AI’s biggest revenue-cycle opportunity is not simply processing existing work faster; it is preventing avoidable denials, delays and rework by connecting the steps that create them. That means carrying accurate eligibility, authorization, documentation and coding information forward into claims—and routing exceptions to people who can resolve them. The distinction matters: in Inovalon’s 2026 research, 93% of organizations still relied on phone, fax or payer portals for prior authorization, while only 7% used dedicated prior-authorization software. Automating a task without fixing its handoffs can leave the underlying friction intact.

For health systems, the practical test is whether an AI-enabled workflow improves measurable outcomes such as denial rate, authorization cycle time, clean-claim rate and cost to collect—not whether it adds automation for its own sake.

Why is operational friction the bigger revenue-cycle problem?

Operational friction is the avoidable effort created when information, decisions or work do not move cleanly between revenue-cycle steps. A missing eligibility detail can become an authorization delay; a documentation gap can become a coding question; and an unresolved question can surface only after a claim is denied. Each handoff can create extra work for staff and delay payment.

The concern is widespread. In the 2026 Guidehouse/HFMA survey, 88% of provider executives ranked payer challenges among their top three concerns. The American Hospital Association reported in 2025 that an AHA survey from 2022 found $6.4 billion in delayed or unpaid claims at least six months old across nearly 800 hospitals. These figures describe different problems and measures, but together they show why reducing the work between a service and a resolved claim matters.

AI can help when it makes relevant information available at the point a decision is needed, flags a likely issue before submission, or organizes an exception for a person to resolve. It is less useful when it merely accelerates one isolated step while leaving the next team without the context or action needed to move the account forward.

Where does friction enter the revenue cycle?

Revenue-cycle friction can arise before care, during documentation and coding, or after a claim is submitted. The AHA/Ailevate end-to-end map groups the work into front-end, middle-cycle and back-end activities.

Part of the cycle Activities Examples of friction to address
Front end Registration, appointment and procedure scheduling, insurance and benefits verification, financial counseling, prior authorization and up-front collections Eligibility information, registration details, authorization requirements or claim data may be incomplete or incorrect.
Middle Denial prevention, charge capture, clinical documentation improvement, utilization review, coding and compliance Clinical support may be missing or inconsistent, or coding may not match the documentation.
Back end Claims submission, denial management, appeals, accounts receivable, billing, collections, revenue integrity and reporting Problems identified only after submission can require research, correction, appeal or follow-up.

Inovalon’s 2026 research identifies eligibility as a leading denial driver across ambulatory, acute and post-acute settings. Registration, prior authorization, and missing or invalid claim data are also prominent. That pattern is important: a denial showing up in the back end may have been caused by information or a decision needed much earlier.

Why doesn’t automating one task solve the problem?

Task automation can shorten one activity without correcting the transfer of information or responsibility to the next activity. A faster authorization request, for example, does not by itself ensure the request used the right payer requirement, included the necessary documentation, or produced a status that the care team can act on.

Prior authorization illustrates the gap. Inovalon’s 2026 survey found that 93% of organizations relied on phone, fax or payer portals, while 7% used dedicated prior-authorization software. Inconsistent health-plan requirements were cited as the core difficulty by 56% of Inovalon respondents. The operational problem is therefore not just the time spent submitting a request; it also includes finding the applicable requirement, assembling support, tracking the response and managing exceptions.

McKinsey’s 2025 survey insights identify interoperability, finances, staff training and change management as barriers to full automation. A tool that cannot exchange information with the relevant EHR, practice-management or payer systems may create another place for staff to check. A workflow that people have not been trained to use may leave manual work running alongside it. The useful question for a buyer is whether a solution removes a complete source of friction across connected steps.

Which AI use cases can prevent denials upstream?

The strongest candidates are workflows where information is available before a preventable error becomes a claim problem. McKinsey’s 2025 survey lists denial management and appeals as a leading AI priority for 57% of respondents, followed by documentation and coding accuracy at 56%. The use cases below connect those priorities to the points where corrections can still be made.

