People with a typical measles case are contagious from four days before the rash appears through four days after it begins. Count the first day of rash as day 0: the general U.S. CDC return point is day 5, after staying home for days 0 through 4. Severe immune compromise can extend infectiousness, so follow your clinician and local health department’s instructions for case-specific timing.

How long measles is contagious

For a typical case, measles can spread from four days before rash onset through four days after rash onset. Because contagiousness starts before the rash, someone may transmit the virus before realizing they have measles. CDC describes this contagious period in its Clinical Overview of Measles and How Measles Spreads.

Measles also spreads through the air. CDC says the virus can remain in an airspace for up to two hours after an infected person leaves. Avoiding direct contact alone does not replace the recommended isolation period.

When it is generally safe to return

CDC advises people with measles to stay home until the fifth day after the rash develops. The counting convention matters: rash onset is day 0, and the four days after onset are days 1 through 4. Example: rash begins on Monday

  • Monday: day 0, the rash-onset day
  • Tuesday: day 1
  • Wednesday: day 2
  • Thursday: day 3
  • Friday: day 4
  • Saturday: day 5, the general earliest return point

This example applies CDC’s day-0 convention and fifth-day instruction. Local health department directions, a clinician’s case-specific advice, and school or employer procedures take precedence.

When the usual return timeline does not apply

People with severe immune compromise may remain infectious longer and may not develop the usual rash. The standard day-5 timing should not be assumed to apply to them. Their treating clinician and public-health authorities should determine how long isolation is needed.

For typical cases, the cited CDC guidance uses a time-based isolation rule. Feeling better, being fever-free, covering the rash, wearing a mask, or using an air purifier does not establish an earlier return date.

If measles is suspected or you were exposed

Suspected or confirmed measles

Call a healthcare provider or hospital before going in person, explain that measles is suspected, and follow their instructions. Calling ahead lets the facility arrange evaluation while limiting exposure to other people.

Exposure without a measles diagnosis

Exposure is not the same as having measles, and an exposed person should not automatically use the infected person’s return-to-school or return-to-work timeline. Contact a healthcare provider promptly. Assessment of immunity, possible post-exposure measures, and whether someone must stay away from a school, workplace, or other setting depend on the circumstances and public-health instructions. CDC’s guidance for people who are sick or exposed explains the recommended first contact.

The CDC notes that up to 90% of close contacts who are not immune may become infected; that figure applies to close contacts and is not a prediction for every exposure. See How Measles Spreads.

Whose instructions to follow

The timing above reflects U.S. CDC guidance. Health departments may direct isolation and contact measures for a particular case or exposure, and schools and employers may have procedures for reporting or return. Check with the relevant local health department and follow case-specific medical advice rather than relying on the calendar example alone.
