A fall-detection device can alert someone after it senses a possible fall, but no device should be treated as a guarantee that a fall will be detected or prevented. For older adults, the useful approach is to choose a system that fits the person’s routine and alert needs, then address fall risks with a clinician.

What fall detection does—and what it does not do

Fall detection uses sensors to recognize a possible fall and trigger an alert. The goal is to help summon assistance after an event; it is different from preventing a fall or assessing whether someone is likely to fall in the future.

Falls are common: the Centers for Disease Control and Prevention (CDC) says more than one out of four older people falls each year, and fewer than half tell their doctor. What kinds of fall-detection systems are available?

Wearable sensors

A wearable system depends on the person having the sensor with them—often as part of a wearable medical alert device. NIH materials describe fall-detection sensors integrated into widely available medical alert devices. Environmental sensors

Environmental approaches use sensors in a space, including cameras or floor sensors, rather than relying only on a device worn by the person. The federal materials cited here identify these as technology categories, but do not establish which approach is more accurate in everyday use. How to choose a system for an older adult

There is no single best type for every person. Compare the actual system and its alert path with the person’s habits, living environment, and preferences. Confirm the details with the manufacturer before buying; the federal sources cited here do not establish current features or terms for particular models.

  • Who receives an alert? Find out whether alerts go to family, another chosen contact, or a monitored response service, and what happens if the first contact does not respond.
  • Where does it work? Check the manufacturer’s stated coverage, including whether the system functions only in the home or also elsewhere.
  • Will it be used consistently? A wearable depends on being worn. Consider comfort and ease of use; an environmental system may suit someone who does not want to wear a device, but its coverage depends on the installed sensors.
  • What does it require? Verify setup, charging or battery needs, connectivity, ongoing service requirements, and who will manage them. These vary by product and are not established by the category-level sources.
  • What happens after a possible fall? Ask how the person can call for help if the system does not detect an event, and make sure they and their contacts understand the alert procedure.

Does fall detection work every time?

No general guarantee is supported by the available evidence. A system’s ability to recognize a possible fall depends on its sensors, design, and circumstances. An alert may also be missed or may not lead to help if the alert pathway is unavailable or not acted on.

One 2014 laboratory study by Liu and Lockhart tested an algorithm with inertial sensors, ten healthy older adults, daily activities, and a defined protocol of slip-induced backward falls. In that experiment, the algorithm reported 100% sensitivity, 95.65% specificity, and a 255-millisecond response time. Those are results for that study and protocol, not a rating for consumer devices or evidence that a product will detect every kind of fall. Liu and Lockhart, 2014.

Fall detection is not the same as fall-risk screening

A fall-detection system tries to identify a possible event as it happens or afterward. A screening tool estimates a person’s future risk; it does not detect a fall. In a 2022 study with 1,563 final respondents, the screening tools examined had sensitivity ranging from 22.5% to 68.7% and specificity from 57.9% to 89.4% for predicting falls. These figures describe those prediction tools, not wearable or environmental device performance. Burns et al., 2022.

Pair a device with fall prevention

Detection may help someone get assistance after a fall, but reducing modifiable risks matters too. CDC’s STEADI initiative recommends that health care providers screen older adults for fall risk, assess those at risk for modifiable factors, and intervene with strategies such as physical therapy, home modification, and medication management.   1. Discuss risk with a clinician. Ask for an assessment, especially after a fall or when balance, mobility, or medicines are a concern.
  2. Review practical interventions. Ask whether physical therapy, a home-safety assessment or modifications, or medication management could address risks identified in the assessment.
  3. Plan how help will be reached. If using fall detection, agree on who will receive alerts and what the person should do if they need help but the system does not alert.
