?

The unit of analysis is the medical-device system in its intended environment of use. Firmware is only one element. A realistic assessment includes the physical device, embedded software, companion applications, accessories, gateways, hospital networks, cloud services, update infrastructure, users, clinical data, third-party components and operational procedures.

Start by writing the intended clinical function in concrete terms: what the device measures or delivers, which outputs clinicians rely on, what timing or availability is required, and what happens if data are delayed, altered or unavailable. Record the intended geography, user groups, deployment model, interfaces and assumptions. Separate manufacturer responsibilities from controls operated by a healthcare delivery organization, while documenting the dependency between them.

Scope items to record

  • Device hardware, firmware, operating systems, embedded libraries and safety-critical functions.
  • Accessories, sensors, chargers, removable media and service tools.
  • Mobile, desktop or web companion applications and their authentication stores.
  • Gateways, wireless links, wired networks, remote-service channels and cloud APIs.
  • Interfaces to electronic health-record, PACS, laboratory, pharmacy or other clinical systems.
  • Users, administrator roles, technicians, support personnel and third-party operators.
  • Protected health information, telemetry, configuration data, logs, keys and credentials.
  • Manufacturing, logistics, installation, maintenance, update, backup, recovery and decommissioning processes.

State assumptions explicitly. FDA’s example is to treat hospital networks as inherently hostile and to assume an adversary may alter, drop or replay packets. An assumption is not a control; it is a condition that must be tested for reasonableness and revisited when the environment changes.

How do I map the system and its trust boundaries?

Draw data-flow and control-flow diagrams before listing threats. Show where data originate, how commands are authenticated, where decisions are made, and which party administers each component. Mark a trust boundary wherever privilege, ownership, protocol, physical access or network exposure changes.

Minimum diagram coverage

  1. Place the device and safety-critical sensors or actuators at the center.
  2. Add companion apps, gateways, clinician workstations, mobile devices and remote-service tools.
  3. Connect EHR, PACS and other interoperability interfaces, identifying protocol and direction of flow.
  4. Show wireless, wired, removable-media and maintenance connections, including dormant ports.
  5. Add cloud services, identity providers, update servers, build systems, package repositories and support vendors.
  6. Label each flow with data type, confidentiality, integrity and timing requirements.
  7. Mark authentication, authorization, key-management and logging points.
  8. Document where the device can continue safely if a link, service or credential is unavailable.
Boundary or dependency Questions to answer Typical failure consequence
Wireless or wired network Can packets be injected, replayed, dropped or reordered? Is mutual authentication used? Incorrect commands, stale measurements or loss of availability.
Interoperability interface Which fields are trusted? How are identities and units validated? What happens on malformed input? Wrong-patient association, corrupted records or unsafe decisions.
Remote service channel Who can connect, from where, for how long and with what audit trail? Unauthorized configuration or persistent access.
Update infrastructure Are packages signed, version-checked and protected against rollback or substitution? Malicious code, loss of safety features or unpatchable exposure.
Supply chain Which commercial and open-source components are included, and how are they maintained? Inherited vulnerability or compromised build artifact.
Physical and removable media Can an attacker reach debug ports, storage or service media? Credential extraction, malware introduction or data disclosure.

What should a medical-device threat model include?

A threat model should identify system risks and mitigations, state assumptions about the environment of use, and capture risks introduced through the supply chain, manufacturing, deployment, interoperation, maintenance and updates, and decommissioning. Treat it as a living engineering artifact linked to requirements, tests and residual-risk decisions.

Choose and explain a method

Use a documented method appropriate to the architecture, such as data-flow-based abuse-case analysis, attack trees, a structured security framework or a combination. The method matters less than traceability: every important asset and boundary should lead to threat events, controls, verification evidence and an owner. Explain why the method covers wireless, cloud, human, physical and third-party attack paths rather than only network vulnerabilities.

