Spoilers for Acts 1–3: The safest way to avoid Baldur’s Gate 3’s worst outcomes is to make a manual save before every major act transition and before any location that visibly advances a companion quest. Recruit everyone you can, talk to companions at camp, finish urgent side quests, bring the relevant companion to their personal quest location, and treat the Shadowfell, the Mind Flayer Colony and late-game transformation choices as stop-and-save checkpoints.

This is advice for a completion-oriented run that preserves the most optional companions, factions and quests—not a claim that one moral route is objectively correct. BG3 is built around consequential choices; a painful ending can be an intended story conclusion rather than a failed quest. Larian describes it as a story-rich, party-based RPG in which your choices shape “a tale of fellowship & betrayal, survival & sacrifice, and the lure of absolute power.”

What counts as a “worst outcome” in BG3?

Not every bad roll or disapproving reaction has the same cost. Judge a decision on four axes:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Party status: Does a companion die, leave permanently or remain recruitable?
  • Quest access: Does a companion, faction or regional quest become unavailable?
  • World-state cost: Are refugees, prisoners, major NPCs or rewards removed?
  • Run status: Is the result tragic but playable, or does it end the game?

That produces a useful severity scale:

  1. True game over: the run ends immediately or after an explicit fatal warning.
  2. Permanent companion loss: a character dies, leaves or can no longer join.
  3. Permanent quest or faction lockout: the party survives, but a branch, reward or region state is closed.
  4. Tragic but playable ending: the companion reaches an intended, painful conclusion such as death or returning to Avernus.
  5. Ordinary approval loss or a failed check: disappointing, but not a permanent content loss.

What should I do before making any major decision?

Use the same preparation routine whenever the game presents a hostile conversation, a new region or a warning that events will advance.

  1. Make a manual save. Use a named slot rather than relying only on quicksave or autosave.
  2. Recruit available companions. Companions can be dismissed and exchanged at camp, so having someone in the roster does not require keeping them in the active party.
  3. Visit camp. Speak to everyone with an exclamation mark or new dialogue. Camp conversations can change approval and unlock the next step of a personal quest.
  4. Resolve urgent side content. Rescue prisoners, deal with time-sensitive quests and finish companion objectives in the current region before crossing its point of no return.
  5. Check your party and inventory. Bring the companion whose quest is about to trigger and carry the key item that quest requires.
  6. Read warnings literally. If a dialogue or location says events may become irreversible, stop, save and finish your checklist first.

A missed approval line is usually recoverable. A companion leaving, a faction being destroyed or a region changing state is not.

What choices permanently lose companions in BG3?

The largest early loss is the Emerald Grove decision. Later losses usually come from entering a major threshold without the right companion or item, or from resolving a personal quest in a way that removes the character from your party.

Do not raid the Emerald Grove if you want the broadest roster

Raiding the Emerald Grove is a valid role-playing route, but it has a large mechanical cost: Wyll and Karlach leave, and Halsin becomes unavailable. It is therefore not merely a choice between two rewards. It changes which companions and future quest lines exist in the run. If preserving the most optional content is your priority, defend the Grove instead of raiding it.

Recruit companions and keep their conversations moving

The principal recruitable Origin Characters are Astarion, Gale, Lae’zel, Shadowheart, Karlach, Wyll and the Dark Urge. Recruit characters when the opportunity appears, then check camp after major conversations or discoveries. Dismissing someone temporarily is safe; ignoring a required conversation while advancing the story is what can close a path.

Resolve Karlach’s early work before leaving the first region

Handle Karlach’s recruitment and infernal-engine work before heading to the Rosymorn Monastery Trail or the Shadow-Cursed Lands. Moving on without dealing with Hunt the Devil can leave her early quest state unresolved and remove opportunities to help her later.

What should I do before entering the Shadowfell?

Treat the Shadowfell as one of BG3’s clearest Act 2 checkpoints. Make a manual save, finish the Act 2 side content you care about, bring Shadowheart, and confirm that the Spear of Night is available before entering.

