Apple Watch Sleep Apnea Notifications can screen for patterns associated with moderate to severe sleep apnea, but they do not diagnose the condition. On supported models, the watch records accelerometer-based breathing disturbances while you sleep, analyzes repeated nights in a 30-day window, and can prompt you to seek clinical evaluation.
You need a supported Apple Watch, current software, a compatible iPhone, authorized regional availability, and at least 10 nights of usable recordings. A notification warrants a conversation with a healthcare professional; no notification does not rule out sleep apnea.
What Apple Watch Sleep Apnea Notifications actually detect
The feature is an opportunistic risk screen. During sleep, the Apple Watch accelerometer measures small wrist movements that can accompany interruptions in otherwise regular respiratory patterns. Apple labels the nightly result Breathing Disturbances and expresses it as events per hour.

The algorithm looks for a sustained pattern that is consistent with signs of moderate to severe sleep apnea. It is designed to encourage medical follow-up, not to replace a sleep study or a clinician’s diagnosis, treatment, or monitoring.

It also is not a live alarm. The watch does not promise to alert you every time breathing pauses during a particular night. Instead, it passively gathers data over multiple nights and evaluates the pattern after enough recordings are available.
Compatibility and prerequisites
Apple’s support requirements list the following combinations. Availability can also depend on authorization in your country or region.
| Requirement | What is supported or required |
|---|---|
| Apple Watch model | Apple Watch Series 9 or later, Apple Watch Ultra 2 or later, or Apple Watch SE 3 |
| Unsupported examples | The first-generation Apple Watch Ultra and older Series models are not listed as supported for Sleep Apnea Notifications |
| Software | The latest available watchOS for the watch, with a compatible iPhone running the required companion software |
| Phone pairing | The watch must be paired with a compatible iPhone |
| Regional availability | The feature must be authorized and available in your country or region |
| Overnight use | Wear the watch consistently during sleep so it can collect usable breathing-disturbance data |
| Minimum data for a notification | At least 10 nights with usable recordings in a 30-day analysis window |
Model eligibility alone is not enough. A supported watch running outdated software, paired to an incompatible iPhone, or used in a region where the feature is not authorized may not offer the notification feature. Apple also does not list the first-generation Ultra or earlier Series watches as compatible, even if they receive other current software features.
How the detection algorithm works
1. The accelerometer records overnight movement
While you sleep, the watch’s accelerometer captures subtle wrist movements. The algorithm uses those movements as an indirect signal of changes associated with breathing interruptions. It does not use a clinical airflow sensor, blood-oxygen measurement, or laboratory polysomnography channel as its primary input for this feature.

2. Each sleep recording produces a breathing-disturbance value
For a usable night, the system produces a Breathing Disturbances result in events per hour. A higher value indicates more movement patterns associated with disrupted breathing during that recording; it is not itself a diagnosis or a clinical apnea-hypopnea index.
3. The watch evaluates a sequential 30-day window
After onboarding, the notification algorithm works through sequential, non-rolling 30-day windows. “Non-rolling” means the evaluation is not recalculated from a continuously sliding 30-day period every time you look at the app. The feature gathers the window’s recordings, applies its rule, and then begins the next window.
4. At least 10 recordings are needed
A window must contain at least 10 sleep recordings with usable breathing-disturbance data. Missing nights, nights when the watch was not worn, or recordings that do not produce usable data cannot substitute for those observations.

If a qualifying window has at least 10 recordings and at least half of those values are elevated, the feature can surface a possible-sleep-apnea notification. The exact threshold used to classify an individual value as elevated is part of Apple’s algorithm; the user-facing result is the notification, not a clinical score to interpret independently.

How long Apple Watch detection takes
The earliest possible notification requires 10 usable nights inside a 30-day window. That does not mean every person will receive a result after exactly 10 nights: the algorithm needs the qualifying pattern, and the watch may need additional nights if recordings are missing or unusable. In practice, consistent overnight wear throughout the window gives the system the best chance to complete its analysis.
Because the windows are sequential rather than continuously rolling, a short burst of nights may not produce an immediate alert followed by daily updates. The feature is intended to identify a persistent pattern, so a single unusual night is not treated as a definitive finding.
Charging and wearing habits matter. Plan charging so the watch is on your wrist for as many nights as possible, and follow Apple’s overnight-wear instructions. If you remove the watch for much of the night or have repeated gaps in recordings, the system may not reach the minimum usable-data requirement.
How accurate is Apple Watch sleep apnea detection?
Apple’s September 2024 clinical validation enrolled 1,499 participants; 1,448 completed the study. The published results report weighted overall sensitivity of 66.3% (95% confidence interval, 62.2%–70.3%) and weighted overall specificity of 98.5% (95% confidence interval, 98.0%–99.0%). Sensitivity for severe sleep apnea was 89.1% (95% confidence interval, 83.7%–93.2%).

| Measure | Published result | How to interpret it |
|---|---|---|
| Overall sensitivity | 66.3% (95% CI 62.2%–70.3%) | Among participants who had the study-defined condition, about two-thirds were identified by the notification algorithm |
| Overall specificity | 98.5% (95% CI 98.0%–99.0%) | Among participants who did not have the study-defined condition, most did not receive a notification |
| Severe sleep-apnea sensitivity | 89.1% (95% CI 83.7%–93.2%) | The algorithm identified severe cases more often than the overall group in this validation |
| Validation population | 1,499 enrolled; 1,448 completed | These results come from Apple’s reported clinical-validation study, not a guarantee for every wearer |
Sensitivity and specificity answer different questions. The high specificity is useful for a recurring passive screen because it is intended to limit false-positive notifications. The lower overall sensitivity means that some people with moderate or severe sleep apnea will not receive a notification. A person should therefore not use a quiet result as evidence that sleep apnea has been excluded.
The FDA submission says the algorithm was trained on more than 11,000 nights of concurrent Apple Watch accelerometer and clinical-reference data, including polysomnography and home sleep-apnea test recordings. Training data and validation results show how the algorithm was developed and evaluated; they do not turn the watch into a diagnostic test for an individual.
Regulatory status and what “cleared” means
The U.S. Food and Drug Administration database lists the Sleep Apnea Notification Feature (SANF) under 510(k) K240929, with a substantially equivalent decision dated September 13, 2024. FDA describes product code QZW as an over-the-counter device intended to provide a notification of sleep-apnea risk in users who have not previously been diagnosed with sleep apnea.

