What if I completed the achievement while offline?
Reconnect before repeating anything. Microsoft Game Development Kit documentation says that when a title unlocks an achievement while the device is offline, the API can queue the offline unlock request. Microsoft’s API reference also describes offline updates on PC and Xbox One being posted after the connection is restored, even if the title is no longer running.
That behavior depends on how the individual title implements achievements. Microsoft also states that achievements do not unlock while the user is offline at the time of the request; the title must later submit a queued request or re-evaluate the condition. Therefore:

Why is the achievement notification missing but the record present?
A toast notification and the underlying achievement record are separate things to verify. The pop-up can be delayed, suppressed, or missed while the account record has already updated.

- When possible, check the same account from another Xbox client.
- Compare the unlock state and timestamp instead of relying only on what appeared on screen.
If another client shows the achievement as unlocked, do not repeat a difficult requirement simply to make a notification appear. Allow the clients to refresh after reconnecting and restarting the game.