  1. Save outside the entrance. Keep a separate slot so you can return to the pre-Shadowfell state.
  2. Finish tiefling, prisoner and rescue work. Side quests and rescue opportunities can be locked when the story advances.
  3. Visit camp. Talk to Shadowheart and any companion whose quest is still active.
  4. Bring Shadowheart. Entering without her can cause her to leave and prevents her from taking part in the sequence built around her faith and identity.
  5. Carry the Spear of Night. Entering without it can prevent the intended progression of Shadowheart’s quest.
  6. Do not let Balthazar retrieve the Nightsong if your goal is to preserve Shadowheart’s continued party involvement. That outcome radically changes the story state and can make her leave.

These instructions preserve options; they do not guarantee a cheerful ending. The Nightsong sequence is designed to force a consequential character decision, and different choices deliberately produce different stories.

Can I still save Wyll’s quest if I missed Mizora?

If Wyll’s story matters, resolve Mizora’s pod sequence in the Mind Flayer Colony before leaving the area. The pod is easy to overlook in the colony’s side chambers, but leaving it unresolved can permanently lock Wyll’s companion quest. Failing to save Mizora—or killing her in the pod—is also recorded as a trigger for losing Wyll.

When you reach the colony, search its side rooms rather than following only the main objective. Bring Wyll if possible, inspect the pod, complete the interaction and then visit camp to check for his follow-up dialogue. Do not assume that recruiting Wyll earlier protects the quest automatically.

Which Shadowheart decisions are irreversible?

Shadowheart’s arc combines a personal quest, a faction conflict and several permanent departure conditions. Bring her to the relevant locations and save before each final confrontation.

  • Entering the Shadowfell without Shadowheart can make her leave.
  • Entering without the Spear of Night can block the intended progression of her quest.
  • Allowing Balthazar to retrieve the Nightsong can make her leave and radically alter the Act 2 state.
  • Offering her to Viconia after she rejects Shar can permanently remove her.
  • Killing her parents after rescuing them is another permanent outcome.

These are not ordinary approval penalties. They change whether Shadowheart remains available and which later scenes can occur. If you want to role-play a different belief system, make the save first so the consequence is deliberate.

How do I get the best companion endings in BG3?

There is no universal “best” ending. For a completion-oriented run, the practical goal is to preserve each companion’s decision point until you have seen the relevant alternatives and resolved the preceding quest.

Wyll and Karlach

Resolve Wyll’s companion quest so he becomes the Blade of Avernus if you want him available to accompany Karlach later. Karlach’s engine storyline can end with her death or with her return to Avernus. Both are narrative endpoints, not automatically failed quests; they differ in emotional cost and in who remains with her.

Astarion

Bring Astarion to the Cazador confrontation. Killing Cazador without Astarion, under certain approval conditions, is recorded as a trigger that can remove Astarion’s quest path. Save before the confrontation and do not treat the boss fight as interchangeable with his personal resolution.

Gale

Gale’s needs and later quest beats must be addressed as they arise. Ignoring them can create severe story consequences, including late-game situations that threaten the run. Speak with him at camp after major Act 2 and Act 3 developments rather than assuming the main quest will advance his arc for you.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Lae’zel

Resolve Lae’zel’s crèche situation before advancing past the crèche. If you leave without dealing with the conflict, her personal quest and later faction choices can be locked or radically changed.

Halsin

Complete the steps needed to lift the Shadow Curse before leaving the Shadow-Cursed Lands if you want Halsin’s full arc. Leaving without fully resolving Lift the Shadow Curse closes that outcome even if Halsin has joined your camp.

Jaheira and Minsc

Protect Jaheira through her Act 2 appearances and recruit her before attempting to secure Minsc. If Jaheira leaves before Minsc is recruited or protected, the Minsc path can be lost. Bring Jaheira to the relevant Act 3 encounters and save before final confrontations.

Which Baldur’s Gate 3 decisions lock me out of quests?

Quest lockouts usually happen when you cross a region threshold, resolve a faction conflict or complete a personal quest without the companion present. The following checklist covers the highest-cost documented examples.

