Do not change production code solely because an AI tool labeled a finding “critical” or supplied a convincing explanation. Treat the report as a lead: verify the affected revision, trace the alleged input to the sensitive operation, reproduce the claimed behavior safely where possible, and establish the security impact before deciding whether and how to fix it.
What counts as verification?
A vulnerability claim is substantiated when evidence shows that the stated behavior occurs in the affected code under the stated conditions, is unintended or violates a security boundary, and has a credible security impact. A severity label, confident narrative, or test written by the same agent that produced the report is not that evidence.
Keep the report’s observations separate from its interpretation. For example, “this parameter reaches the database call without an apparent check” is an observation to investigate; “this is a critical SQL injection” is a conclusion that still needs proof of attacker control, exploitability, and impact.
OWASP’s AI guidance calls for qualified human review and heightened scrutiny of security-critical changes. It also warns that untrusted repository content, issue text, pull-request comments, links, and tool output can influence an AI agent. Treat those materials as untrusted input when an agent consumes them, and do not let the report’s own assumptions dictate the verification.

Normalize the report into testable claims
Before opening a fix, turn the finding into a short statement that another engineer could check without relying on the report’s rhetoric. Capture these details:
- Location: affected component, file or call path, and exact code revision or package version.
- Entry point: the input, state, or action alleged to be attacker-controlled, and how an attacker can reach it.
- Prerequisites: required account, role, network position, user interaction, configuration, or deployment condition.
- Expected behavior: what the application is meant to permit or reject, based on the relevant policy and design.
- Observed behavior: what actually happens, including the specific security boundary allegedly crossed.
- Impact and proposal: what an attacker could do if the claim holds, and what change the report proposes.
Ask the generator or reporter for a minimal reproduction and the evidence behind the claim if either is missing. Do not treat a generated proof of concept or a package/version suggestion as trustworthy just because it is specific.

Check the exact code path and assumptions
- Pin the version. Inspect the exact commit, build, dependency lockfile, and relevant configuration. A finding against another branch or a newer package release does not establish exposure in the deployed version.
- Trace reachability. Follow the claimed input through routing, parsing, validation, authorization, and any transformations to the sensitive operation. Confirm that the stated attacker can reach that operation under the stated prerequisites.
- Check the boundary. Identify what the code is supposed to protect—such as a user’s own records, privileged actions, secrets, or trusted data—and whether the observed path crosses that boundary.
- Verify dependencies independently. For a package finding, confirm the package exists, the application actually uses the affected version, and the reported vulnerability applies to that version and configuration. Cross-check against a recognized vulnerability database or the project’s advisory rather than relying on model memory.
- Compare with intended behavior. A surprising result is not automatically a vulnerability. Check the relevant access policy, API contract, and documented behavior before deciding that the behavior is unintended.
Reproduce the claim without putting production at risk
Use an authorized development or staging environment that matches the affected code and relevant configuration as closely as practical. Do not run untrusted proof-of-concept content against production or in a privileged environment. Use the smallest test that can distinguish the claimed vulnerable behavior from the expected safe behavior.
- Record the revision, configuration, dependencies, and test environment.
- State the attacker’s prerequisites and provide the minimal input or action needed to exercise the path.
- Run the test and capture the command or request, relevant logs, and actual result. Protect secrets and personal data in any retained artifacts.
- Compare the result with the expected behavior and note which part of the claim it does—or does not—demonstrate.
If a safe reproduction is unavailable, do not imply that the vulnerability has been reproduced. Use code review and controlled tests as substitute evidence, and record what remains uncertain. NIST SP 800-216, Recommendations for Federal Vulnerability Disclosure Guidelines (published May 24, 2023), addresses handling and communicating vulnerability reports; NIST’s software verification guidance recognizes varied methods, including static and dynamic analysis, black-box and structural testing, regression testing, and fuzzing.

Corroborate with checks suited to the claim
Independent checks are valuable because they can expose errors in the original report’s assumptions. Choose methods based on the suspected weakness; several methods may be appropriate for a security-critical claim.

Passing tests do not prove that code is secure; they show only that the tested behavior did not fail under the conditions exercised. For critical behavior, have a qualified human reviewer independently assess the report and the evidence. OWASP cautions against relying on AI-generated security tests without independent verification, especially when an agent both writes security-critical code and its tests.

Assess impact before accepting severity
Describe impact in terms of an attacker’s achievable action, not the tool’s risk label. Record the access or interaction required, the assets or users affected, and the difference between observed and intended behavior. Then consider whether those facts support the proposed severity under your organization’s severity framework and threat model.
A scanner’s critical finding should not be merged around casually. OWASP’s AI Security Verification Standard (AISVS) says a critical finding should block a pull request from merging; bypass requires a written exception approved by an authorized human. Apply the standard in the context of your organization’s policy and the finding’s evidence, rather than treating the label itself as proof.

Make a decision, then fix only what the evidence supports
Use one of three plain-language outcomes so the team knows whether it is acting on established evidence or uncertainty:
- Substantiated: evidence confirms the behavior, boundary violation, and material impact under stated conditions.
- Disproven: the relevant code path, version, prerequisite, or alleged behavior does not match the claim, with the reason documented.
- Uncertain: available evidence is insufficient to confirm or rule out the claim; state the missing evidence and who will resolve it.
For a substantiated issue, make the smallest change that addresses the demonstrated cause, then add a regression test that fails before the fix and passes afterward. Review for side effects, including whether the change weakens another control or breaks intended behavior. If a critical finding is not fixed before merge, follow the written-exception process described by AISVS and your own security policy.
Keep an evidence trail from report to deployment
Retain enough information for another reviewer to understand and, where safe, repeat the decision. A compact record should include:
- The original report and the normalized claim, including affected revision and configuration.
- Code-path analysis, attacker prerequisites, intended behavior, and the security boundary considered.
- Reproduction steps or substitute evidence, test inputs, commands, relevant logs, and results.
- Independent checks performed and their scope, including any limitations.
- The impact and severity rationale, decision, reviewer, and any approved exception.
- The remediation commit, regression test result, build, and deployment reference.
AISVS discusses correlation and replay across the prompt, response, commit, build, and deployment. NIST SP 800-216 provides guidance on vulnerability-report handling and communication. Preserving the links among those records makes it possible to revisit a decision if the code, threat model, or deployment changes.
Apply standards as process guidance, not proof
OWASP AISVS 1.0, released in June 2026, describes 191 requirements across 12 chapters and three appendices. Those figures describe the standard’s scope, not a measure of its accuracy or the effectiveness of any individual verification. Standards can structure review and escalation; they do not replace evidence about the affected application.
