Put a human checkpoint in front of every consequential tool call, save the agent’s exact state and approved arguments, and make the effect idempotent before execution. If the action later fails or must be undone, restore a saved version, send a compensating request, or route the case to a clearly marked non-reversible workflow. This combination—approval interruption, durable checkpoint, atomic decision consumption, and tool-specific compensation—lets an agent wait for hours or days without losing context or sending the same email, payment, SQL write, or cancellation twice.
What are approval gates and rollback in an AI agent workflow?
An approval gate is a deliberate pause between an agent’s proposed tool call and the tool’s side effect. The model can reason, draft arguments, and ask for a decision, but your server—not the model or a browser button—decides whether execution may proceed. Rollback is the recovery path after an approved action has changed something: restore a prior version when possible, issue a compensating action for an external effect, or declare the operation non-reversible and require stronger controls before it runs.
A safe implementation treats the pause as a first-class workflow state rather than as an HTTP request that happens to be waiting. The server records the run, workflow version, agent identity, tool, validated arguments, policy result, and an idempotency key. A reviewer then approves, edits, rejects, or responds. The server atomically consumes that decision and resumes the original run from its checkpoint.
The lifecycle at a glance
- The agent proposes a tool call.
- Policy and automatic guardrails classify and validate it.
- If the call has a consequential side effect, execution is withheld.
- A server-owned checkpoint stores the pending state and exact arguments.
- An authenticated, authorized reviewer sees a least-privilege explanation and chooses a decision.
- The pending decision is consumed once, atomically.
- The original root run resumes from the stored state.
- The tool executes with an idempotency key and records its result.
- If recovery is needed, a tool-specific rollback or compensation workflow runs and records its outcome.
Which AI agent actions should require human approval?
Use automatic execution for low-risk reads and human review for side effects. OpenAI’s 2026 guidance summarizes the division as: “Use guardrails for automatic checks and human review for approval decisions.” It specifically calls out cancellations, edits, shell commands, and sensitive MCP actions as examples that warrant review.

| Action class | Typical treatment | Reason |
|---|---|---|
| Read-only queries and retrieval | Automatic, with input and output guardrails | They inspect data without changing an external system. |
| Record creation or edits | Approval required | The change can affect customers, compliance records, or downstream automation. |
| External messages such as email | Approval required | Once delivered, the message may be impossible to retract. |
| Payments, refunds, transfers, or order cancellation | Approval required; consider a higher threshold | These create financial or contractual consequences. |
| SQL that writes, deletes, or changes schema | Approval required and tightly scoped | The blast radius can exceed the model’s apparent intent. |
| Shell commands and infrastructure changes | Approval required; use an isolated execution context | Commands can affect data, credentials, or availability. |
| Sensitive MCP or privileged tools | Approval required | They may expose secrets or cross a trust boundary. |
Classify by the effect of the tool, not by its name. A tool called update_customer is still high risk if it can alter billing status; a read endpoint may be safe to automate even when it belongs to a sensitive system. Keep the policy beside the tool definition and evaluate it on every call, including calls produced after a retry or a nested handoff.
How should the approval checkpoint be designed?
The checkpoint must be owned by a trusted server and sufficient to resume safely without trusting data returned by a browser. Capture it immediately before the side effect, after arguments have passed validation and policy evaluation.
Store the minimum complete state
- Run identity: the root run ID and the pending interruption ID.
- Workflow identity: an immutable workflow-definition version and tool-schema version.
- Agent identity: the agent, tenant, and authorization context that generated the proposal.
- Tool identity: the exact tool name and version.
- Validated arguments: the canonical, server-validated payload that will be executed; do not replace it with untrusted browser input.
- Prior state: the serialized run state and a reference to the last safe checkpoint.
- Policy decision: which rule caused the gate, its risk classification, and any required approval level.
- Idempotency key: a stable key derived for this intended effect, not regenerated on every retry.
- Integrity data: a checkpoint hash or equivalent tamper-evident fingerprint.
Encrypt sensitive state, restrict access by tenant and role, and keep snapshots server-side. A reviewer needs enough context to make a decision, but not unrestricted secrets or unrelated customer records.
How do I implement an approval gate step by step?
1. Define a risk policy for every tool
Add an explicit needsApproval-style policy or equivalent middleware rule to each tool. Make the rule deterministic where possible: a refund above a threshold, a delete operation, a production shell command, or any message to an external recipient should produce a gate. Include the policy version in the checkpoint so a later policy change cannot silently reinterpret an old proposal.
2. Run automatic guardrails before the gate
Input guardrails validate types, authorization scope, destination, and data-handling rules. Tool guardrails check that the proposed operation is allowed for the current tenant and environment. Output guardrails can detect unsafe or malformed model output before it becomes a tool argument. These checks reduce reviewer workload; they do not replace the approval decision for a consequential effect.
3. Validate and canonicalize arguments
Resolve aliases, normalize dates and identifiers, apply allowlists, and reject fields the tool does not support. Store the canonical result. If a reviewer edits a value, run the same validation and policy checks again before creating a new approved payload.
4. Persist the interruption before returning to the caller
Write the checkpoint and pending-approval record in a transaction or an equivalent durable operation. The record should be visible to the reviewer only after it is complete. If persistence fails, do not expose an approval button and do not execute the tool.

