For generally healthy adults, U.S. guidance does not set a daily EPA-and-DHA dose for heart health. The American Heart Association’s food-first advice is to eat fish—preferably fatty fish—one to two times a week as part of a healthy eating pattern. Advice differs for people with coronary heart disease or high triglycerides, and the prescription dose used to treat high triglycerides is not a general fish-oil supplement target.

First, distinguish ALA from EPA and DHA

“Omega-3” refers to several fatty acids, not one interchangeable ingredient. ALA (alpha-linolenic acid) is found mainly in plant foods and oils. EPA and DHA are long-chain omega-3s found in fatty fish and seafood, as well as fish-oil and some algal-oil supplements.

For adults, the National Academy of Medicine’s adequate intake values reported by the NIH Office of Dietary Supplements (ODS) are 1.6 g of ALA per day for men and 1.1 g per day for women. Those figures apply to ALA; they are not EPA/DHA targets for heart health. The body converts only small amounts of ALA to EPA and then DHA. ODS says recommended amounts for EPA and DHA have not been established for the general population. So a label’s EPA and DHA amounts cannot be compared with the ALA adequate-intake figures to determine whether you have met a heart-health requirement.

For most people, focus on weekly seafood—not a daily capsule dose

The AHA recommends at least two fish servings a week, preferably fatty fish. Its consumer guidance describes one to two servings weekly. The AHA’s seafood advisory defines a serving as 3.5 oz and estimates that one 4 oz serving of fatty fish such as salmon each week supplies about 250 mg per day of long-chain omega-3 on average. Whether the fish is baked, broiled, or otherwise prepared, it belongs in an overall healthy eating pattern. The dietary choice matters partly because seafood can replace less healthy foods; the recommendation is not simply to add a supplement to an otherwise unchanged diet. If you do not eat fish, talk with a health-care provider about suitable food or supplement options rather than assuming a particular capsule dose is required. Lichtenstein saying supplements are “a completely viable option for people who don’t eat oily fish”; that is an expert’s reported comment, not a universal AHA supplement recommendation. When heart disease or high triglycerides change the advice

Existing coronary heart disease

AHA guidance summarized by NIH ODS recommends approximately 1 g per day of EPA plus DHA for people with existing coronary heart disease, preferably from oily fish. A supplement may be considered under a physician’s or other health-care provider’s direction. This is clinical-population guidance, not a daily target for generally healthy adults. Elevated triglycerides

The AHA’s 2019 science advisory says 4 g per day of prescription omega-3—an EPA-only or EPA-plus-DHA formulation—can lower triglycerides by approximately 20–30%. This is treatment under medical care. Prescription products are not interchangeable with ordinary over-the-counter fish-oil capsules, and the 4 g figure is not a general supplement recommendation. AHA hypertriglyceridemia advisory

Do omega-3 supplements prevent heart disease?

Evidence for preventing cardiovascular events is mixed and depends on the formulation, population, comparison treatment, and outcome measured. NIH ODS summarizes a high-dose EPA-only trial with a positive result and a high-dose EPA-plus-DHA trial that did not find significant cardiovascular benefit. These findings do not support a blanket claim that routine fish-oil supplements prevent heart disease. ODS reports the FDA-qualified wording: “Supportive but not conclusive research shows that consumption of EPA and DHA omega-3 fatty acids may reduce the risk of coronary heart disease.” NIH ODS health professional fact sheet

What to check before using a supplement

  • Read the EPA and DHA amounts. Total “fish oil” milligrams do not tell you how much EPA plus DHA a serving contains.
  • Keep food and supplements in perspective. A supplement is not required for every healthy person, and it is not a substitute for prescription treatment when that is indicated.
  • Review medicines with a clinician. ODS notes potential medication interactions and advises people who take regular medication to discuss supplements with a health-care provider.
  • Know the common side effects. Reported, usually mild effects include unpleasant taste, bad breath, heartburn, nausea, gastrointestinal discomfort, diarrhea, headache, and odoriferous sweat.
  • Do not treat a safety limit as a target. ODS says the FDA recommends no more than 5 g per day of combined EPA and DHA from dietary supplements. That ceiling is not a recommended intake or assurance that unsupervised high-dose use is appropriate.

High-dose use deserves particular care: two large trials using 4 g per day for several years found a slight increase in atrial fibrillation among people with cardiovascular disease or high cardiovascular risk.
