You cannot guarantee that a general-purpose language model will never hallucinate. You can, however, build an agent that rejects unsupported plans, unauthorized tool calls, impossible state changes, and unverified results before they become actions. The practical design is a fail-closed loop: the model proposes a typed plan, a parser turns it into an abstract syntax tree (AST), deterministic negative assertions inspect that tree, feasibility and evidence checks test it against the current world, and only an authorized, accepted tree reaches execution.
This distinction matters. Ziwei Xu, Sanjay Jain, and Mohan Kankanhalli write in Hallucination is Inevitable that “we show that it is impossible to eliminate hallucination in LLMs.” Treat zero hallucination as a bounded operating envelope and an explicit rejection policy, not as a property of the model.
What does a zero-hallucination AI agent actually guarantee?
A defensible guarantee is narrower than perfect truth: no proposal that violates your declared policy, lacks required evidence, or fails a live precondition is allowed to execute. The model may still generate an incorrect candidate, but the control loop must make that candidate harmless, explain why it was rejected, and preserve the rejection for testing.
Define the operating envelope before writing prompts. Specify which tools exist, which data may leave the system, which state transitions are permitted, what evidence a claim requires, maximum steps and time, and which actions require a person. Outside that envelope the agent abstains or asks for review. Inside it, every claim and action remains subject to validation.

General models cannot guarantee consistency with every computable ground truth. Xu, Jain, and Kankanhalli formalize hallucination as inconsistency with a computable ground truth and show why a general LLM problem solver inevitably encounters it. A fail-closed architecture changes the question from “Is the model always right?” to “Can an unsupported or unauthorized proposal cross the execution boundary?”
How does the fail-closed architecture work?
The model is a proposal generator, never the authority that performs an action. Each stage narrows what can proceed and emits a structured reason when it stops.
- User request: Capture the requested outcome, actor, scope, and any deadlines or approval requirements.
- Typed proposal: Ask the model for a plan object with fixed fields for intent, tool calls, arguments, preconditions, evidence references, expected changes, and rollback behavior.
- Parser and AST builder: Parse the object with a strict grammar and construct a typed tree. Reject unknown node types, duplicate fields, ambiguous values, and malformed nesting.
- Negative assertions: Evaluate machine-checkable predicates over AST nodes and edges. These assertions describe forbidden behavior or missing prerequisites.
- Feasibility and evidence evaluation: Check current state, permissions, resource availability, and support for every factual claim.
- Authorization: Bind least-privilege credentials and approval tokens to the accepted AST, not to the model’s explanation.
- Sandboxed or transactional execution: Apply bounded time, step limits, idempotency keys, and a kill switch.
- Outcome verification: Read the external state again and compare it with the declared postcondition. Log the tree, predicates, evidence, result, and any rejection.
A proposal that fails any mandatory stage stops. A repair loop may ask the model for a new proposal, but the original rejected tree and reason remain immutable audit records.
What should the agent’s typed proposal contract contain?
Keep policy-relevant values separate from free-form explanation so a parser can make an unambiguous decision. A minimal intermediate representation can contain the following fields.
| Field | Purpose | Typical validation |
|---|---|---|
intent |
Desired outcome in a controlled vocabulary | Must map to an enabled operation |
tool |
Exact tool or API identifier | Allowlist membership; version match |
arguments |
Typed inputs for the tool | Schema, range, secret, and sink checks |
preconditions |
Facts that must be true before execution | Evaluate against a fresh state snapshot |
evidence_ids |
Sources supporting claims or decisions | Every claim requires at least one valid reference |
expected_state_change |
Declared postcondition | Must be observable and comparable |
max_steps and timeout_ms |
Resource bounds | Within policy ceilings; neither may be unbounded |
authorization |
Approval or capability token | Scope, issuer, expiry, and AST hash match |
rollback or stop_behavior |
Recovery if execution fails | Required for reversible or multi-step changes |
Do not let prose override these fields. A sentence such as “the user approved this” is not an approval token. A token should identify the approved operation, resource scope, expiration, and a digest of the parsed tree.