That intended use is narrower than diagnosis. Apple states: “Your Apple Watch is not a medical device and is not intended to replace talking with your doctor for diagnosis, treatment, or monitoring of sleep apnea.” The FDA clearance supports a risk notification, not a confirmed diagnosis, severity grading for an individual, or treatment recommendation.

What to do if the watch sends a notification
- Read the notification as a screening result. It indicates that the recorded pattern is consistent with possible sleep apnea, not that the condition has been proven.
- Contact a healthcare professional. Share the notification and describe relevant symptoms, sleep quality, and medical history. A clinician can assess risk factors and decide what evaluation is appropriate.
- Complete the test your clinician recommends. Depending on your circumstances, that may be a home sleep-apnea test or an overnight laboratory sleep study. Those tests, rather than the watch notification, provide the clinical evidence used for diagnosis.
- Follow through even if you feel well. Sleep apnea can be underrecognized, and the purpose of the notification is to prompt a conversation about a condition that may otherwise go unassessed.
Sairam Parthasarathy, M.D., professor and director at the University of Arizona Health Sciences Center for Sleep, Circadian, and Neurosciences, described the potential benefit this way: “Empowering consumers everywhere to have the ability to reliably identify the presence of abnormal breathing patterns during sleep can help uncover a woefully underdiagnosed and serious medical condition such as sleep apnea.” The practical implication is referral and testing, not self-treatment based on the watch alone.
What a missing notification means
No notification can have several explanations: the window may not contain enough usable nights, fewer than half of the recorded values may have been elevated, or the pattern may fall below the algorithm’s notification threshold. It can also reflect the limits shown by the validation study, including the fact that overall sensitivity was 66.3%.
Therefore, the absence of an alert does not rule out sleep apnea. If you have symptoms or concerns, discuss them with a healthcare professional regardless of what the watch reports. The feature is an additional screening signal, not a clearance result.
Apple Watch versus clinical sleep testing
When considering any wearable or test, compare the things that determine what the result can actually tell you: the regulatory intended use, sensors, amount of overnight data, published performance, compatibility, and who interprets the result.
| Comparison point | Apple Watch Sleep Apnea Notifications | Home sleep-apnea test | Laboratory sleep study |
|---|---|---|---|
| Intended use | Notification of possible sleep-apnea risk | Clinical evaluation selected and ordered by a healthcare professional | Clinical evaluation selected and interpreted by a healthcare professional |
| Primary data source | Accelerometer-derived wrist movements summarized as Breathing Disturbances | Clinical sensors specified for the prescribed test | Comprehensive laboratory recording configured by the sleep service |
| Overnight data requirement | At least 10 usable nights within a 30-day window for a possible notification | Duration and number of nights depend on the test protocol | Usually an attended or monitored study night under the laboratory’s protocol |
| Result interpretation | Algorithm notification; clinician follow-up is still needed | Clinician interprets the recorded study | Sleep professionals interpret the study and clinical context |
| Can it diagnose sleep apnea by itself? | No | Used by clinicians as part of diagnosis | Used by clinicians as part of diagnosis |
The watch is convenient because it can collect data during normal sleep without a separate test appointment, but convenience does not remove the need for clinical interpretation. A prescribed home or laboratory study answers a different question and should be chosen with a clinician.
Troubleshooting an unavailable or delayed notification
The feature is missing from the watch or iPhone
- Verify that the watch is Series 9 or later, Ultra 2 or later, or SE 3.
- Check that the watch and paired iPhone have the latest available compatible software.
- Confirm that the watch is paired with a compatible iPhone.
- Check whether Sleep Apnea Notifications are authorized in your country or region.
The watch has recorded sleep but no notification appears
- Confirm that at least 10 nights with usable breathing-disturbance data have accumulated in the current 30-day window.
- Continue wearing the watch consistently; an incomplete window may not yet satisfy the algorithm’s data rule.
- Remember that at least half of the qualifying values must be elevated before the feature can notify you.
- Do not interpret the absence of a notification as proof that breathing is normal; sensitivity is not 100%.
You received a notification but are unsure what to do
Save or show the result to your healthcare professional and ask whether a home sleep-apnea test or laboratory study is appropriate. Do not attempt to confirm or treat the condition solely from the watch’s Breathing Disturbances value.
Quick Recap
Key points to remember
- Supported hardware, current software, a compatible iPhone, regional authorization, and regular overnight wear are all required.
- The watch uses accelerometer-derived movement patterns and reports Breathing Disturbances in events per hour.
- Analysis requires at least 10 usable nights in a sequential 30-day window; it is not an instant breathing-pause alarm.
- Apple’s published validation reported 66.3% overall sensitivity, 98.5% overall specificity, and 89.1% sensitivity for severe sleep apnea.
- FDA clearance covers notification of risk, not diagnosis.
- A notification should lead to medical follow-up, while no notification cannot exclude sleep apnea.
