AI can find potentially suitable clinical trials faster, but it should not enroll patients or replace research staff. The safest model is clinician-supervised: software searches trial catalogs and patient records, translates eligibility rules into checks, ranks possible matches, and shows the evidence behind each result. Qualified clinicians and coordinators then verify the record, resolve missing or contradictory information, explain options to the patient, and obtain informed consent.

That distinction matters because matching accuracy is only one part of recruitment. A system can save screening time yet miss people whose records are incomplete, reproduce historic access disparities, or expose sensitive data if governance is weak. Current evidence supports AI as an assistant for discovery and prescreening—not as an autonomous decision-maker.

What can AI actually do in clinical trial recruitment?

AI recruitment tools reduce the manual work between a trial opening and a patient conversation. They can search thousands of protocol records, interpret free-text eligibility criteria, scan structured and unstructured clinical data, and produce a ranked list of possible studies. The output is a recommendation with supporting evidence, not a final eligibility determination.

Trial discovery

A system can query authoritative trial registries and a research site’s own trial-management systems using disease, biomarker, treatment-history, location, age, and other attributes. Natural-language processing is useful when a protocol says “adequate organ function” or describes a prior therapy in prose rather than a single database field.

Criterion-level prescreening

Instead of returning only a trial title, a well-designed tool evaluates each inclusion and exclusion criterion separately. It should show the matching laboratory value, diagnosis code, note excerpt, genomic result, or the fact that no usable evidence was found. “Unknown” is materially different from “does not qualify.”

Prioritizing human work

Recruiters can focus first on patients with the strongest evidence of a possible match, while still reviewing borderline and incomplete cases. This changes the coordinator’s task from searching every chart manually to validating a transparent shortlist and contacting patients appropriately.

How accurate is AI trial matching?

Accuracy depends on the model, the protocol, the data available, and the quality of human review. The strongest published result in the available data is NIH’s 2024 TrialGPT evaluation: it was compared with three clinicians on more than 1,000 patient-criterion pairs and achieved nearly the same accuracy. In a small pilot involving six patients, clinicians using TrialGPT spent 40% less time screening while maintaining the same accuracy.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Those findings are encouraging but bounded. A six-patient pilot is evidence of workflow efficiency, not proof of better enrollment across diseases, sites, or populations. The benchmark measured criterion-level matching; it did not establish that AI causes more completed consents or successful treatment starts.

Measure What was reported How to interpret it
Criterion-level accuracy TrialGPT was close to three clinicians across more than 1,000 patient-criterion pairs (NIH, 2024). Useful evidence for prescreening; not a license for automatic enrollment.
Screening time 40% less time in a six-patient clinician pilot (NIH, 2024). Suggests efficiency under pilot conditions; performance may differ in routine operations.
Operational activity Ohio State reported 87 active users, 242 active trial queries, and more than 3,000 patients screened from August 2023 through February 2024. Shows adoption and throughput, not a measured causal enrollment increase.
Enrollment outcome Ohio State said enrollment outcomes were pending data-capture tools. No universal enrollment-lift percentage has been established.

When comparing systems, ask for false-negative and false-positive rates by disease area, protocol, site, and patient subgroup. A high overall match rate can hide missed candidates in a small population or a site whose data is less complete.

Will AI replace clinical research coordinators?

No. AI can automate searching and organize evidence, while coordinators retain the responsibilities that require context, judgment, and trust. NIH’s TrialGPT workflow produces an annotated, ranked list for clinicians to discuss with patients; it does not make the consent decision or autonomously enroll anyone.

Work that remains human

  • Confirming that a diagnosis, treatment history, laboratory result, or imaging finding is current and clinically meaningful.
  • Investigating conflicting notes, outdated medication lists, and protocol exceptions.
  • Explaining risks, alternatives, travel requirements, randomization, and what participation involves.
  • Obtaining informed consent and documenting that the patient understood the discussion.
  • Deciding how to approach a patient respectfully, including whether contact is appropriate.
  • Communicating with investigators, sponsors, laboratories, and referring clinicians when a protocol question is ambiguous.

AI may change staffing mixes: fewer hours can be spent on repetitive chart searches and more on patient conversations, eligibility confirmation, retention, and complex cases. That is augmentation, not elimination.

What happens when the medical record is incomplete?