Checkpoint What can be lost Completion-oriented safeguard
Emerald Grove raid Wyll and Karlach leave; Halsin becomes unavailable Defend the Grove if you want the broadest companion roster
Rosymorn Monastery Trail or Shadow-Cursed Lands Karlach’s early infernal-engine work can remain unresolved Finish Hunt the Devil first
Shadowfell entrance Shadowheart’s quest and other Act 2 side quests can be locked Save, clear side content, bring Shadowheart and carry the Spear of Night
Nightsong and Balthazar Shadowheart can leave; Act 2 state changes radically Do not let Balthazar retrieve the Nightsong when preserving her arc
Mind Flayer Colony Wyll’s companion quest can be permanently locked Resolve Mizora’s pod before leaving
Leaving the crèche Lae’zel’s personal and faction path can be altered Complete the crèche situation before moving on
Leaving the Shadow-Cursed Lands Lift the Shadow Curse outcome and parts of Halsin’s arc Finish the required steps before departure
Act 3 personal confrontations Astarion, Gale, Jaheira, Minsc or Minthara content Bring the relevant companion and save before the final encounter

What are the point-of-no-return choices in Baldur’s Gate 3?

Points of no return are not limited to one button or warning. They include entering a new act, moving through the Shadowfell, leaving a region after a major objective and accepting a companion’s final personal choice. The game may advance several quests at once when you cross one of these boundaries.

Use this sequence before every threshold:

  • Check the journal for unfinished companion and rescue objectives.
  • Speak to every companion at camp.
  • Confirm the active party includes the character whose quest is about to trigger.
  • Carry quest-specific equipment such as the Spear of Night.
  • Make a manual save and name it for the location.
  • Proceed only after you are comfortable losing the unfinished content shown in the journal.

In Honour Mode, this preparation is especially important because the mode is not designed around reloading to erase a consequence.

What choices can cause a genuine game over?

Some late Act 2 and Act 3 decisions can end the run rather than merely removing a companion. The game-over conditions documented for these transitions include ignoring explicit warnings after major Act 2 progression and certain late-game decisions involving Gale or mind-flayer hunger.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A game-over result is different from Karlach returning to Avernus, Shadowheart leaving or a faction becoming hostile. Those outcomes can be devastating but still leave a playable campaign; a true game over cannot.

How should I handle a tragic companion ending?

Separate content preservation from emotional preference. Karlach’s death or return to Avernus, a companion rejecting the party, and a character choosing a painful fate can all be intentional conclusions that reflect earlier role-playing decisions.

If you want to see alternatives, save before the final conversation and keep the companion present. If you prefer the consequences of your current role-play, accept the ending without labeling it a failure. The purpose of this checklist is to prevent accidental lockouts—not to flatten BG3’s deliberately different stories into one approved morality route.

What is the most reliable save-and-checklist routine?

For a long campaign, use named saves tied to the decision rather than dozens of anonymous quicksaves.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. “Before Grove decision”: save before committing to the goblin conflict’s final route.
  2. “Before region exit”: save before leaving the first region, the crèche or the Shadow-Cursed Lands.
  3. “Before Shadowfell”: verify Shadowheart, the Spear of Night and all desired Act 2 side quests.
  4. “Before colony exit”: verify Mizora’s pod and Wyll’s follow-up.
  5. “Before personal boss”: bring Astarion, Gale, Lae’zel, Jaheira, Minsc, Minthara or another relevant companion to their confrontation.
  6. “Before ending choice”: save before transformation, sacrifice and companion-fate dialogues that display an explicit warning.

This routine cannot make every outcome happy, but it makes the consequences legible and prevents the most common accidental losses.

How we chose these warnings

The checklist prioritizes documented triggers that remove a companion, close a quest or faction branch, change a major world state or end the run. Ordinary approval loss, one failed ability check and deliberately tragic but playable endings are listed separately so they are not confused with permanent content loss.

Frequently Asked Questions

Does recruiting a companion guarantee that their quest will remain available?

No. Recruitment only places the character in your roster. You still need to bring them to relevant locations, answer their camp conversations and complete their required steps before advancing a region.

Can I dismiss a companion without losing them permanently?

Usually, yes: companions can be exchanged at camp. Permanent loss is tied to specific deaths, departures, dismissals at documented story points or choices that reject the character—not to ordinary party rotation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Is there a way to undo a bad decision in Honour Mode?

Honour Mode is designed without the normal safety of reloading after a consequence. Plan around warnings and thresholds before acting; a manual save is useful for reference, but it should not be treated as a guaranteed reversal.

Why did a quest disappear after I entered a new region?

Crossing an act or regional threshold can advance unresolved objectives and change the world state. Check your journal and camp dialogue, then use a pre-threshold save if the missing quest is important to your run.