How do negative constraint assertions stop unsafe plans?
Negative constraints are executable predicates that reject a tree when a forbidden condition is present or a required condition is missing. They are stronger than style instructions because they have deterministic inputs, outputs, and reason codes.
| Assertion family | Reject when… | Reason code example |
|---|---|---|
| Tool authorization | A tool or API is not on the allowlist, or its version is disabled | FORBIDDEN_TOOL |
| Data handling | An argument sends secrets or regulated data to an unapproved sink | FORBIDDEN_DATA_SINK |
| Evidence | A factual claim has no resolvable evidence reference | MISSING_EVIDENCE |
| State safety | A precondition is false or a transition is not permitted from the current state | STATE_PRECONDITION_FALSE |
| Resource bounds | A loop, recursion, step count, or timeout is unbounded | UNBOUNDED_LOOP |
| Human control | A delete, publish, payment, or other irreversible node lacks an approval marker | HUMAN_APPROVAL_REQUIRED |
| Input integrity | Arguments contain untrusted interpolation, hidden instructions, or a type mismatch | UNTRUSTED_INTERPOLATION |
Assertions should describe behavior, not vague preferences. “Use a professional tone” is difficult to enforce at an action boundary; “a publish node must reference an approved document version” is testable.

How do AST gates enforce meaning rather than just syntax?
An AST gate parses the proposal into typed nodes and then evaluates predicates over those nodes and their relationships. It can inspect semantics that a flat JSON validator cannot, such as whether a publish node is preceded by a review node or whether a data export reaches an approved destination.

- Walk nodes and edges: Visit every tool, claim, branch, loop, read, write, and state-transition node. Check both local properties and ordering relationships.
- Apply deny predicates first: A single forbidden tool, secret sink, missing approval, or false precondition should fail the whole tree unless policy explicitly allows partial execution.
- Emit an explainable decision: Return accepted or rejected, the exact AST path, the predicate, and a remediation category such as supply evidence, request approval, or re-plan.
Research on ChopChop is a useful model for this boundary: its semantic constrained-decoding formulation defines constraints over ASTs and monitors prefixes while asking whether a valid completion remains possible. In production, you can use the same idea either during generation or as a post-generation gate; the post-generation gate remains the authoritative policy check.
Typical tree predicates include:
- A
tool_callnode must name an allowlisted tool and have arguments matching that tool’s schema. - A
writenode must descend from an approved capability and target a resource in scope. - A
claimnode must point to evidence whose freshness and authority meet the claim type. - A
transitionnode must have a precondition that evaluates true in a fresh snapshot. - A
loopnode must carry a finite limit and timeout, and its body must be idempotent or compensatable. - A
publish, delete, or payment node must include a human-approval marker bound to the tree digest.
Does JSON schema or constrained decoding prevent hallucinations?
No. A schema can guarantee that output is parseable and has the required fields, but it cannot prove that a value is true, feasible, authorized, or safe. Constrained decoding works by masking invalid tokens and renormalizing the remaining probabilities; a syntactically valid continuation can still contain a false date, an impossible target, or a fabricated identifier.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Reddy and colleagues describe this failure mode and report that constrained decoding can distort generation when valid continuations have low probability. Their 2026 ICML work on draft-conditioned constrained decoding reports strict structured-accuracy gains of up to 24 percentage points, including an increase from 15.2% to 39.0% on GSM8K with a 1B model. Those are benchmark-specific measurements, not a production guarantee against hallucination.
Use schema enforcement for wire-format reliability, then rely on AST assertions, evidence checks, live state validation, and authorization for semantic safety. A valid object with an unapproved tool is still unsafe; a complete claim with no evidence is still unsupported.
How should feasibility and evidence be evaluated?
Separate two questions: can the model support the statement, and can the requested action succeed in the current world? Use deterministic checks whenever the relevant state is available, and treat learned evaluators as additional signals rather than final authorities.