An incomplete record should produce an explicit uncertainty state and a human follow-up task, not an automatic rejection. Missing pathology, outside-hospital records, recent laboratory values, race or ethnicity fields, or genomic results can make a suitable patient look ineligible.

A safe incomplete-data workflow

  1. Label the gap. Show which criterion lacks evidence and distinguish missing data from a failed criterion.
  2. Show the date and source. A value from an old encounter or an external document may not satisfy a time-window requirement.
  3. Request confirmation. Route the case to a coordinator or investigator for chart retrieval, a laboratory order, or a direct clinical question.
  4. Preserve the partial match. Do not discard the patient merely because one field is unresolved.
  5. Record the resolution. Capture who verified the criterion, what evidence was used, and when the decision was made.

Systems should also expose contradictions—for example, a structured medication list that disagrees with a recent note—rather than silently choosing one source. Human reviewers need access to the original context and a way to correct the underlying data.

Can AI make clinical trials more diverse?

It can help identify overlooked candidates, but it cannot make recruitment equitable by itself. FDA’s December 2025 guidance connects representative enrollment with safety and effectiveness evidence that applies to the people who will use a product. It calls attention to demographic and clinical characteristics including sex, race, ethnicity, age, geography, organ dysfunction, comorbidities, disabilities, weight extremes, and low-prevalence diseases.

AI may search across records that staff would not have time to review, surface patients outside a trial’s usual referral network, and reveal where outreach is thin. The same system can also reproduce documentation gaps or historical disparities: patients with incomplete demographic fields, fewer specialist visits, limited internet access, or less local testing may be ranked lower or excluded.

Fairness checks that belong in production

  • Measure match, referral, screening, consent, and enrollment rates for relevant subgroups.
  • Compare false negatives and time-to-review across sites and populations, not only overall accuracy.
  • Display missing or uncertain demographic and clinical data instead of inferring sensitive attributes without a valid basis.
  • Have staff review borderline cases and cases from historically underrepresented groups.
  • Audit whether outreach channels, language support, transportation assistance, and visit schedules create barriers after a match is found.

Higher referral volume is not the same as representative enrollment. The equity test extends through consent, retention, and completion.

How did one health system implement AI matching?

Ohio State University Comprehensive Cancer Center selected Deep6 AI after a two-year evaluation and began implementation in January 2023 across 17 disease teams. Its workflows combined structured records, unstructured notes, genomics, registries, and trial-management data. Staff could build cohorts, search records, inspect keywords in context, see partial matches and exclusions, and validate queries manually.

From August 2023 to February 2024, the implementation document reported 87 active users across 12 disease teams, 242 active trial queries, and more than 3,000 patients screened. Enrollment outcomes were still awaiting data-capture tools, so these figures demonstrate feasibility and use rather than a proven enrollment lift. The document also gave historical context: 3% interventional enrollment nationally versus 26% at the center, with 14% therapeutic enrollment in the cited 2021 comparison. Those figures are context from that document and year, not a current national benchmark.

What should an organization evaluate before deployment?

Start with a narrowly defined use case and test it prospectively with research staff. A tool for finding candidates in an oncology service has different data, risks, and validation requirements from a tool for site feasibility or patient outreach prioritization.

Eight evaluation dimensions

Dimension Questions to ask
Eligibility performance What are precision, false negatives, and false positives for each protocol and subgroup?
Workload How much screening time is saved, and does review time simply move to another queue?
Data coverage Can it use structured fields, notes, pathology, imaging summaries, genomics, registries, and external records?
Explainability Does every criterion have an evidence link, timestamp, source, and uncertainty label?
Fairness Are performance and referral rates monitored across demographic and clinical groups?
Privacy and security Who can access data, where is it processed, how is it encrypted, and what audit logs and retention controls exist?
Interoperability Can it connect to the electronic health record, clinical-trial management system, registries, genomics systems, and ClinicalTrials.gov without duplicate manual entry?
Operations What training, query validation, change control, support, and ongoing monitoring are required?

Require a documented rollback or disablement procedure. A protocol amendment, data-source change, or model update can alter results even when the interface looks unchanged.

What is the human-first operating model?

