AI researchers test safety-rule compliance by turning written policies into observable behaviors, evaluating those behaviors on ordinary and adversarial cases, and checking whether the test itself is trustworthy. They test the complete configuration—model, prompts, tools, context, safeguards, and resource limits—not merely an abstract model name. Results can reveal weaknesses or support a bounded deployment decision, but no finite test proves that a model will obey every rule in every future interaction.
What does it mean for a model to follow a safety rule?
A safety rule is an intended boundary: refuse a prohibited request, protect sensitive information, follow a higher-priority instruction when directions conflict, or take a required protective action. A policy document states that intention in human language. An evaluation team must translate it into behaviors that an observer can identify and score.
Turn policy language into observable claims
Teams write a claim narrow enough to test. Examples include: “The model declines this category of disallowed request,” “The model does not reveal a protected value when the user asks indirectly,” or “The model follows the system instruction when a later user message conflicts with it.” The claim should specify what counts as success, what counts as failure, and what an acceptable borderline response looks like.
Anthropic’s public Constitution illustrates the distinction. It describes intended behavior for Claude while explicitly warning that training does not guarantee behavior will always reflect those ideals. A test therefore measures observed conduct against a defined rule; it does not assume the written rule is proof of compliance.

State the scope of the claim
A useful claim names the model version, interface, language, tools, context, and threat scenario being examined. “The model is safe” is too broad to evaluate. “In this chat configuration, the model resisted these tested attempts to obtain restricted instructions” is a bounded claim that a report can support or challenge.
How are safety-evaluation cases built?
Researchers assemble prompts, tasks, and interactive scenarios that cover normal use, known edge cases, and plausible ways a rule could fail. They combine several ways of producing cases because each exposes different weaknesses.
Curated routine evaluations
A routine evaluation uses a defined set of examples with a known scoring method. Cases may represent ordinary user requests, instruction conflicts, ambiguous wording, or context shifts. Holding the cases and procedure constant lets a team compare two model versions or measure whether a safeguard changed behavior.
Recommended Free Tools
Tasks can be scored by an automated grader, by human annotators, or by both. Automated scoring is fast and repeatable when the success condition is clear. Human review is useful when safety depends on nuance, intent, or the quality of an explanation rather than on a single phrase.
Human and expert assessments
Anthropic’s discussion of evaluation methods describes multiple-choice benchmarks, crowdworker A/B judgments, domain-expert red teaming, and audits for dangerous capabilities. Crowd judgments can reveal which answer users perceive as safer or more helpful, while subject-matter experts are better positioned to spot a subtle medical, legal, cybersecurity, or privacy failure. Each method needs a clear rubric and quality checks; agreement among reviewers does not automatically make a task valid.
Interactive scenarios
Some failures emerge only after several turns. A scenario can begin with an innocent request, introduce a conflicting instruction, add a retrieved document, and then ask the model to act through a tool. The evaluator records not just the final text but also whether the model took an unsafe intermediate action, exposed data, or ignored a required confirmation.

What is the difference between an ordinary evaluation and red teaming?
Ordinary evaluations measure behavior on a defined test set. Red teaming deliberately searches for ways the system may fail under adversarial, abusive, or unexpected inputs. They are complementary, not competing, methods.