Threat categories to examine

  • Identity and privilege: default or shared credentials, weak recovery, excessive administrator rights, stolen tokens and undocumented service accounts.
  • Interfaces and protocols: parser flaws, unauthenticated commands, replay, downgrade, unsafe error handling and insecure legacy protocols.
  • Cryptography and secrets: hard-coded keys, weak randomness, exposed certificates, poor rotation and plaintext sensitive data.
  • Software supply chain: vulnerable or abandoned open-source packages, compromised dependencies, build-system access and incomplete component inventories.
  • Update and rollback: unsigned packages, interrupted updates, downgrade paths, unavailable patches and unsafe recovery modes.
  • Operations and people: misconfiguration, phishing of support staff, shared workstations, inadequate logs and untrained users.
  • Physical exposure: debug interfaces, accessible storage, theft, tampering and service-port abuse.
  • Safety behavior: whether a security failure causes a hazardous output, masks an alarm, corrupts a measurement or prevents a safe manual fallback.
  • Lifecycle transitions: factory provisioning, installation, transfer between facilities, loaner equipment, repair, resale and secure retirement.

How should exploitability and patient-safety impact be scored?

Do not reduce cybersecurity to a conventional probability-of-harm calculation. FDA notes that cybersecurity incidents are difficult to predict probabilistically. Assess how feasible an attack is and what consequences could follow, then explain the evidence and uncertainty behind each rating.

Exploitability dimensions

  • Access required: internet, adjacent network, local network, authenticated user, technician or physical access.
  • Attack complexity: skill, specialized equipment, timing, race conditions and knowledge of the architecture.
  • Privileges and credentials: whether a normal account, elevated role, stolen key or no authentication is needed.
  • Interaction and persistence: whether a victim must open a file or approve a prompt, and whether access survives reboot or update.
  • Detectability and containment: quality of logging, network segmentation, rate limits and the ability to revoke access.
  • Exposure and reach: number of deployed units, shared services, remote connectivity and concentration of common components.

Clinical and operational consequence dimensions

  • Safety: possibility of injury, incorrect therapy, missed alarm or unsafe actuation.
  • Effectiveness: degraded diagnostic accuracy, treatment delay or loss of intended clinical benefit.
  • Availability: downtime, denial of service, battery drain or inability to recover quickly.
  • Integrity: altered measurements, settings, patient identity, timestamps or audit records.
  • Confidentiality: disclosure of health, identity, location or operational data.
  • Clinical operations: workload, diversion, manual procedures, cross-system disruption and delayed care.

Record pre-mitigation and post-mitigation ratings, the rationale, confidence level, acceptance criteria and decision authority. A low-likelihood event with catastrophic clinical consequences still needs strong treatment if exploitation is feasible. Conversely, a highly visible nuisance with no plausible path to patient harm may be managed through monitoring and recovery rather than a redesign. Do not hide uncertainty by assigning a precise number that the evidence cannot support.

How do I select and verify cybersecurity controls?

Select controls against the modeled attack paths and clinical hazards, then verify them in the intended clinical environment. A control is incomplete until its operating assumptions, test method, pass criteria, owner and failure behavior are documented.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Control area Implementation questions Evidence to retain
Secure design Are services minimized, inputs validated and privilege separated? Architecture, threat traces, code review and negative tests.
Authentication and authorization Are unique identities, strong authentication, least privilege and time-limited service access enforced? Role matrix, configuration tests, credential-lifecycle records and audit logs.
Segmentation Can a compromised workstation reach safety-critical functions or update services? Network diagrams, firewall rules and isolation testing.
Secure update and rollback Are packages signed, versions checked, keys protected and interrupted updates recoverable? Signature, anti-rollback, power-loss and recovery test results.
Monitoring and response Are security-relevant events logged, time-synchronized, protected and actionable? Log samples, alert tests, retention settings and response playbooks.
Backup and fallback Can clinicians continue safely or restore trusted configuration after compromise? Recovery-time tests, manual procedures and clinical usability validation.
Disclosure and patching Is there a vulnerability-reporting channel, triage process and supported update path? Policy, service-level targets, advisory workflow and release records.