Check current feasibility
Read inventory, permissions, resource status, version compatibility, and dependencies immediately before execution. A plan that was feasible at generation time may become invalid after another process changes the state. Require a fresh snapshot or conditional API operation for resources that can change concurrently.

Require evidence for claims
Attach evidence references to each externally verifiable claim. Validate source identity, retrieval time, scope, and whether the evidence actually entails the claim. If evidence is missing or contradictory, reject, narrow the claim, or abstain instead of allowing confident prose to pass.
Distinguish ignorance from unjustified certainty
Google’s hallucination taxonomy separates cases where the model lacks the needed knowledge from cases where it knows but answers with unjustified certainty. The first calls for retrieval or a required evidence reference; the second calls for calibrated confidence, abstention thresholds, or an explicit uncertainty state. One generic confidence score will not reliably address both.
Use learned target-feasibility evaluators carefully
Zhao and colleagues, in Rejecting Hallucinated State Targets during Planning, describe a planning agent with a learned target-feasibility evaluator. Their design uses off-policy-compatible learning, a distributional architecture, and hindsight relabeling to reject infeasible targets that can otherwise cause delusional behavior and safety concerns. In an operational agent, calibrate such a model on held-out state transitions, monitor false accepts and false rejects, and keep deterministic policy checks mandatory.

How do you make the agent fail closed?
Fail closed means that uncertainty, parser errors, policy service outages, and missing approvals result in no side effect. Design every failure path explicitly instead of letting the executor interpret a partial plan.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errors- Reject: Stop immediately for forbidden tools, invalid signatures, secret leakage, false preconditions, or malformed trees.
- Repair: Ask for a new proposal containing only the missing or corrected fields. Never patch an unsafe AST in place without re-parsing and re-running every gate.
- Abstain: Return a bounded explanation when evidence or state is unavailable. Do not fill a required value with a guess.
- Escalate: Route irreversible or high-impact operations to a human with the exact proposed change, evidence, and reason for review.
- Degrade safely: If the policy engine, evidence store, or state reader is unavailable, disable actions that depend on it. Read-only status may remain available if separately authorized.
Cap repair attempts and total elapsed time. After the cap, return a stable failure code rather than allowing an endless model conversation to smuggle an action through a later turn.
How should authorization and execution be isolated from the model?
Authorization must be computed from the accepted AST. The model’s explanation is informational and must not grant permission.
- Issue short-lived capabilities scoped to specific tools, resources, operations, and limits.
- Bind the capability to a canonical AST digest so changing an argument invalidates approval.
- Run tools in a sandbox or transaction with least-privilege credentials and network egress restrictions.
- Use idempotency keys for retries and a compensating action or rollback boundary for multi-step work.
- Enforce wall-clock, CPU, memory, token, and step limits outside the model.
- Provide an operator kill switch that bypasses the model and revokes capabilities.
Keep side effects behind a single tool router. The router should accept only an accepted, signed tree and should reject direct model text, unparsed JSON, or calls assembled by a prompt template.
How do you verify that execution produced the claimed result?
A passing pre-execution gate certifies only that the proposal was permitted at that moment. It does not prove that an external service accepted the request or that the intended state now exists.
- Capture the tool’s raw response and validate it against a response schema.
- Perform a fresh read of the affected resource, using a consistency token or version when available.
- Compare observed state with the proposal’s
expected_state_change. - On mismatch, mark the operation uncertain, stop dependent actions, and start the declared recovery path.
- Record request and response digests, timestamps, identity, policy version, and verification result.
For irreversible actions, require an independent check or human confirmation even when the API response is successful. A successful HTTP status is not equivalent to a verified business outcome.
What should you measure to know whether the gate works?