| Method | Primary question | Typical elicitation | Common scoring | Main limitation |
|---|---|---|---|---|
| Routine policy evaluation | Does the system meet specified behavior on known cases? | Static prompts or scripted conversations | Automated grader, rubric, or human label | May miss failure modes outside the test set |
| Red teaming | Can an attacker or unexpected user discover a weakness? | Adaptive probing, abuse attempts, or creative reframing | Expert judgment, documented exploit, or reproduced failure | Coverage is hard to quantify and results depend on the team’s skill |
| Capability evaluation | Can the model perform a capability that could create risk? | Benchmarks, tasks, or tool-using workflows | Task success, accuracy, or completion criteria | Capability is not itself a measure of unsafe intent or safeguards |
| Human comparative assessment | Which of two outputs better satisfies a safety or quality rubric? | Blind A/B judgments or expert review | Pairwise preference or rubric score | Judgments can vary with wording, reviewer training, and context |
OpenAI’s red-teaming guidance summarizes the distinction: use evaluations to measure whether an AI system behaves as intended, and red teaming to probe behavior under adversarial, abusive, or unexpected inputs. A strong program uses routine cases to track known requirements and red teaming to discover cases the routine set did not anticipate.
Why must researchers test the whole system instead of only the base model?
For a tool-using or multi-step model, the surrounding harness can change the result. Tool permissions, retrieved context, conversation history, workflow logic, time or token budget, monitoring, and refusal safeguards all influence what the model can do and how it responds.
Document the tested configuration
A credible report identifies the exact model and version, system and developer instructions, user prompts, available tools, tool permissions, context supplied at each step, stopping conditions, resource budget, and safeguards enabled during the run. It also explains how the behavior was elicited. A score from a bare chat prompt should not be presented as evidence about an agent that can send messages, execute code, or access private records.
Keep comparisons equivalent
When comparing models or versions, hold the harness, tools, prompts, sampling settings, and scoring procedure as constant as practical. If one system receives a different context window or more opportunities to retry, the resulting difference may reflect the setup rather than the model’s safety behavior.

How are safety-test results scored?
Scoring follows the claim. A binary label may be appropriate when a request is clearly allowed or disallowed. More complex rubrics can separately score refusal quality, leakage of sensitive details, harmful assistance, explanation, and whether the model offered a safe alternative.

Automated graders
Automated graders provide scale and consistency, but they can reward superficial cues. A model might include refusal language while still revealing the dangerous information, or avoid a keyword while giving equivalent instructions. Teams should validate graders against human-reviewed examples and inspect disagreements rather than treating a single automated number as definitive.
Human and expert scoring
Human annotation can capture intent, context, and partial compliance. Reviewers need written criteria, examples of borderline cases, and a process for resolving disagreements. Domain experts are especially important when a response’s safety depends on technical details that a general annotator cannot judge.
Report the denominator and conditions
A percentage is meaningful only with its test population and conditions: number and type of cases, model configuration, scoring rule, and whether failures were counted per prompt, conversation, or attempted attack. A pass rate on a curated set is not the same as a probability that any future user will receive a safe answer.
Which validity problems can make a safety score misleading?
Before interpreting a result, evaluators examine whether the task, model behavior, and measurement process support the intended conclusion. OpenAI’s shared playbook for trustworthy third-party evaluations highlights several recurring hazards.
Contamination and benchmark familiarity
If test material or close paraphrases appeared in training data, a high score may reflect memorization rather than robust rule-following. Held-out cases, fresh adversarial variants, and careful documentation of data provenance reduce—but do not eliminate—this concern.
Refusal masking
A blanket refusal can make a model appear safe while hiding whether it understands the underlying task. For a capability evaluation, refusal may prevent the capability from being measured at all. Reports should distinguish a genuine safe response from an inability or refusal that makes the task untestable.

Broken or unfair tasks
An ambiguous prompt, incorrect answer key, inaccessible tool, or unrealistic scenario can create failures unrelated to the safety rule. Teams should audit examples, verify tool operation, and record exclusions rather than silently dropping inconvenient cases.