Test controls under realistic constraints: intermittent connectivity, shared clinical workstations, alarm timing, battery limits, emergency access and concurrent patient care. A security setting that blocks urgent treatment or creates alarm fatigue can introduce a safety hazard. Feed that interaction into the safety-risk process rather than treating cybersecurity and safety as separate checklists.

How should cybersecurity findings connect to ISO 14971 safety risk management?

Transfer security-related hazards that can cause diagnostic, therapeutic or operational harm into the device’s safety-risk process. Keep the analyses linked but distinct: cybersecurity exploitability describes attack feasibility, while ISO 14971 processes address safety hazards and risk controls under their applicable assumptions. Do not substitute an ISO 14971 probability estimate for an attack-path analysis.

For each security scenario, trace the chain from threat event to compromised asset, hazardous situation, foreseeable sequence of events, harm, control and verification. Reassess the safety file when architecture, software, deployment environment, interoperability partner or update mechanism changes. Confirm that residual risk is acceptable under the manufacturer’s criteria and that information needed by healthcare organizations is communicated.

What does the FDA expect in a U.S. cyber-device submission?

For a U.S. cyber-device submission, prepare evidence that a reviewer can trace from system scope and threat model through controls, testing and residual-risk decisions. Section 524B applies to 510(k), PMA, Product Development Protocol, De Novo and Humanitarian Device Exemption submissions that meet the statutory cyber-device definition.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Core submission evidence

  • System description, intended use, operating environment, data flows and trust boundaries.
  • Threat model with assumptions and coverage of supply chain, manufacturing, deployment, interoperability, maintenance, updates and decommissioning.
  • Cybersecurity risk assessment showing exploitability, clinical and operational consequences, pre- and post-mitigation ratings and acceptance rationale.
  • Security requirements, architecture, implemented controls and verification or validation results.
  • Component and dependency information, including a cybersecurity bill of materials or equivalent information where applicable.
  • Secure update, rollback, access-control, logging, backup, recovery and incident-response capabilities.
  • Postmarket monitoring, coordinated vulnerability disclosure, vulnerability triage and patch/update plans.
  • Traceability to safety-risk controls and an explanation of residual-risk governance.

FDA guidance is generally nonbinding unless incorporated into law or a specific submission requirement. Confirm the device’s jurisdiction, classification, submission type and the current FDA expectations before filing. Manufacturers remain responsible for identifying risks and hazards, including cybersecurity risks; a hospital’s network controls do not transfer that responsibility.

How do IEC 81001-5-1 and NIST SP 800-30 fit the assessment?

IEC 81001-5-1 Edition 1.0 (2021-12) provides a structured secure health-software lifecycle. It can organize planning, security requirements, architecture, implementation, verification, release, maintenance and problem resolution. FDA recognizes it as a complete consensus standard for secure health-software lifecycle processes, but conformance alone may not satisfy every Section 524B or guidance requirement. Treat it as lifecycle structure, not as a safety certificate.

NIST SP 800-30 Rev. 1, published in September 2012, offers a general framework for preparing, conducting, communicating and maintaining risk assessments. It is useful for organizing the work, documenting assumptions and communicating uncertainty, but it does not replace medical-device-specific clinical impact analysis, FDA evidence or safety-risk integration.

How do postmarket monitoring and incident response change the assessment?

Assessment continues after release. FDA’s postmarket expectations span design, development, production, distribution, deployment and maintenance because new vulnerabilities, attack techniques, integrations and clinical uses can change risk.

Operate a feedback loop

  1. Monitor vulnerability advisories, component inventories, threat intelligence, support tickets, logs and reports from users or researchers.
  2. Provide a coordinated vulnerability-disclosure channel and acknowledge, triage and prioritize reports.
  3. Reproduce the issue, identify affected versions and environments, and assess exploitability and clinical consequences.
  4. Contain exposure through segmentation, credential revocation, configuration guidance or temporary service restrictions when needed.
  5. Develop, verify and safely deploy a patch or compensating control, including rollback and recovery testing.
  6. Notify affected customers and authorities when required, with clear instructions and residual-risk information.
  7. Update the threat model, safety analysis, component information, test evidence and release documentation.