Eligibility and coverage discovery

AI-supported eligibility workflows can check coverage and write verified information into the patient account so downstream teams can use it. This is most useful when a check does more than display a result: the relevant coverage detail should be available to the people handling scheduling, authorization and billing. Banner Health is cited by the AHA as an example in this category; that example does not establish a universal outcome for other organizations.

Prior-authorization workflow

A connected workflow can identify requirements early, assemble relevant documentation, track status and send exceptions to staff. Because requirements vary by health plan, the process needs to show which requirement applies and what information supports a submission. Automation should reduce avoidable chasing, not make a disputed medical-necessity judgment without appropriate human review.

Documentation and coding quality

AI can flag missing or inconsistent clinical support before a claim is submitted, giving the responsible clinician, coder or documentation specialist a chance to review it. The objective is to surface a specific gap while it can still be addressed—not to treat a generated suggestion as proof that the record supports a code. McKinsey’s 2025 finding that 56% of surveyed leaders prioritized documentation and coding accuracy reflects the importance of this work, not a guaranteed benefit from any particular tool.

Pre-submission denial prevention

Rules and historical payment information can help identify claims that appear likely to be denied, so staff can correct the record before filing. The AHA’s 2024 use-case account from a Fresno community-health network reported a 22% reduction in commercial prior-authorization denials and an 18% reduction in denials for services not covered after pre-submission review. These are reported results from that case, not a forecast or benchmark for every hospital.

Denial and appeal orchestration

After a denial, AI can classify the reason, gather relevant evidence and help draft an appeal, while routing cases that require judgment to trained staff. This can organize recovery work, but it should also feed patterns back to upstream teams: recurring denial reasons may point to an eligibility, authorization, documentation or claim-data problem that should be prevented earlier. McKinsey’s 2025 survey found denial management and appeals were an AI priority for 57% of respondents.

How should a hospital choose where to start?

Start with a recurring, measurable friction point and trace it across the handoffs that create or resolve it. A useful pilot has a defined workflow, an accountable operational owner, a baseline and an outcome that can be measured without confusing activity volume with improvement.

  1. Map the problem end to end. Follow a representative account from the first relevant step—such as eligibility or authorization—to claim submission and resolution. Record where information is missing, which team owns each action and where work is repeated.
  2. Choose a preventable issue with a clear owner. Use existing denial and workflow information to identify a focused problem, such as a recurring eligibility error or documentation gap. Define which team can act on a warning and what correction is expected.
  3. Check data and integration readiness. Confirm that the workflow can access the systems and information it needs, including relevant EHR, practice-management and payer data. Establish how corrections and status updates will flow back to the people doing the work.
  4. Set the baseline and success measures. Select measures tied to the problem—such as denial rate, authorization cycle time, clean-claim rate or cost to collect—and document the measurement period and population. Also track operational effects such as staff work and exception volume so a faster step does not conceal added burden elsewhere.
  5. Test with human review and clear escalation. Define which cases can be handled through the workflow and which require staff review, especially disputed or judgment-heavy decisions. Make it possible for staff to see the reason for a flag, correct bad data and record the disposition.
  6. Review results before expanding. Compare outcomes with the baseline and examine exceptions, data quality and effects across relevant payer or patient groups. Expand only if the workflow improves the intended measure without creating unacceptable errors or new work.

This sequence helps distinguish a useful end-to-end intervention from a narrow automation project. It also makes the implementation burden visible: a workflow can have a plausible use case and still fail if integration, training or ownership is inadequate.

How can buyers compare revenue-cycle AI solutions?

Compare the work a solution changes, the information it can use, the evidence it provides and the operational burden it creates. A feature list alone does not establish that a product prevents denials or lowers the cost to collect.