A system may optimize for what the grader rewards instead of the intended behavior—for example, producing a formulaic refusal that still contains actionable harmful details. Inspecting samples, testing paraphrases, and using independent checks help detect this shortcut.
Evaluation awareness and deliberate underperformance
Models can behave differently when they infer that they are being tested, and an evaluator can unintentionally signal the desired answer. Hidden test cases, varied wording, and independent replication make it harder for a system or operator to game the measurement. Deliberate underperformance is another possibility: a model may conceal a capability or fail selectively, so one low score should not be treated as proof that the capability is absent.
How do researchers use failures to improve safeguards?
A failure is a diagnostic signal, not automatically a reason to discard the entire system. Teams trace it to the layer that needs work: training data, policy wording, system prompts, tool permissions, confirmation steps, monitoring, or the evaluation itself.
- Reproduce the behavior. Save the exact prompt sequence, configuration, tool calls, and output so another evaluator can confirm the issue.
- Classify the failure. Decide whether it was a policy violation, a capability exposure, an unsafe tool action, a scoring error, or an untestable case.
- Apply a mitigation. Possible changes include additional training, a narrower tool permission, stronger input or output filtering, a confirmation gate, or improved monitoring.
- Retest the original and expanded cases. A fix that passes one example but fails paraphrases or multi-turn variants is not yet robust evidence.
- Make the decision explicit. The team records whether more testing, a stronger safeguard, a restricted release, or a deployment pause is warranted.
OpenAI’s Preparedness Framework is an example of an organization-specific approach that links measured capability risk and safeguard effectiveness to operational thresholds. Those thresholds belong to that framework; there is no single universal industry pass score that certifies every model.
What can safety evaluations prove—and what can’t they?
Carefully designed tests can show how a model behaved on a particular distribution of cases, in a particular configuration, under a particular scoring method. They can reveal weaknesses, compare systems under equivalent conditions, or provide evidence that specified safeguards resisted the attacks that were tested.

They cannot establish flawless compliance in every future context. Possible interactions are effectively open-ended, test sets are incomplete samples, and small design choices can change measured performance. Anthropic reported in 2023 that the MMLU benchmark covers 57 tasks, including areas such as mathematics, history, and law, and that simple formatting changes could shift measured accuracy by about 5 percent. Those figures describe benchmark scope and sensitivity, not a safety-compliance rate or a general error margin for all evaluations.
What should a trustworthy evaluation report disclose?
Readers should be able to reconstruct what was tested and understand the limits of the conclusion. A practical report includes:
- The behavioral or capability claim and the policy rule it maps to.
- Model name, version, interface, system instructions, and sampling settings.
- Tools, permissions, context, workflow, safeguards, and resource limits.
- How cases were selected, including held-out or adversarial material.
- Scoring rubric, grader type, human-review procedure, and treatment of disagreements.
- Counts of cases and failures, with the unit of analysis clearly defined.
- Validity checks for contamination, broken tasks, refusal masking, scoring shortcuts, and evaluation awareness.
- Known limitations and the deployment or mitigation decision the evidence supports.
Google DeepMind’s 2024 work on holistic safety and responsibility evaluations emphasizes that advanced-model safety assessment is diverse and still evolving. That is why a portfolio of methods, with transparent limits, is more informative than a single headline score.
Frequently Asked Questions
Can a model pass a safety evaluation and still fail users later?
Yes. A pass means the model met the stated criterion on the tested cases and configuration. New languages, tools, contexts, attack strategies, or model updates can expose behavior that the original evaluation did not cover, so organizations need regression testing and ongoing monitoring.
Are red-team findings reproducible?
Often, but not always. A well-documented finding includes the full conversation, system setup, tool state, and steps needed to trigger it; adaptive attacks may depend on timing or hidden context and require several attempts to reproduce.
What is a false positive in a safety test?
It is a case labeled as a safety failure even though the model followed the rule, commonly because the prompt was ambiguous, the task was broken, or the grader misunderstood a qualified answer. Reviewing disputed examples and correcting the rubric prevents these labels from distorting later decisions.
Should organizations publish every adversarial prompt they use?
Not necessarily. Publishing enough methodological detail to audit the claim improves accountability, but releasing an effective exploit verbatim can make abuse easier. Teams can share sanitized examples, aggregate results, and access-controlled artifacts while protecting sensitive attack details.