5. Show a least-privilege review screen
Display the requested action, affected resource, normalized arguments, expected impact, policy reason, and links to the minimum evidence needed for review. Mask tokens, credentials, and unrelated records. Authenticate the reviewer, authorize the reviewer against tenant and action scope, and require re-authentication or a second approver for especially sensitive operations.
6. Support explicit decision types
- Approve: execute the stored, validated arguments.
- Edit: accept a constrained change, then revalidate and record the revised arguments as a new decision payload.
- Reject: end the pending call and return a structured denial to the agent.
- Respond: provide information when the human is acting as the tool rather than authorizing the original call.
LangGraph documents these four decisions—approve, edit, reject, and respond—for interrupting tool execution. Treat an edit as a new policy evaluation, not as a cosmetic change to the display.
7. Consume the decision atomically
Use a compare-and-set update, row lock, or equivalent transaction that changes the pending record from pending to consumed exactly once. Check the interruption ID, run ID, reviewer authorization, and checkpoint hash in that transaction. A second browser tab, webhook retry, or competing worker must receive an “already consumed” result and must not resume the run again.
8. Resume the original root run
Resume the same run from its stored state rather than starting a new conversation that attempts to reconstruct history. OpenAI’s Agents SDK guide describes this behavior directly: when a tool call requires approval, the SDK pauses the run, returns interruptions, and lets you resume later from the same RunState. The documented flow evaluates needsApproval, returns pending interruption items without executing the tool, applies state.approve(interruption) or state.reject(interruption), and resumes the original top-level run.
Recommended Free Tools
Approvals from nested handoffs and agents exposed through agent.asTool() surface on the outer run. Your persistence layer therefore needs a root-run relationship and must prevent a child worker from bypassing the parent’s pending decision.
9. Execute with an idempotency key
Pass the stored key to the downstream API when supported. For your own database writes, enforce a uniqueness constraint or use a transactional outbox: commit the intended effect and an outbound event together, then let a dispatcher retry delivery. If the worker crashes after the provider accepted the request but before your process recorded success, a retry with the same key must return the original result rather than create a second effect.
How does the OpenAI Agents SDK pause and resume an approved tool call?
The SDK’s human-in-the-loop model is an interruption in the run, not a second agent invocation. A tool declares when approval is needed; the runner returns interruption data; your application obtains a decision; and the original RunState is resumed after approval or rejection.

- Evaluate the tool’s approval predicate for the proposed call.
- If no prior decision exists, withhold execution and return the interruption.
- Persist the serialized state and interruption metadata on your server.
- After reviewer authorization, call the state’s approve or reject operation for that interruption.
- Pass the updated state back to the runner and continue the root run.
For an approval that may wait beyond the lifetime of a web request, serialize RunState and store it durably. Authenticate and authorize every resume request, validate that the state has not been superseded, and atomically consume the pending decision before invoking the runner. Do not place an unsigned state blob in a client cookie or accept replacement arguments from a client-side approval link.
How does LangGraph handle human review?
LangGraph middleware pauses tool execution with an interrupt. Its documented decision model lets a reviewer approve the original call, edit arguments conservatively, reject the call, or respond as the tool. A persistent checkpointer is required for production-grade waits because the graph must be reconstructed after a worker or request process disappears.
For an edited call, retain both the original proposal and the revised payload, record who changed it, rerun authorization and tool validation, and generate a fresh hash and idempotency key if the external effect has materially changed. For a rejection, resume the graph with a structured outcome so the agent can explain the denial or choose a safe alternative without retrying the blocked call.
What does Amazon Bedrock session management provide?
AWS documents session-management APIs that checkpoint workflow stages for iterative testing and human-in-the-loop workflows, resume conversations and tasks after interruption, and provide session logs for analysis and debugging. Session access is controlled with IAM and encrypted with AWS-managed or customer KMS keys.
AWS labels these APIs preview. Before adopting them for a production approval system, perform an API-stability review, confirm retention and regional behavior, and keep an application-level checkpoint and decision ledger so a preview service is not your only recovery mechanism.
When should I use a durable orchestrator?
Use durable orchestration when an approval can outlive a request process or worker, or when timers, retries, and recovery must be coordinated across services. OpenAI’s running-agents documentation names Dapr, Temporal, and Restate integrations for durable, long-running agents with human-in-the-loop tasks.
A durable orchestrator should wake the workflow when a decision arrives, enforce a timeout or escalation policy, and resume from a recorded checkpoint rather than replaying the entire trajectory. Keep the tool side effect idempotent even when the orchestrator offers workflow-level exactly-once semantics; the downstream provider may still receive a request twice after a network failure.