What to evaluate Questions to ask
Prevention versus recovery Does the workflow address a cause before submission, organize work after denial, or do both? What handoff changes?
End-to-end coverage Can it connect patient access, authorization, documentation, coding, claims and appeals where those steps matter to the use case?
Interoperability How does it exchange data with the organization’s EHR, practice-management and payer systems? Where will staff still need to re-enter or reconcile information?
Explainability and oversight Can users see why an item was flagged, review the supporting information and audit actions? How are judgment-heavy cases routed to people?
Measured outcomes What changes in denial rate, authorization cycle time, clean-claim rate and cost to collect will be measured, for which population and over what period?
Implementation and staffing What integration, training and change-management work is required? Which existing tasks may shrink, and which new review tasks may be added?
Total cost What is the full cost of implementation and operation, and how will staffing impact be included in the business case?

Ask vendors to distinguish their product’s capabilities from measured outcomes at customer sites. The AHA’s Fresno case demonstrates that local gains have been reported, but it does not show that results transfer automatically to another payer mix, workflow or organization. A hospital should validate its own baseline and define the evidence it needs before scaling.

What risks require human oversight?

AI can scale a flawed rule, amplify poor data or make an opaque process harder to challenge. Health Affairs’ 2026 analysis of AI in utilization review identifies opacity, automation bias, underperformance and unintended consequences as governance risks, and notes that older algorithms have produced large-scale errors.

  • Keep consequential judgment reviewable. Disputed medical-necessity decisions should have human oversight rather than being treated as routine data processing.
  • Require understandable reasons. Staff need transparent reason codes or explanations that show why a claim, authorization or record was flagged.
  • Maintain an audit trail. Record relevant inputs, recommendations, human actions and outcomes so errors can be investigated and corrected.
  • Control data quality. Monitor whether incomplete or inaccurate source data is producing unreliable recommendations or downstream work.
  • Monitor for uneven performance. Check for payer or population bias and unintended effects, rather than assuming performance is consistent across all cases.

Human review should be designed into the workflow, not added only after an error. Staff need a clear route to question a recommendation, correct underlying information and escalate a case that the system cannot resolve safely.

Does AI actually reduce cost to collect?

It can contribute to lower cost to collect when it reduces avoidable work—such as repeated data correction, status chasing or manual appeal preparation—without shifting the burden to a different team. The available figures here do not establish a general cost reduction or a guaranteed return on investment. Cost to collect should be measured in the organization’s own workflow alongside denial, authorization and clean-claim outcomes.

A credible business case should account for the full operating model: integration, implementation, staff training, exception review and ongoing oversight as well as any reduction in manual effort. If a tool increases the number of alerts or creates a new reconciliation step, those costs belong in the calculation too. The right outcome is not maximal automation; it is less total effort to produce a clean, appropriately supported claim and resolve exceptions.

Frequently Asked Questions

How can patients help prevent revenue-cycle delays?

Patients can help by providing current insurance details and promptly responding when the provider requests information needed to verify coverage or arrange authorization. They should ask the provider’s billing or financial-counseling team which details are needed for their specific visit rather than assuming an automated check has resolved every coverage question.

What should a revenue-cycle AI pilot measure besides denials?

Track the time required to complete the targeted workflow, the volume and type of cases routed to staff, and whether work shifts to another team. Pair those measures with a defined population and baseline so an apparent improvement is not just a change in case mix or workload location.

Who should own an AI revenue-cycle workflow?

Ownership should sit with the operational team accountable for the targeted workflow, with appropriate involvement from clinical documentation, IT, compliance and data governance. A named owner is important because someone must decide how exceptions are handled and whether the system’s recommendations remain appropriate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can AI make medical-necessity decisions on its own?

The evidence cited here supports human oversight for disputed medical-necessity decisions, not unsupervised automated determinations. A workflow should make its rationale reviewable and provide a path for qualified staff to assess cases that require judgment.