Measure the control loop, not only model accuracy. The following metrics expose where unsupported proposals are entering or being stopped.
| Metric | Definition | What a change can indicate |
|---|---|---|
| Proposal acceptance rate | Accepted ASTs divided by all parsed proposals | Policy strictness, prompt quality, or model drift |
| Rejection reason distribution | Counts by stable predicate code | Which constraint is most often violated |
| Unsupported-claim rate | Claims lacking valid evidence per proposal or per accepted plan | Retrieval gaps or overconfident generation |
| Tool-authorization violations | Attempts to invoke disallowed tools or scopes | Prompt injection, policy mismatch, or model regressions |
| Repair-loop rate and depth | Share of proposals requiring repair and attempts per request | Contract ambiguity or an overly permissive generator |
| False-accept rate | Accepted plans later found unsupported or infeasible | Gate blind spots; prioritize new predicates |
| False-reject rate | Valid plans rejected after human or deterministic review | Overly strict rules or stale state |
| Post-execution incidents | Verified mismatches or unauthorized side effects | Failures at authorization, execution, or verification layers |
No universal benchmark number exists for this exact architecture. Set an initial baseline on representative workloads, stratify by tool and risk class, and publish confidence intervals rather than a single headline rate. Keep rejected trees as test cases so every policy or model change can be replayed.
How does AST gating compare with other hallucination controls?
| Control | Guarantee layer | Enforcement point | Typical failure behavior | Observability |
|---|---|---|---|---|
| Prompt-only instruction | Behavioral intent | Generation | Model may ignore or reinterpret it | Weak unless separately logged |
| Grammar or JSON schema | Syntax and field shape | Generation or parsing | Retry malformed output | Parse errors are visible; truth is not |
| AST semantic gate | Structure, policy, and relationships | Before execution | Reject, repair, abstain, or escalate | Exact node and predicate can be recorded |
| Evidence evaluator | Factual support | Before execution or publication | Require source, narrow claim, or abstain | Claim-to-source links and entailment result |
| Feasibility evaluator | World-state attainability | Planning and pre-execution | Re-plan or reject target | State snapshot and feasibility score |
| Runtime verification | Observed postcondition | After execution | Rollback, stop dependents, or escalate | Before/after state comparison |
These layers are complementary. Prompt-only controls are inexpensive but hard to audit. AST gates are deterministic for the structures they cover, while evidence and runtime checks address risks that syntax cannot see.

What is a practical implementation sequence?
Build the smallest useful policy surface first, then expand it from observed failures.
- Inventory actions: List every tool, data sink, state transition, and irreversible operation. Classify each by risk and required approval.
- Define the IR: Version the proposal schema and make ambiguous values impossible to represent. Include limits, evidence, preconditions, postconditions, and stop behavior.
- Implement the parser: Produce typed AST nodes with source locations and canonical identifiers. Reject unknown syntax rather than dropping it.
- Write deny predicates: Start with tool allowlists, secret detection, evidence requirements, approval checks, state preconditions, and finite resource limits.
- Add a policy decision object: Return
accepted,reason_code,node_path,policy_version, and remediation guidance. - Connect authorization: Mint capabilities only after all mandatory predicates pass; bind them to the AST digest.
- Sandbox execution: Enforce limits in infrastructure, not in prompt text. Add idempotency and a kill switch.
- Verify outcomes: Read the resulting state and compare it with the declared postcondition.
- Replay and test: Build fixtures from every rejection and incident. Run them against new models, prompts, parser versions, and policies.
A simplified controller looks like this:
proposal = model.generate(request, typed_schema)
ast = parse_and_normalize(proposal)
decision = policy_gate(ast, current_state, evidence_store)
if not decision.accepted:
audit.reject(ast, decision)
return abstain_or_repair(decision)
capability = authorize(ast, decision)
result = execute_in_sandbox(ast, capability)
verified = verify_postcondition(ast, result, fresh_state())
audit.record(ast, decision, result, verified)
return verified
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. What trade-offs should you expect?
Strict gates reduce unsafe actions but can reject useful plans when state is stale, evidence systems are incomplete, or policies are too broad. They also add parsing, policy evaluation, storage, and verification latency. Keep read-only and low-risk paths lightweight while applying stronger checks to writes and irreversible operations.