Plan secure decommissioning: revoke certificates and remote accounts, erase or cryptographically destroy stored data, remove the device from management systems, disable update or service channels and retain records needed for safety and regulatory traceability.

How can I compare assessment approaches or external providers?

Compare deliverables and coverage, not marketing labels or scan counts. Ask each candidate to show how its work addresses the following dimensions.

Comparison axis Evidence of a strong approach
System boundary Includes device, software, cloud, networks, users, accessories and clinical workflows.
Threat-model technique Documented method with assets, boundaries, assumptions, attack paths and mitigations.
Lifecycle coverage Addresses supply chain, manufacturing, deployment, maintenance, updates and retirement.
Exploitability scoring Explains access, complexity, privileges, interaction, persistence and uncertainty.
Patient-safety linkage Traces cyber events into hazardous situations and verifies controls clinically.
Interoperability and supply chain Tests external interfaces and maintains component/dependency information.
Clinical-environment validation Exercises emergency access, downtime, alarms, segmentation and recovery under realistic conditions.
Regulatory evidence Produces traceable artifacts suitable for the applicable submission and jurisdiction.
Residual-risk governance Defines acceptance criteria, decision authority, exceptions and reassessment triggers.
Postmarket support Includes disclosure, monitoring, triage, patch delivery, communications and incident response.

What are the most common assessment failures?

  • Scanning only the firmware: The largest risks may sit in cloud administration, interoperability, remote service or hospital deployment.
  • Assuming the hospital network is trusted: A compromised workstation or unmanaged device can reach exposed interfaces.
  • Counting vulnerabilities without clinical context: Severity scores alone do not show whether a measurement, therapy or alarm can be affected.
  • Using certification as a verdict: IEC 81001-5-1 conformance or a penetration-test report does not prove safe operation in every environment.
  • Ignoring updates and retirement: An effective control that cannot be patched, rolled back or securely decommissioned will age into risk.
  • Failing to test recovery: A theoretically secure device may leave clinicians without a safe fallback during an outage.
  • Leaving assumptions undocumented: Unstated dependencies make residual-risk decisions impossible to review.
  • Confusing manufacturer and provider duties: Both parties need defined controls, handoffs and information, but their obligations are not identical.

What should the final assessment package contain?

Before release or a major change, assemble one traceable package: scope and intended-use statement; system and data-flow diagrams; trust-boundary register; assumptions; threat model; vulnerability and threat-event analysis; exploitability and clinical-impact ratings; security and safety requirements; control implementation and verification evidence; component information; residual-risk decisions; postmarket and disclosure plans; update, rollback, backup and recovery procedures; incident-response contacts; and decommissioning instructions.

Assign an owner and review date to every open risk. Trigger reassessment after a material architecture change, new cloud or interoperability dependency, newly discovered vulnerability, changed clinical workflow, new deployment geography or update-mechanism change. This makes the assessment an operating control rather than a one-time document.

Frequently Asked Questions

Is a penetration test enough to assess a connected medical device?

No. A penetration test samples exploitable weaknesses at a point in time. It does not replace system scoping, clinical-impact analysis, supply-chain review, safety integration, lifecycle evidence or a plan for monitoring and patching new issues.

Who is responsible for cybersecurity after a device is installed in a hospital?

The manufacturer remains responsible for identifying and managing risks associated with the device and for maintaining appropriate update and disclosure capabilities. The healthcare organization operates local controls such as segmentation, identity management, asset inventory, monitoring and incident response; the handoff and assumptions should be documented.

How often should a medical-device cybersecurity risk assessment be updated?

Update it whenever a change can alter attack paths or clinical consequences, including software or architecture changes, new interfaces, deployment-environment changes and newly discovered vulnerabilities. Continuous postmarket monitoring should identify those triggers rather than relying only on a calendar review.

What is a cybersecurity bill of materials used for?

It identifies software components and dependencies so a manufacturer can determine which released versions are affected when a vulnerability is disclosed. It supports triage, customer communication, patch prioritization and regulatory evidence, but it is not itself a risk assessment or proof that components are secure.