How do the main framework approaches compare?
| Approach | Approval behavior | Checkpoint and resume evidence | Distinctive considerations |
|---|---|---|---|
| OpenAI Agents SDK | Per tool call through an approval predicate; approve or reject an interruption | Resume the same serialized RunState; interruptions are returned by the paused run |
Nested handoffs and agent.asTool() approvals appear on the outer run |
| LangGraph | Interrupt-based review with approve, edit, reject, or respond | Persistent checkpointer is documented for production human-in-the-loop workflows | Edit decisions require conservative argument changes and renewed validation |
| Amazon Bedrock session management | Session checkpoints support interruption and human-in-the-loop stages | Session APIs resume conversations and tasks and provide session logs | APIs are labeled preview; IAM and AWS-managed or customer KMS encryption control access |
| Dapr, Temporal, or Restate integrations | Workflow-level human tasks coordinated by a durable engine | Designed for long waits, timers, retries, and worker recovery | Still requires application-level authorization, idempotency, and compensation logic |
Choose based on the boundary you need to control. Framework interruptions are useful inside an agent run; a durable orchestrator is useful when the run must survive infrastructure turnover and coordinate several services. Neither choice removes the need for a server-owned approval record and a tool-specific rollback plan.
How should rollback be designed for each kind of tool?
There is no universal undo command. Capture a checkpoint immediately before every effect and choose the recovery mechanism when you design the tool, not after an incident.

On a post-approval failure, resume from the last safe checkpoint or enter a compensation workflow. Do not blindly replay the entire agent trajectory: that can repeat earlier successful effects while attempting to fix a later failure.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How do I prevent a rejected or retried call from running twice?
Separate approval identity from execution identity. A rejection closes the pending approval and produces a terminal result for that proposal; it must not leave a resumable record that a stale worker can consume. An approval gets one durable execution key, and every retry uses the same key.
Use a transactional outbox for asynchronous effects
- In one database transaction, mark the approval consumed, record the intended effect, and insert an outbox event keyed by the idempotency key.
- A dispatcher sends the event to the provider and records the provider response.
- If delivery times out, retry the same event and key.
- Reconcile ambiguous outcomes with the provider’s idempotency lookup or request-status endpoint before deciding that a compensation is needed.
For synchronous tools, the same principle applies: persist the consumed decision before calling the provider, then pass the stable key. If a crash occurs between those operations, a recovery worker can inspect execution status instead of creating a fresh call.
What security controls belong around the reviewer and resume endpoint?
- Authenticate every reviewer and resume request; do not rely on an unguessable URL alone.
- Authorize against tenant, resource, environment, and approval level at decision time.
- Expire or revoke pending approvals when credentials, policy versions, or workflow versions change.
- Bind a decision to the checkpoint hash, interruption ID, and canonical arguments.
- Redact secrets from the review screen and audit payloads while retaining enough data to explain the decision.
- Use separate credentials for the reviewer service and the tool executor, with least privilege for each.
- For high-impact operations, require two independent reviewers or a second factor and prevent self-approval when separation of duties is required.
Never trust a client-supplied “approved” flag, tool name, or argument object. The server should look up the pending record, verify its integrity and status, and derive the executable payload from stored data.

Log decision and execution events separately so a reviewer action cannot be mistaken for a successful side effect. Correlate nested-agent events with the root run. Apply retention and access controls appropriate to the data, and make audit records tamper evident so an incident review can distinguish an original proposal from a later edit.
How should I test approval, waiting, retries, and rollback?
- Approve a normal call and verify exactly one downstream effect.
- Reject a call and confirm the tool is never invoked and the agent receives a structured denial.
- Edit an argument into an unauthorized value and verify that the edit is blocked.
- Open two reviewer sessions and confirm only one can consume the decision.
- Deliver the same approval webhook multiple times and confirm only one resume occurs.
- Kill the worker while the run is waiting, restart it, and resume from the persisted state.
- Kill the worker after provider acceptance but before recording success, then retry with the same idempotency key.
- Force a downstream failure after approval and verify that the workflow enters compensation rather than replaying prior steps.
- Change the workflow or tool schema and confirm old checkpoints are rejected or migrated explicitly.
- Verify that audit records contain the full lifecycle without exposing credentials.
Exercise these cases in a staging environment that mirrors production authorization and provider behavior. A green unit test for the approval predicate is not evidence that the distributed resume and duplicate-prevention path is safe.
Frequently Asked Questions
Can an AI agent stay paused for several days while waiting for approval?
Yes. Persist the serialized run state and pending decision in durable storage, then use a worker or durable orchestrator to resume it when the decision arrives. Add expiration, escalation, and policy-version checks so an old request does not execute after the underlying authorization has changed.

No. Treat the link as a reference to a server-side pending record. The server should retrieve canonical arguments, verify the checkpoint hash and reviewer authorization, and ignore any replacement payload supplied by the browser.
What happens if a tool has no undo operation?
Classify it as non-reversible before deployment. Require stronger approval—often a higher role or two-person review—capture the final payload and recipients, and provide a post-action incident procedure rather than promising automatic rollback.
Is a database transaction enough to prevent duplicate emails or payments?
No. A local transaction cannot guarantee that an external provider saw a request exactly once. Use the provider’s idempotency facility when available, or combine a transactional outbox with request-status reconciliation and a stable execution key.