A dependable program separates automated retrieval from accountable clinical decisions. Use this sequence:

  1. Define the intended use. State whether the system supports discovery, prescreening, site feasibility, or outreach prioritization. Do not expand the purpose silently.
  2. Translate criteria with clinical staff. Validate computable definitions, time windows, units, medication synonyms, and exception rules with investigators and coordinators.
  3. Retrieve candidates. Search authoritative registries and approved local data sources, recording the version or date of each protocol.
  4. Present evidence. Show criterion-level matches, exclusions, partial matches, missing fields, and source passages in context.
  5. Require qualified review. A clinician or trained research professional confirms the possible match before patient communication.
  6. Communicate and consent. Staff discuss the opportunity, answer questions, and follow the approved informed-consent process.
  7. Measure outcomes. Track false negatives, false positives, time saved, consent outcomes, subgroup representation, and model drift.
  8. Revalidate changes. Repeat testing after protocol amendments, data-source changes, or model updates.

Is AI recruitment safe and compliant?

Safety is a system property, not a marketing feature. It depends on validated data flows, role-based access, auditability, explainable outputs, human accountability, and a process for correcting errors. Recruitment software handles protected health information and can influence who gets access to experimental treatments, so privacy and fairness failures are clinical risks as well as technical ones.

FDA’s April 29, 2026 Federal Register notice proposed an AI-enabled early-phase clinical-trial pilot covering possible uses such as recruitment, dose escalation, safety monitoring, adaptive designs, biomarker assessment, biomarker-based selection, and endpoint validation. The notice describes trustworthy AI as valid, safe, secure, accountable, explainable, privacy-protective, and fair, and asks how to evaluate robustness, drift, subgroup performance, usability, and stakeholder trust.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

That proposal signals regulatory direction, not a blanket approval for any recruitment product. Organizations still need institutional review, privacy and security controls, documented validation, human oversight, and procedures aligned with applicable clinical-trial and health-information rules.

What are the most common failure modes?

Most problems arise at the boundary between a model’s output and real clinical work.

  • False reassurance: A ranked result is treated as confirmed eligibility. Fix it by labeling outputs as recommendations and requiring criterion-level review.
  • Silent missingness: Blank fields are interpreted as “no.” Fix it by separating unknown, partial, and failed states.
  • Stale protocols: The model uses an old eligibility version after an amendment. Fix it with versioned protocols and revalidation triggers.
  • Data-source bias: Patients documented outside the health system disappear from the candidate pool. Fix it by incorporating approved external records and measuring coverage.
  • Keyword overreach: A word in a note is mistaken for a confirmed condition or treatment. Fix it with context windows, source links, and reviewer validation.
  • Activity mistaken for impact: More queries or screened patients are reported as enrollment success. Fix it by connecting screening to consent, enrollment, and retention data.
  • Automation without ownership: Staff do not know who resolves an uncertain match. Fix it with named roles, escalation paths, and service-level expectations.

What does the evidence support today?

The evidence supports using AI to accelerate discovery and prescreening under clinician supervision. NIH’s TrialGPT results show that an annotated matching assistant can approach clinician accuracy in a benchmark and reduce screening time in a small pilot. Ohio State’s implementation shows that combining many data types across disease teams is operationally feasible, while also illustrating why adoption metrics must not be confused with causal enrollment results.

The practical conclusion is human-first, AI-enhanced recruitment: let software search broadly and expose evidence; let trained people interpret uncertainty, communicate with patients, protect privacy, and make accountable decisions. Organizations that measure subgroup performance and real downstream outcomes will learn whether a tool improves access rather than merely producing more candidate lists.

Frequently Asked Questions

Can a patient use AI to find a trial without a doctor?

AI-powered public search can help a person discover studies, but a potential match still needs confirmation against the current protocol and medical record. Eligibility can depend on laboratory values, prior therapies, timing, and exclusions that a public tool cannot verify.

What information improves an AI trial match?

Current diagnoses, medications, treatment dates, laboratory results, pathology, genomic findings, age, location, and prior trial history are especially useful. Records from outside hospitals and clearly dated source documents reduce the chance that a system mistakes missing information for ineligibility.

How should patients challenge an AI-generated rejection?

Ask the research team which criterion was considered unmet and what evidence supported that conclusion. You can provide updated records or request investigator review, particularly when the result was based on an unknown or outdated field.

Does AI recruitment require a new consent from every patient?

The consent requirement depends on how the system is used, the data involved, and applicable institutional and legal rules. Using an approved clinical record for internal prescreening is different from contacting a person for research or sharing data with an outside service, so the site’s privacy and ethics teams should define the process.