Constrained decoding may lower fluency or probability of a desired continuation when the valid set is narrow. Draft-conditioned methods aim to recover quality, but their reported gains are benchmark results, not evidence that a gate is universally safe.
Decoding interventions can improve factuality without replacing policy enforcement. Chuang and colleagues report that DoLa improved TruthfulQA by 12–17 percentage points for LLaMA-family models in work published by Microsoft Research in 2023 and at ICLR 2024. Their result shows that generation strategy matters; it does not show that AST gating alone prevents fabricated tool calls or unsupported actions.
Plan for policy evolution. Version schemas, predicates, evidence rules, and authorization issuers independently, and retain enough metadata to reproduce a decision after a model or tool changes.
What common designs fail, and how can you fix them?
| Failure pattern | Why it fails | Fix |
|---|---|---|
| Giving the model direct tool access | Prose and execution share one trust boundary | Require an accepted AST and capability-bound router call |
| Trusting a valid JSON object | Shape does not establish truth or permission | Add semantic predicates, evidence checks, and authorization |
| Checking only the final answer | Unsafe intermediate calls may already have side effects | Gate every tool node before execution |
| Using one global confidence score | Ignorance and unjustified certainty have different causes | Separate retrieval/evidence requirements from abstention calibration |
| Auto-repairing a rejected tree | A patch can preserve a hidden forbidden node | Generate a new proposal, parse again, and rerun all gates |
| Ignoring stale state | A once-valid precondition can become false | Use fresh reads, conditional operations, and postcondition checks |
| Logging only successful actions | Blind spots and near misses disappear | Persist rejected trees, predicate paths, and policy versions |
| Allowing unbounded loops | A benign plan can consume resources or repeat side effects | Require finite limits, timeouts, idempotency, and cancellation |
Which concepts should an engineering team learn first?
Teams implementing AST gates benefit from compiler fundamentals: lexical and syntactic parsing, abstract syntax trees, type checking, semantic analysis, and interpreters. Introduction to Compiler Design by Torben Ægidius Mogensen, third edition (2024), covers compiler construction, syntax and semantic processing, type checking, interpretation, and implementation techniques. It is a useful compiler design textbook for engineers who need to make policy decisions over trees rather than over raw text.
Pair that foundation with threat modeling for prompt injection, capability-based authorization, transactional APIs, evidence provenance, and statistical calibration. The safest architecture treats each as a separate control, because no single parser, prompt, decoder, or evaluator can establish every property.
What is the bottom-line design rule?
Make the model propose, make deterministic gates decide, make authorization bind to the accepted AST, and make verification check the real world afterward. You will not eliminate every hallucinated token, but you can prevent unsupported or unauthorized proposals from becoming actions and make every rejection explainable and testable.

Should a rejected proposal always be sent back to the model for another try?
No. Retry only for repairable issues such as a missing field or evidence reference, and cap attempts and time. Security violations, expired approvals, and unavailable policy dependencies should fail closed or escalate rather than trigger blind retries.
How can I test whether a new model bypasses my constraints?
Replay a corpus of accepted plans, rejected plans, prompt-injection attempts, malformed trees, and stale-state cases. Compare predicate decisions and post-execution outcomes across model versions, and treat any newly accepted forbidden case as a release-blocking regression.
What should an audit record contain for a disputed action?
Store the canonical AST and digest, schema and policy versions, model identifier, request and prompt hashes, evidence references, authorization scope and expiry, state snapshot, tool responses, verification result, and every rejection or repair decision. This lets an investigator reproduce what the system knew and why it acted.

Can an AST gate protect actions performed outside the tool router?
No. The guarantee applies only to effects that are forced through the governed execution boundary. Remove ambient credentials, restrict network and filesystem access, and monitor for out-of-band side effects so the model cannot bypass the gate through an ungoverned channel.
